        In-line 마그네트론 스퍼터 장치를 사용하여 산소 분위기에서 제작한 Ag 박막의 특성

        A study was made to examine the electrical, compositional, structural and morphological properties of Ag thin films deposited by DC magnetron sputtering in $O_2$ atmosphere with deposition temperature from room temperature to 15$0^{\circ}C$ using in-line sputter system. The Ag films deposited at temperature above $100^{\circ}C$ in oxygen atmosphere gave a similar specific resistivity to and even lower oxygen content than those deposited using pure Ar sputter gas The Ag films deposited with pure Ar gas was mainly composed of crystallites with [111] preferred orientation, while, for those deposited in oxygen atmosphere, more than 50% of the volume was composed of crystallites with [100] orientation. The difference in the micro structure did not cause any difference in the specific resistivity of Ag films. The results showed that the transparent conducting oxide films and the Ag films could be processed sequentially in the same deposition chamber with careful control of deposition temperature, which might result in a cost reduction for constructing the large scale in-line deposition system.

        In Hegel, philosophical thinking that has not system, can not be a science. Hegel's system of science does not mean a collection of diverse Knowledge, but it means a systematic knowledge that all partial knowledge necessarly form a inner, progressive, organic relationship in the one unity-Already Kant, Fichte Schelling, etc., willed to construct a systematic knowledge, but those system is not in a inner, progressive, organic relationship, and so, those system can-not be a true organic unity of all partial knowledge. To construct a systematic knowledge, Hegel critisizes the organon, or the medium as a instrument in cognition, which divided existence and knowledge into two, and which was popular in those days. But to recognize The Absolute, Hegel lays claim-to monism in cognition that reconsiles existence with knowledge in one. Hegel insists to remove all preconception in nature of knowledge, and to reveal knowledge of Erscheinung. The very first knowledge is natural consciousness which rises from Erscheinung, and so , in Hegel's Phenomenology, natural consciousness makes progress from it to a true absolute knowledge. In its progress, we gradually understand that natural consciouness is not true knowledge, and this self-awakening is repeated over and over again, and so , in the end, we reach to the absolute knowledge. Therefore, Consciousness has various forms of knowledge, and these various forms of one in a inner, progressive, organic relationship, can be a systematic one Hegel's method to systematize knowledge is dialect ice, namely, Hegel's system relies on relationship with three moment of thinking movement. Three moment of thinking movement is following : Thinking of abstract understanding is not settled, unchangeable, ultimate, but this aufhebens itself continually, and in the end, this is altered to its opposition. On the contrary, thinking of positive reason comprises positively these two oppositions, these two contradiction, in itself to a higher dimension. Development of conscious in Hegel's phenomenology of Mind has relationship with these logical triple rhythm of thinking movement. Of course, self-awakening process of conscious- ness in Phenomenology of Mind is genuinly contemplative, descriptive, passive but we can find this triple framework in the result of phenomenological cognition's process throughout the Hegel's Phenomenology of Mind Wherever.

