      • 자궁내막증 백서모형에서 Danazol과 Decapeptyl의 치료효과에 관한 연구

        본 연구의 목적은 장간막에 자궁내막 조직을 자가이식하여 수술적으로 유도된 자궁내막증 백서모형에서 자궁내막증의 치료를 위해 danazol과 decapetyl의 치료효과를 평가하는 것이다. 백서(n=48)들은 무작위적으로 정상대조군(n=12)과 수술적으로 유도된 자궁내막증군(n=36)으로 구분하였다. 후자는 비치료군과, danazol 치료군과 decapeptyl 치료군으로 세분하였다(각 군의 n=12). Danazol(8mg/일)은 수술후 4째주부터 구강을 통해 투여되었다. Decapeptyl(0.2mg/2주)은 근주하였다. 처음투약후 10일, 20일, 40일, 60일째에 각군의 2마리의 쥐를 희생시켰다. 저자들은 자궁내막증 이식체와 난소의 크기를 측정하여 다음의 결론를 얻었다. 1. 자궁내막증 이식체의 크기는 비치료군에 비해(3.91±1.08mm) danazol 치료군과 decapeptyl치료군에서 (각각2.28±0.17mm 과 2.78±0.97mm), 현저히 감소되었다(p<0.01). 2. danazol과 decapeptyl 두 치료군에서 장기치료가 단기치료보다 자궁내막증 이식체의 크기를 감소시키는데 더 효과적이었다(p<0.01). 3. 난소의 크기는 정상대조군과, 비치료군과 decapeptyl 치료군에서 유의하게 차이를 보이지 않은데 반해(각각 4.46±0.85mm, 4.77±0.82mm과 4.97±0.92mm), danazol 치료군에서는 유의하게 감소하였다(3.97±0.63mm) (p<0.63mm). 백서모형에서 수술적으로 이식된 자궁내막조직의 성장과 발달은 자궁내막증의 병인 이해와 신약개발에 유용한 연구모델을 제공할 수 있을 것 이다. The purpose of this study was to surgically induce endometriosis in the rat by autotransplanting endometrial tissue to the mesentery and to evaluate the efficacy of danazol and decapeptyl for the treatment of endometriosis. Rats(n=48) were randomly assigned to normal control group(n=12) and surgically induced endometriosis group(n=36). The latter was further divided into the untreated, danazol treated and decapeptyl treated groups (n=12 for each group). Danazol(8mg/day) was orally administrated from 4weeks after operation. Decapeptyl(O.2ml/2weeks) was injected intramuscularly. Two rats of each group are sacrificed on the 10th, 20th, 40th, 60th, 80th and lOOth day after the first drug administration. We measured sizes of endometriotic implants and ovaries. The results were as follows; 1. The sizes of endometriotic implants were significantly decreased(P<O.Ol) in the danazol and decapeptyl treated groups (2.28±O.71mm and 2.78±O.89mm, respectively) compared with untreated group(3.91±l.08mm). 2. Long term treatment was more effective than short term treatment in decreasing the size of endometriotic implants in both danazol and decapeptyl treated groups(P<O.Ol). 3. The size of ovaries was not significantly different among the normal control, untreated and decapeptyl treated groups(4.65±O.83mm, 4.77±O.82mm and 4.97±O.92mm, respectively) but was significantly decreased(P<O.Ol) in the danazol treated group(3.97±O.63mm). The successful growth and development of surgically transplanted endometrial tissue in the rat may offer an useful research model that can be used to understand pathogenesis of endometriosis and to explore new drug therapies.

      • 정상인에서 베타3-아드레날린 수용체 유전자의 변이가 복부 비만도 및 지질대사에 미치는 영향

        The β₃-adrenergic receptor is expressed in visceral adipose tissue in humans and is thought to contribute to the regulation of the resting metabolic rate and lipolysis. We studied the influence of a mutation in the β₃ -adrenergic receptor gene(Trp64Arg) on body fat distribution, central obesity, lipid metabolism in 65 healthy young male adults. One out of 65 subjects were homozygous (Arg/Arg) for the trp64Arg mutation, 17 subjects were heterozygous (Trp/Arg), and 47 lacked the mutation(Trp/Trp). The body weight, height, body mass index, waist-to-hip ratio were similar between the subjects with- and without the mutation. Total body fat, abdominal fat amount, body fat distribution, serum total- and HDL-cholesterol, triglyceride, glucose, and insulin concentrations were also not different according to the presence or absence of the mutation. These results suggested that Trp64Arg mutation in β₃-adrenergic receptor gene is not a major contributing factor for central obesity or change in lipid metabolism in Korean young adults.

