      • 거대적아구성빈혈의 임상적 고찰

        Megaloblastic anemias are disorders caused by impaired DNA synthesis and chara-cterized by the presence of megaloblastic cells. Clinical data on these disorders in Korea are limited. The aim of this study was to analyze clinical characteristics of megaloblastic anemias caused by vitamin B12 deficiency. We identified 40 cases who were diagnosed as megaloblastic anemia from January 1993 to December 2002 in a retrospective study on the basis of chart review. We investigated the annual distribution of diagnosed patients, clinical manifestations and response to treatment. 19 cases were complicated with a previous total gastrectomy(gastrectomy group) and 21 cases had no history of surgical operation(non-gastrectomy group). The most common presenting complaint was generalized weakness. The median age at diagnosis was 7(range: 38-79) years in non-gastrectomy group. The mean pos-toperative duration was 7(range: 4-25)years in group of total gastrectomy and mean level of serum vitamin B12 was lower than 100 pg/mL in both group. Peripheral blood revealed thrombocy-topenia, macrocytic anemia and hypersegmented neutrophil in all cases. After treatment of pareliteral vitamin B12, Their clinical symptoms and hematologic findings improved except two patients who had bone marrow with lower cellularity. Cobalamin replacement after total gastrectomy is still missing. Diagnoses are made too late in a population of patients, resulting in irreversible neurological sequale, and we suggest that Pernicious anemia seems not to be very rare in Korea.

      • Anabaena PCC 7120의 窒素固定活性에 미치는 環境要因의 影響

        The growth and yield of heterocyst of nitrogen fixing cyanobacterium Anabaena PCC 7120 by the minimal medium of Allen and Arnon(l/8) were twice higher than those by the medium of BG 11 under optimum environmental conditions. The induction of the heterocyst after 24 hours of cultivation of vegetative cells with the starvation of nitrogen sources resulted in the initiation of the nitrogenase activity. The optimum nitrogenase activity was obtained in the nitrogen free minimal cultures with anaerobic, pH8, 35℃, 7000lux and 120rpm conditions. A significant inhibition was appeared by the aeration, low pH, low temperatures and dark conditions. The addition of NH^+_4, NO^-_3 and NaCl inhibited the nitrogenase activity of Anabaena PCC 7120 and the concentrations for 50% inhibition of the activities were 0.04, 0.20 and 50mM, respectively. The treatments of several toxic heavy metals inhibited significantly the nitrogenase activity by the range of 0.1∼0.6ppm levels and the toxicity was in the order of Co^++>Cd^++>Hg^++>Zn^++>Pb^++.

      • 濟州道의 導入牛에 있어서 고사리 中毒의 發生 狀況 및 그 對策에 對한 調査 硏究

        The authors investigated the status of an outbreak of bracken poisoning, and counter-measures, among recently introduced cattle (from Nov., 1976 to Oct., 1979) belonging to 19 ranches on Jeju Island. 1. 364 head of a total of 2396 head (Aberdeen Angus 948, Brahman 188, santa 231, Shorthorn 334, Simmental 26, Hereford 101, Holstein 568) showed the symptoms of bracken poisoning, and 159 head died between May, 1979 and Oct., 1980. 2. It was recognized that Holstein, Shorthorn, Angus were sensitive, and Brahman, Santa, Here ford were resistant to the bracken toxicity. 3. As counter-measures against bracken poisoning, 1) pasture improvement and it's reasonable management, 2) optimal time (October-November) for introducing the cattle, 3) proper calf management, and 4) the early diagnosis of bracken poisoning were suggested.

        정신치료 집단 Supervision 경험

        The author reviewed the 32 tape recordings of successive series of psychotherapy group supervision sessions, and studied the reaction of supervisees through a questionnaire. The group supervision sessions were conducted by a psychotherapy supervisor of Kyung-pook National University Hospital from February 1988 to July 1989. Twenty-one psychiatric residents and one co-supervisor participated. Each session ran 90 minutes once every other week. A supervisee's psychotherapy case was reported and it was followed by group discussion, supervisor's interventions and summary remarks. The tape recordings were analyzed in terms of most frequent mistakes and shortcomings of psychotherapy practice. The author distributed a questionnaire to find the reactions of the participants to the supervision sessions. The results could be summarized as follows ; The frequent mistake and the lack of therapeutic ability indicated by the supervisor were therapeutic interchange without clear understanding of the patient's nuclear emotions or central dynamics, the therapist's inability for empathic understanding and the therapist's lack of genuine interest in the patient. In terms of the technical shortcomings of the resident therapists, insufficient clarifications of the important issues, the tendency to neglect the formal history taking, frequent changes of the topic by the therapists because of their own needs and anxiety, premature interpretations and closed question methods. In the answer of the questionnaire composed of 18 items, all 21 participants responded that the supervision experience was helpful in understanding theoretical as well as practical aspects of psychotherapy, but some reported that the experience was perceived as provoking undue anxiety and even hostility. The author concludes that the group psychotherapy supervision model is suitable for the resident training in psychotherapy and the competence of the supervisor is the prerequisite condition for the group supervision.

