        Politeness Strategies in Healthcare Communication at" Difficult Times": A Pragmatic Analysis of the Manga Discourse in Nnrse Aoi

        This paper examines the ways in which healthcare professionals interact with patients`` family members, and/or colleagues. The data are from healthcare discourses at difficult times found in the manga series entitled Nurse AOI. As the first step, we selected several communication scenes for analysis in terms of politeness strategies. From these scenes, we extracted individual events that exhibit anyone of five main constraints/ maxims that follow Leech``s (2009) Grand Strategy of Politeness (GSP) framework- generosity/tact, approbation/modesty, obligation, opinion, and feeling. Then in-depth analysis was conducted using the following three perspectives: the contextual background of the communication, the relationship with patients`` family member and/or colleagues, and the degree of face-threatening acts. This last perspective was evaluated by Politeness Theory (Brown & Levinson, 1987), judging the imposition of face-threatening acts, social distance, and relative power. Each communication event was examined in terms of politeness strategies (Brown and Levinson, 1987; Yoshioka, 2007). At difficult times optimal politeness strategies are required while simultaneously providing urgent medical care for patients or victims. The findings of this study indicate both potential problems with healthcare communication at difficult times such as disasters and also suggest the ways in which the quality of communication may be enhanced.

        Electrohydraulic Lithotripsy for Difficult Bile Duct Stones under Endoscopic Retrograde Cholangiopancreatography and Peroral Transluminal Cholangioscopy Guidance

        Background/Aims: Electrohydraulic lithotripsy (EHL) under endoscopic retrograde cholangiopancreatography (ERCP) guidance can be an option to treat difficult stones. Recently, a digital, single-operator cholangioscope (SPY-DS) has become available. Peroral transluminal cholangioscopy (PTLC) using SPY-DS has also been reported. In this retrospective study, the technical feasibility and clinical effectiveness of EHL for difficult bile duct stones under ERCP guidance and under PTLC guidance was examined. Methods: In this pilot study, patients with difficult bile duct stones between July 2016 and July 2017 were retrospectively enrolled. Results: Forty-two consecutive patients underwent EHL using a SPY-DS; 34 patients underwent EHL under ERCP guidance, and the other 8 patients underwent EHL under PTLC guidance. Median procedure time was 31 minutes (range, 19 to 66 minutes). The median number of EHL sessions was 1 (range, 1 to 2), and that of ERCP sessions was also 1 (range, 1 to 3). The rate of complete stone clearance was 98% (41/42). Adverse events such as cholangitis and acute pancreatitis were seen in 14% (6/42), which could be treated conservatively. Conclusions: EHL using SPY-DS was technically feasible, not only under ERCP guidance, but also PTLC guidance. A prospective clinical study of EHL using SPY-DS is needed. (Gut Liver 2018;12:457-462)

        Communication Apprehension among Japanese College Students

        The main purposes of this study are: (a) to identify the constructs that may generate communication apprehension and to investigate how they influence communication apprehension and reduce the level of willingness to communicate; and (b) to explore the basic mentality underlying these constructs. The data were obtained from semi-structured interviews with 10 Japanese female college students and one Vietnamese male student who provided a contrast for the data from the Japanese students. The study suggests that constructs such as perfectionism, competitiveness, ``good-student`` mentality and face-protecting orientation may generate communication apprehension and the basic mentality underlying them may be other-directedness.

      • Japanese University Students` Willingness to Communicate in English: The Serendipitous Effect of Oral Presentations

        This study examines the ways in which college students in Japan overcame sensitivity to external evaluation and increased their willingness to communicate in English. It is not uncommon for university students in Japan, who are otherwise proficient speakers of English and motivated to learn, fail to exhibit English competency in real communication situations. In interviews with students and teachers we discovered that sociocultural factors impede spontaneous communication, resulting in a reduced level of willingness to communicate in the students` L2, English. The social norms of the language classroom at a Japanese university create a milieu where individual students are sensitized to the social appropriateness of their communication acts. Since these norms are related to both the speech community of college students as well as larger societal expectations, including the concept of ‘seken’ (public eye), we suggest that language educators need to consider carefully the social context of the Japanese student language community when facilitating individuals` development of English language competence. Serendipitously we discovered that communication apprehension decreased when socially appropriate acts of communication were encouraged. The participants for this study read books (graded readers) of their own selection individually as a part of reading classes and gave in-class presentations, an idea self-initiated by the students themselves. Without any explicit attempt at boosting their levels of willingness to communicate, the participants of this study reported that they gained higher self-perceived communicative competence and felt more comfortable in speaking.

