<P><B>Purpose</B></P><P>Transrectal ultrasound-guided prostate biopsy is the procedure of choice for diagnosing prostate cancer. We compared with pain-relieving effect of acetaminophen, a known drug for enhancing the pain-relieving effect of tramadol, and eutectic mixture of local anesthetics (EMLA), a local anesthetic agent, with that of the conventional periprostatic nerve block method.</P><P><B>Materials and Methods</B></P><P>This was a prospective, randomized, single-blinded study. A total of 430 patients were randomly assigned to three groups. Group 1 received a periprostatic nerve block with 1% lidocaine, group 2 received acetaminophen 650 mg, and group 3 received EMLA cream for pain control. All patients were given 50 mg of tramadol intravenously 30 minutes before the procedure. At 3 hours after completion of the procedure, the patients were asked to grade their pain on a horizontal visual analogue scale (VAS). The patients were also asked whether they were willing to undergo future biopsy if required.</P><P><B>Results</B></P><P>There were no significant differences between the three groups in terms of age, prostate-specific antigen, prostate size, or numbers of biopsy cores. The pain scores for groups 2 and group 3, which were 3.47±1.92 and 3.50±1.36, respectively, were similar and were significantly lower than that of group 1, which was 5.24±2.07.</P><P><B>Conclusions</B></P><P>Acetaminophen and EMLA cream with intravenous injection of tramadol are safe, easy, and effective methods of controlling pain during the procedure. These methods were more effective for pain relief than was the conventional periprostatic nerve block method.</P>
A case of terra firma-forme dermatosis treated with salicylic acid peeling

Terra firma-forme dermatosis(TFFD) is a bizarre, acquired and idiopathic dermatosis that etiology has still not been fully defined. It is characterized by the presence of asymptomatic, brownish dirt-like lesion maybe due to disorder of keratinization. These lesions cannot be removed with ordinary cleansing, therefore TFFD can be differentiated from dermatosis neglecta. Patient was a 17-year-old man with brownish lesion on his face for 1 month. The patient had a history of regular washing habit with soap and water. The lesions were asymptomatic however, the patient wanted to solve the problem due to cosmetic reason. Punch biopsy revealed that hyperkeratosis and fungal spore in stratum cornium. We had performed salicylic acid peeling with alcohol base. It disappears completely on gentle swabbing with peeling. In this point, the diagnosis of TFFD could be considered. This disease can be confused with dermatosis neglecta, therefore we report this case with literature review.

Objectives: The aim of this study is to quantify and characterize the patients visiting the ED of our hospital with skin complaints in recent 6 years. Methods: We performed a retrospective study of 464 patients who visited our ED in Myongji Hospital with treatment by a dermatologist from January 2010 to December 2015. Results: The mean age of patients was 41.2 years and there were more female patients (57%) than males (43%). The peak incidence occured in June (11.4%), and the lowest was in November (4%). In arrival time distribution, the peak incidence occurred in PM 18~21hour (23.7%) and the lowest incidence in AM 3~6hour (2.1%). In residence time distribution, the peak incidence occurred in less than 2 hours (73.2%), followed by in 2~4 hours 23.7%). Herpes zoster was most common complaint 31.6%), followed by urticaria/angioedema (12.9%), contact dermatitis (10.3%). 23.7%(n=110) of enrolled patients were admitted, mainly for herpes zoster (n=67, 60.9%). In admission rate, erysipelas/cellulitis was the highest (50%, 7/14), followed by herpes zoster (45.5%, 67/147). Conclusion: Though urticaria/angioedema and herpes zoster were most common in ED for skin complaints, appropriate diagnosis and management for other skin diseases is necessary.
A case of actinic granuloma on the face and anterior neck

Actinic granuloma (O’Brien) is an annular granulomatous lesion that develops mainly in sun exposed area. This rare skin disorder was first described in 1975 by O’Brien. This disease presents in middle aged individuals with asymptomatic annular plaque with mild central atrophy. Patient was a 76-year-old woman with yellowish, annular and crust like lesion on the face and anterior neck for 6 months. The lesions gradually increased with mild pruritus. Microscopic examination of skin scraping was negative for fungal elements. Punch biopsy showed granulmatous reaction and giant cells with degenerative elastic fibers. The histopathologic findings and photodistribution were consistent with actinic granuloma. We report a case of actinic granuloma that showed typical clinical and histopathological findings.

By saponification and extraction of corn oil foots abandoned as waste during oil refining, a mixture of phytosterols was obtained, and its major components were determined as .betha.-sitosterol, campesterol and stigmasterol by gas chromatographic analysis. The mixture is very cheap and regarded as an excellent substrate for direct fermentation of C-17 keto steroid intermediate for various steroid pharmaceuticals.
A case of SAPHO syndrome with acne scar

SAPHO (synovitis, acne, pustulosis, hyperostosis, osteitis) syndrome is a systemic inflammatory disorder characterized by involving the skin, bone and joints. This osteoarticular and dermatological inflammatory disease was first reported 1987. Inflammatory osteitis can occur in sternoclavicular and sacroiliac joints with pain and tenderness; these symptoms can be a clue for diagnosis. In dermatologic field, acne on face and pustulosis on palm or sole can be seen. Patient was a 53-year-old woman with neck, chest and back tenderness who was consulted from rheumatologic department for evaluation of pustulosis on the palm and sole. Patient had acne scar on the face. Radiologic evaluation revealed that sternoclavicular osteitis without any infectious sign. Skin biopsy had been obtained from the palm and revealed that intraepidermal cavity filled with polymorphonuclear leukocytes associated with spongiform changes within the surrounding epidermis. The diagnosis of SAPHO syndrome was considered because of these clinical findings. We report a case of SAPHO syndrome, because it is rare and can be helpful for dermatologic doctors to diagnosis this disease.
