      • Tumor Diameter for Prediction of Recurrence, Disease Free and Overall Survival in Endometrial Cancer Cases

        Aims: To analyse the predictors of recurrence, disease free survival and overall survival in cases with endometrial cancer. Materials and Methods: A total of 152 women diagnosed with endometrial cancer were screened using a prospectively collected database including age, smoking history, menopausal status, body mass index, CA125, systemic disorders, tumor histology, tumor grade, lymphovascular space invasion, tumor diameter, cervical involvement, myometrial invasion, adnexal metastases, positive cytology, serosal involvement, other pelvic metastases, type of surgery, fertility sparing approach to assess their ability to predict recurrence, disease free survival and overall survival. Results: In ROC analyses tumor diameter was a significant predictor of recurrence (AUC:0.771, P<0.001). The optimal cut off value was 3.75 with 82% sensitivity and 63% specificity. In correlation analyses tumor grade (r=0.267, p=0.001), tumor diameter (r=0.297, p<0.001) and the serosal involvement (r=0.464, p<0.001) were found to significantly correlate with the recurrence. In Cox regression analyses when some different combinations of variables included in the model which are found to be significantly associated with the presence of recurrence, tumor diameter was found to be a significant confounder for disease free survival (OR=1.2(95 CI,1.016-1.394, P=0.031). On Cox regression for overall survival only serosal involvement was found to be a significant predictor (OR=20.8 (95 % CI 2.4-179.2, P=0.006). In univariate analysis of tumor diameter > 3.75 cm and the recurrence, there was 14 (21.9 %) cases with recurrence in group with high tumor diameter where as only 3 (3.4 %) cases group with smaller tumor size (Odds ratio:7.9 (95 %CI 2.2-28.9, p<0.001). Conclusions: Although most of the significantly correlated variables are part of the FIGO staging, tumor diameter was also found to be predictor for recurrence with higher values than generally accepted.

        YouTube: A good source for retrograde intrarenal surgery?

        Purpose: To evaluate the quality of videos for retrograde intrarenal surgery (RIRS) on YouTube (Google, LLC) from the perspective of both patients and physicians. Materials and Methods: All videos longer than 2 minutes returned by the YouTube search engine in response to the keyword search “retrograde intrarenal surgery” were included in this study. The quality of content was analyzed by using the validated Journal of the American Medical Association Benchmark Score (JAMAS) and the Global Quality Score (GQS). Two surgeons developed the RIRS Scoring System (RIRSSS) to evaluate the technical quality of the videos. A video power index (VPI) was used to score the popularity of the videos. Results: A total of 63 videos with a median of 389 views were included in the present study. Forty-three videos (68.3%) were provided by health care professionals and 53 videos (84.1%) included technical aspects about RIRS. The median (interquartile range) GQS, JAMAS, RIRSSS, and VPI scores were 2 (1–3), 1 (1–2), 2 (1–5), and 0.41 (0.08–1.29), respectively. Videos with audio had significantly higher GQS and RIRSSS scores than did with videos with no audio (p<0.001, p=0.039, respectively). The GQS of videos providing general information about RIRS was higher, whereas RIRSSS scores were higher for videos detailing technical aspects (p=0.027, p=0.038, respectively). Conclusions: The quality of YouTube videos containing information about RIRS evaluated in this study was very low. It is necessary for health care organizations to prepare online materials and upload these materials to popular social media platforms to convey accurate information to patients.

        Investigation of linear and nonlinear of behaviours of reinforced concrete cantilever retaining walls according to the earthquake loads considering soil-structures interactions

        It is known that retaining walls were severely damaged as well in the most recent earthquakes having occurred in the countries in the active seismic belts of the world. This damage can be ascribed to the calculation methods used for the designs of retaining walls in the event of their constructions and employment having been accurately carried out. Generally simplified pseudo-static methods are used in the analysis of retaining walls with analytical methods and soil-structure interaction are not considered. In view of these circumstances, in this article by taking soil interaction into consideration, linear and nonlinear behaviours of retaining walls are analyzed with the assistance of LUSAS which is one of the structural analysis programs. This investigations are carried out per LUSAS which employs the finite element method as to the Erzincan (1992) Earthquake North-South component and the obtained findings are compared with the ones obtained from the method suggested in Eurocode-8, which is still effective today, and Mononobe-Okabe method. Not only do the obtained results indicate the distribution and magnitude of soil pressures are depend on the filling soil but on the foundation soil as well and nonlinear effects should be considered in designs of these walls.

