        A Guidewire May Save the Day

        Endoscopic gallbladder drainage (EGBD) has been used to treat acute cholecystitis or to relieve malignant biliary obstruction as an alternative to percutaneous gallbladder drainage and patient's are poor surgical candidates. This is currently being performed by placement of lumen apposing metallic stent (LAMS) with electrocautery mounted tip delivery system also called as "hot" technique. We had reported a case of self-expanding metallic stent (SEMS) within LAMS after stent migration during EGBD using "hot" technique and propose routine use of guidewire in patients undergoing the procedure.         Liberalized Exchange Rate Management System and Devaluation in India: Trade Balance Effect

        This paper examines the short run trade balance effects of the recent exchange rate policies in India in terms of the extended Jones-Corden [1976] model. With rigidity of money wage rate as the target of concurrent fiscal policy, a change in the LERMS formula improves trade balance only if non-trade-ables are relatively labour-intensive. A devaluation might still fail to improve trade balance, on the other hand, in presence of imported input. The Jones-Corden condition thus gets modified in presence of imported input.

        Underwater Endoscopic Mucosal Resection for 10 mm or Larger Nonpedunculated Colorectal Polyps: A Systematic Review and Meta-Analysis

        Background/Aims: Recent studies have reported the favorable outcomes of underwater endoscopic mucosal resection (UEMR)for colorectal polyps. We performed a systematic review and meta-analysis evaluating the efficacy and safety of UEMR fornonpedunculated polyps ≥10 mm. Methods: We performed a comprehensive search of multiple databases (through May 2020) to identify studies reporting theoutcomes of UEMR for ≥10 mm nonpedunculated colorectal polyps. The assessed outcomes were recurrence rate on the first followup,en bloc resection, incomplete resection, and adverse events after UEMR. Results: A total of 1276 polyps from 16 articles were included in our study. The recurrence rate was 7.3% (95% confidence interval[CI], 4.3–12) and 5.9% (95% CI, 3.6–9.4) for nonpedunculated polyps ≥10 and ≥20 mm, respectively. For nonpedunculated polyps≥10 mm, the en bloc resection, R0 resection, and incomplete resection rates were 57.7% (95% CI, 42.4–71.6), 58.9% (95% CI, 42.4–73.6), and 1.5% (95% CI, 0.8–2.6), respectively. The rates of pooled adverse events, intraprocedural bleeding, and delayed bleedingwere 7.0%, 5.4%, and 2.9%, respectively. The rate of perforation and postpolypectomy syndrome was 0.8%. Conclusions: Our systematic review and meta-analysis demonstrates that UEMR for nonpedunculated colorectal polyps ≥10 mm issafe and effective with a low rate of recurrence.

        Synthesis and photoluminescence properties of silver nanowires

        Silver nanowires of 50–190 nm in diameters along with silver nanoparticles in the size range of 60–200 nm in prismatic and hexagonal shapes are synthesized through chemical process. The lengths of the silver nanowires lie between 40 and 1000 ㎛. The characterizations of the synthesized samples are done by X-ray diffraction (XRD), scanning electron microscopy (SEM) and UV–visible absorption spectroscopy. The syntheses have been done by using two processes. In the first process, relatively thicker and longer silver nanowires are synthesized by a soft template liquid phase method at a reaction temperature of 70 ℃ with methanol as solvent. In the second process, thinner silver nanowires along with silver nanoparticles are prepared through a polymer mediated polyol process at a reaction temperature of 210 ℃with ethylene glycol as solvent. The variations of photoluminescence (PL) emission from the silver nanocluster dispersed in methanol as well as in ethylene glycol are recorded at room temperature under excitation wavelengths lying in between 300 and 414 nm. The blue–green PL emission is observed from the prepared samples and these emissions are assigned to radiative recombination of Fermi level electrons and sp- or d-band holes.

