        Mapping Regional Laryngopharyngeal Mechanoreceptor Response

        Objectives. To map mechanoreceptor response in various regions of the laryngopharynx. Methods. Five patients with suspected laryngopharyngeal reflux and six healthy control subjects underwent stimulation of mechanoreceptors in the hypopharynx, interarytenoid area, arytenoids, aryepiglottic folds, and pyriform sinuses. The threshold stimuli evoking sensation and eliciting laryngeal adductor reflex were recorded. Results. In controls, an air pulse with 2 mmHg pressure evoked mechanoreceptor response in all regions, except bilateral aryepiglottic folds of one control. In patients, stimulus intensity to elicit mechanoreceptor response ranged between 2 mmHg and 10 mmHg and varied among the regions. Air pulse intensity differed between right and left sides of la- ryngopharyngeal regions in the majority of patients. Conclusion. Laryngopharyngeal mechanoreceptor response was uniform among regions and subjects in the healthy group. Patients with suspected laryngopharyngeal reflux showed inter- and intra-regional variations in mechanoreceptor re- sponse. Laryngopharyngeal sensory deficit in patients with suspected laryngopharyngeal reflux is not limited to ary- epiglottic folds.

      • Cost and Effectiveness Comparison of Immediate Colposcopy Versus Human Papillomavirus DNA Testing in Management of Atypical Squamous Cells of Undetermined Significance in Turkish Women

        Background: A small but significant proportion of cases with atypical squamous cells of undetermined significance (ASCUS) may harbour CIN 2-3, or even invasive carcinoma. Although immediate colposcopy, HPV-DNA testing or expectant management are three recommended options in ASCUS triage, a consensus does not currently exist on which one of these approaches is the most efficient. In this study, we aimed to compare the performance and cost of immediate colposcopy and colposcopy based on the human papillomavirus (HPV) testing for detecting histologically confirmed high-grade cervical intraepithelial neoplasia (CIN) in women with ASCUS. Materials and Methods: Records of 594 women with an index Papanicolaou smear showing ASCUS were retrospectively analyzed. Women in the immediate colposcopy arm were referred directly to colposcopy (immediate colposcopy group, n=255) and those in the HPV triage arm were proceeded to colposcopy if the high-risk HPV (hrHPV) test was positive (HPV triage group, n=339). High grade CIN (CIN2+) detection rate and treatment costs were compared between the groups. Results: The detected rate of CIN2+ was higher in the HPV triage group compared to immediate colposcopy group (8% vs. 1.6%, p=0.011). In the HPV triage group, the total cost, cost per patient, and the cost for detecting one case of high grade CIN were higher than the immediate colposcopy group (p<0.001). Conclusions: In women with ASCUS cytology, HPV DNA testing followed by colposcopy is more costly than immediate colposcopy, but this approach is associated with a higher rate of CIN2+ detection. This findings suggest that HPV DNA testing combined with cervical cytology could reduce the referral rate to colposcopy.

        Soil-pile interaction effects in wharf structures under lateral loads

        Wharfs are essential to shipping and support very large gravity loads on both a short-term and long-term basis which cause quite large seismic internal forces. Therefore, these structures are vulnerable to seismic activities. As they are supported on vertical and/or batter piles, soil-pile interaction effects under earthquake events have a great importance in seismic resistance which is not yet fully understood. Seismic design codes have become more stringent and suggest the use of new design methods, such as Performance Based Design principles. According to Turkish Code for Coastal and Port Structures (TCCS 2008), the interaction between soil and pile should somehow be considered in the nonlinear analysis in an accurate manner. This study aims to explore the lateral load carrying capacity of recently designed wharf structures considering soil-pile interaction effects for different soil conditions. For this purpose, nonlinear structure analysis according to TCCS (2008) has been performed comparing simplified and detailed modeling results.

        Poly(amidoamine) Functionalized Graphene Oxide Incorporated Carbon/Epoxy Prepreg Composites for Enhanced Electrical and Thermal Properties