        Celay/In-Ceram, Conventional In-Ceram, Empress 2 전부도재관의 변연적합도에 관한 비교 연구

        There have been many studies about marginal discrepancy of single restorations made by various systems and materials. But many of statistical inferences are not definite because of sample size, measurement number, measuring instruments. etc. The purpose of this study was to compare the marginal adaptations of the anterior single restorations made by different systems and to consider more desirable statistical methods in analysing the marginal fit. The in vitro marginal discrepancies of three different all-ceramic crown systems (Celay In-Ceram. Conventional In-Ceram. IPS Empress 2 layering technique) and one control group (PFM) were evaluated and compared. The crowns were made from one extracted maxillary central incisor prepared with a 1mm shoulder margin and $6^{\circ}$ taper walls by milling machine. 10 crowns per each system were fabricated. Measurements or a crown were recorded at 50 points that were randomly selected for marginal gap evaluation. Non-parametric statistical analysis was performed for the results. Within the limits of this study, the following conclusions were drawn: 1 Mean gap dimensions and standard deviations at the marginal opening for the maxillary incisor crowns were $98.2{\pm}40.6{\mu}m$ for PFM, $83.5{\pm}18.7{\mu}m$ for Celay In-Ceram, $104.9{\pm}44.1{\mu}m$ for conventional In-Ceram, and $45.5{\pm}11.5{\mu}m$ for IPS Empress 2 layering technique. The IPS Empress 2 system showed the smallest marginal gap (P<0.05). The marginal openings of the other three groups were not significantly different (P<0.05). 2 The marginal discrepancies found in this study were all within clinically acceptable standards ($100\sim150{\mu}m$). 3. When the variable is so controlled that the system may be the only one, mean value is interpreted to be the marginal discrepancy of a restoration which is made by each system and standard deviation is to be technique-sensitivity of each one. 4. From the standard deviations. the copy-milling technique (Celay/In-Ceram) was not considered to be technique-sensitive in comparison with other methods. 5. Parametric analysis is more reliable than non-parametric one in interpretation of the mean and standard deviation. The sample size of each group has to be more than 30 to use parametric statistics. The level of clinically acceptable marginal fit has not been established. Further studies are needed.

        <i>In vitro</i> and <i>in vivo</i> evaluation of a novel polymer-free everolimus-eluting stent by nitrogen-doped titanium dioxide film deposition

        <P><B>Abstract</B></P> <P>Inflammation and thrombosis are linked to the use of polymer-based drug-eluting stents (DES). The aim of this study was to develop a polymer-free everolimus (EVL)-eluting stent using nitrogen-doped titanium dioxide (N-TiO<SUB>2</SUB>) and verify its efficacy by <I>in vitro</I> and <I>in vivo</I> assessment in a porcine coronary model. Various analytical approaches such as scanning electron microscopy and atomic force microscopy, electron spectroscopy, Fourier transform infrared spectrometry and contact angle measurement were employed for the characterization. As a part of biocompatibility assessment, platelet adhesion and smooth muscle cell (SMC) proliferation were examined. Bare metal stent (BMS), N-TiO<SUB>2</SUB> stent, everolimus-eluting N-TiO<SUB>2</SUB> (N-TiO<SUB>2</SUB>-EVL) stent, and commercialized EVL-eluting stent (EES) were randomly placed in forty coronary arteries in twenty pigs. After four weeks of implantation, the stents were subjected to histological and quantitative analysis. The N-TiO<SUB>2</SUB> film used in this study was well coated without any cracks or peeling. Surface hydrophilicity (88.8% of angle decrement) could be associated with the decrease in surface roughness post N-TiO<SUB>2</SUB> deposition (37.0%). The platelet adhesion on the N-TiO<SUB>2</SUB> surfaces was less than that on the BMS surface. The proliferation of SMC was suppressed in the N-TiO<SUB>2</SUB>-EVL group (30.2%) but not in the BMS group. In the animal study, the percent area restenosis was significantly decreased in the N-TiO<SUB>2</SUB>-EVL group compared to that in the BMS group. The results (BMS; 47.0 ± 11.00%, N-TiO<SUB>2</SUB>-EVL; 31.7 ± 10.50%, and EES; 29.1 ± 11.21%, <I>n</I> = 10, <I>p</I> < 0.05) were almost at par with those of the commercialized EVL-eluting stent. The introduction of N-TiO<SUB>2</SUB> deposition during fabrication of polymer-free DES may be an efficient accessorial process for preventing in-stent restenosis and thrombosis.</P> <P><B>Highlights</B></P> <P> <UL> <LI> N-TiO<SUB>2</SUB> surfaces can help to reduce the platelet adhesion. </LI> <LI> In porcine model, N-TiO<SUB>2</SUB> everolimus decreased in-stent restenosis and fibrin deposition. </LI> </UL> </P>