      • 그레이브스 병 치료 후 재발 위험 인자에 관한 연구

        연구 배경 : 그레이브스 병에 대한 항갑상선제 치료후 적절한 치료 여부 평가 및 재발 위험성을 예측하기 위하여 여러 가지 예후 판정 인자들이 연구되어 왔으며, 이러한 인자들 중 TRH(thyrotropin-releasing hormone) 자극 시험이 가장 믿을 만한 검사중에 하나로 알려져 있다. 그러나 TRH 자극 검사가 정상으로 나와도 재발하는 경우가 많으며, 현재로는 이러한 경우에 재발을 예측하는 예후 인자에 대한 연구는 전무한 실정이다. 금번 연구에서는 TRH 자극 시험 후 TSH(thyroid stimulating hormone)가 정상적인 반응을 보인 환자를 대상으로 재발 및 관해여부를 예측할 수 있는 인자들을 찾고자 하였다. 대상 및 방법 : 항갑상선제제 치료 종료후 TRH 자극 시험을 시행하였던 그레이브스 병 환자 총 84명 중, 정상적인 TSH 반응을 보이면서 1년 이상 추적 관찰이 가능하였던 32명의 환자를 대상으로 하였다. 처음 진단 시와 치료 종료 시에 T3(triiodothyronine), T4(thyroxine), T3/T4 비, TSH, TRAb, Anti-TPO Ab(anti-thyroperoxidase antibody)를 측정하였으며, 치료 종료 2개월후에는 TRH 자극 검사를 하여 TSH 증가폭(△TSH) 및 유리 T_(3) 증가폭(△FT_(3))을 구하였다. 모든 측정치는 평균±표준편차로 표시하였으며, SPSS software를 이용하여 Student's paired t-test, chi-square test, Wilcoxon signed ranks test를 시행하였고, 유의 수준 0.05 이하를 통계적으로 유의하다고 판단하였다. 결과 1) 총 32명의 환자 중 재발한 환자(Group Ⅰ)는 22명(69%) 이었으며, 재발한 환자(Group Ⅱ)는 10명(31%) 이었다. 2) 진단 초기 총T3 치는 Ⅰ 군에서 342.0±126.2ng/dl, Ⅱ 군에서 493.0 ±142.2 ng/dl로 Ⅱ군에 비해 유의하게 증가되었다(p<0.05). 3) Anti-TPO Ab는 Ⅰ군에서 2.9±4.4, Ⅱ군에서 22.3±22.3 U/mL로 나타나 Ⅱ군에서 유의하게 증가되었다.(p<0.05) 4) 양군에서 치료 종료후 재발시 대부분의 환자가 1년 이내에 재발하였다. 5) TRH 자극· 검사후 유리 T3 증가폭 및 TSH 증가폭은 양군간에 차이가 업ㅅ었다(1.5±0.9 vs 1.2±1.0 pmol/L, 12.54±7.56 vs 11.23±4.21 mU/L, p>0.05) 결론 : TRH 자극 검사후 정상적인 TSH 반응을 보인 환자에서 재발 및 관해여부를 치료 초기 T3 및 치료 종료시 Anti-TPO Ab로 예측할 수 있었으며, 이들 측정치가 증가해 있을수록 재발의 가능성이 많음을 알 수 있었다. 또한 투약 중단후 초기 12개원의 추적기간 동안 관해가 유지되면 그 이후엔 재발의 가능성이 현저히 줄어 들었으며, 이 후 좀더 많은 환자들을 대상으로 하는 장기적인 추적연구가 진행 되어야 할 것으로 사료된다. Background : Many studies were undertaken to find parameters that could predict the clinical course of patients with Graves' disease after withdrawal of antithyroid drugs. In many parameters, TRH (thyrotropin-releasing hormone) stimulation test and T3 suppression test were considered to be one of the most reliable for predicting remission. But recently, some studies showed that the measurement of free T3 and FT3 are to be a more accurate than other thyroid function test. In this study, we measured T_(3), T_(4), T_(3)/T_(4), TSH (thyroid stimulating hormone), TRAb (TSH receptor antibody), FT3 and other known useful parameters to compare the factors influencing the relapse of Graves disease after antithyroid medication. Patients and methods : Among the 84 Graves' disease patients who had done the TRH stimulation test after withdrawal of antithyroid medication, we examined 32 patients who showed normal TSH response and followed up more than one year. T_(3), T_(4), T_(3)/T_(4), TSH and TRAb levels were measured at the time of diagnosis and at the end of therapy. Serum TSH and FT_(3) levels were measured after TRH stimulation, after two months of withdrawal of medication. Statistical evaluation was Performed by SPSS software in combination with Student's Paired t-test, chi-square test, Wlicoxon signed-ranks test. p-value less than 0.05 was considered to be significant. Results 1. In the 32 patients with Graves disease, 10 patients(31%) relapsed(Group Ⅰ) and 22 Patients(69%) remained in remission(Group Ⅱ). 2. Initial serum T3 level was significantly higher in group Ⅰ than group Ⅱ(342.0 ±126.2 vs. 493.0±142.2 ng/dl, p<0.05). 3. Anti-TPO Ab level was significantly higher in group Ⅰ than group Ⅱ(22.3±22.3 vs 2.9±4.4 U/mL, p<0.05). 4. In group Ⅰ and Ⅱ most patients showed relapse within 1 year after antithyroid medication. 5. After TRH stimulation test, serum FT_(3), and TSH level were not significantly different between group I and Ⅱ(p>0.05). 6. Serum T_(4), T_(3)/T_(4), TSH, TRAb levels were not significantly different from group Ⅰ and Ⅱ. Conclusion : In this study, we found increased relapse rate in the patients with Graves' disease whose initial serum T3 and anti-TPO Ab levels were high, so these parameters could be used to predict the relapse of Graves' disease. In the case of more than 12 months remission period after withdrawal of antithyroid medication, no relapse wsa observed in this study, but study should be done in the large population of patients with Graves' disease.

