      • 제 3형 급성 견봉-쇄골 관절 분리 환자의 수술적 치료 결과

        목적 : 본 연구를 통하여 Rockwood 제 3형 급성 견봉-쇄골 관절 분리 환자에서 Phemister 술식과 modified Phemister 술식의 치료 결과를 비교하고자 한다. 대상 및 방법 : 1992년 2월부터 2001년 8월까지 제 3형 급성 견봉-쇄골 관절 분리로 수술적 치료를 받고 1년 이상 추시 가능하였던 45명, 45예의 환자를 대상으로 하였다. 평균 연령은 28.1세였으며, 남자가 42명, 여자가 3명이었다. 진단은 이학적 검사와 단순 방사선 부하 촬영법을 이용하였으며, 수상일로부터 수술적 치료까지의 평균 기간은 7.8일이었다. 술장 소견상 오구-쇄골인대 봉합술이 어려운 15예에서는 Phemister 술식을 시행하였고 (Ⅰ군), 인대 봉합이 가능한 30예에서는 modified Phemister 술식을 시행하였다(Ⅱ군). 수술 후 추시 기간은 평균 16.2개월이었고, 결과 판정은 최종 추시 때의 임상 소견과 이학적 검사 및 방사선 촬영을 통해 UCLA shoulder scoring system과 acromio-clavicular separation scoring system을 이용하였다. 결과 : 술후 합병증은 Ⅱ군에만 표재성 감염이 2예, K-강선 이주가 1예에서 발생하였다. 최종 추시상 전예에서 동통, 관절 운동 범위의 제한은 없었고 , Ⅱ군에서만 방사선 촬영상 2예에서 아탈구가 관찰되었다. UCLA shoulder scoring system은 Ⅱ군에서 우수 93.3%, 양호 6.7%였고, acromio-clavicular shoulder scoring system은 Ⅱ군에서 우수 90%, 양호 10%였다. 결론 : 활동적인 연령에서 발생한 제 3형 급성 견봉-쇄골 관절 손상의 환자에서 Phemister 술식만으로도 좋은 결과를 보일 수 있을 것으로 사료된다. Purpose : The purpose of this study is to compare the Phemister technique with the modified Phemister technique for the patients with Rockwood type 3, acromio-clavicular separation. Materials and Methods : The 45 cases of 45 patients received surgical treatment for Rockwood type 3, acute acromio-clavicular separation in our hospital from Feb. 1992 to Aug. 2001 later with the follow-up study were selected as subjects. The average ages were 28.1 years old, male and female were 42, 3 persons, respectively. Physical examination and plain radiography were used for their diagnosis and the intervals between injury and surgical treatment were 7.8 days. In intraoperative finding, we performed Phemister technique in 15 cases according not to be able to repair coraco-clavicular ligament (groupⅠ), modified Phemister technique in 30 cases according to be able to repair that (groupⅡ). The average follow up period was 16.2 months, and the UCLA shoulder scoring system and the acromio-clavicular separation scoring system were used to obtain clinical results. Results : Only in Group Ⅱ, the complication after surgery were associated with superficial infection in two cases and K-wire migration in one case. At last follow up, there were no pain and limitation of range of motion in all cases, and two cases in Group Ⅱ were found to be subluxation in radiography. Clinical results revealed excellent was 93.3%, good was 6.7% in UCLA shoulder scoring system in both groups, and excellent was 90%, good was 10% for group Ⅱ in acromio-clavicular separation scoring system. Conclusion : The results are considered to be food with only Phemister technique in type 3, acute injury occurred in working ages.

