        Effects of excessive tea consumption on pregnancy weight gain and neonatal birth weight

        Objective Tea lovers are increasing worldwide. We hope that this report is the first to discuss the possible impacts of high black tea consumption on gestational weight gain (GWG) and birth parameters. Methods Throughout one year, a total of 7,063 pregnant ladies coming for first antenatal visit were screened in a major tertiary center. Of them, 1,138 were involved and divided according to their preference into 3 groups: excessive tea (ET), usual tea (UT), and mixed beverages group. The study included women who gave birth to healthy neonates. Results The rate of ET consumption was 4.13% with a total of 41 cases. The UT group (controls) comprised 94 women. ET was significantly associated (P<0.05) with maternal age, parity, occupation, smoking, and poor GWG starting from 30 weeks’ gestation until delivery, low birth weight, and small for gestational age (SGA). Poor GWG had a higher relative risk (with 95% confidence interval) in the ET group than in the UT group in crude (1.84 [0.85-2.43]) and risk adjusted models (1.25 [0.28-2.26]). Further, similar results were obtained for SGA in the crude and 3 adjusted models, where the first model was adjusted for bio-obstetrical variables, the second for social parameters, and the third for all factors included in the previous models (1.53 [0.62-2.81], 1.52 [0.71-2.50], and 1.46 [0.78-2.39]), respectively. Conclusion Consumption of large amounts of daily black tea during pregnancy (≥1,500 mL) is a significant cause of poor GWG and SGA.

        ON THE STRUCTURE OF A k-ANNIHILATING IDEAL HYPERGRAPH OF COMMUTATIVE RINGS

        In this paper we obtain a new structure of a k-annihilating ideal hypergraph of a reduced ring R, by determine the order and size of a hypergraph 𝒜𝒢<sub>k</sub>(R). Also we describe and count the degree of every nontrivial ideal of a ring R containing in vertex set 𝒜(R, k) of a hypergraph 𝒜𝒢<sub>k</sub>(R). Furthermore, we prove the diameter of 𝒜𝒢<sub>k</sub>(R) must be less than or equal to 2. Finally, we determine the minimal dominating set of a k-annihilating ideal hypergraph of a ring R.

        Safety and Efcacy of Cerebrolysin in Infants with Communication Defects due to Severe Perinatal Brain Insult: A Randomized Controlled Clinical Trial

        Background and Purpose Te neuroregenerative drug Cerebrolysin has demonstrated efcacy in improving cognition in adults with stroke and Alzheimer’s disease. Te aim of this study was to determine the efcacy and safety of Cerebrolysin in the treatment of communication defects in infants with severe perinatal brain insult. Methods A randomized placebo-controlled clinical trial was conducted in which 158 infants (age 6–21 months) with communication defects due to severe perinatal brain insult were enrolled; 120 infants completed the study. The Cerebrolysin group (n=60) received twice-weekly Cerebrolysin injections of 0.1 mL/kg body weight for 5 weeks (total of ten injections). Te placebo group (n=60) received the same amount and number of normal saline injections. Results Te baseline Communication and Symbolic-Behavior-Scale-Developmental Profle scores were comparable between the two groups. Afer 3 months, the placebo group exhibited improvements in the social (p<0.01) and speech composite (p=0.02) scores, with 10% and 1.5% increases from baseline, respectively. Te scores of the Cerebrolysin group changed from concern to no concern, with increases of 65.44%, 45.54%, 358.06%, and 96.00% from baseline in the social (p<0.001), speech (p<0.001), symbolic (p<0.001), and total (p<0.001) scores. Conclusions Cerebrolysin dramatically improved infants’ communication especially symbolic behavior which positively afected social interaction. Tese fndings suggest that cerebrolysin may be an efective and feasible way equivalent to stem cell therapy.