      • In-hospital mortality in patients with STEMI admitted during off hours

        Conflicting results exist on the outcome of off hours PCI in ST elevation myocardial infarction (STEMI). However, there were only a few studies that have focused on the clinical characteristics and outcomes of off hours PCI in STEMI. So, we studied the clinical characteristics and hospital mortality in STEMI patients treated with primary PCI during regular hours (weekdays 9:00 AM to 6:00 PM) versus off hours Weekdays 6:01 PM to 8:59 AM, weekends, and holidays) in Korea Acute Myocardial Infarction Registry. We analyzed in hospital and one year mortality among 5,665 consecutive ST segment elevation myocardial infarction patients treated with primary PCI between November 2005 to January 2008. Total 2,848 (50.2%) patients were treated during off hours. Baseline finding were similar, although regular hours patients were older. Median symptom to balloon time (304 min, IQR 175 to 750 vs. 270 min, IQR 145 to 551, p=0.001) were longer for regular hours primary PCI. Median door to balloon time (71min, IQR: 48 to 132 vs. 59 min, IQR 39 to 110 min, p=0.001) were longer for off hours pPCI. Also, Cardiac enzyme such as Max CK-MB (212.1±299.3 vs 194.7±303.4, p=0.031) and max TnI (72.6±239.5 vs. 58.9±94.4, p=0.013) were increased in off hours pPCI. However, unadjusted in hospital (6.0% off hours vs. 6.0% regular hours, p=0.946) and one year cardiac mortality (11.3% off hours vs. 11.7% regular hours, p=0.661) were comparable. In multivariate analysis, off hours primary PCI did not predict an adverse outcome. In conclusion, when primary PCI was performed within an appropriate reperfusion strategy, the clinical effectiveness of either off hours or regular hours pPCI is comparable.

        정신지체 특수학교 전공과 및 전환교육의 운영실태와 개선방향

        본 연구의 목적은 정신지체 특수학교의 전공과 및 전환교육의 운영실태와 개선방향을 탐색하는데 주안점을 두었다. 전공과 및 전환교육의 운영실태는 16개 영역으로 조사하고 문제점과 개선방향은 10개 영역을 중심으로 세부적인 내용을 조사 분석하였다. 여기서 조사분석된 내용은 정신지체 직업 · 전환교육에 좋은 지침이 될 수 있을 것이며 타장애영역 학생들의 직업진로 및 전환교육에 실질적인 도움을 줄 수 있을 것이다. The aims of this study are finding out the reality of the operation of the majored subjects and problems and finding out the improved methods about them. And 1 surveyed special teachers who are attending 17 special schools in Korea in which have the majored subjects about the reality of operating them and improved methods about them. First, about surveying the reality of operation, as to the majored subjects, they are mainly teaching students in the form of work training, guiding job, preparing job and daily Iiving skills through classes. The selection of majored subjects conceming jobs are made through school's direction and the cooperation among teachers, students and parents, and teaching jobs are now changed or will be changed because of the maladjustment to student' s aptitude, the assessment on their job aptitude is not being made now. There are many qualified teachers who have the certification in special school and they think that their roles as directors, guiders are important. The majoring students don’t have enough practicing places and factories. Even though they are received training about job in school, they get jobs unrelated with their received training. And they have verγ low employment as a whole. Though the transitional education program whose aims are selection in jobs, employment and enjoying jobs, it is started especially in the middle and high school courses. And it is thought that official and individualized education program is very important, also needed the cooperation among related authorities and it’s major program is used as the programs on job training. programs on job training. Next, as to the problems and improved methods, the problems about the majored subjects are the unrelatedness with job training and unreality and only one line of selection in job training. The problem about the transitional education program is the shortage in social skills, in the basement of job training and in carrγing out ability. The professional ability which is required in job guiding teacher is job training guide, many methods, the ability about assessing job aptitude, and testing their ability. The construction of the curriculum is not applying the guided curriculum rather than transforming the curriculum according to the atmosphere, and their school year should not be stuck to any one standard but be varied according to their ability and improvement. In the operation of majored subjects, it is necessarγ for them to provide real career education, the opportunity to receive various job training and the supported employments in which they can be included in their society actively.

