      • Midazolam Inhibits Proinflammatory Mediators in the Lipopolysaccharide-activated Macrophage

        BACKGROUND:: Midazolam, a benzodiazepine, has a hypnotic effect and is widely used as a sedative. The role of midazolam in activation of macrophages during sepsis is not known. The aim of this study was to evaluate the antiinflammatory actions of midazolam in cultured macrophages. METHODS:: Using a macrophage cell line, RAW264.7 cells, the effect of midazolam on proinflammatory mediators and activation of mitogen-activated protein kinase was measured by Western blot. Nuclear factor-&kgr;B (NF-&kgr;B) activation and translocation of p65 subunit of NF-&kgr;B was measured using luciferase assay and immunocytochemistry. Superoxide production was measured by lucigenin chemiluminescence. RESULTS:: Midazolam significantly inhibited lipopolysaccharide-induced up-regulation of both cyclooxygenase 2 and inducible nitric oxide synthase in a dose-dependent manner (approximately 3–30 μm). I&kgr;B-α degradation and NF-&kgr;B transcriptional activity induced by lipopolysaccharide were also suppressed by the midazolam. Nuclear translocation of the p65 subunit of NF-&kgr;B was inhibited by midazolam. Furthermore, midazolam suppressed phosphorylation of p38 mitogen-activated protein kinase and also inhibited lipopolysaccharide-induced superoxide production in macrophages. CONCLUSIONS:: These results suggest that midazolam has an antiinflammatory action by inhibiting inducible nitric oxide synthase and cyclooxygenase-2 expression, possibly through suppression of NF-&kgr;B and p38 mitogen-activated protein kinase activation.

        미숙아에서 모세 혈관종의 임상양상

        Purpose: Capillary hemangiomas occur more frequently in preterm infants. We aimed to describe the clinical course of capillary hemangiomas in preterm infants. Methods: The records of preterm infants with a gestational age (GA) of <35 weeks who were admitted to two tertiary neonatal intensive care units from January 2004 to December 2013 and had capillary hemangiomas were reviewed retrospectively. Subgroup analysis of between infants of GA <30 weeks and GA 30-34+6 weeks were done and ad hoc analysis comparing study population and matched preterm infants without hemangioma for investigation of differences in clinical characteristics. Results: Of the 2,772 preterm infants, 112 (4%) infants developed capillary hemangiomas. The majority (91.9 %) of them had a solitary hemangiomas with the trunk was the most commonly involved site (43%). Three quarters of the patients were treated with topical corticosteroid, propranolol or laser treatment. When we divided this population as who were born before or after GA 30 weeks, there was no difference at postmenstrual age (PMA) of onset of capillary hemangiomas (median [IQR], 36+4 [30+5-40+5] vs. 36+2 [33+6-41+1] weeks, P= 0.275). The age at involution of capillary hemangiomas was also not differ between two groups (median [IQR], 7.75 [3.75-12.25] vs. 7.5 [4-13.75] months, P=0.425). There were no statistical differences between preterm infants with capillary hemangiomas and their age, weight and sex matched control preterm infants without hemangiomas in the neonatal and maternal factors. Conclusion: The development of capillary hemangiomas occurred at approximately 36 to 37weeks of PMA regardless of prematurity in preterm infants. Capillary hemangiomas of preterm infants resolved spontaneously and disappear completely by around 7 months of corrected age.

        Alternative Isoforms of TonEBP with Variable N-termini are Expressed in Mammalian Cells

        Hypertonicity imposes a great deal of stress to cells since it causes rise in cellular ionic strength, which can be reduced by the accumulation of compatible osmolytes. TonEBP plays a central role in the cellular accumulation of compatible osmolytes via transcriptional stimulation of membrane transporters and aldose reductase. Alternatively spliced forms of TonEBP mRNA have previously been reported and two of them showed different transcriptional activity. In the present study, isoform-specific antibodies were produced to confirm the translation of the spliced mRNA to protein. TonEBP was immunoprecipitated by using anti-TonEBP antibody and then immunoblotted using anti-TonEBP or isoform specific antibodies to find out the expression profile of TonEBP isoforms in basal or stimulated condition. From these results, we conclude that all TonEBP isoforms are expressed in mammalian cells and their expression patterns are not same in every cells.

