      • 반복치료로 호전된 호산구성 폐렴 유사 폐흡충증 1예

        우리나라에서 인기 있는 게장등의 섭취로2) 성인뿐만 아니라 소아에서도 지속적으로 보고되고 있는 폐흡충증은 다양한 임상양상을 보일 수 있다. 저자들은 방사선 소견상 만성 호산구성 폐렴과 유사하였고, 반복적인 치료로 호전된 폐흡충증 1예를 경험하였기에 보고하는 바이다. Paragonimiasis mostly occurs by ingestion of raw or undercooked freshwater crabs or crayfish. Recently, re-emergence of paragonimiasis is reported because of ingestion of raw food such as freshwater fish and Gejang (marinated raw crabs). As the radiological features as well as the laboratory findings are similar to those of chronic eosinophilic pneumonia, the diagnosis of pulmonary paragonimiasis is often difficult and delayed. A 43 year-old female was visited our hospital for migrating and new developing consolidative lung lesions and peripheral blood eosinophilia. The eosinophil profile of leukocyte was 32% (absolute count 2,690 /mm3). Histologic finding was organizing pneumonia pattern with severe infiltration of eosinophils. Parasite specific antibody IgG forParagonimus westermani was positive (titer:0.61, normal range<0.20). After treatment with praziquantel (75mg/Kg once a day for 2days), the chest x-ray was clearly resolved. Follow-up study showed right pleural effusion, newly developed focal infiltration, and several nodules. After retreatment, we couldn't find any recurrent evidence.

        The epoxy resin plate and acryl plate (Both thichness:3mm) overlapped are loaded by Pin-Joint. The stress distribution, stress intensity factors and correction factor at crack tip with various demension of the specimen are as follows; 1. When the pin is loaded, the K₁value is increased remarkable in case that length of the crack is above 6mm. 2. Correction factor(F₁) is greatly dependent on the value of a/d, but not on the value of a/w or a/r. 3. When the pin is loaded, K₁value is highly decreased with decrease in crack length because of the compression load developed between pin and specimen. 4. When the pin is loaded correction factor(F) is remakable incresed as the distance of two holes are decreased.

        Because of the anatomical form of posterior teeth, the amount of tooth reduction at the cuspal area in the typical intracoronal cavity will be inevitably maximal, while stress development on the restoration in this area will be relatively minimal. In an attempt to decrease the amount of tooth reduction at the cuspal area, two modified cavities were newly designed in this study. They had a decreased pupal depth of 1mm at the cuspal area and offsets with the depth of 0.5 ~ 1.0mm were formed at the central fossa area. The Von Mises stresses developed in the gold restoratives and in the teeth were compared with that in the typical intracoronal cavity, which had a simple flat pulpal floor with 1.5mm pulpal depth at the central fossa area. Stress analysis was done with two dimensional finite technique. The obtained results were as follows: 1. The newly modified cavity designs were found to be more advantageous in stress distribution. 2. The effect of offset depth was not significant, but some relationship between the offset depth and stress distribution was found.

      • 체위변화에 의한 SaO_2의 변화

        To investigate the SaO_2 changes according to positional change in general anesthetic patients, 80 ASA physical state class I-II patients were studied. Each group classified supine position group, group to change from supine position into sitting position, group to change from supine position into prone position and group to change supine position into lateral position. After intubation, in the supine position SaO₂was checked and then positional changes performed. After that we checked Sa0_2 in 1, 5, 10, 15, 30, 60 minutes respectively. The results were as follows. 1) In supine position, time-dependent SaO_2 change was not significant statistically(p>0.05). 2) In the change into sitting position, time-dependent SaO_2 change was not significant statistically(p>0.05). 3) In the change into prone position, time-dependent SaO_2 change was not significant statistically(p>0.05). 4) In lateral positional change, time-dependent SaO_2 change was not significant statistically(p>0. 05). 5) Among the groups there were no significant SaO_2 changes according to positional change compared with supine position statistically. In conclusion, this experiments revealed that the change in SaO_2 according to positional change was not significant.

        Fully reversed pull-push low cycle fatigue test under triangle cyclic waveform mode of constant strain control were conducted for SM15C and copper at the room temperature in air. Cyclic strain hardening exponent, n' was obtained from the cyclic δ-ε curve defined as the locus of tips of the stable hysteresis loops. Equivalent cyclic strain hardening exponent, n'_(eq), was obtained from the outer trace of a single stable hysteresis loop. Modified equivalent cyclic strain hardening exponent, n'_(eqm) , was obtained from the outer trace of a single stable hysteresis loop in the modified coordinate system. The relationship between n'_(eq), n'_(eqm), and n' was investigated with the test results. Equivalent cyclic strain hardening exponent n'_(eq) is considerable dependent upon the controlled strain range Δ^(ε) and decrease with increase of Δ^(ε). If Modified equivalent cyclic strain hardening exponent n'_(eqm) is calculated from the stable hysteresis loop at the modified coordinate system translated along the elastic slope, it closely approach to cyclic strain hardening exponent n'.

