        Cloning and characterization of polyA- RNA transcripts encoded by activated B1-Like retrotransposons in mouse erythroleukemia MEL cells exposed to methylation inhibitors

        We have previously identified a DNA silent region located downstream of the 3?-end of the βmajor globin gene (designated B1-559) that contains a B1 retrotransposon, consensus binding sites for erythroid specific transcription factors and shares the capacity to act as promoter in hematopoietic cells interacting with β-globin gene LCR sequences in vitro. In this study, we have cloned four new non-polyA RNA transcripts being detected upon blockade of murine erythroleukemia (MEL) cell differentiation to erythroid maturation by methylation inhibitors and demonstrated that two of them share high structural homology with sequences of B1 element found within the B1-559 region. Although it is not clear yet whether and how these RNAs interfere with induction of erythroid maturation, these data provide evidence for the first time showing that methylation inhibitors can activate silent repetitive DNA sequences in MEL cells and may have implications in cancer chemotherapy using demethylating drugs as antineoplastic agents.         Bilateral Type IIIa Endoleak and Disconnection of Both Limbs after Evar with an Endurant II Endograft

        A 75-year-old patient with severe comorbidities was treated with an Endurant®(Medtronic, USA) II endograft due to a ruptured abdominal aortic aneurysm (AAA). After four years of unremarkable follow-up, bilateral limb separation was detected. The patient underwent endovascular bridging without any complication. Althoughrarely detected in newer grafts, late bilateral type IIIa endoleaks can present andshould be promptly repaired. Complex or ruptured AAAs treated with off-label useof endografts should be under closer surveillance using imaging tools for potentialendoleaks or aneurysm sac growth.

        Deep Learning for Radio Propagation: Using Image-Driven Regression to estimate path loss in urban areas

        Radio propagation modeling and path loss prediction have been the subject of many machine learning-based estimation attempts. Our current work uses deep learning for the task in question, trying to exploit the potential of applying convolutional neural networks in order to perform predictions based on images. A comparison between data-driven and image-driven estimations has been carried out in order to assess the proposed method. The results show that an appropriately chosen image can, per se, be treated as an alternative to a vector of tabular data and produce reliable predictions. The effect of the image’s size has also been examined.

      • A Parallel Personalized Recommendation Algorithm using Bipartite Graphs

        BDM-NBI algorithm is proposed in this paper. It focuses on the analysis of a personalized recommendation algorithm that utilizes a weighted bipartite graph suitable for processing big data. To improve the performance of this recommendation algorithm through parallel processing techniques, a sparse matrix partitioning algorithm is then developed that uses the bipartite graph as input. Our algorithm adopts bipartite graph partitioning using a vertex separator method that partitions a high-dimensional sparse matrix into a pseudo-block based diagonal matrix. Then, the recommendation algorithm analyzes all weighted sub-matrices in parallel. We produce the global recommendation weighted matrix by merging all of the sub-matrices in parallel. Experiments with Hadoop show that our algorithm has good approximation for small matrices and excellent scalability.

        Role of Pelvic Ischemia in Human Lower Urinary Tract Symptoms and Sexual Function Among Patients With Common Iliac Artery Obstruction Undergoing Revascularization Surgery

        Purpose: In this case-control study, we explored the relationships among pelvic ischemia, lower urinary tract symptoms (LUTS), and sexual function in patients with common iliac artery steno-occlusive disease, along with the potential therapeutic role of revascularization. Methods: We recruited 33 men diagnosed with radiologically documented common iliac artery stenosis (>80%) who underwent endovascular revascularization, and 33 healthy controls. Five patients had obstruction of the abdominal aorta (Leriche syndrome). The International Prostate Symptom Score (IPSS), Overactive Bladder Questionnaire, and International Index of Erectile Function were used to evaluate LUTS and erectile function. Medical history, anthropometrics, urinalysis, and blood tests, including levels of serum prostate-specific antigen, urea, creatinine, triglycerides, cholesterol, low-density lipoprotein, high-density lipoprotein, and hemoglobin A1c, were recorded. Uroflow (maximum flow, average flow, voided volume, and voiding time) and ultrasound parameters (prostate volume and postvoid residual [PVR]) were also measured. Patients with moderate-to-severe LUTS (IPSS>7) underwent complete urodynamic investigation. Patients were examined at baseline and 6 months postoperatively. Results: Patients exhibited poorer total IPSS (P<0.001), storage (P=0.001) and voiding symptom (P<0.001) subscores, as well as worse OAB-bother (P=0.015), OAB-sleep (P<0.001), OAB-coping (P<0.001), and OAB-total (P<0.001) scores than control participants. Additionally, erectile function (P=0.002), sexual desire (P<0.001), and satisfaction from intercourse (P=0.016) deteriorated in the patient group. Six months postoperatively, significant improvements were observed in erectile function (P=0.008), orgasm (P=0.021), and desire (P=0.014). Similarly, PVR significantly improved (P=0.012), while fewer patients experienced increased bladder sensation (P=0.035) and detrusor overactivity (P=0.035) upon postoperative urodynamic study. No significant differences were found between patients with bilateral and unilateral obstruction or between either of those groups and Leriche syndrome patients. Conclusions: Patients with steno-occlusive disease of the common iliac artery experienced more severe LUTS and sexual dysfunction than healthy controls. Endovascular revascularization alleviated LUTS in patients with moderate-to-severe symptoms and improved bladder and erectile function.

