        Utilization of Marble Dust, Fly Ash and Waste Sand (Silt-Quartz) in Road Subbase Filling Materials

        Large volumes of earthen materials are used in construction each year in Turkey and elsewhere. The wastes generated from these materials can be utilised in various applications including road subbase filling materials. In this study, three different types of wastes namely fly ash, marble dust and waste sand are used. These wastes were mixed with natural soils as a potential alternative filling materials in the road subbase. Two types of natural soils were replaced with 0%, 5%, 10%, 15%, 20% of fly ash, marble dust and waste sand. Standard compaction, permeability and saturated California Bearing Ratio (CBR) tests, X-Ray Diffraction (XRD) and Scanning Electron Microscopy (SEM) analysis were performed on two types of natural soils, containing three industrial waste types in different ratios. The study indicates that the fly ash, marble dust and waste sand are fairly good additive materials in road subbase fill and reaction substantially improves their CBR, swelling ratio and water conductivity. It was found that a clear optimum replacement level of 15% for all of these by products for medium and low plasticity type of soils.

        Monte Carlo analysis of LWR spent fuel transmutation in a fusion-fi ssion hybrid reactor system

        The aim of this paper is to determine neutronic performances of the light water reactor (LWR) spent fuelmixed with fertile thorium fuel in a FFHR. Time dependent three dimensional calculations for majortechnical data, such as blanket energy multiplication, tritium breeding ratio, cumulative fissile fuelenrichment and burnup have been performed by using Monte Carlo Neutron-Particle Transport codeMCNP5 1.4, coupled with a novel interface code MCNPAS, which is developed by our research group. Aself-sustaining tritium breeding ratio (TBR>1.05) has been kept throughout the calculations. The studyhas shown that the fissile fuel quality will be improved in the course of the transmutation of the LWRspent in the FFHR. The latter has gained the reusable fuel enrichment level conventional LWRs betweenone and two years. Furthermore, LWR spent fuel - thorium mixture provides higher burn-up values thanin light water reactors.

        Effect of Type 2 Diabetes Mellitus on Prognosis of Nonmetastatic Renal Cell Cancer

        Purpose: We evaluated the prognostic value of type 2 diabetes mellitus (DM) in patients treated surgically for localized renal cell carcinoma (RCC). Materials and Methods: Between 1995 and 2011, 588 patients with renal tumor diagnoses were treated surgically and 492 patients with pathologically confirmed nonmetastatic RCC diagnoses were included in the study. The associations of clinical and pathologic parameters with a type 2 DM diagnosis were evaluated. Kaplan-Meier estimations for disease-specific survival (DSS) and overall survival (OS) were generated according to type 2 DM diagnosis, and the log-rank test was used to compare survival according to the variables. Results: The mean age of the patients was 56.7±12 years (range, 15 to 84 years; median, 58 years) and the mean length of follow-up was 35.9±28 months (range, 1 to 145 months; median, 34.3 months). Of the 492 patients, 62 (12.6%) had a diagnosis of DM at the time of surgery (group I) and 430 did not have DM (group II). The mean age and the incidence of clear cell RCC histological subtype were significantly higher in group I than in group II (p<0.001 and p=0.036, respectively). Although DSS and OS were lower in group I, this difference was not significant. Type 2 DM was not detected as an independent prognostic factor for DSS and OS. Conclusions: This study investigated the role and effect of DM on the prognosis of localized RCC that was treated surgically. The present study did not detect DM as an independent prognostic factor for RCC.

        Hepatic and splenic sonographic and sonoelastographic findings in pulmonary arterial hypertension

        Purpose: The aim of this study was to evaluate the associations of sonographic and sonoelastographic parameters with clinical cardiac parameters, as well as to assess their value in predicting survival in patients with pulmonary arterial hypertension (PAH). Methods: Thirty-six patients with PAH and normal liver function were prospectively enrolled in this prospective study along with 26 healthy controls, all of whom underwent ultrasound and point shear wave elastography examinations. Additionally, the portal vein pulsatility index (PVPI), inferior vena cava collapsibility index, and clinical cardiac variables were obtained in PAH patients. The values of hepatic (LVs) and splenic shear wave velocity (SVs) were compared between PAH patients and controls. The relationships between all sonographic and clinical parameters in the PAH patients were analyzed. Furthermore, their prognostic value in predicting survival was investigated. Results: LVs values in PAH patients (median, 1.62 m/s) were significantly higher than in controls (median, 0.99 m/s), while no significant difference was observed in SVs values. Patients with higher grades of tricuspid regurgitation (TR) had significantly different values of PVPI (P=0.010) and sonoelastographic parameters (P<0.001 for LVs and P=0.004 for SVs) compared to those with less severe TR. Tricuspid annular plane systolic excursion values were the only investigated parameter found to be associated with survival (hazard ratio, 0.814; 95% confidence interval, 0.694 to 0.954; P=0.011). Conclusion: Our results demonstrated a direct association between cardiac congestion (i.e., the severity of TR) and liver stiffness, which should be kept in mind during the assessment of fibrosis in patients with PAH.