      • Comparison of Two Step LEEP and Cold Conisation For Cervical Intraepithelial Lesions to Decrease Positive Surgical Margins

        Purpose: To assess the success rates of two step loop electrosurgical excision procedure (LEEP) compared with conventional cold conization procedures for decreasing positive surgical margins. Materials and Methods: This study was conducted on 70 patients who underwent colposcopic evaluation in Zeynep Kamil Women and Children's Health Training and Research Hospital between 2013-2015 with indications of CIN 2/3 or persistent CIN 1 for more than 2 years. The study included age matched groups of patients with similar histopathololical lesions who underwent cold conization (n=40) or LEEP (N=30). Results: Comparison of tissue characteristics between the two groups revealed significantly higher deepest depth and lower volume of tissue removed by the two step LEEP. Ectocervical positivity rate was similar between groups (1/39 versus 0/29, P>0.05), while endocervical surgical margin positivity rate was significantly higher in the cold conization group (9/39 versus 0/29, P<0.05). Surgical margin positive cases were significantly older than the cases with negative margins (P<0.05). Conclusions: Two step LEEP made it easier to reach the squamocolumnar junction in the endocervical region with lower blood loss and applicability in office settings. Our study suggests to use two step approach in cases with high grade and glandular CIN.

      • A Single-Phase, Zero-Torque, Drivetrain Integrated On-Board Charger Control Method for Plug-In Hybrid and Electric Vehicles

        The drivetrain components of an electric vehicle can be utilized to obtain an on-board battery charger (OBC) to save cost, volume and weight. Many integrated OBC topologies have been proposed in the past years, which involve a wide range of machine types, inverter topologies, and power levels. None of these ideas provide a universally acceptable alternative due to the disadvantages they inherit. Therefore, ”drivetrain integrated on-board chargers” is still an interesting research topic. In this paper a single-phase, zero-torque integrated on-board charger control method, which utilizes the machine inductance very effectively and requires minimal intrusion to an existing drivetrain design with a 3-phase permanent-magnet synchronous machine and a three-phase inverter for plug-in hybrid and electric vehicles is investigated.

        Evaluation of Larynx Cancer via Chemometrics Assisted Raman Spectroscopy

        Larynx cancer is a potentially terminal and severe type of neck and head cancer in which malignant cells start to grow and spread upwards in the larynx, or voice box. Smoking tobacco, drinking hot beverages and drinking alcohol are the main risk factors for these tumors. In this study, we aimed to develop a precise, accurate and rapid chemometrics assisted Raman spectroscopy method for diagnosis of larynx cancer in deparaffinized tissue samples. In the proposed method, samples were deparaffinized and 20 microns of each tissue were located on a coverslip. Both healthy (n = 13) and cancerous tissues (n = 13) were exposed to a Raman laser (785 nm) and excitations were recorded between wavenumbers of 50~1500 cm -1. An Orthogonal Partial Least Square algorithm was applied to evaluate the Raman spectrum obtained. Sensitivity and specificity of the proposed method is high enough with the aid of Principal Component Analysis (PCA) to test the whole model. Healthy and cancerous tissues were accurately and precisely clustered. A rapid, easy and precise diagnosis algorithm was developed for larynx cancer. By this method, some useful data about differences in biomolecules of each group (phospholipids, amides, tyrosine, phenylalanine collagen etc.) was also obtained from the spectra. It is claimed that the optimized method has a great potential for clustering and separating tumor tissues from healthy ones. This novel, rapid, precise and objective diagnosis method may be an alternative for the conventional methods in literature for diagnosis of larynx cancer.

