A wind power heat generation system that converts wind power directly to heat instead of electric power is considered in this study. The system consists of a wind turbine part and a heat generation part. The heat generation part is materialized by a hydraulic system including a hydraulic pump, a flow control valve, a hydraulic oil tank, etc. The flow control valve primarily converts hydraulic energy generated in the pump to heat energy. It should have a function of overspeed protection under excessive wind speeds. In this study, a novel flow control valve design is proposed for excellent flow control characteristics under excessive pump driving torque (excessive wind speed). The performance of the suggested valve is analyzed using numerical simulation.

Flow and pressure ripple in swash plate type piston pumps is largely dependent on the design of notches(silencing grooves) in the valve plate. In uni-directional pumps, the basic design concept for notches in the valve plate could be said to be established. It is easily deduced that the design concept for notches in uni-directional pumps can not be simply applied to bi-directional pumps requested for EHA(electric hydrostatic actuators). To carry out systematic research on technological issues regarding notch applications to bi-directional piston pumps, five notch designs are devised. The effects of the notch designs on the characteristics of the pump are investigated by numerical simulations and experiments. Through this study, basic concepts about notch design for bi-directional piston pumps are suggested.

<P>Symmetric bifrontal uptake of bone-seeking agents is usually considered as the main feature of hyperostosis frontalis interna in postmenopausal elderly women. This finding is not uncommon in elderly women because of the change in their hormonal level. However, in the present case, a 66-year-old woman with intra-axial brain metastases of breast cancer showed symmetric bifrontal uptake on bone scintigraphy. Therefore, symmetric bifrontal uptake should not always be considered as a definite indicator of hyperostosis frontalis interna. Further evaluation such as SPECT/CT is needed for evaluation of brain metastases especially in cancer patients.</P>

<P>Barcelona Clinic Liver Cancer (BCLC) stage C hepatocellular carcinoma (HCC) consists of a heterogeneous group of patients with a wide range of survival times, requiring further prognostic stratification to facilitate treatment allocation. We evaluated the prognostic value of F-18-FDG uptake on PET/CT at the time of presentation in patients with BCLC stage C HCC. Methods: A total of 291 patients with BCLC stage C HCC who underwent F-18-FDG PET/ CT between 2009 and 2010 for staging were retrospectively enrolled from 7 university hospitals. The patients were further divided into 2 groups according to the extent of disease, as intrahepatic or extrahepatic. Tumor-to-liver SUV ratio (TLR) of the primary tumor was measured on F-18-FDG PET/CT. Prognostic values of TLR and other clinical variables were analyzed to predict overall survival (OS) in univariate and multivariate analyses. Differences in the OS stratified by TLR were examined by the Kaplan Meier method. Results: Higher TLR was associated with extrahepatic disease (P = 0.018). On multivariate analysis, Child Pugh classification and TLR were independent prognostic factors in the intrahepatic disease group (all P < 0.05), whereas TLR was the only independent prognostic factor in the extrahepatic disease group (P < 0.05). Patients with high TLR showed a significantly worse OS than those with low TLR (P < 0.05) in both groups. Conclusion: In patients with BCLC stage C HCC, F-18-FDG uptake in the primary tumor was significantly higher in patients with extrahepatic disease than in those with intrahepatic disease. In addition, F-18-FDG uptake on pretreatment PET/CT had an incremental prognostic value for OS in both intrahepatic and extrahepatic disease groups.</P>

Perilla is an Asian grain that consumed for food ingredient, oil crops, medicinal materials through the process of roasting. A 49-year-old male has been roasting grains for 10 years, who met with shortness of breath during the roasting perilla seeds, but not in other situations. Serum total immunoglobulin E (IgE) level, serum eosinophil count and skin prick test didn`t showed significant results. Methacholine bronchial provocation test was positive (PC20 0.31 mg/mL). Specific bronchial provocation test with inhaling smoke from roasting perilla seeds showed a dual asthmatic response. Measured peak expiratory flow rate on his work place showed the result of 37% decrease at the end of work and full recovery at 6 hours after the end of work. Skin prick test to row perilla showed positive response with late symptoms. We diagnosed that the patient had an occupational asthma caused by inhaling smoke from roasting perilla seeds with IgE mediated mechanism. (Allergy Asthma Respir Dis 1(1):90-93, 2013)

위험인자가 없이 발생한 근위심부정맥혈전증 및 폐색전증 환자에서는 고호모시스테인혈증을 포함한 혈전성향을 확인하는 것이 필요하다. 저자들은 엽산 결핍에 의한 고호모시스테인혈증으로 인해 근위심부정맥혈전증및 폐색전증이 발생한 것을 진단하고 엽산과 항혈소판제의 투여를 유지하여 혈중 호모시스테인의 수치를 정상화하고 혈전증을 치료한 환자를 경험하여 보고하는 바이다. Most cases of venous thrombosis occur at underlying condition. If a patient is diagnosed with venous thrombosis without any apparent risk factor, evaluation of thrombophilia maybe needed. Hyperhomocysteinemia is associated with hypercoagulability and cardiovascular disease including venous thrombosis, the leading cause of it is vitamin deficiency. We report a case of pulmonary embolism and both internal iliac vein thrombosis with hyperhomocysteinemia due to folate deficiency. A 29-year-old male patient presented with chest pain. Chest and lower extremity CT scans revealed pulmonary embolism and thrombosis of both internal iliac veins. His plasma homocysteine level was elevated to 14.8μmol/L, folic acid level was decreased to 2.30 ng/mL. He was treated with folic acid and warfarin for six months. After treatment, plasma homocysteine level was normalized and the evidence of thrombosis has disappeared.