        School illumination with full volume of light and adequate brightness is designed to help students better focus on their education and produce an optimum atmosphere where students can apply their utmost to learning. This paper presents study on actual illumination environment of schools in order to figure out illumination problems. Furthermore. it attempts to suggest an optimum design for schoolroom illumination through computer simulation. We try to attain qualitative improvement effect in school illumination under the illuminaion plan which is based on illumination maintenance and uniformity ratio that K.S. illumination standard sets.

      • 위상이동기법을 이용한 반점사진 계측법 Ⅰ : 이론 Theory

        물체표면의 아주 작은 변형이나 움직임을 재는데 많이 응용되는 반점사진 계측법에서 반점사진을 빠르고도 정밀하게 읽어낼 수 있는 새로운 방법을 제시하였다. 이 방법은 공간주파수 여과법과 위상이동기법을 결합한 것이다. 공간주파수 여과법을 쓰면 반점사진 전체에 걸친 반점의 이동벡터 분포를 한꺼번에 볼 수 있으나, 그 측정정밀도는 떨어진다. 정밀도를 높이려면 헤테로다인 기법을 쓰면 되지만, 이 때는 한번에 한 곳에서만 이동벡터를 잴 수 있으므로 반점사진 전체를 읽어내는데는 시간이 오래 걸린다. 공간주파수 여과법과 위상이동기법을 결합시키면 반점사진 전체를 한꺼번에 읽어내면서도 반점들의 이동벡터의 측정밀도를 높여 거의 헤테로다인 기법을 써야 얻을 수 있는 수준까지 올릴 수 있다. 이 새로운 방법을을 적용시킬 수 있는 실험장치를 고안하고 그 장치의 작동원리를 설명하였다. A new method of the reading out specklegrams, which can provide both the speed and precision simultaneously to the measurement of speckle displacement is proposed. The method is based on the combined application of spatial frequency filtering technique and phase stepping technique. With the spatial frequency filtering technique one can quickly obtain the distribution of speckle displacement vectors over a whole specklegram, yet the precision of the measurement is not so good. By additionally applying the phase stepping technique the precision can be increase to the level of the precision obtainable with heterodyne technique, while preserving the measurement speed. An experimental layout for realizing this new method is devised and the theoretical basis of its operation is explained.

      • 창원시 농촌지역 간이상수도 오염실태 조사

        Contamination of small water systems at rural areas of Changwon city was investigated in this study. The results of this study showed that the KDWQS compliance rate by small water systems in the study area was better than national average, but worse than the US average. Microbial standards were more frequently violated than any other standard. However, small water systems in the ,study area were found to be relatively less contaminated than groundwaters in other areas of Korea. No heavy metal was detected in small water systems in the study area. 4 % and 1 % of small water systems in the study area violated the NO₃-N and F^(-) standards. NO₃-N was found to be closely related to the planting area. No organic standard was violated and no pesticide/herbicide was detected. A few VOC (TCE, bensene, toluene, ethylbenzene) were detected, but their concentrations were low.