        북극 스발바드 군도 스피츠베르겐섬 콩스피요르드에서의 다환 방향족 탄화수소화합물의 분포 특성

        In order to elucidate the polycyclic aromatic hydrocarbon concentration and its origin in arctic area, four arctic brown algae (Laminaria saccharina, L. digita, Alaria esculenta, Desmarestia aculeata), one marine invertebrate (Echinoidea) and sediments were collected from Kongsfjorden in Spitsbergen from the late July to early August. 2003. In case of macroalgae, the young blade part above growth point and the old stipes and blades beneath growth point were separated and analyzed for polycyclic aromatic hydrocarbons (PAHs) in an attempt to check the mechanism of uptake in macroalgae to accumulate PAN. There was no difference in PAH concentrations between sampling sites (Stations B and C), species. and blades beneath and above growth point. PAH concentrations in all samples collected in this study were relatively higher than those reported in other areas of arctic. Especially, station C, which is known as an unpolluted area, showed 10 times higher PAH concentration (8,765 ng/g) in sediment than station A (694 ng/g) around harbor. In addition high PAH concentration, station C had very higher proportion of methylated PAH to parent PAH in sediment than station A. Source analysis using PAH isomer pair ratios as indicators showed that Kongsfjorden area seemed to be relatively contaminated with PAH derived from direct petroleum input.

      • 돈지, 들깨유 및 달맞이꽃 종자유의 혼합급이가 흰쥐의 간장, 뇌 및 고환의 지질 성분에 미치는 영향

        돈지 및 들깨유와 달맞이꽃 종자유등 2종 식물 종자유와의 혼합급이가 흰쥐의 체내 조직의 지질 성분에 미치는 영향을 구명하기 위해 S·D계 숫쥐에게 돈지 2.55및 들깨유와 달맞이꽃 종자유의 혼합비율을 달리하여 4주간 급이 사육한 후 간장,뇌 및 고환의 지질 성분을 분석 검토한 결과는 다음과 같다. 간장의 총콜레스테롤 함량은 D군이 대조군에 비해 유의적으로 낮았으며, 인지질 함량은 각 군간의 유의적인 차이가 없었고, 중성지질 함량은 B및 C군이 대조군에 비해 현저하게 낮았다. 콜레스테롤 급이군의 간장 총콜레스테롤 함량은 대조군에 비해 c,d및 e군이 유의적으로 낮았고, 인지질 및 중성지질의 함량은 b군이 낮게 나타났다. 뇌의 총콜레스테롤 함량은 대조군에 비해 B및 C군이 유의적으로 낮았으며, 인지질 및 중성지질의 함량은 대조군에 비해 전 군이 유의적으로 낮데 나타났다. 콜레스테롤 급이군에 있어 뇌의 총콜레스테롤 함량은 각 군간의 유의적인 차이가 없었고, 인지질 함량은 c군이 중성지질 함량은 e군이 대조군에 비해 낮게 나타났다. 고환의 총콜레스테롤 및 인지질 함량은 각 군간의 유의적인 차이가 없었으나, 중성지질 함량은 대조군에 비해 전 군이 유의적으로 낮았다. 콜레스테롤 급이군은 콜레스테롤 무급이군과 거의 유사한 경향을 나타내었다. In order to observe the effects of the feeds mixed with the lard and two vegetable seed oils, which mixed with 2.5% lard and various level of perilla oil and evening primrose oil were administered to the male rats of Sprague Dawley for 4 weeks on lipid components of liver, brain and testes. In livers , total cholesterol contents were remarkably lower in the D group(2.5% lard+2.5% perilla oil+5.0% evening primrose oil,free cholesterol) than in the control group , and phospholipid contents were not significant difference in each groups. Triglyceride contents were remarkably lower in the B(2.5% lard+7.5% perilla oil+5.0% ,free cholesterol) and C(2.5% lard+5.0% perilla oil+2.5% evening primrose oil,free cholesterol) group than in the control group. In the supplemented cholesterol groups, total cholesterol contents were significantly lower in c(C group+cholesterol diet), d(D group+cholesterol diet) and e(2.5% lard+7.5% evening primrose oil,free cholesterol) group, phhspholipid and triglyceride contents were lower in the b group(B group+cholesterol diet) than in the control groups than in the control group, phospholipid and triglyceride contents in brain were considerably lower in the other groups than in the control group. In the supplemented cholesterol groups, total cholesterol contents were not significant difference in each groups. Phospholipid and triglyceride contents were conspicuously lower in the c and e groups. respectively. In testes, total cholesterol and phospholipid contents were not significant difference in the groups, but triglyceride contents were lower in the all groups except for control group The contents of total cholesterlo, phospholipid and triglyceride were similar between the supplimented cholesterol groups and the nonsupplemented cholesterol groups.

