      • Initial Experience with Intragastric Balloon Lexbal (Domestic Industry) in the Treatment of Patients with Mild to Moderate Obesity (type I-Ii)

        Background: Objective: To evaluate the effectiveness and response Gastric balloon ( Lexbal ) in the treatment of mild to moderate obesity Observational and retrospective : Experimental design Methods: Intervention: We conducted in two clinics (Our Lady of Pilar and Clinica B & S ) an observational, retrospective study. We have compiled the results of 10 follow intragastric balloons (Balon Lexbal ) in obese patients with mild to moderate type I- II (BMI between 28 and 34.9 kg/m2 ) placed in 2012 and 2013 losses have been achieved. over 70 % of excess weight. Furthermore, it has been observed satisfaction of our patients Measurements: Descriptive observational study in which the sample is made up of the 10 patients treated with balloon LEXBAL in our midst. The variables studied were age, sex, weight, BMI, percentage of weight lost, fi ll volume, tolerance, satisfaction and dietary monitoring by patients Results: Over 80 % degree of patient satisfaction, 70 % decrease in weight above the average ( over 12 kilos ) better response in those presenting adherence to nutritional treatment and no differences were observed in the volume of fi lling the balloon. Conclusions: Treatment with intragastric balloon, along with a nutritional monitoring allows us to re-educate the patient, and change their eating habits. · Just for gradual diet, and to adapt each phase as tolerated by the patient, helps us to improve dietary behavior and facilitates greater weight loss The intragastric balloon is a safe, well tolerated, with few adverse effects and relatively simple in the hands accustomed to endoscopic practice. We believe it can be considered an effective adjunctive therapy in selected cases of mild / moderate obesity (type I-II).         Neurologic swallowing disorders are more frequently in rehabilitation medicine most other medical specialties. Stroke is the leading cause of neurologic dysphagia. Objective: To evaluate effectiveness of the neuroelectroestimulation (NMES) with use of VocaSTIM in the correction of swallowing disorders. Ml & ms From March 2011 until May of 2013 were 40 treatments, only 28 completed all of the treatment 28 Patients were included (18 men, 10 women) average age between 32 to 78 years. neurological sequelae other causes were esophageal bolus. We average between 8-10 sessions of 20 minutes average 2 times a week, was conducted all (VFSS): initial and control as well as a home electrodiagnosis (ED) to objectively assess degree of denervation in some cases esophageal manometry and (FEES), Methods: To establish comparisons all held you a (VFSS) initial and control as well as an electro-Diagnostics of home and its completion to objectively assess the degree of denervation and x-ray of thorax and VEDA and nasal fi beroptic endoscopic evaluation of swallowing (FEES), in the majority of cases esophageal manometry and in 2 cases Impedanciometry in 6 cases pH 24/metry. Results: Response was assessed according to degree of clinical and scale as well as the corresponding (ED)dysphagia.. Patient satisfaction /Values of ED (fi nals) /Score of speech /grade of dysphagia evaluation It was noted a degree of positive response (degree of satisfaction) in more of the 80 % patients as well as a superior response to the 90 % (ED), speech evaluation is considered good to very good in more of the 80 % of the patients. High degree of response in diseases with neurological lesions (was observed in diseases neurodegenerative (ALS, Parkinson,) the response was considered poor (regular/good) Conclusions: VocaSTIM® can produce a different degree of satisfaction functional response in the majority of patients with dysphagia.