        SAFE TECHNOLOGY WITH A NOVEL REAR COLLISION AVOIDANCE SYSTEM OF VEHICLES

        This study aims to reduce the severity of injuries caused by and the frequency of rear-end vehicle collisions. A better collision prediction method is proposed by means of millimeter-wave radar sensors able to rapidly detect potential collisions and alert drivers. Implementation of the system would bring about a reduction in accidents and potentially reduce the severity of the accidents that cannot be avoided. The proposed collision avoidance system adopted millimeter-wave radar sensors to collect information on vehicle’s rear surroundings. The designed experimental device has several unique features. The device has a wide monitoring range, incorporates a continuous detection process, and contains a rapid warning system. A series of real-world experiments were performed on cars (Model: Tercel 1.5, Toyota) to demonstrate the monitoring efficiency and warning frequency of this device under several operating conditions. This study furthered the development of a safe technology for avoidance of rear-end collisions, and this offers a novel system which can be used in a variety of vehicles to reduce the frequency of rear-end vehicle collisions.

        The Homologous Region 3 from Bombyx mori Nucleopolyhedrovirus Enhancing the Transcriptional Activity of Drosophila hsp70 Promoter

        Drosophila melanogaster heat shock protein 70 gene promoter (Dhsp70p) is widely used in transgenic insect to drive exogenous gene, and the homologous region 3 from Bombyx mori nucleopolyhedrovirus (BmNPVhr3) functions as an enhancer for several promoters. To test whether BmNPVhr3 can enhance the Dhsp70ps transcriptional activity, the reporter plasmids, which contain the Dhsp70p, the reporter $\beta$-galactosidase gene with SV40 terminator and BmNPVhr3 fragment, are constructed and transfected into the insect cell lines (Bm-N cells and Sf-21 cells) by lipofectin-mediated method. The results from the transient expression assay show that BmNPVhr3 significantly increases transcriptional activity of Dhsp70p both under the normal condition and under the heat-shock treatment, although the effects are significantly different between in Bm-N cells and in sf-21 cells. The enhancing behavior of BmNPVhr3 on the Dhsp70p is in an orientation-independent manner. Meanwhile, the effects of heat-shock treatment on Dhsp70p alone or Dhsp70p/BmNPVhr3 combination present no significant difference, indicating that BmNPVhr3 only enhances the transcriptional activity of Dhsp70p, but cant alter its characteristic of the response to the heat-shock stress. The above results suggest that the Dhsp70p/BmNPVhr3 combination is more effective one to drive exogenous gene for transgene or stable cell expression system in insects.

        Apoptosis induced in vivo by new type gosling viral enteritis virus

        In this study, apoptosis was induced by new type gosling viral enteritis virus (NGVEV) in experimentally infected goslings is reported in detail for the first time. After 3-day-old goslings were orally inoculated with a NGVEV-CN strain suspension, the time course of NGVEV effects on apoptotic morphological changes of the internal tissues was evaluated. These changes were observed by histological analysis with light microscopy and ultrastructural analysis with transmission electron microscopy. DNA fragmentation was assessed with a terminal deoxynucleotidyl transferase-mediated dUTP nick end-labeling (TUNEL) assay and DNA ladder analysis. A series of characteristic apoptotic morphological changes including chromatin condensation and margination, cytoplasmic shrinkage, plasma membrane blebbing, and formation of apoptotic bodies were noted. Apoptosis was readily observed in the lymphoid and gastrointestinal organs, and sporadically occurred in other organs after 3 days post-infection (PI). The presence and quantity of TUNEL-positive cells increased with infection time until 9 days PI. DNA extracted from the NGVEV-infected gosling cells displayed characteristic 180~200 bp ladders. Apoptotic cells were ubiquitously distributed, especially among lymphocytes, macrophages, monocytes, and epithelial and intestinal cells. Necrosis was subsequently detected during the late NGVEV-infection phase, which was characterized by cell swelling, plasma membrane collapse, and rapidly lysis. Our results suggested that apoptosis may play an important role in the pathogenesis of NGVE disease.