      • 개심술(開心術)의 임상적 고찰 : 치험 200예 200 Cases

        Background and Objectives: Two hundred cases of the open heart surgery due to congenital and acquired heart disease were done using the cardiopulmonary bypass in the Department of Thoracic and Cardiovascular surgery, Chosun University Hospital from November, 1980 to June, 2001. Material and Methods: The number of congenital heart disease was 132 cases. 68 cases were acquired heart disease. 'The age of the congenital heart disease was from 22 days to 45 years. In the acquired heart disease, the age was from 14 to 72 years. Result: Congenital heart disease consisted of 113 congenital acyanotic heart disease, 19 congenital cyanotic heart disease. Corrective operation was done for congenital heart disease with 4. 5 % operative mortality. Of 68 cases acquired heart disease, 44 patients were valvular heart disease, 3 cardiac tumors, 9 ischemic heart diseases, 12 aortic dissections and etc. The operative mortality was 13%. Conclusion: The postoperative complications were appeared in 68 cases and the complications were wound infection, occipital alopecia, low cardiac output syndrome, arrhythmia, septicemia etc. The mortality after open heart surgery was 15 cases and the causes of death were low cardiac output syndrome, septicemia, respiratory failure and left ventricle rupture.

        Nimodipine as a Potential Pharmacological Tool for Characterizing R-Type Calcium Currents

        <P> Nimopidine, one of dihydropyridine derivatives, has been widely used to pharmacologically identify L-type Ca currents. In this study, it was tested if nimodipine is a selective blocker for L-type Ca currents in sensory neurons and heterologous system. In mouse dorsal root ganglion neurons (DRG), low concentrations of nimodipine (<10μM), mainly targeting L-type Ca currents, blocked high-voltage-activated calcium channel currents by ∼38%. Interestingly, high concentrations of nimodipine (>10μM) further reduced the "residual" currents in DRG neurons from α<SUB>1E</SUB> knock-out mice, after blocking L-, N- and P/Q-type Ca currents with 10μM nimodipine, 1μM ω-conotoxin GVIA and 200 nM ω-agatoxin IVA, indicating inhibitory effects of nimodipine on R-type Ca currents. Nimodipine (>10μM) also produced the inhibition of both low-voltage-activated calcium channel currents in DRG neurons and α<SUB>1B</SUB> and α<SUB>1E</SUB> subunit based Ca channel currents in heterologous system. These results suggest that higher nimodipine (>10μM) is not necessarily selective for L-type Ca currents. While care should be taken in using nimodipine for pharmacologically defining L-type Ca currents from native macroscopic Ca currents, nimodipine (>10μM) could be a useful pharmacological tool for characterizing R-type Ca currents when combined with toxins blocking other types of Ca channels.