      • 慶南地方의 實效濕度 分布에 關하여 : 釜山, 蔚山 및 忠武地方, 1951∼80 Busan, Ulsan, and Chungmu, 1951∼'80

        This study was carried out to estimate the distribution and change of the mean effective humidity during thirty years(Jan. 1951-Dec. 1980) in Busans, Ulsan, and Chungmu in Gyeongman, Korea. The basic meteorological data of relative humidity(R.H.) used in order to calculate the effective humidity H_eff were observed by the Busan Meterological Office(35°06'N, 129°02'E. H=69.2m), Ulsan Met. Station(35°33'N, 129°19'E, H=31.5m), and Chungmu Met. Station(34°50'N, 128°26'E, H=32.2m) The results of this study obtained were as follows: Annual mean effective humidity was respectively about H_eff=4.37%(in Busan), H_eff=46.5%(in Ulsan), and H_eff=47.6%(in Chungmu) The mean effective humidity in spring were about H_eff=42.9%(in Busan),45.6%(in Ulsan). and 46.1%(in Chungmu), in summer were about H_eff=53.8%(in Busan), 53.6%(in Ulsan). and 55.6%(in Chungmu), in autumn were about H_eff=43.5%(in Busan),48.3%(in Ulsan). and 48.0%(in Chungmu), in winterwere about H_eff=34.6%(in Busan),38.7%(in Ulsan). and 40.6%(in Chungmu) respectively. The distributions of the appeared frequency day under H_eff=50.0% in a year were about 274 days(in Busan), 243 days(in Ulsan), and 243 days(in Chungmu) respectively. The climatic characteristics in the view point of humidity were apparently shown dry-air ; as following the effective humidity H_eff=31.7∼39.1%(in Jan., Feb., Nov., and Dec., in Busan), H_eff= 35.5-40.1%(in Jan., Fed., and Dec., in Ulsan), and H_eff=38.6-39.2%(in Jan., Feb., and Dec., in Chungmu) all around the year.