        Computed tomographic analysis of maxillary sinus anatomy relevant to sinus lift procedures in edentulous ridges in Taiwanese patients

        Purpose: To analyze the maxillary sinus anatomy over edentulous ridges in the bilateral posterior maxillary area in Taiwanese patients using cone-beam computed tomography (CBCT). Methods: In total, 101 anatomical sites from 61 patients, including 32 premolar and 69 molar regions, were analyzed using CBCT. Measurements were made of the width and height of edentulous ridges, the thickness of the lateral wall of the maxillary sinus, and the presence of a sinus septum and the posterior superior alveolar artery (PSAA). A statistical analysis of the measurements was performed, and correlations among the measurements were assessed. Results: The average ridge width was 10.26±3.16 mm, with a significantly greater ridge width in the second molar region than in the premolar region. The mean residual ridge height was 8.55±4.09 mm, and ridge height showed an opposite trend from ridge width for the premolar and molar regions. A sinus septum was present at 5.9% of the sites, and the PSAA was observed in 24.5%. The average thickness of the lateral wall of the maxillary sinus was 2.08±0.94 mm, with no significant difference between the tooth position and lateral wall thickness. Conclusions: This study presents the anatomical features of the maxillary sinus, which should be considered in sinus lift procedures for implant placement, in the Taiwanese population. The use of CBCT is recommended to avoid intraoperative complications.

        Non-monotonic Magnetoresistivity in Two-dimensional Electron Systems

        The connection being studied is the one between the non-monotonic magnetoresistivity (MR) andthe electron-electron interaction (EEI) correction in weakly-disordered two-dimensional electronsystems (2DESs) in the ballistic region kBT/~ > 1, where kB, T, , and ~ are the Boltzmannconstant, the temperature, the scattering time, and the reduced Planck constant, respectively. Atzero magnetic field, a transition of the resistivity (T) from the insulating region d/dT < 0 tothe metallic region d/dT > 0 is observed. The MR shows a maximum, and with increasing T,the position of the MR maximum in B increases for both GaAs-based (sample A) and GaN-based(sample B) 2DESs. Our data suggest that the EEI plays an important role in such a non-monotonicMR effect and in the temperature dependence of the resistivity.

      • Depletion of LINE1-ORF1P Causes GV Arrest in Mouse Oocytes In Vitro

        LINE-1 is an autonomous non-LTR retrotransposon in mammalian genomes and encodes ORF1P and ORF2P. ORF2P has been clearly identified as the enzyme supplier needed in LINE-1 retrotransposition. However, the role of ORF1P is not well explored and requires further elucidation. In this study, we depleted ORF1P to investigate its role in mouse oocyte meiotic maturation. The results showed that depletion of ORF1P caused oocyte arrest at the germinal vesicle (GV) stage as well as down-regulation of Cdc2 and Cyclin B1, components of the maturation promoting factor (MPF). Further analysis demonstrated a decreased expression of the P21 upstream factors Smad4 and Dcp1a after ORF1P depletion. However, SMAD4 and DCP1A became accumulated in the nucleus. This translocation would up-regulate the expression of P21. Furthermore, ORF1P knockdown also increased the expression of microRNA-494, which could decrease the expression of Cyclin B1 and Cdc2. Propidium Iodide (PI) staining after ORF1P depletion revealed abnormal chromatin configuration in the GV of mouse oocytes. Taken together, these results indicate that ORF1P is involved in the TGF-β pathway to modulate the GV breakdown (GVBD) during mouse oocyte meiotic maturation.

        FSCB phosphorylation in mouse spermatozoa capacitation

        It is generally accepted that spermatozoa capacitation is associated with protein kinase A-mediated tyrosine phosphorylation. In our previous study, we identified the fibrous sheath CABYR binding protein (FSCB), which was phosphorylated by PKA. However, the phosphorylation status of FSCB protein during spermatozoa capacitation should be further investigated. To this aim, in this study, we found that phosphorylation of this 270-kDa protein occurred as early as 1 min after mouse spermatozoa capacitation, which increased over time and remained stable after 60 min. Immunoprecipitation assays demonstrated that the tyrosine and Ser/Thr phosphorylation of FSCB occurred during spermatozoa capacitation. The extent of phosphorylation and was closely associated with the PKA activity and spermatozoa motility characteristics. FSCB phosphorylation could be induced by PKA agonist DB-cAMP, but was blocked by PKA antagonist H-89.Therefore, FSCB contributes to spermatozoa capacitation in a tyrosine-phosphorylated format, which may help in further elucidating the molecular mechanism of spermatozoa capacitation.