        A Rare Case of Lipomatous Hypertrophy of the Interventricular Septum

        Asymmetrical left ventricular hypertrophy secondary to interventricular septum hypertrophy is usually considered a typical phenotype of hypertrophic cardiomyopathy. In rare cases other conditions such as tumors or lipomatous hypertrophy of the interventricular septum may have a similar presentation. We present a case of a male patient who presented for routine cardiology work up and was diagnosed of having ventricular septal hypertrophy secondary to localized lipomatous hypertrophy.

        Game Theoretical Models of the Cooperative Carrier Behavior

        This paper presents a multi-level modeling approach which captures the cooperative behavior of carriers in maritime freighttransportation networks. Ocean carriers, land carriers and port terminal operators are considered. Port terminal operators are regardedas a special type of carrier for modeling purposes. Ocean carriers are the leaders in a maritime shipping market. Port terminaloperators are the followers of ocean carriers as well as the leaders of land carriers. For cooperating ocean and land carriers,compensation principle is used to find the optimal service charge and routing pattern that maximize their total profit, while portterminal operators act competitively. The concept of Stackelberg game is applied to a multi-level game, assuming a single oceanshipping company (leader) through an alliance. Subsequently, the paper considers cooperation between ocean carriers and portterminal operators, while individual carriers within the same group are considered to act competitively. Cooperative games within thesame carrier group and between different groups of carriers are examined and compared with competitive games. A numericalexample is presented to demonstrate the validity of the developed model.

        Cerebro-/Cardiovascular Collateral Damage During the COVID-19 Pandemic: Fact or Fiction?

        Numerous observational studies have identified a decline in cerebro-/cardiovascular (CV) admissions during the initial phase of the COVID-19 pandemic. Recent studies and meta-analyses indicated that the overall decrease was smaller than that found in initial studies during the first months of 2020. Two years later we still do not have clear evidence about the potential causes and impacts of the reduction of CV hospitalizations during the COVID-19 pandemic. It has becoming increasingly evident that collateral damage (i.e., incidental damage to the public and patients) from the COVID-19 outbreak is the main underlying cause that at least somewhat reflects the effects of imposed measures such as social distancing and self-isolation. However, a smaller true decline in CV events in the community due to a lack of triggers associated with such acute syndromes cannot be excluded. There is currently indirect epidemiological evidence about the immediate impact that the collateral damage had on excess mortality, but possible late consequences including a rebound increase in CV events are yet to be observed. In the present narrative review, we present the reporting milestones in the literature of the rates of CV admissions and collateral damage during the last 2 years, and discuss all possible factors contributing to the decline in CV hospitalizations during the COVID-19 pandemic. Healthcare systems need to be prepared so that they can cope with the increased hospitalization rates for CV events in the near future.

        Swallowing and Aspiration Risk: A Critical Review of Non Instrumental Bedside Screening Tests

        Background and Purpose The presence of dysphagia and aspiration in stroke patients is associated with increased mortality and morbidity. Early recognition and management of these two conditions via reliable, minimally invasive bedside procedures before complications arise remains challenging in everyday clinical practice. This study reviews the available bedside screening tools for detecting swallowing status and aspiration risk in acute stroke by qualitatively observing reference population study design, clinical flexibility, reliability and applicability to acute-care settings. Methods The primary search was conducted using the PubMed, Embase, and Cochrane Library databases. The search was limited to papers on humans written in English and published from 1991 to 2016. Eligibility criteria included the consecutive enrollment of acute-stroke inpatients and the development of a protocol for screening aspiration risk during oral feeding in this population. Results Of the 652 sources identified, 75 articles were reviewed in full however, only 12 fulfilled the selection criteria. Notable deficiencies in most of the bedside screening protocols included poor methodological designs and inadequate predictive values for aspiration risk which render clinicians to be more conservative in making dietary recommendations. Conclusions The literature is dense with screening methods for assessing the presence of dysphagia but with low predictive value for aspiration risk after acute stroke. A standard, practical, and cost-effective screening tool that can be applied at the bedside and interpreted by a wide range of hospital personnel remains to be developed. This need is highlighted in settings where neither trained personnel in evaluating dysphagia nor clinical instrumentation procedures are available.