      • Investigation of the effects on earthquake behavior and rough construction costs of the slab type in reinforced concrete buildings

        In the architectural design process, the selection and configuration of the structural system significantly affect the earthquake behaviours of the reinforced concrete buildings. The main purpose of this study, the effects on the earthquake performances and the rough construction cost of the buildings of the slab type in reinforced concrete buildings are to examine comparatively for different local soil classes. The results obtained from this study have been determined that the building model having slabs with beams is safer compared to other types of slabs, especially when considering the vertical bearing structural elements (columns). It also shows that other types of slab, except for slab with beams, reduce the earthquake performances of reinforced concrete buildings, increase the displacement values, 1st natural vibration period values and the cost of rough construction. This matter reveals that slab type is quite important and the preference of beamed slabs in reinforced concrete buildings to be constructed in earthquake zones would be more appropriate in terms of safety and cost.

        Preventive Effect of Probiotics and α-Tocopherol on Ethanol-Induced Gastric Mucosal Injury in Rats

        The protective effect of a probiotic mixture of 13 different bacteria and α-tocopherol on 98% ethanol-induced gastric mucosal injury was evaluated. Levels of gastric mucosal pro- and anti-inflammatory cytokines, malondialdehyde, and secretory immunglobulin A were measured. Rats were allocated into four groups: control, ethanol, probiotic, and α-tocopherol. The control and ethanol groups received skim milk for 14 days. Probiotic and α-tocopherol groups were administered probiotic mixture suspended in skim milk and 100mg/kg α-tocopherol, respectively, by daily gavage for 14 days. On Day 15, gastric lesions were induced by administration of ethanol 98% (1mL) to all rats except those in the control group. Probiotic, but not α-tocopherol, seemed to inhibit ethanol-induced gastric mucosal tumor necrosis factor-α, interferon-γ, and interleukin-2 production (P>.05). Ethanol caused the elevation of mucosal interleukin-4 level (compared to the control, P<.05). Probiotic pretreatment significantly suppressed the ethanol-induced increase of gastric mucosal interleukin-4 levels. Pretreatment with either probiotic or α-tocopherol inhibited the ethanol-induced increase of mucosal malondialdehyde concentration (P<.01 and P<.05, respectively). Probiotic pretreatment enhanced the gastric mucosal secretory immunoglobulin A concentration (P<.001). In conclusion, probiotic mixture and α-tocopherol reduced ethanol-induced gastric mucosal lipid peroxidation, suggesting that they may be beneficial for gastric lesions induced by lower ethanol concentration.

        The Effects of Cold Application to the Perineum on Pain Relief After Vaginal Birth

        Purpose: Perineal pain developing during the postpartum period affects women's relationships with their families and infants. The aim of this study was to determine the efficacy of cold gel pad application for relieving perineal pain and possibly increasing mothers' comfort after vaginal delivery. Methods: This experimental randomized controlled study was conducted in the postpartum department of obstetrics and gynecology hospital. A total of 200 mothers were included in the study. Cold gel pads were applied to the perineum of mothers in the experimental group for 20 minutes in the postpartum first 2 hours and 4 hours after the first application. All the data were collected by using an information form, the visual analog scale, and the postpartum comfort questionnaire. Results: In the experimental group, the first visual analog scale score was 6.73 ± 1.68; after cold gel pad application, the pain levels decreased to 2.59 ± 1.20 in both primiparous and multiparous mothers. In addition, the postpartum comfort questionnaire score increased from 2.58 ± 0.14 to 2.69 ± 0.14 in the second assessment after the cold gel pad application and the difference was statistically significant (p < .001). Conclusion: The application of the cold gel pad to the perineum relieved perineal pain and increased postpartum comfort in all the women. The pain felt by the women during the recovery and the daily activities decreased. Postpartum perineal pain adversely affected daily activities such as lying down, sitting, and walking; infant care, breastfeeding, and urination; and comfort levels of the postpartum women.