        Carbon fiber (CF) reinforced epoxy matrix composites were incorporated with 0.5, 1, 2, 4, 7, 10, and 15 wt%poly(amidoamine) (PAMAM) functionalized graphene oxide (GO) via matrix modification. GO was synthesized by chemicaloxidation and PAMAM dendrimer of generation 0.5 (G0.5) was used within the study. Surface chemistry of graphene wasanalyzed by X-ray photoelectron spectroscopy (XPS), which confirmed the oxidation and PAMAM functionalization(PAMAM-GO). After prepregging, laminated composites were manufactured by hot pressing. So produced CF/epoxy-PAMAM-GO composites were thermally analyzed by thermogravimetric analyses (TGA-DTG). The electrical conductivityof the composites was measured in three directions (fiber, transverse, and through-thickness). Mechanical properties wereexamined in means of interlaminar shear strength (ILSS) determined by short beam shear (SBS) tests. The neat graphene,GO, PAMAM-GO, and the fractured composite surfaces were imaged using scanning electron microscopy (SEM). As aresult, PAMAM-GO incorporation significantly enhanced the electrical properties of CF/epoxy composites. Thermal stabilityincreased while the ILSS values decreased.

        Intraovarian platelet-rich plasma administration could improve blastocyst euploidy rates in women undergoing in vitro fertilization

        Objective: Platelet-rich plasma (PRP) therapy has received a considerable attention as an adjunct to fertility treatments, especially in women with very low ovarian reserve and premature ovarian insufficiency. Although recent studies have demonstrated that PRP led to improvements in folliculogenesis and biomarkers of ovarian reserve, the effect of intraovarian PRP administration on embryo genetics has not been studied. Methods: We report a pilot study of patients who had preimplantation genetic testing for aneuploidy (PGT-A) before and then within 3 months following PRP administration. Twelve infertile women with at least one prior failed in vitro fertilization (IVF) cycle underwent ovarian stimulation (cycle 1) with a gentle stimulation protocol and PGT-A performed at the blastocyst stage. Following cycle 1, autologous intraovarian PRP administration was performed. Within 3 months following PRP administration, the patients underwent cycle 2 and produced blastocysts for PGT-A. The percentage of euploid embryos between both cycles was compared. Results: The mean age of all participants was 40.08±1.46 years, and their mean body mass index was 26.18±1.18 kg/m<sup>2</sup>. The number of good-quality embryos formed at the blastocyst stage was similar between cycle 1 and cycle 2 (3.08±0.88 vs. 2.17±0.49, respectively; p=0.11). Among all patients in cycle 1, 3 of 37 embryos were euploid (8.11%) while in cycle 2, 11 out of 28 embryos were euploid (39.28%, p=0.002). Three clinical pregnancies were noted among this patient group. Conclusion: This novel study is the first to present an improvement in the embryo euploidy rate following intraovarian PRP application in infertile women with prior failed IVF cycles. The growth factors present in PRP may exhibit a local paracrine effect that could improve meiotic aberrations in human oocytes and thus improve euploidy rates. Whether PRP improves live birth rates and lowers miscarriage rates remains to be determined in large trials.

      • Effect of Hydronephrosis on Survival in Advanced Stage Cervical Cancer

        Background: Hydronephrosis is frequently encountered in advanced stage cervical cancers, and may be associated with mortality. In the present study, we aimed to demonstrate the effect of hydronephrosis on survival in patients with inoperable advanced stage cervical cancer. Materials and Methods: The study data were acquired by retrospective analysis of the patient records belonging to 165 women with FIGO (International Federation of Gynecology and Obstetrics) stage-IIIB or more advanced cervical cancer, which were not surgical candidates. Parameters including patient age, pathological diagnosis, disease stage, pelvic sidewall extension, presence of hydronephrosis and administration of chemoradiation were analyzed. Further, the effects of these variables on survival were assessed. P values less than 0.05 were considered statistically significant. Results: The distribution of the study patients according to disease stage was as follows: 131 (79.4%) had stage-IIIB, 18 (10.9%) had stage-IVB and 16 (% 9.7) patients had stage-IVA disease. Hydronephrosis was not evident in 91 (55.2%) of these patients, whereas 41 (24.8%) had unilateral and 33 (20%) patients had bilateral hydronephrosis. When compared to mean survival in patients who did not have hydronephrosis, survival was significantly shortened in patients who had bilateral and unilateral hydronephrosis (p<0.05). There was no significant survival difference between patients with unilateral and bilateral hydronephrosis (p>0.05). Although patient age, pathological type, pelvic involvement, and chemotherapy treatment rates were similar (p>0.05), radiotherapy requirement rate and disease stage were significantly different among the study groups (p<0.05). Conclusions: Hydronephrosis was found to be a significant predictor of poor survival in patients with advanced stage cervical cancer, irrespective of unilateral or bilateral involvement.While waiting for future studies with larger sample sizes, we believe that the FIGO stages in advanced cervical cancer could further be stratified into subgroups according to presence or absence of hydronephrosis.

