Interferon (IFN)-associated psychiatric disorders can be managed without interruption to hepatitis C virus (HCV) treatment. The limited number of cases in the literature reporting psychotic depression as an adverse drug reaction to IFN resulted in discontinuation of HCV therapy. The author reports a case of a 49 year-old man with chronic HCV genotype 1a treated with pegylated interferon-alpha and ribavirin developing major depressive disorder with psychotic features. The patient was successfully treated with both an antidepressant and antipsychotic for this suspected IFN-associated adverse drug effect while continuing 12 months of uninterrupted HCV treatment and subsequently achieving sustained hepatitis C virological response. Although IFN can cause distressing psychiatric disturbances, appropriate treatment with psychotropic agents and careful monitoring allows patients to be maintained on a full course of HCV treatment.
Characteristics and 10 key components of interpersonal caring: a narrative review

This paper aims to help people understand better the lives of people who are mentally ill by describing the general concept of the Interpersonal Caring Theory (ICT) and deducing 10 key components of interpersonal caring. The literature review described the definition of interpersonal caring, and its assumptions and characteristics. Furthermore, the authors’ experience with patient care suggested the critical components of interpersonal caring, which is the compassion-based therapeutic actions/behaviors through the collaborative partnership developed between nurse and client. Essential characteristics of interpersonal caring include the following: person-to-person interaction between nurse and patient, genuine love and concern toward the person, conveying trust and hope, transcending space, time, and culture, holistic approach expressed through a comprehensive and dynamic mode of communication, helping the patient focus on their self-worth, and providing culturally relevant and sensitive nursing. Ten key components of interpersonal caring in ICT include noticing, participating, sharing, active listening, companioning, complimenting, comforting, hoping, forgiving, and accepting. Interpersonal caring results from the blended understanding of the empirical, aesthetic, ethical, and intuitive aspects of a given clinical situation, and a nexus of pre-conditions, content, feelings, and sense of self-worth/self-esteem.

Congenital tuberculosis is a very rare disease and the affected infants are often delivered prematurely. Even though proper treatment, the mortality rate is high. The preterm male infant born at 24 weeks of gestational age with birth weight of 760 g was admitted to neonatal intensive care unit (NICU) for intensive treatment. One month after delivery, his mother visited to outpatient clinic for headache and nausea and later she was diagnosed with tuberculous meningitis. Then, the preterm infant who was already hospitalized was examined for the possibility of congenital infection. As a result, congenital tuberculosis infection was confirmed by positive reaction with tuberculosis in acidfast bacilli stain, culture and polymerase chain reaction. The infant was completely isolated for 2 weeks and antituberculosis drugs were administrated. We performed chest radiography and skin reaction test for the other 29 infants and 60 coworkers in the NICU during the same period. The infants were treated with prophylactic antituberculous drug. Three months later, no abnormal findings were observed in any infants and any coworkers during the follow up period. We report the experience of successful treatment and isolation for congenital tuberculosis in premature infants.