        한국인 당뇨병 및 비당뇨병 환자에서의 뇌혈관 질환 유무에 따른 PAI-1 촉진자 유전자형과 인슐린저항성에 관한 연구

        연구배경 : 혈전현상을 특징으로 하는 질환에서는 Plasminogen activator inhibitor-1 (PAI-1) 이 높은 활성도를 보이는데, PAI-1 치는 당뇨병, 심근경색증, 비만 등에서 높다고 밝혀진 바 있다. 또한 당뇨병 환자들의 합병증의 주요한 병인은 죽상경화증으로 혈전현상이 특징인 질환에서 증가하는 PAI-1이 당뇨병 환자에서 높다. 목적 : 정상인에서의 PAI-1 유전자 촉진자의 유전자형의 분포 및 혈액농도를 관찰하고, 당뇨병 및 뇌혈관 질환 환자군에서의 PAI-1 유전자 촉진자 유전자형의 분포 및 혈액농도를 측정하여 정상인과 차이점을 알아본다. 당뇨병 환자군에서의 혈장 PAI-1 치와 인슐린 저항성, 전구 인슐린 등과의 상관관계를 살펴보고, 인슐린저항성과 대혈관질환의 지표로 사용될 수 있는지 알아보았다. 방법 : 대상으로는 정상인 76명, 제2형 당뇨병 환자 56명, 뇌혈관질환이 동반된 제2형 당뇨병 환자 48명, 뇌혈관질환 환자 51명을 선택하여, 환자의 혈액에서 인슐린, 공복시 혈당, 전구인슐린, 중성지방, 총콜레스테롤 및 기타 생화학 검사 및 이학적 검사를 시행하였다. 환자의 DNA를 채취하여 전사개시 -675bp를 포함하는 대립형질 특이 시발체를 사용하여 중합효소 연쇄 반응을 실시하여, 그 유전자형을 판독하였다. 결과 : 정상 대조군 76명 (46.4±11.1세), 2형 당뇨병 환자 56명 (58.3±12.6세), 뇌경색증 환자 51명 (63.1±13.2세) 대상으로 하였다. PAI-1 촉진자 유전자형의 (4G/4G, 4G/5G, 5G/5G)빈도는 정상 대조군이 각각 23.7%, 75.0%, 1.3%, 뇌경색 환자군이 19.6%, 66.7%, 13.7%, 뇌경색이 동반된 당뇨병 환자군이 33.3%, 58.3%, 8.3% 였다. (X2=12.6, p=0.05). 이러한 사실은 서구인에 비해 4G/4G, 5G/5G 동형 유전자형이 낮은 결과였다. 각 군별 혈장 PAI-1 농도는 정상 대조군 13.4, 1.8 ~ 65.2 ng/mL (중앙값 , 범위 ) 2형 당뇨병 환자군 14.4, 2.9 ~ 47.8 ng/mL, 뇌경색 환자군 21.9, 6.2 ~ 154.7 ng/mL , 뇌경색이 동반된 2형 당뇨병 환자군 28.8, 3.2 ~ 139.3 ng/mL 로 차이를 보였다 (p=0.000). 전체 대상에서 PAI-1 촉진자 부위의 유전자형에 따른 PAI-1 활성도와 항원 농도는 차이를 보이지 않았다. 그러나 PAI-1 활성도는 혈중 중성지방, 전구인슐린, 체질량지수와 독립적인 상관관계를 보였다 (p=0.000, p=0.000 and p=0.005). 결론 : 결론적으로 PAI-1 촉진자 부위의 유전자형은 뇌경색증의 지표는 아니며, PAI-1 활성도를 결정짓는 인자는 유전적 요인보다는 혈중 중성지방, 전구 인슐린, 체질량지수와 같은 대사적 요인으로 생각된다. Plasminogen activator inhibitor-1 (PAI-1) is known be related to insulin resistance and several components of the large vascular disease. Notably, the high frequencies of diseases such as coronary heart disease or stroke are related to type 2 diabetes complications. We studied to find out whether the PAI-1 promother genotype could be a marker for cerebral infarction in type 2 patients. Subject patients were; 56 type 2 diabetics (age 58.3±12.6), 51 patients with cerebral infarction (age 63.1±13.2), 48 type 2 diabetics with cerebral infarction (age 64.8±9.3) , and 76 healthy control (age 46.4±11.1). The 4G/5G genotype of PAI-1 promoter was evaluated by polymerase chain reaction and endonuclease digestion. PAI-1 promoter genotype frequency (4G/4G, 4G/5G, 5G/5G) was 23.7%, 75.0% and 1.3% in healthy control, 17.9%, 67.9% and 14.3% in type 2 diabetes patients, 19.6%, 66.7% and 13.7% in cerebral infarction patients, 33.3%, 58.3% and 8.3% in type 2 diabetics with cerebral infarction (X^2=12.6, p=0.05). This finding is lower in frequency of 5G/5G homozygote than that reported in Caucasians. The plasma PAI-1 concentrations according to the disease were 13.4, 1.8 ~ 65.2 ng/mL (median, range) for healthy control, 14.4, 2.9 ~ 47.8 ng/mL for type 2 diabetes, 21.9 6.2 ~ 154.7 ng/mL for cerebral infarction , and 28.8, 3.2 ~ 139.3 ng/mL, for cerebral infarction with type 2 diabetes (p=0.000). In the all subjects, PAI-1 concentration and activity of PAI-1 promoter genotype did not show any significant difference. However, the PAI-1 activity was independently associated with serum triglyceride level, plasma proinsulin and BMI (p=0.000, p=0.000 and p=0.005 respectively). We concluded that PAI-1 genotype is not a marker for the cerebral infarction ; however, the genotype is related to PAI-1 concentration , and therefore it seems to be that metabolic factors such as triglyceride level or plasma proinsulin or BMI are more in relations with determining the PAI-1 concentration than the genotype.

        A cable dome structure that is a kind of soft structure is very sensitive to the initial stress, therefore if a tensile load of the member is changed, a tensile stress and location of the another member is also changed. And in order that this process reach the shape of real design, trial and error should be performed continually in the process of erection. But it is very difficult to adjust the internal stress in the real space exactly as the design shape. In the study, a behavior of cable structure is investigated by using control a stress and displacement, and analyze it numerically.