        Discourse Analysis of Encouragement in Healthcare Manga

        This article examines how healthcare professionals use encouragement. Focusing on GAMBARU [``to try hard``], forty-one scenes were collected from healthcare manga. Each scene of encouragement was analyzed from three perspectives; the contextual background of the communication, the relationship with the patients and the patients`` response to the encouragement, and the evaluation of the imposition of face-threatening acts, social distance, and relative power (Politeness Theory). Further analysis was conducted using Leech``s (2009) Grand Strategy of Politeness, and each chosen communication event was categorized among ten types of speech acts. The findings suggest some implications to enhance the quality of healthcare communication that offers patients encouragement.

        Motivation Constructs in Learning English Among Japanese College Students

        In this study, a questionnaire was administered to a sample of 226 college students to investigate the motivational constructs of learning English in the Japanese college contexts. The exploratory principal components analysis was performed several times with a varimax rotation in order to extract the factors to probe the internal structure of motivation. The results showed eight factors; integrativeness, anxiety, instrumentality, self-confidence, attitudes, motivational intensity (effort), intrinsic motivation, and personal trait with reasonable values of reliability except for personal trait which has only one item.

        多文化共生に向けた対話型オンライン地域日本語活動の試み

        近年、多様なエスニシティの増加に伴い多文化共生に向けた取り組みが喫緊の課題とされている。こうした背景を受け、人びとの生活に密接に関わる地域日本語教育は今後益々重要になると考えられる。そこで筆者は愛知県にある犬山国際交流協会と協力し、参加者が対話を通して互いの異なりや共通性に気付き、共に暮らしやすい社会を創るためのヒントを見出す「対話型オンライン地域日本語活動(以下:多文化おしゃべりクラブ)」を試みた。多文化おしゃべりクラブで重視したのは、言語学習ではなく、誰もが社会的存在として参加し、その人らしく発言できること、つまり共生のためのウェルフェアの概念に基づいた対話の場づくりである。そのための工夫として本実践では教育のユニバーサルデザイン(以下、UD)の環境調整を行った。本稿では、参加者の日本語非母語話者と日本語母語話者各1名の語りから、多文化おしゃべりクラブにおける“参加者間の関係性”“参加の意義”“参加を通して得たもの”を明らかにし、共生のためのウェルフェアの概念に基づいた対話の場づくりにはどのような工夫が必要なのか考察することを目的とする。SCAT (Steps for Coding and Theorization)を用いて分析した結果、双方向の意見交換ができ互いに尊重される環境において対等性が育みやすいとわかった。また他参加者からの気付きや学びが個人の情意的·行動的変容を生じさせる可能性も窺えたが、そのためには多様な参加者の存在が欠かせず、教育のUDにおける「快適さ」に関わる工夫や配慮がとりわけ重要であることが示唆された。 In recent years, efforts toward multicultural conviviality have become an urgent issue as the number of diverse ethnic groups has increased. Against this background, local Japanese language education, which is closely related to people's lives, is expected to become increasingly important in the future. Therefore, in cooperation with the Inuyama International Association in Aichi Prefecture, the author attempted an “interactive online Japanese language activity” (hereafter referred to as “TABUNKA-OSHABERI CLUB”), in which participants could notice differences and similarities among themselves through dialogue and find hints for creating a society in which it is easy to live together. The emphasis of the Multicultural Chat Club was not language learning, but the creation of a place for dialogue based on the concept of “convivial welfare,” where everyone can participate as a social being and speak up in his/her own way. As a device to achieve this, this practice adjusted the environment for universal design of education (hereinafter referred to as “UD”). In this paper, we use the narratives of one non-native Japanese speaker and one native Japanese speaker to clarify the “relationship among participants,” “significance of participation,” and “what they gained through participation” in the multicultural chatting club. The results were analyzed using SCAT (Steps for Coding and Theorization), The results of the SCAT analysis showed that equality is easily fostered in a mutually respectful environment where there is an interactive exchange of ideas and opinions. The study also showed that the awareness of and learning from other participants can lead to emotional and behavioral changes in individuals, and that the presence of diverse participants is essential for this, suggesting that it is particularly important to consider the ‘comfort’ of UD in education.