        Relationship between liver iron concentration determined by R2-MRI, serum ferritin, and liver enzymes in patients with thalassemia intermedia

        BackgroundIron overload is a risk factor affecting all patients with thalassemia intermedia (TI). We aimed to determine whether there is a relationship of serum ferritin (SF) and alanine ami-notransferase (ALT) with liver iron concentration (LIC) determined by R2 magnetic reso-nance imaging (R2-MRI), to estimate the most relevant degree of iron overload and best time to chelate in patients with TI.MethodsIn this cross-sectional study, 119 patients with TI (mean age years) were randomly se-lected and compared with 120 patients who had a diagnosis of thalassemia major (TM). Correlations of LIC, as determined by R2-MRI, with SF and ALT levels, were assessed in all participants. A P-value <0.05 was considered statistically significant. ResultsSF and LIC levels were lower in patients with TI than in those with TM; only ferritin values were significant. We found a statistically significant relationship between SF and LIC, with cut-off estimates of SF in patients with TI who had splenectomy and those who entered puberty spontaneously (916 and 940 ng/mL, respectively) with LIC >5 mg Fe/g dry weight (P<0.0001). A significant relationship was also found for patients with TI who had ele-vated ALT level (63.5 U/L), of 3.15 times the upper normal laboratory limit, using a cut-off for LIC ≥5 mg Fe/g dry weight.ConclusionWe determined the cut-off values for ALT and SF indicating the best time to start iron chela-tion therapy in patients with TI, and found significant correlations among iron overload, SF, and ALT.

        An outbreak of Motile Aeromonas Septicemia in cultured Nile tilapia, Oreochromis niloticus with reference to hematological, biochemical and histopathological alterations

        The current investigation dealing with the causative agent of mass mortalities in cultured Oreochromis niloticus. The diseased fish showed external hemorrhage, unilateral and bilateral eye opacity, ended by blindness and fish death. The postmortem lesions revealed congested friable kidney and spleen, and liver has yellow nodules. Obtained isolates were identified as Aeromonas hydrophila (the causative agent of Motile Aeromonas Septicemia) and found to be highly pathogenic as they contained hemolysin virulence gene causing mortality reached to 100 and 70% in intraperitoneal and intramuscular infection. The prevalence of MAS was 80% among the surveyed O. niloticus. Blood and serum were collected from naturally diseased, intraperitoneal and intramuscular injected O. niloticus for hematological and biochemical examination. Similarly, gills, musculature, kidney, liver and spleen were collected for histopathological evaluation, and micropathomorphological analysis of spleen was done. Macrocytic hypochromic anemia was recorded in the intraperitoneal infection. Serum protein, albumin and globulin were decrease only in naturally diseased fish. Leucocytosis with heterophilia and lymphocytosis were observed in naturally diseased and intraperitoneal infected fish. There were severe degenerative changes and hemorrhagic necrosis in the examined tissues which were more obvious in intraperitoneal than intramuscular infection. Activation and proliferation of melanocytes macrophages centers with severe hemosiderosis were recorded in spleen of naturally diseased and experimentally infected fish.

        Hypodectes propus (Acarina: Hypoderatidae) in a rufous turtle dove,Streptopelia orientalis (Aves: Columbiformes), in Japan

        An adult male rufous turtle dove, Streptopelia (S.) orientalis (Aves: Columbiformes), was found dead in Yorii-machi Town, Osato District 369-1217, Saitama Prefecture, Japan,and subjected to necropsy. A large number of immobile hypopi (deutonymphs) of the hypoderatid mite, Hypodectes (H.) propus (Acarina: Hypoderatidae), were found individually encapsulated subcutaneously primarily in the adipose tissue. The mites were 1.43 mm in length and 0.44 mm in width on average, and had provoked mild inflammatory reactions that predominantly manifested as foamy macrophages and lymphoplasmocytes. PCR analysis using ribosomal DNA extracted from paraffin-blocked tissues produced a 240 bp band specific for hypoderatids. Based on the morphological features (distinct coxal apodemes,especially in the anterior portion) and PCR-based findings,the hypopi were identified as H. propus. To the best of our knowledge, this is the first case describing the subcutaneous mite H. propus in a rufous turtle dove, S. orientalis, in Japan. This study also highlights the use of paraffin blocks as a source of tissue DNA for molecular evaluation.

