        Atrial Fibrillation in Heart Failure: a Therapeutic Challenge of Our Times

        Atrial fibrillation (AF) and heart failure (HF) are growing cardiovascular disease epidemics worldwide. There has been an exponential increase in the prevalence of AF and HF correlating with an increased burden of cardiac risk factors and improved survival rates in patients with structural heart disease. AF is associated with adverse prognostic outcomes in HF and is most evident in mild-to-moderate left ventricular (LV) dysfunction where the loss of “atrial kick” translates into poorer quality of life and increased mortality. In the absence of underlying structural heart disease, arrhythmia can independently contribute to the development of cardiomyopathy. Together, these 2 conditions carry a high risk of thromboembolism due to stasis, inflammation and cellular dysfunction. Stroke prevention with oral anticoagulation (OAC) remains a mainstay of treatment. Pharmacologic rate and rhythm control remain limited by variable efficacy, intolerance and adverse reactions. Catheter ablation for AF has resulted in a paradigm shift with evidence indicating superiority over medical therapy. While its therapeutic success is high for paroxysmal AF, it remains suboptimal in persistent AF. A better mechanistic understanding of AF as well as innovations in ablation technology may improve patient outcomes in the future. Refractory cases may benefit from atrioventricular junction ablation and biventricular pacing. The value of risk factor modification, especially with regard to obesity, sleep apnea, hypertension and diabetes, cannot be emphasized enough. Close interdisciplinary collaboration between HF specialists and electrophysiologists is an essential component of good long-term outcomes in this challenging population.

        Clinical and Microbiologic Efficacy and Safety of Imipenem/Cilastatin/Relebactam in Complicated Infections: A Meta-analysis

        Background: Antimicrobial resistance is on the rise. The use of redundant and inappropriate antibiotics is contributing to recurrent infections and resistance. Newer antibiotics with more robust coverage for Gram-negative bacteria are in great demand for complicated urinary tract infections (cUTIs), complicated intra-abdominal infections (cIAIs), hospital-acquired bacterial pneumonia (HABP), and ventilator-associated bacterial pneumonia (VABP). Materials and Methods: We performed this meta-analysis to evaluate the efficacy and safety profile of a new antibiotic, Imipenem/cilastatin/relebactam, compared to other broad-spectrum antibiotics for complicated infections. We conducted a systemic review search on PubMed, Embase, and Central Cochrane Registry. We included randomized clinical trials-with the standard of care as comparator arm with Imipenem/cilastatin/relebactam as intervention arm. For continuous variables, the mean difference was used. For discrete variables, we used the odds ratio. For effect sizes, we used a confidence interval of 95%. A P-value of less than 0.05 was used for statistical significance. Analysis was done using a random-effects model irrespective of heterogeneity. Heterogeneity was evaluated using the I2 statistic. Results: The authors observed similar efficacy at clinical and microbiologic response levels on early follow-up and late follow-up compared to the established standard of care. The incidence of drug-related adverse events, serious adverse events, and drug discontinuation due to adverse events were comparable across both groups. Conclusion: Imipenem/cilastatin/relebactam has a non-inferior safety and efficacy profile compared to peer antibiotics to treat severe bacterial infections (cUTIs, cIAIs, HABP, VABP).

        Improvement of Dialysis Dosing Using Big Data Analytics

        Objectives: Large amounts of healthcare data are now generated via patient health records, records of diagnosis and treatment,smart devices, and wearables. Extracting insights from such data can transform healthcare from a traditional, symptom-driven practice into precisely personalized medicine. Dialysis treatments generate a vast amount of data, with more than100 parameters that must be regulated for ideal treatment outcomes. When complications occur, understanding electrolyteparameters and predicting their outcomes to deliver the optimal dialysis dosing for each patient is a challenge. This studyfocused on refining dialysis dosing by utilizing emerging data from the growing number of dialysis patients to improve patients’quality of life and well-being. Methods: Exploratory data analysis and data prediction approaches were performed togather insights from patients’ vital electrolytes on how to improve the patients’ dialysis dosing. Four predictive models wereconstructed to predict electrolyte levels through various dialysis parameters. Results: The decision tree model showed excellentperformance and more accurate results than the support vector machine, linear regression, and neural network models. Conclusions: The predictive models identified that pre-dialysis blood urea nitrogen, pre-weight, dry weight, anticoagulation,and sex had the most significant effects on electrolyte concentrations. Such models could fine-tune dialysis dosing levels forthe growing number of dialysis patients to improve each patient’s quality of life, life expectancy, and well-being, and to reducecosts, efforts, and time consumption for both patients and physicians. The study’s results need to be validated on a largerscale.

