Endovascular Management of Long-Segmental Petrocavernous Internal Carotid Artery (Carotid S) Occlusion

Background and Purpose Long-segmental thrombotic occlusion of the distal internal carotid artery (ICA) sparing the cervical segment proximally and the supraclinoid segment distally, which could be termed ’Carotid S occlusion’, has an unusual clinical presentation. However, endovascular management of this lesion is challenging. The purpose of our study is to report our endovascular treatment clinical experience of the disease. Methods From March 2008 to June 2013, we could identify 14 patients (average age: 62.1, median age: 62, range: 50-79) with ‘Carotid S occlusion’, who underwent endovascular recanalization procedures. Patient’s clinical presentations were collected and the imaging findings also analyzed. The technical success rate, 24-hour and follow-up imaging outcome, and the clinical outcome using the 90-day mRS (modified Rankin scale) score were evaluated. Results Patients presented with gradually progressing (n=8), fluctuating (n=3), transient ischemic attack (n=2) and stationary (n=1) symptoms. DWI showed internal and external border-zone lesions in six patients, only internal ICA border-zone lesions in three patients, and only external border-zone lesions in two patients. Underlying distal ICA stenosis was noted in 12 patients. The technical success rate was 92.8% (13/14). Luminal patency was noted in all patients (100%) after 24 hours and in nine of 10 (90%) on long-term follow-up (median: 6.5, average: 15.1, range: 1-39 months). A 90-day, good functional outcome (mRS≤2) was noted in 13 of 14 patients (92.8%). Conclusions ‘Carotid S occlusion’ usually presented with border-zone infarction and endovascular management of the lesions was feasible. A relatively successful clinical outcome could be achieved after successful revascularization.

The intluence of double pilot on combustion noise, soot, NOx and BSFC has been confirmed on a 2.0L HSDI diesel engine equipped with a common rail fuel injection system. By examining the exhausted gases of NEDC emission cycle on chassis roller test and by measuring of part load test on engine lest bed, extremely higher values of NOx and soot show on acceleration range and EUDC area of NEDC cycle. Optimal multiple injections strategy in viewpoint of the combustion noise and soot reduction was proposed, by inspecting post2 timing and quantity swing Multiple injections can be utilized to reduce soot and combustion noise without deteriorating fuel economy and NOx.

The Use of Protection Device in Landmark-wire Technique of Symptomatic Subclavian Artery Occlusion with Combined Approach via Trans-femoral vs. Trans-brachial Arteries: Technical note

<P><B>Purpose</B></P><P>Since we reported about a landmark technique to reopen an occluded subclavian artery, we have faced difficulty in using protection devices in the vertebral artery to protect against thromboembolism from the reversed steal phenomenon after angioplasty and stenting. Therefore, we are presenting an optimal solution in using a protection device while recanalizing the occluded subclavian artery.</P><P><B>Materials and Methods</B></P><P>Among 21 cases of stenting for subclavian artery steno-occlusion, we applied the landmark technique at the opposite end of an occluded segment in 4 patients and used a protection device in two patients. Because the embolic protection device was placed in the vertebral artery via the brachial artery, optimal angioplasty and stenting via the brachial route were limited. Therefore, angioplasty via the trans-brachial approach was needed to be followed by stenting through a trans-femoral approach. We estimated the safe and optimal steps for placement and retrieval of the protection devices in addition to stenting.</P><P><B>Results</B></P><P>The procedure was safely performed when a stent was introduced via the femoral artery and a protection device was used via the brachial artery. However, in cases when a guidewire wasn't passed via the transfemoral route, simultaneous use of two systems via the brachial route could cause friction of devices or trapping of protection devices in a stent. When a protection device was trapped in a deployed stent, we retrieved the protection device with a 4F angiocatheter by selectively rotating the catheter tip. To avoid such procedural difficulty, we recommend using a transbrachial angioplasty followed by trans-femoral stenting while placing the protection device in the vertebral artery via the trans-brachial route.</P><P><B>Conclusion</B></P><P>If a guidewire is not passed through using a trans-femoral approach while performing the landmark technique, changing the stenting route from brachial to the femoral artery can be useful after securing the lumen in the occluded subclavian artery after angioplasty via the brachial artery.</P>

글로벌 가치사슬과 생산 변동성

이 연구는 글로벌 가치사슬(GVC) 참여와 생산 변동성의 관계를 분석하였다. GVC 참여도가 높을수록 해외에서 발생하는 불안정성과 위험에 노출되는 정도가 크기 때문에 생산 변동성이 높아진다. 한편, GVC 참여를 통해 중간재의 공급과 조달을 국내에 한정하지 않고 전세계로 확대하여 다양화하고, GVC에 참여하는 생산자가 그렇지 않은 생산자에 비해 효율적이기 때문에 생산 흐름이 원활하게 작동함으로써 생산 변동성이 축소된다. GVC 참여가 생산 변동성에 미치는 최종적인 영향은 이러한 두 가지 상반된 요인의 상호작용으로 결정된다. 무역 개방과 생산의 변동성을 분석한 대부분의 연구는 국가 차원으로 분석에 한정되어 있는데, 각 국가의 경제는 서로 이질적인 산업으로 구성되어 있는 점을 고려하여 국가-산업 차원의 분석을 수행하였다. 세계투입산출표를 이용하여 2000-2014년 세계 43개국 22개 산업을 대상으로 분석한 결과, GVC 참여가 생산 변동성을 줄이는데 기여하는 것으로 나타났다. 이러한 결과는 분석 기간을 달리하여도, 그리고 분위회귀분석방법을 적용해도 변하지 않고 강건한 것으로 나타났다. This paper investigates the impact of participation in global value chains (GVC) on output volatility. GVC participation can increase volatility by encouraging specialization and raising exposure to foreign shocks. On the other hand, GVC participation allows countries to diversify the demand and supply chains for intermediate goods across countries. Furthermore, GVC participation reduces the failure intermediate suppliers to deliver their products on schedule, because producers participating GVC are more efficient than others. The ultimate effects of GVC participation on output volatility are determined by the interaction of these two forces. Using the World Input Output database covering 43 countries, 22 manufacturing industries and the period 2000- 2014, this paper finds that GVC participation reduces output volatility. This finding is robust to changes in the time period measuring output volatility and alternative estimator.