        The Diagnostic Value of Malondialdehyde, Superoxide Dismutase and Catalase Activity in Drug Naïve, First Episode, Non-Smoker Generalized Anxiety Disorder Patients

        Objective: Generalized anxiety disorder (GAD) is a common anxiety disorder. Although lots of research done to reveal neurobiological basis of GAD, it is still unclear. Diagnosis of GAD depends on subjective complaints of patients, thus the need for a biological marker is constantly emerging. In this study, we aimed to investigate diagnostic value of malondialdehyde (MDA), superoxide dismutase (SOD) and catalase (CAT) in GAD. Methods: We evaluated MDA, SOD, and CAT levels in peripheral blood of 46 patients and 45 controls. MDA was measured with Ohkawa’s methods, SOD was measured with Fridovich method, and CAT was measured with Beutler’s method. Results: MDA was significantly increased in patients than controls, medians 4.05 nmol/mg and 1.71 nmol/mg respectively, p<0.001; SOD and CAT activity was significantly decreased in patients than controls, medians of SOD were 159.07 U/mg and 301.87 U/mg, p<0.001 respectively, medians for CAT were 138.47 U/mg and 160.60 U/mg respectively. We found high correlation between Hamilton Anxiety Rating Scale and SOD, MDA r values were 0.723 and 0.715 respectively, p<0.001 for both. Receiver operator characteristic (ROC) curve analysis showed high diagnostic performance for MDA and SOD, low diagnostic performance for CAT, areas under curve were 1.0, 1.0, and 0.648 respectively. Conclusion: Our results reveal possible diagnostic value of MDA, less likely of SOD but not CAT. Future studies should investigate diagnostic value of oxidants and antioxidantn enzymes in larger samples and include diagnostic value of these parameters.

        Functional Outcomes and Long-term Durability of Artificial Urinary Sphincter Application: Review of 56 Patients With Long-term Follow-up

        Purpose: To evaluate the long-term outcomes of artificial urinary sphincter (AUS) implantation and to report the complication rates, including mechanical failure, erosion,and infection. Materials and Methods: From June 1990 to May 2011, AUS (AMS 800) implantations were performed in 56 adult males by one surgeon. Various demographic and preoperative variables, surgical variables, and postoperative outcomes, including success and complication rates with a median follow-up of 96 months, were recorded retrospectively. Results: The mean age of the patients at the time of AUS implantation was 61.8 (±14.2)years. During the follow-up period, the total complication rate was 41.1% (23 patients). The incidence of complications was significantly lower during the follow-up period after 48 months (p<0.05). Kaplan-Meier analysis revealed that 5- and 10-year failure-free rates were 50.3% and 45.2%, respectively. Conclusions: Long-term durability and functional outcomes are achievable for the AMS 800, but there are appreciable complication rates for erosion, mechanical failure, and infection of up to 30%.

        Predictive factors for flexible ureterorenoscopy requirement after rigid ureterorenoscopy in cases with renal pelvic stones sized 1 to 2 cm

        Purpose: To evaluate the outcomes of rigid ureterorenoscopy (URS) for renal pelvic stones (RPS) sized 1 to 2 cm and to determinethe predictive factors for the requirement for flexible URS (F-URS) when rigid URS fails. Materials and Methods: A total of 88 patients were included into the study. In 48 patients, the RPS were totally fragmented withrigid URS and F-URS was not required (group 1). In 40 patients, rigid URS was not able to access the renal pelvis or fragmentationof the stones was not completed owing to stone position or displacement and F-URS was utilized for retrograde intrarenal surgery(RIRS) (group 2). The predictive factors for F-URS requirement during RIRS for RPS were evaluated. Both groups were compared regardingage, height, sex, body mass index, stone size, stone opacity, hydronephrosis, and previous treatments. Results: The mean patient age was 48.6±16.5 years and the mean follow-period was 39±11.5 weeks. The overall stone-free rate inthe study population was 85% (75 patients). In groups 1 and 2, the overall stone-free rates were 83% (40 patients) and 87% (35 patients),respectively (p>0.05). The independent predictors of requirement for F-URS during RIRS were male gender, patient height,and higher degree of hydronephrosis. Conclusions: Rigid URS can be utilized in selected patients for the fragmentation of RPS sized 1 to 2 cm with outcomes similar tothat of F-URS. In case of failure of rigid URS, F-URS can be performed successfully in this group of patients.

The present manuscript is about an unusual case of a 16-year-old CAHpatient due to 11β-hydroxylase deficiency. He underwent testicle biopsy because of testicletumor suspicion and diagnosed with TART.

        Comparison of the analgesic efficacy of the ultrasound-guided transversalis fascia plane block and erector spinae plane block in patients undergoing open inguinal hernia repair under spinal anesthesia

        Background: Open inguinal hernia repair (OIHR) surgery is a common surgical procedure, and ultrasound guided interfascial plane blocks can also be included in current approaches to postoperative multimodal analgesia regimens. This study aimed to compare the postoperative analgesic efficacy of the erector spinae plane block (ESPB) and transversalis fascia plane block (TFPB) in patients undergoing OIHR.Methods: This prospective, randomized, assessor-blinded comparative study was conducted in the postoperative recovery room and ward of a tertiary hospital. A total of 80 patients with American Society of Anesthesiologists physical status I–III were enrolled and allocated equally to either the ESPB or TFPB group. The patients received standard multimodal analgesia in addition to an ultrasound-guided ESPB or TFPB. During the first 24 h postoperatively, tramadol consumption was assessed and pain levels at rest and during movement were compared using numeric rating scale (NRS) scores at 1, 3, 6, 9, 12, 18, and 24 h postoperatively.Results: The results showed no difference in NRS scores at any time point between the groups, except for NRS at rest in the third hour. However, tramadol consumption was lower in the TFPB group than in the ESPB group overall (88 ± 75.2 vs. 131 ± 93.7 mg, respectively; P = 0.027, mean difference: −43, 95% CI [−80.82, −5.18]).Conclusions: The TFPB leads to lower tramadol requirements in the first 24 h postoperatively than the ESPB in patients undergoing OIHR.