        Granulocytopenia, which can be seen in patients with Graves' disease during treatment with antithyroid agents, could be a self resolving transient episode or can imply the beginning of life threatening agranulocytosis requiring a change in treatment modality. Transient granulocytopenia could be a manifestation of hyperthyroidism itself, or a mild side effect of antithyroid drugs. Aganulocytosis is a rare, but major complications of the termination drug, propylthiouracil (PTU), requiring prompt termination of the medication, and intensive care. Therefore, differentiation of agranulocytosis and transient granulocytopenia, is important, but is not practically easy. We introduce a case of transient granulocytopenia, which was detected in a patient with Graves' Disease, accompanied by underlying type 1 diabetes mellitus, during treatment with PTU. Diagnosis of transient granulocytopenia was made by a normal granulocyte count following a single injection of G-SCF*, and the patient was treated with conservative therapy. This case confirms a diagnostic tool for differentiating transient granulocytopenia and PTU-induced agranulocytosis

      • 새로운 진단기준(ADA)에 따른 임신성 당뇨병의 빈도 및 특징

        연구배경 : 최근에 미국당뇨병학회(ADA)에서 임신성 당뇨병의 진단기준으로 Carpenter와 Coustan 등의 기준을 채택하였다. 본 연구는 우리나라 산모를 대상으로 진단 기준의 변화에 따른 유병률의 변화와 임상적 특징을 고찰해 보고 자 하였다. 방법 : 1998년 10월부터 1999년 12월까지 포천중문의과대학 부속 분당차병원에 내원한 임신24~28주의 산모 2,682명 모두에서 50 g 경구당부하검사를 시행하였고, 양성을 보인 451명에서 100 g 경구당부하검사를 시행하였다. 최종적으로 정상내당능으로 확인된 산모를 A군, NDDG 기준에 의해 임신성 당뇨병으로 진단된 산모를 B군, ADA 기준을 적용함으로써 추가로 임신성 당뇨병으로 진단된 산모를 C군으로 하여 세 군 간의 임상적·생화학적 특징, 주산기 합병증 등을 비교하였다. 결과 : 1) NDDG 진단기준에 의한 임신성 당뇨병의 유병률은 2.1%였으며 ADA 진단기준을 적용하면 3.4%로 1.6배의 증가를 보였다. 2) C군의 임신전 체중, 신체질량지수(BMI), 임신횟수, 출산횟수, 공복시 혈당은 B군과 차이가 없었으나, 정상내당능을 보인 A군과는 유의한 차이를 보였다. 3) 신생아의 평균체중, 거대아, 과체중 출생아, 저체중 출생아, 미숙아 및 제왕절개술의 빈도, 1분 및 5분 APGAR 점수에 있어서는 세 군간에 유의한 차이를 보이지 않았다. 결론 : 임신성 당뇨병의 유병률은 NDDG 기준에 의하면 2.1%였으며, ADA 기준을 적용하면 3.4%로 1.6배의 증가를 보였다. ADA 진단기준을 적용함으로 인하여 추가로 임신성 당뇨병으로 진단된 군의 임상적·생화학적 특징이 정상 내당능을 보인 군과는 유의한 차이를 보였으며, 기존의 NDDG 진단기준에 의하여 임신성 당뇨병으로 분류된 군과는 차이가 없었다. Background: Recently, American Diabetes Association(ADA) accepted the Carpenterand Coustan's criteria for the diagnosis of GDM. This report will examine the change of the prevalence and clinical characteristics of GDM in Korean pregnant women according to either NDDG or ADA criteria. Method: 2,682 pregnant women were universally screened for GDM 451 positively screened women were taken 100gm OGTT and classified into three groups. The group A was women with NGT, group B was women with GDM by NDDG criteria and group C was women with GDM additionally diagnosed by ADA criteria. We examined the prevalence and compared the clinical and biochemical characteristics of the three groups. Results: 10 The prevalence of GDM was increased from 2.1% to 3.4% according to the change of the diagnostic criteria. 2) The pregestational weight, BMI, gravida, partiy, fasting plasma glucose level in group C were not different from group B, but significantly different from group A. 3) The rates of macrosornia large or small for gestational age, prematurity, 1 and 5 minute APGAR scores and cesarean section rates were not different among three groups.