      • Unknown Parameter Identifier Design of Discrete-Time DC Servo Motor Using Artificial Neural Networks

        This paper introduces a high-performance speed control system based on artificial neural networks(ANN) to estimate unknown parameters of a DC servo motor. The goal of this research is to keep the rotor speed of the DC servo motor to follow an arbitrary selected trajectory. In detail, the aim is to obtain accurate trajectory control of the speed, specially when the motor and load parameters are unknown. By using an artificial neural network, we can acquire unknown nonlinear dynamics of the motor and the load. A trained neural network identifier combined with a reference model can be used to achieve the trajectory control. The performance of the identification and the control algorithm are evaluated through the simulation and experiment of nonlinear dynamics of the motor and the load using a typical DC servo motor model.

        Isolation of Medullary Dorsal Horn Neurons and Characteristics of Membrane Currents

        Medullary dorsal horn (MDH)neurons receive noxious inputs from primary afferent fibers in the orofacial area, integrate modulatory influences from descending and local circuits and send the information to the thalamus and somatosensory cortex. In addition, this region contains many putative neurotransmitters, including excitatory and inhibitory amino acids and peptides. Because of the importance of MDH neurons in orofacial pain transmission, the modulation of activities of these cells is very interesting. In this study, we tried to establish a method for isolating and preparing MDH neurons for the purpose of sutdying the pain modulation mechanism in future, and identify if the acutely isolated cells preserve their electrophysiological activities using patch-clamp and fura-2-based microfluorometry techniques. Transverse slices (400 ㎛) of lower brinstem, 2-3 mm caudal to the obex, from neonatal rats (1-2 weeks old)were sequentially digested with two proteases (pronase 1 mg/ 5 ml, and thermolysin 1 mg/5 ml) at 32℃ for 20 min. The MDH region was then dessected out and mechanically dissociated under a stereomicroscope using a series of fire-polished glass pipettes with a variety of orifice sizes. Single isolated cells were used for the experinemt. This method yields multipolar, bipolar, unipolar, ellipse and pyramidal cells. These cells seem to be marginal cells of lamina I, stalked cells or islet cells in lamina II and pyramidal cells in lamina III, respectively. Fura-2-based microfluorometry showed that the intracellular Ca^2+ concentration ([Ca^2+]_i) increased in response to the membrane depolarization induced by high K^+, and the increase of [Ca^2+]_i was caused by the influx of extracellular Ca^2+, demonstrating the expression of voltage-sensitive calcium channels (VSCC) in MDH neuroms. From whole-cell patch-clamp techniques, voltage-sensitive Na^+and Ca^2+ currents were directly recorded, the results were similar to those studied in in vivo or slice preparations. These facts indicate that electrophysiological activities are well preserved in the acutely isolated neurons, and these cells can be effectively used for biophysical and pharmacological studies, including understanding the pain modulation mechanism mediated by VSCC in the MDH.

        Effect of Capsaicin and Its Derivative on the Voltage Dependent Ca^2+ Currents in Spinal Dorsal Root and Trigeminal Ganglion Neurons

        Capsaicin produces initial irritation followed by insensitivity to noxious stimuli. It can not be easily used in clinical pain control due to its toxic effects. Recently, a capsaicin derivative (KR) was developed to use as an analgesic with fewer toxic effects. However, the mechanism of action and clinical effects of KR are not well understood. In this experiment we tried to explore the cellular mechanisms of the analgesic effect induced by capsaicin and KR in DRG or TG neurons. The DRG and TG neurons were acutely isolated into single neuronal cells from neonatal or adult rats with trypsin treatment and mechanical trituration. Voltage dependent calcium currents were measured with whole-cell patch clamp configuration in small diameter sensory neurons (between 10 μm and 20μm). On the basis of activation threshold, Ca^2+ currents of DRG and TG neurons were classified as LVA (T-type) and HVA I_ca. HVA I_ca were subclassified into L-type and N-type. The amplitude of L-type calcium current was remarkably inhibited by application of 1 μM KR in DRG. The inhibition of calcium current recovered significantly at five minutes after application of KR. In most capsaicin-sensitive neurons in TG, capsaicin irreversibly inhibited voltage dependent calcium currents. Capsaicin and KR-induced inhibition of voltage dependent calcium currents would be expected to reduce neurotransmitter release from both central and peripheral terminals of the sensitive primary sensory neurons. This can be proposed as one possible mechanism for the analgesic and anti-inflammatory effects induced by them.