Purpose: To investigate the effect of nocturnal dip, carotid artery blood flow, and brain ischemic change on the progression of glaucomatous visual field defect in open-angle glaucoma (OAG) when IOP is less than the target pressure. Methods: We classified OAG patients (74 patients, 148 eyes) who maintained IOP less than the target pressure as normal tension glaucoma (NTG; 52 patients, 104 eyes) or primary OAG (POAG; 22 patients, 44 eyes). Additionally, we performed 24-hr ambulatory blood pressure monitoring (24-hr ABPM), carotid artery color Doppler U/S (CAD), brain MRI, and visual field (V/F) tests on the patients. Nocturnal dips less than 10% were classified as non-dippers, and dips greater than 10% as dippers. The relationships among nocturnal dip, carotid artery blood flow, brain ischemic change, and progression of glaucomatous V/F defect were examined. Results: In the case of dippers, glaucomatous V/F defects were aggravated, with a relative risk of approximately 1.74 (NTG) and 2.91 (POAG) times that of non-dippers. In NTG, decreased carotid artery blood flow and brain ischemic change furthered glaucomatous V/F defects, with a relative risk of approximately 2.40 and 2.54 times that of normal carotid artery blood flow and brain MRI findings, respectively. However, in POAG, decreased carotid artery blood flow and brain ischemic change were not influenced by the progression of glaucomatous V/F defects. Conclusions: In dippers, decreased carotid artery blood flow and brain ischemic change caused a progression of glaucomatous V/F defects in NTG and POAG patients. Thus, performing 24-hr ABPM, CAD, and brain MRI should be helpful for glaucoma patients with progression of glaucomatous V/F defects even when the IOP is less than the target pressure. In addition, this analysis provides useful information regarding glaucoma diagnosis and treatment.

Purpose: To investigate the effect of nocturnal dip influence on the progression of glaucomatous visual field defect. Methods: We performed 24hr ABPM and V/F tests on patients diagnosed with NTG (140 patients, 280 eyes) and POAG (84 patients, 168 eyes). Nocturnal dips below 10% were classified as non-dippers, and those above 10% were noted as dippers. The correlations among nocturnal dip, progression of glaucomatous visual field defect, and hypertension treatment were examined. Results: In NTG, dippers in both systolic and diastolic blood pressure furthered glaucomatous visual field defects, with a relative risk of approximately three times that of non-dippers. Hypertension treatment was not influenced by the progression of glaucomatous visual field defect but was influenced by dips in the systolic and diastolic pressures in NTG and in diastolic pressure in POAG. Nocturnal dips were more frequent in the group with progression of the visual field compared to those in the group with non-progression of the visual field in NTG. Conclusions: Dipper caused a progression of glaucomatous visual field defects in NTG and was influenced by hypertension treatment in NTG and POAG. Performing 24hr ABPM should be helpful for glaucoma patients with progression of a glaucomatous visual field defect even when the IOP is less than the target pressure. The Clinical Usefulness of $^{18}F$-FDG PET/CT in Patients with Systemic Autoimmune Disease

Purpose Individuals with systemic autoimmune disease have an increased susceptibility to both inflammation and malignancy. The aim of this study was to evaluate the clinical usefulness of $^{18}F$-FDG PET/CT in patients with systemic autoimmune disease. Methods Forty patients diagnosed with systemic autoimmune disease were enrolled. Diagnostic accuracy of FDG PET/CT for detecting malignancy was assessed. FDG PET/CT findings, including maximum standardized uptake (SUVmax) of lymphadenopathy (LAP), liver, bone marrow, spleen, joint and muscles, were considered for the characterization of LAPs. Results FDG PET/CT could detect metabolically activated lesions in 36 out of 40 patients (90%) including inflammatory lesions in 28 out of 32 patients (88%). The sensitivity, specificity, accuracy, positive predictive value, and negative predictive value of FDG PET/CT for the detection of malignancy were 100, 67, 70, 25, and 100%, respectively. Multiple LAPs were found in 25 of 40 patients (63%), and comprised three malignancies, four cases of tuberculosis, and 18 reactive changes. A SUVmax ratio of bone marrow to liver below 0.78 could distinguish malignancy from tuberculosis + reactive change (AUC=1.000, sensitivity: 100%, specificity: 100%). The SUVmax ratio of spleen to liver in the reactive group was also significantly higher than that in the malignancy group (P=0.014). SUVmax of LAP in the TB group was significantly higher than that in the reactive group (P=0.040). Conclusions PET/CT is useful in detecting and differentiating inflammation and malignancy in patients with systemic autoimmune disease. Frequent false-positive interpretations can be minimized by consideration of FDG uptake in bone marrow and spleen.