        Analysis of Changes in the Glomerular Filtration Rate as Measured by the Cockroft-Gault Formula in the Early Period after Percutaneous Nephrolithotomy

        Purpose: We aimed to analyze the changes in kidney function during the postoperative early period caused by the application of percutaneous nephrolithotomy (PNL), which is commonly used in kidney stone surgery. Materials and Methods: PNL was performed in 80 patients (48 men, 32 women) with kidney stones. The mean age of the patients was 43.71 years (range, 18 to 71 years). Preoperative and postoperative values for stone size, glomerular filtration rate (GFR), serum creatinine, urea, electrolytes (Na, K, Cl), and Hb were compared in 80 patients in whom PNL surgery was performed. The formula of Cockroft-Gault was used to calculate the GFR during the early postoperative period (72 to 96 hours). Results: Statistically significant decreases after PNL were observed in average stone size (preoperative, 627.75±375.10 mm2; postoperative, 81.70±16.15 mm2), serum urea (preoperative, 38.40±17.26 mg/dl; postoperative, 33.28±16.98 mg/dl), and creatinine (preoperative, 1.03±0.53 mg/dl; postoperative, 0.97±0.55 mg/dl) and an increase was observed in GFR (preoperative, 104.30±37.30 ml/min; postoperative, 112.38±40.1 ml/min). No changes were detected in the serum electrolyte values (Na, K, Cl). Multiple access, operation time, and type of lithotripter did not have any significant effects on the change in the GFR. Conclusions: In light of our results, PNL for kidney stone operations appears to be a reliable and efficient method that provides recovery of kidney functions in the early post-operative period by increasing the GFR and with high stone-free rates.       • Roles of Neutrophil/Lymphocyte and Platelet/Lymphocyte Ratios in the Early Diagnosis of Malignant Ovarian Masses

        Background: The present study aimed to investigate the utility and importance of the various parameters of complete blood count panel for benign-malignant differentiation of adnexal masses. Materials and Methods: This retrospective study involved 316 patients with documented benign and 253 patients with malignant adnexal masses who underwent primary surgical treatment at a tertiary referral center. Prior to the study, all benign and malignant cases were compared within their own groups and then the benign and malignant cases were compared to each other. For all cases, cut-off, sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) were calculated for the neutrophil lymphocyte ratio (NLR), platelet lymphocyte ratio (PLR), neutrophil, lymphocyte, platelet and CA-125 parameters, and the results were compared in regards to the groups. Results: NLR, PLR, neutrophil, CA-125, and platelet values were higher in the malignant compared to the benign cases (p<0.01). The lymphocyte value was lower in the malignant cases (p<0.01). No significant differences were found for basophils and eosinophils (p > 0.05). For CA-125, the sensitivity, specificity, PPV and NPV for all cases were 78%, 62%, 62% and 78%, respectively. For NLR, they were 65.6%, 72.1%, 65.3%, and 72.3%, and for PLR, 48%, 81%, 67%, and 66%. Additionally, the sensitivity and specificity were 55% and 77% for CA-125, 66% and 58% for NLR, and 61% and 58% for PLR in early malignant cases. Conclusions: NLR and PLR appear to be useful methods that can be applied together with CA-125 due to the relatively high sensitivity values for the malign-benign differentiation of ovarian masses. Although the specificity of these parameters is lower than CA-125, especially in cases with early malignant ovarian pathology, their sensitivity being higher is promising for the early diagnosis of ovarian cancer. It can be used to detect ovarian malignancies in the early stages, and it will increase the treatment options and improve survival rates.

        Behavior and design of stainless steel tubular member welded end connections

        Among the various alternatives to make a steel tubular member connection, making a slotted and gusset plate welded connection is one of the most frequently preferred alternatives. This type of connection is essentially an end connection that is made by slotting the tube longitudinally, inserting the gusset plate and then placing longitudinal fillet welds at the tube-to-plate interface. In this paper an experimental study on the behaviour of such connections in stainless steel is presented. 24 specimens were tested under concentrically applied axial tensile forces for varying tube-to-gusset plate weld lengths. Both circular and box section members were considered in the test program. Load-deformation curves were obtained and comparisons were made in terms of strength and ductility. The results obtained from the study were then critically examined and compared with currently available design guidance for slotted gusset plate welded tubular end connections. It is noted that no specific rules exist in international specifications on structural stainless steel which cover the design of such connections. Therefore, the results of this study are compared with the existing design rules for carbon steel.