        류마티스 관절염 환자의 T세포 수용체 Vβ 유전자 레퍼토리 분석

        목적: 류마티스 관절염(rheumatoid arthritis; RA)의 병인에 있어 중추적인 역할을 하는 것으로 알려진 T세포에 관한 연구가 최근에 관심의 초점이 되고 있으나, 현재까지 한국인을 대항으로 시행된 보고는 없다. 한국인 류마티스 관절염 환자의 관절병변부위에서 T세포 수용체(TCR)의 Vβ유전자 사용의 빈도를 검색하여 한국인 류마티스 관절염 환자에서 사용되는 빈도가 높은 유전자계를 찾는데 목적을 두었다. 방법: 이에 저자는 T세포에 관한 연구의 일환으로 류마티스 관절염 환자 3명과 정상인 4명을 대상으로 말초 혈액과 활액 T세포로부터 추출한 RNA를 이용하여 cDNA를 합성한뒤 cDNA를 주형을 Vβ family specific oligonucleotide를 시발체(primer)로하여 반정량적 역전사 연쇄중합반응(semiquantative RT-PCR)을 시행하여 T세포 수용체(T cell receptor; TCR) Vβ 레퍼토리를 분석하여 다음과 같은 결과를 얻었다. 결과: 1. 정상인 4명의 말초혈액에서 T세포 수용체 Vβ유전자 평균사용빈도는 Vβ7(8.68±3.20%), Vβ3(7.83±2.03%), Vβ(6.74±1.43%)의 순서로 빈도가 높았으며, HLA-DR4 양성군(3명)에서는 Vβ8(7.39±1.71%), Vβ2(7.31±2.30%), Vβ1(7.22±1.54%)의 순서로 빈도가 높았고, HLA-DR4 음성(1명)에서는 Vβ3(17.79%), Vβ4(11.41%), Vβ24(9.8%)의 순서로 사용빈도가 높았다. 2. 류마티스 관절염환자 3명의 말초 혈액에서는 Vβ16(6.90±1.81%), Vβ18(6.89±0.80%), Vβ14(6.58±0.65%)의 순서로 빈도가 높았다. HLA-DR4 양성 환자군에서는 Vβ16(7.52±2.06%), Vβ14(6.96±0.04%), Vβ22(6.78±0.18%)의 순서로 빈도가 높았다. 각각 환자의 결과를 보면 첫번째 환자(HLA-DR4 양성)는 Vβ16(8.98%), Vβ14(6.99%), Vβ22(6.91%)의 순서로 빈도가 높았고, 두번째 환자(HLA-DR13 양성)는 Vβ18(7.79%), Vβ20(5.98%), Vβ24(5.90%)의 순서로 빈도가 높았다. 이를 종합해 보면 류마티스 관절염 환자의 말초혈액에서 Vβ16, Vβ20, Vβ14, Vβ18유전자계가 제한되어 사용되었다. 3. 류마티스 관절염환자의 활액의 T세포에서 Vβ유전자 평균발현빈도는 Vβ20(7.39±1.77%), Vβ18(5.60±1.31%), Vβ12(5.56±1.77%)순으로 높았으며, HLA-DR4양성인 환자군에서는 Vβ12(6.56±0.58%), Vβ20(6.44±0.94%), Vβ4(5.30±0.49%)순으로 빈도가 높았으며, HLA-DR4음성 환자에서는 Vβ20(9.29%), Vβ19(7.57%), Vβ18(7.07%)의 순서로 빈도가 높았다. 각각 환자의 결과를 보면 첫번째 환자(HLA-DR4 양성)는 Vβ12(6.15%), Vβ20(5.77%), Vβ7(5.74%)의 순서로 빈도가 높았고, 두번째 환자(HLA-DR4 양성)는 Vβ20(7.10%), Vβ12(6.98%), Vβ24(5.83%)의 순서이었고, 세번째 환자(HLA-DR13 양성)는 Vβ20(9.29%), Vβ19(7.57%), Vβ18(7.06%)의 순서로 빈도가 높았다. 활액 T세포에서는 Vβ20, Vβ12유전자계가 제한되어 사용되었다. 4. 말초 혈액에 비해 활액 T세포에서 TCR Vβ유전자 사용빈도가 1.5배 이상인 경우는 Vβ5.2, Vβ9, Vβ23 이었으나, 이들 유전자계가 활액 T세포 전체에서 차지하는 사용빈도에 있어서는 각각 Vβ5.2는 1.07±1.26%, Vβ9는 2.17±1.42%, Vβ23는 3.84+1.97%로 낮은 수치를 나타내었다. 각각 환자에서 비교했을때에 첫번째 환자(HLA-DR4 양성)는 Vβ23(3.69%:1.12%), Vβ5.1(3.51%:1.40%), Vβ12(6.15%:3.29%), Vβ6(3.35%:2.10%), Vβ2(5.74%:3.73%)의 유전자계에서, 두번째 환자(HLA-DR4 양성)는 Vβ9(3.58%:0.74%), Vβ19(4.67%:1.52%), Vβ5.2(2.52%:0.87%), Vβ10(4.25%:1.75%), Vβ1(5.74%:2.35%)의 유전자계에세, 세번째 환자(HLA-DR13 양성)는 Vβ6(2.68%:0.15%) Vβ9(2.17%:0.73%), Vβ11(2.30%:1.10%), Vβ20(9.29%:5.98%)의 유전자계에서 활액에서 말초혈액보다 1.5배이상 증가되어 사용됨을 볼수 있었다. 즉 활액에서 말초혈액보다 의미 있게 편향되어 사용되는 Vβ 유전자계는 각각 환자마다 다른 결과를 나타내었다. 5. 류마티스 관절염환자의 말초 혈액과 정상인의 말초 혈액의 T세포의 Vβ 유전자 사용빈도를 비교했을때 류마티스 관절염 환자의 말초혈액 T세포에서 Vβ16(6.90±1.81%:3.23±1.21%), Vβ18(6.89±0.80%:2.72±0.70%) 유전자계가 2배 이상으로 빈도가 높았고, HLA-DR4양성군만 비교하였을때도 Vβ16(7.52±2.06%:3.69±1.46%), Vβ18(6.45±0.28%:2.61±0.86%) 유전자계가 2배 이상 사용이 많았다. 