        Background: The intragastric balloon can be used for weight reduction for mild to moderate obesity, weight loss or as a pre- or preoperative choice for morbid obesity -super- obesity. The intragastric balloon is a temporary treatment for obese patients. Methods: Prospective, double-blind Obesity in 30 patients with type I and type 2 obesity: 23 women and 7 men, 43.9 ± 10, 120.3 ± 17 kg, and body mass index (BMI) of 44, 2 ± 5 kg/m2, 10 placing gastric balloons (Lexbal®). 20 -BIB intragastric balloons Allergan ®). fi lled with 700 ml of saline. Both balls were placed with conscious sedation and general anesthesia withdrawn at 6 months. Intravenous and oral medications are given to control symptoms for 48-72 h. Complications, symptoms, weight and quality of life evaluated by Life Index (GIQLI) every 30/45 days were recorded. Results: At 6 months, the balloons were removed endoscopically, with a mean weight loss (12.8 ± 8 vs 14.1 ± 8 kg), BMI loss (4.6 ± 3 vs 5.5 ± 3 kg/m2) showed no signifi - cant differences between groups. The withdrawal of the balloons 1 balloon Lexbal not found in the stomach, and three patients required removal of the BIB balloon endoscopy. Tolerance was good in both groups Conclusions: Both intragastric balloons achieved signifi cant weight loss with good tolerance in obese patients and no serious complications were observed correlated with any of the balloons.

        Genetic discontinuity of Digenea (Rhodomelaceae, Rhodophyta) from Mexico supports recognition of two new species, D. mexicana and D. rafaelii

        Genetic continuity of Digenea simplex outside the Atlantic waters remains less studied, despite its long exposure to oriental folk medicine and wide distribution in tropical waters. Mitochondrial COI-5P and plastid rbcL sequences and morphology were investigated for plants from Mexico and additional specimens from Japan. Two new species, Digenea mexicana and Digenea rafaelii, are described for plants that would previously have been recognized as D. simplex in Mexico. D. mexicana grows to 10.5 cm tall and has cylindrical axes, irregular or di-, trichotomous branches, simple to branched determinate branchlets with eight pericentral cells, and tetrasporangia on inflated upper parts of determinate branchlets. It occurs intertidally in Quintana Roo, Yucatan Peninsula. D. rafaelii grows to 5 cm tall and has cylindrical axes, irregular or di-, trichotomous branches, and simple determinate branchlets with ten pericentral cells. It occurs intertidally in the Gulf of California. Both COI-5P and rbcL sequences revealed the genetic discontinuity between D. mexicana and D. rafaelii.

        Effect of mechanically deboned poultry meat content on technological properties and sensory characteristics of lamb and mutton sausages

        Objective: This study aimed to develop a value-added product concerning technological and sensory characteristics changes of the use of mechanically deboned poultry meat (MDPM) as meat replacer in lamb and mutton emulsion-type sausages (mortadella). Methods: Sausages were produced with lamb and mutton and with different contents of MDPM. Six treatments, using lamb or mutton and 0%, 30%, and 60% of MDPM in relation to the meat batter, were produced and analyzed for pH, proximal composition, calcium and residual nitrite content, water activity, 2-thiobarbituric acid reactive substances (TBARS), instrumental color and texture profile. The sensory profile of the mortadella's was also evaluated by acceptance test and check-all-that-applies (CATA) analysis. Results: The MDPM addition increased (p<0.05) fat, residual nitrite and calcium content in the all sausage formulations, but mutton sausage had (p<0.05) higher fat and lower moisture content than lamb sausage. The pH, water activity, TBARS index and color was not affected by MDPM additions, while the mutton sausages were significantly redder (higher $a^*$, $C^*$, and lower $h^{\circ}$) and darker (lower $L^*$) than lamb sausages. Adding up to 60% of MDPM reduced (p<0.05) sausages hardness and chewiness. Overall, the meat replacement by MDPM increased the sausages acceptance, but the mutton sausage with 30% of MDPM replacer were the most preferred. Consumers related that pink color, glossy appearance, poultry meat-like taste, soft texture, juicy and greasy mouth feel to all sausages contain MDPM according to CATA analysis. Conclusion: Mutton from culled ewes can be utilized for mortadella production with 30% replacement of lean mutton and fat by MDPM.