        Effcacy and Safety of Lumen-Apposing Stents for Management of Pancreatic Fluid Collections in a Community Hospital Setting

        Background/Aims: Endoscopic ultrasound-guided transmural drainage and necrosectomy employing lumen-apposing metal stent(LAMS) are used for treating pancreatic fluid collections (PFCs) with excellent results from academic centers. Herein, we report theeffcacy and safety of LAMS in the treatment of PFCs at a community hospital. Methods: We retrospectively reviewed the etiology of pancreatitis, type and size of PFCs, length of procedure, technical success,clinical success, adverse events, and stent removal. The primary outcome was the rate of clinical success, and secondary outcomes weretechnical success and adverse events. Results: Twenty-seven patients with a mean age of 54.1±6.5 years were included, 44% of which were men. The mean size of the PFCswas 9.7±5.0 cm (range, 3–21). The most common etiology of pancreatitis was alcohol (44%) followed by idiopathic causes (30%) andpresence of gallstones (22%). The diagnosis was pseudocyst in 44.4% (12/27) and walled off necrosis in 55.6% (15/27) of patients. Therewas 100% technical success without any complications. Clinical success was achieved in 22 of 27 patients (81.5%) who underwent stentremoval. Conclusions: Our study is the first to report that endoscopic therapy of PFCs using LAMS is safe and effective even in a communityhospital setting with limited resources and support compared to large academic centers.

        Segmentation Analysis of Domestic Tourists – A Case Study

        This paper presents the segmentation analysis of domestic leisure tourists in India. Samples were taken from visitors to 6 cities categorised by ‘leisure value’ and the population was further segmented based on ‘household income’ and ‘trip frequency’. Key salient features were substantial numbers of tourists belonging to middle-income group, and major share going to train. Logit analysis was carried out. NL structure was justified for seven categorisations out of nine. The proposed sub-categorisation was found to be significant and statically different then the parent population. Factors like household income, comfort and convenience of use of a travel mode, service levels in travel modes, budgetary constraints, presence of children and older persons in a family and affordability of travel mode were found to be influencing the leisure travel. Consideration set got influenced by user and destination connectivity. Frequency of travel was affected by economic affordability and status of the tourist. Value of travel time kept shifting judiciously across the sub-categories. The tourists valued their time highly if visiting spiritual locations or made less number of leisure trips in a year. Middle income group was found aspiring to avail higher facilities on leisure travel and was willing to pay more for the same. Policy initiatives are proposed based on the work. The approach is applicable to any city across the countries, which has either cultural, religious or waterfront value.

        INTRINSIC STRUCTURE AND HIGH SPIN POSITIVE PARITY STATES IN DOUBLY EVEN SE ISOTOPES

        A systematic study of nuclear structure properties of proton rich even-even positive parity 80,82,84Se isotopes have been studied within twobody self-consistent quantum mechanical framework known as Projected Shell Model (PSM)which incorporates quadrupole-quadrupole, monopole and quadrupole pairing interactions. The description of band structures of even-even 80,82,84Se nuclei based on the band diagrams indicates the presence of multiquasi-particle structure which provides us a unified understanding of ever accumulating high spin structure of these nuclei. The back-bending in moment of inertia, yrast spectra and g-factor have also been obtained and are also compared with the available experimental data.

        Rheological investigations of water based drilling fluid system developed using synthesized nanocomposite

        In the present study, polyacrylamide grafted xanthan gum/multiwalled carbon nanotubes (PA-g-XG/ MWCNT) nanocomposite was synthesized by free radical polymerization technique using potassium persulfate as an initiator. The polyacrylamide was grafted on xanthan gum backbone in the presence of MWCNT. The synthesized nanocomposite was characterized by X-ray diffraction technique (XRD), and Fourier transform infrared spectroscopy analysis (FT-IR). The morphological characteristics of the nanocomposite were analyzed by field emission scanning electron microscopy (FESEM) and atomic force microscopy (AFM) analyses. Also, its temperature resistance property was observed with Thermogravimetric analysis (TGA). The effect of nanocomposite on the rheological properties of the developed drilling fluid system was analyzed with a strain controlled rheometer and Fann viscometer. Flow curves were drawn for the developed water based drilling fluid system at elevated temperatures. The experimental data were fitted to Bingham, power-law, and Herschel Bulkley flow models. It was observed that the Herschel Bulkley flow model predict the flow behavior of the developed system more accurately. Further, nanocomposite exhibited non-Newtonian shear thinning flow behavior in the developed drilling fluid system. Nanocomposite showed high temperature stability and had a significant effect on the rheological properties of the developed drilling fluid system as compared to conventionally used partially hydrolyzed polyacrylamide (PHPA) polymer.