      • 複合組織鋼의 摩擦 熔接部의 疲勞破壞特性에 관한 硏究

        This study has been carried out to investigate the characteristics of fatigue crack of welded zone and heat affected zone of friction welded dual phase steel. The results obtained from the experiments are summarized as follows. Microstructure of welded zone is changed to ferrite and pearlite from ferrite and martensite by wel-ding heat. Micro-vickers hardness of welded zone is higher than that of base metal zone and heat affec-ted zone. Micro-vickers hardness of heat affected zone is lower than that of welded zone and microstructure of heat affected zone is changed to fine martensite and ferrite by heating cycle of welding. Fatigue limits of heat affected zone is a little higher than that of welded zone and base metal zone. There are no difference of fatigue limit of base metal zone and welded zone. Fatigue strength of welded zone is similar to that of base metal zone. At high △K region(△K>19MPa·??), fatigue crack propagation velocity of heat affected zone is slower than that of welded zone and base metal zone, and the difference of fatigue crack propagation velocity is smaller than that of high △K region. The crack occurs to the notch in heat affected zone first, but the final fracture surface is the surface of welded zone. It is abvious that heat affected zone of friction welded dual phase steel superior to welded zone and base metal zone in fatigue strength and resistance of fatigue crack propagation. The above mentioned facts are contrary to theory that heat affected zone of general metals is weaker than welded zone and base metal zone.

      • SB41鋼 潛弧熔接部의 平滑 및 側面홈에 관한 疲勞크랙 傳播特性 : 大氣雰圍氣下에서의 擧動 at Room Temperature in Air

        Fatigue test was conducted on the smooth and side-grooved specimens fabricated from submerged are welded plate of SB41 steel. Test conditions were constant-loan-amplitude fatigue crack growth test with stress ratio 0.1, loading frequency 5㎐, sinusoidal waveform at room temperature in air. The results are summarized as follows: 1) Fatigue crack growth rate of weld metal is slightly lower than that of base metal. 2) Due to the effect of plastic constraint, side-grooved specimen shows somewhat faster fatigue crack growth rate than smooth specimen. 3) Welding residual stress affects fatigue crack growth rate.

      • 수술중 수혈환자의 임상적 고찰 (Ⅱ)

        Over the past several decades, blood and blood components have been increasingly recognized as medication rather than innocuous fluids used to replace modest blood lose at operation. But blood transfusions have the potential for many harmful side effects. Therefore, modern hemotherapy practice focuses on the benefits and risks, along with profer dosage, items with which blood component transfusion is concerned, when blgod transfusions are indicated. only that fraction of the blood that the patient requires should be administered. Most blood donations are separated into components after collection : red blood cells, platelet concentrates, and/or cryoprecipitates and plasma. No longer is "fresh whole blood" an appropriate response to a hemotherapeutic problem. This study was undertaken to evaluate transfusion according to surgical department, sex, preoperative hemoglobin values, blood type, transfusion amounts and blood component therapy. 3847 transfused patients among the total 21371 surgical cases done at the Chung-Nam National University Hospital from 1985 to 1989 were reviewed. The results are summerized as follow : 1. The numbers of transfused patients among the 21371 operative cases were 3847 cases (18%). 2. The number of male transfused was greater than that of female and third decade patient was the greatest incidence among the transfused patients. 3. Mostly hemoglobin value in preoperative state was maintained more than 10g/dl. 4. The blood types of transfused blood were in the great order of A. B, O, AB type. 5. The percentage of one pint transfusion was gradualy decreased and 2-5 pint transfusion was greatest percentage namely 57.94% 6. As the amount of blood transfused was increased, the percentage of blood compoent therapy was increased, and mixed case of PRC and FFP was the greatest incidence. 7. The greatest percentage of blood component therapy was in the cases of the orthopedic surgical operation.

        치과기공사 국가시험 과목 타당성 연구

        This study prepared a basic framework for the development and improvement of Korean Dental Technicians' Licensing Examination, based on actual test questions. A peer review was conducted to ensure relevance to current practices in dental technology. For the statistical analysis, 1000 dental laboratory technicians were selected; specialists in dental laboratory technology (laboratory owners, educators, etc.) were involved in creating valid and reliable questions. Results indicated that examination subjects should be divided into three categories: basic dental laboratory theory, dental laboratory specialties, and a practical examination. To ensure relevance to current practice, there should be less emphasis on basic dental laboratory theory, including health-related laws, and more emphasis on dental laboratory specialties. Introduction to dental anatomy should be separated from oral anatomy and tooth morphology; and fixed prosthodontics should be separated from crown and bridge technology and dental ceramics technology. Removable orthodontic appliance technology should be renamed 'orthodontic laboratory technology'. There should be less questions related to health related law, oral anatomy, dental hygiene, dental materials science and inlay, while the distribution ratio of questions related to tooth morphology should be maintained. There should be a decrease in the distribution ratio of questions related to crown and bridge technology, dental ceramics technology, complete dentures and removable partial dentures technology, and orthodontic laboratory technology. In the practical examination, the current multiple choice test should be replaced with tooth carving using wax or plaster. In dental laboratory specialties, subjects related to contemporary dental laboratory technology should be included in the test items.