        Modified Orbitozygomatic Approach without Orbital Roof Removal for Middle Fossa Lesions

        Objective : The purpose of the present study was to describe an OrBitoZygomatic (OBZ) surgical variant that implies the drilling of the orbital roof and lateral wall of the orbit without orbitotomy. Methods : Design : cross-sectional study. Between January 2010 and December 2014, 18 patients with middle fossa lesions underwent the previously mentioned OBZ surgical variant. Gender, age, histopathological diagnosis, complications, and percentage of resection were registered. The detailed surgical technique is described. Results : Of the 18 cases listed in the study, nine were males and nine females. Seventeen cases (94.5%) were diagnosed as primary tumoral lesions, one case (5.5%) presented with metastasis of a carcinoma, and an additional one had a fibrous dysplasia. Age ranged between 27 and 73 years. Early complications were developed in four cases, but all of these were completely resolved. None developed enophthalmos. Conclusion : The present study illustrates a novel surgical OBZ approach that allows for the performance of a simpler and faster procedure with fewer complications, and without increasing surgical time or cerebral manipulation, for reaching lesions of the middle fossa. Thorough knowledge of the anatomy and surgical technique is essential for successful completion of the procedure.

      • Salient and Relevant Colors in Digital Images of Paintings

        The painters in their works not only try to faithfully shape a scene, but to submit this representation to the painter`s mind, according to his training and aesthetic taste. We find of great interest to characterize what we can understand as relevant information in a painting. Techniques for determining "salient" information in an image have recently been developed. It is a question of determining in an image the objects or areas in which an observer focus its attention. With these techniques we can establish which the fundamental colors of a painting are. We have applied the Graph-Based Visual Saliency algorithm (GBVS) to a set of digital images of six painters (Titian, El Greco, Rubens, Velázquez, Rembrandt and Vermeer). We have obtained the number of salient colors in each image after applying a method for counting discernible colors. We have taken a step further by applying thresholds at different levels to determine which the really relevant colors in a scene are, so that we approach the subjective judgments that would give an observer of the colors that stand out in a certain painting. When we evaluate the percentage of reduction of the number of discernible colors in the salient areas, this parameter is less for Rembrandt, Velázquez and Vermeer than for Titian, El Greco and Rubens. This can show a difference in the use of colors with the period that is with the evolution from the renaissance to baroque style. However, when we have evaluated the relevant colors, there is a slight variation in the average values obtained, 15 to 20. We could conclude that the number of relevant colors in the paintings do not change so significantly as the total number of discernible colors according to authors and according to paintings.

        Gigantomastia as a Cause of Pulmonary Hypertension

        Reduction mammaplasty is the gold standard treatment for gigantomastia. We report one female patient with juvenile gigantomastia associated with severe pulmonary hypertension where her pulmonary pressure decreased significantly after the surgery, improving her quality of life. A 22-year-old female patient with gigantomastia since 10 years old, tricuspid regurgitation, and pulmonary thromboembolism antecedent was admitted to the emergency department. Her oxygen saturation was 89%. Acute heart failure management was initiated. An echocardiogram reported left ventricle ejection fraction (LVEF) of 70% with severe right heart dilation, contractile dysfunction, and arterial pulmonary pressure (PASP) of 110mm Hg. A multidisciplinary team considered gigantomastia could generate a restrictive pattern, so a Thorek reduction mammoplasty with Wise pattern was performed. Presurgical measurements were: sternal notch to nipple-areola complex, right 59 cm, left 56 cm. Three days after surgery, the patient could breathe without oxygen support. In the outpatient followup, patient referred reduction of her respiratory symptoms and marked improvement in her quality of life. Sixmonths after surgery, a control echocardiogram showed a LVEF of 62% and PASP of 85mm Hg. Pulmonary hypertension may be present in patients with gigantomastia. Reduction mammoplasty may be a feasible alternative to improve the cardiac signs and symptoms in patients with medical refractory management.