        A comparative study of two unani regimens in diabetic vulovaginal candidiasis

        To compare the efficacy of two Unani regimen in diabetic vulovaginal candidiasis (VVC). A prospective, randomized, single-blind, comparative trial was conducted at Nizamia Tibbia College. Group A (n = 23) medicine included 100 ml of Joshanda [Ushba Desi (5 g) and Tukhme Hayat (5 g)] with 3 g Sufuf [Mur Makki (1 g), Afsanteen (2 g), and Nagandbabari (3 g)] orally twice daily half an hour before meal. Locally, douche (Aab Barg Bakayan (5 g) and Sufuf Katha (1 g) followed by humul [Roghan Neem (10 ml), Luab Gheegawar (2 g) and Sufuf Zard Chobb (1 g)] was given daily once. Group B (n = 23) received 100 ml of aforementioned Joshanda with 3 g sufuf [Kutki Siyaha (2 g), Kali Zeeri (2 g) and Tukhme Kasni (2 g)] orally twice daily half an hour before meal. Locally, douche (Gulanar (5 g) and Barg babool 5 (g)] was given daily once. Both groups received the treatment for 10 days for three consecutive months. The primary outcomes included change in vaginal discharge and wet mount test. The secondary outcomes included other clinical features of VVC and blood glucose level. The results were analyzed. Group A and B showed absence of vaginal discharge and fungal spores/hyphae in 91.30% and 86.95% respectively, statistically not significant (P = 0.3), whereas pre- and post-test comparison was statistically significant in both groups (P < 0.001). Inter-group comparison was statistically non significant (P > 0.05) for remission of other clinical features and blood glucose level, whereas pre- and post-test comparison showed statistically significant difference in both groups (P < 0.001). Both groups were equally effective for the treatment of diabetic VVC.

      • A Smart and Efficient Interior Home Designing Framework using Augmented Reality with ARCore and SLAM

        Currently, scientific advancements and innovations are growing very rapidly and there is an extreme need for individuals to surround oneself with state-of-the-art digital gadgets. So, as a solution, an Augmented Reality (AR) which is an interactive combination of real and virtual reality is used to meet this level of demand. Nowadays, it makes simple and easy for individuals to shop due to its magnificent work in designing home interiors. With the help of AR, people can virtually envision the designed models of various interior items in a real-time physical environment by doing actions such as zooming in and out, dragging, and rotating. The platforms such as Google ARCore and Marker-less based AR technologies are used to assist design and implementation of AR. Different peoples can easily perceive this artificially created physical environment. But most of the applications do not fully satisfy their users as they are not providing a sufficient set of features for people such as SayDuck. In this work, we are presenting the solution in a real environment along with the functionality of items purchasing in interior designing applications.

        N-Acetyl-D-Glucosamine Kinase Is a Component of Nuclear Speckles and Paraspeckles

        Protein O-GlcNAcylation, dictated by cellular UDP-Nacetylglucosamine (UDP-GlcNAc) levels, plays a crucial role in posttranslational modifications. The enzyme GlcNAc kinase (NAGK, E.C. 2.7.1.59) catalyzes the formation of GlcNAc-6-phosphate, which is a major substrate for the biosynthesis of UDP-GlcNAc. Recent studies have revealed the expression of NAGK in different types of cells especially in neuronal dendrites. Here, by immunocytochemistry (ICC) and immunonucleochemistry (INC) of cultured rat hippocampal neurons, HEK293T and GT1-7 cells, we have showed that NAGK immuno-reactive punctae being present in the nucleoplasm colocalized with small nuclear ribonucleoproteinassociated protein N (snRNPN) and p54NRB, which are speckle and paraspeckle markers, respectively. Furthermore, NAGK IR cluster was also found to be colocalized with GTF2H5 (general transcription factor IIH, polypeptide 5) immuno reactive punctae. In addition, relative localization to the ring of nuclear lamin matrix and to GlcNAc, which is highly enriched in nuclear pore complexes, showed that NAGK surrounds the nucleus at the cytoplasmic face of the nuclear outer membrane. By in situ proximity ligation assay (PLA) we confirmed the colocalization of NAGK with snRNPN in the nucleus and in dendrites, while we also verified the interactions of NAGK with p54NRB, and with GTF2H5 in the nucleus. These associations between NAGK with speckle, paraspeckle and general transcription factor suggest its regulatory roles in gene expression.

        Comparing Trackers for Multiple Targets in Warehouses

        Multiple object tracking is a complex and challenging computer vision problem. In industrial premises like warehouses, a robust multiple object tracking framework could provide useful information on lift truck and pedestrian movements. This information could be utilized for process improvement. It could also potentially promote greater safety awareness in the facility. We evaluate and analyze selected model-based and deep feature-based tracking mechanisms suitable for warehouse environments. Objects (or targets) are forklift trucks or people. Our experimental comparison and discussion facilitates useful insights into the design of robust multiple target trackers in warehouses, their limitations, and future research directions.