        Clinical and molecular characteristics of infantile-onset diabetes mellitus in Egypt

        Purpose: In patients diagnosed with diabetes mellitus (DM) before the age of 12 months, there is an increasing recognition of diabetes caused by single-gene mutations, also known as monogenic diabetes of infancy or neonatal DM (NDM). This study aimed to classify patients at Alexandria University Children’s Hospital (AUCH) diagnosed with infantile-onset DM into type 1 DM (T1DM) or NDM and to detect differences in molecular characteristics of NDM patients at our center in comparison to other countries. Methods: This retrospective/prospective observational study was conducted on 39 patients diagnosed with infantile-onset DM (age of onset ≤1 year) at AUCH from January 2003 to November 2020. The patients were divided into 2 groups according to age at the onset of DM: ≤6 months and >6–12 months. Molecular testing was done in patients diagnosed with DM at ≤6 months and those with negative autoantibodies. Results: Twelve patients were diagnosed with DM at age ≤6 months and 27 patients were diagnosed between 6–12 months. Seventeen patients (43.6%) had T1DM, whereas 9 patients (23.1%) had genetically confirmed NDM, including 3 harboring novel mutations. The most common genetic causes of NDM were EIF2AK3 mutations (n=3), followed by KCNJ11 (n=2) and ABCC8 (n=2). Other mutations included SLC19A2 (n=1) and INS (n=1). Three patients with potassium ATP channel mutations were transferred from insulin to sulfonylurea treatment. Conclusion: It is essential to identify patients with NDM clinically and confirm the diagnosis by molecular testing to distinguish them from T1DM as it helps in refining their management, predicting prognosis, and guiding genetic counseling.

        Toolkit to Compute Time-Based Elixhauser Comorbidity Indices and Extension to Common Data Models

        Objectives: The time-dependent study of comorbidities provides insight into disease progression and trajectory. We hypothesizethat understanding longitudinal disease characteristics can lead to more timely intervention and improve clinicaloutcomes. As a first step, we developed an efficient and easy-to-install toolkit, the Time-based Elixhauser Comorbidity Index(TECI), which pre-calculates time-based Elixhauser comorbidities and can be extended to common data models (CDMs). Methods: A Structured Query Language (SQL)-based toolkit, TECI, was built to pre-calculate time-specific Elixhauser comorbidityindices using data from a clinical data repository (CDR). Then it was extended to the Informatics for IntegratingBiology and the Bedside (I2B2) and Observational Medical Outcomes Partnership (OMOP) CDMs. Results: At the Universityof Arkansas for Medical Sciences (UAMS), the TECI toolkit was successfully installed to compute the indices from CDRdata, and the scores were integrated into the I2B2 and OMOP CDMs. Comorbidity scores calculated by TECI were validatedagainst: scores available in the 2015 quarter 1–3 Nationwide Readmissions Database (NRD) and scores calculated usingthe comorbidities using a previously validated algorithm on the 2015 quarter 4 NRD. Furthermore, TECI identified 18,846UAMS patients that had changes in comorbidity scores over time (year 2013 to 2019). Comorbidities for a random sample ofpatients were independently reviewed, and in all cases, the results were found to be 100% accurate. Conclusions: TECI facilitatesthe study of comorbidities within a time-dependent context, allowing better understanding of disease associations andtrajectories, which has the potential to improve clinical outcomes.

        Testing and evaluation of the corrosion behavior of Aluminum/Alumina bulk composites fabricated via combined stir casting and APB process

        In this study, AA1060/Alumina composites were fabricated by combined stir casting and accumulative press bonding (APB). The APB process was repeated up to six press bonding steps at 300Ċ. As the novelty, potential dynamic polarization in 3.5Wt% NaCl solution was used to study the corrosion properties of these composites. The corrosion behavior of these samples was compared and studied with that of the annealed aluminum alloy 1060 and versus the number of APB steps. So, as a result of enhancing influence on the number of APB process, this experimental investigation showed a significant enhancement in the main electrochemical parameters and the inert character of the Alumina particles. Together with Reducing the active zones of the material surfaces could delay the corrosion process. Also, at higher number of steps, the corrosion resistance of composites improved. The sample produced after six number of steps had a low corrosion density in comparison with high corrosion density of annealed specimens. Also, the scanning electron microscopy (SEM), was used to study the corrosion surface of samples.