<P>We report the case of a 55-year-old woman who presented with a severe pain and a mass in the proximal portion of the medial calf. On sonography, the lesion appeared as a poorly defined hypoechoic lesion with interspersed muscle bundles. A total excision of the lesion was performed. On microscopic examination, the specimen demonstrated cystic lesions surrounded by fibrous walls within the muscle bundles. In a digital age, the demand for comprehensive travel reviews on online platforms has surged. However, writing detailed reviews can be intimidating for users, impacting the quality of feedback. This paper proposes the services to assist users in creating in-depth and valuable travel reviews with Generative pre-trained transformer (GPT)model and OpenAI API. The solutions make the review writing process easier, offering topic suggestions and posing relevant questions. Therefore it not only enhances the quantity and quality of reviews but also lightens the load on users. Performance tests confirm increased efficiency, offering promising transformations for travelers, travel agencies, and product sellers. This innovative approach not only streamlines the review process but also fosters well-informed decision-making, ultimately enriching the overall travel experience.

화약 사출을 이용한 KNSB 아마추어 로켓 피드백

전국대학교로켓연합회(NURA)에서는 매년 로켓 발사대회를 개최한다. 본 동아리에서는 아마추어 로켓의 동체, 추진, 사출, 소프트웨어를 연구해왔지만, 전체 시스템을 통합하지 못했다. 따라서 지속적인 대회 참가 및 도전 과제 수행을 위해, 본 동아리의 표준 발사체를 정립할 필요가 있었다. 본 연구에서는 KNSB를 이용한 약 300N급 아마추어 고체 로켓을 설계 및 제작하고, 안정적인 로켓 회수를 위해 피스톤과 화약을 이용한 낙하산 사출장치를 제작했다. TMS(Thrust Measurement System)를 이용하여 모터의 추력과 안정성을 확인했다. 아두이노 나노를 이용해서 사출장치 동작을 확인하고, 실험용 동체를 제작하여 지상에서 화약 사출 실험을 수행했다. 또한 통합된 모델을 발사 및 회수했으며, 아두이노 메가를 이용해서 비행 데이터를 확보했다. An annual amateur rocket launch contest is held by ‘National Universities Rocket Association’ called NURA. The GNU(Gyeongsang National University) Rocket team has been studying an amateur rocket motor, parachute ejection and software, respectively. However, failed to integrate all the systems as a single rocket. Therefore, to develop the standard model rocket had been highly required in order to consistently participate in the ‘Amateur Rocket Contest’ and also to impose a creative mission to the rocket. In this study, KNSB propellant had used for the rocket motor and the parachute ejection system which enables the rocket to be safely recover has designed to use a piston and gunpowder. The performance of the motor was verified by the TMS(Thrust Measurement System) experiment. Arduino Nano is used to operate the parachute deployment system and the ejection experiment had been performed with the test model. The integrated flight model(FM) lifted off and safely recovered. The flight data had successfully obtained by Arduino Mega.

글로벌 아웃소싱이 고용에 미치는 영향 : 한국 제조업을 중심으로

본 논문은 1988년부터 2014년까지 21개 산업을 대상으로 글로벌 아웃소싱이 고용에 미치는 영향을 분석하였다. 이를 위해 기존연구에서 고려하지 못했던 내생성 문제를 해결하기 위해 시스템GMM을 분석에 사용하였고 외환위기 이전과 이후를 구분하여 추정하였다. 또한 글로벌 아웃소싱의 대용변수로 총중간재에서 차지하는 해당산업의 수입중간재 비중을 사용하였다. 분석결과 전체기간을 대상으로 하였을 때 글로벌 아웃소싱이 고용에 미치는 영향을 발견하지 못하였다. 외환위기 이전과 이후를 구분하여 분석한 결과 또한 글로벌 아웃소싱과 고용간의 유의한 결과를 나타내지 못하여 글로벌 아웃소싱의 확대가 고용에 특별한 영향을 미치지 않는 것으로 나타났다. This study analyzed the impact of global outsourcing on employment for the period 1988 to 2014 using. To analyze the impact, we employed the System GMM and IV methods to address the endogeneity problem that had not been considered in previous studies. The results show that global outsourcing has never had an impact on employment for a full period. Additionally, we analyzed it divided into before and after the financial crisis in 1997. The result for the additional analysis also shows that the global outsourcing has no impact on employment.