결론: 이상의 성적으로 미루어 보아 류마티스 관절염 각각 환자마다 다른 Vβ 유전자계의 제한적 사용과 편향됨을 볼 수 있었으나, 공통된 유전자계의 증식은 볼 수 없었다. 이러한 결과는 다른 연구보고와는 다른 결과를 보였고, 이것은 대상 환자의 질병의 유병기간이 다르고, 또 유전적인 배경, 생존 환경, 적용된 방법의 차이로 기인한다고 하는 기존의 보고와 부합된다고 사료되는데 질병의 진행단계에 따라 주로 면역반응을 일으키는 항원의 에피토프가 달라짐에 따라 여기에 대항하는 T세포들의 수용체도 이들 에피토프에 반응할 수 있는 수용체를 가진 코론들이 증식하게 된다는 epitope spreading theory에 부합되며, 이는 아주 초기에 질병을 시작하게 유도하는 항원의 특성을 규명하기 위해서는 환자선택이 연구결과에 결정적인 역할을 할 것으로 사료된다. 또 다른 가능성은 각각 환자마다 다른 Vβ유전자계의 증식된 클론들의 CDR3 연기 서열을 규명하여 비록 유전자 서열이 다를 지라도 항원과 결합하는 같은 성상을 갖는 아미노산 motif를 가질 가능성에 대해서도 연구가 필요하리라 사료된다. Objectives: Polymerase chain reaction (PCR) technology was eamine synovial fluid and peripheral T cells in patients with rheumatoid arthritis (RA) to determine the preferential usage of the T cell receptor(TCR) variable region (V) gene. Methods: Oligonucleotide primers specific for individual TCR Vβ gene families were used to amplify the TCR gene products in a semiquantitative assay of their relative utilization in unselected T cell populations. Results: The result of Vβ utilization was generally heterogenous, similar with previous reports. However, the mean expression of Vβ16 and Vβ18 in RA was more preferentially utilized compared to normal donors. The usage of Vβ in peripheral blood from 3 patients with RA demonstrated restrictions in Vβ16, Vβ20 and Vβ18 genes, respectively. Analyses of synovial fluid resulted in restriction in β12, Vβ20 and Vβ20, respectively. Although there was no significant pattern of skewed Vβ gene mean usage when comparing the synovial fluids with the peripheral blood T cells from RA patients, there were significant biased Vβ genes, Vβ12, Vβl and Vβ20, each 3 patients. As the HLA type is a determining factor in shaping TCR repertoire of peripheral T cells, we compared the Vβ utilization in HLA-DR4 expressing groups that have susceptibility and gene dosage effect in disease progression. It was a little different that comparing the pattern of Vβ usage in peripheral blood and synovial fluid from RA patients between HLA-DR4 positive and negative group. Conclusion: The results were consistent with the conclusion that the increased Vβ family T cells infiltrate synovium and are dependent on each patient and may be involved in inducing and maintaining the synovitis that characterizes RA The different outcome of each patient may be due to the difference in disease duration, genetic background and geographic region. A more important factor may be the stage of disease, because epitope induced immune reaction may change over time. Therefore, selecting patients early in the course of disease may be important and may facilitate the need for more in-depth TCR analysis in the future.
