        Clinical characteristics and treatment outcomes of primary autoimmune hemolytic anemia: a single center study from South India

        BackgroundAutoimmune hemolytic anemia (AIHA) is a less recognized, potentially fatal condition. There is a scarcity of data on clinicoserological characteristics and response to therapy concerning this disease from South India.MethodsData for 33 patients with primary AIHA recorded from July 2009 to June 2015 were retro-spectively analyzed for clinical presentation, response to frontline therapy, durability of response, time to next treatment (TTNT), and response to second-line agents.ResultsThe median follow-up period was 50 months. Among 33 patients, 48% of the cases were warm autoimmune hemolytic anemia (WAIHA), 46% were cold agglutinin disease (CAD), and 6% were atypical. Three-fourth of patients had severe anemia (<8 g/dL hemoglobin [Hb]) at onset; younger patients (age <40 yr) had more severe anemia. All of the patients who required treatment received oral prednisolone at 1.5 mg/kg/d as a frontline therapy, and the response rate was 90% (62% complete response [CR] and 28% partial response [PR]). The overall response to corticosteroids in WAIHA and CAD was 87% and 92%, respectively. The median corticosteroid duration was 14 months, and 50% of the patients required second-line agents. Fourteen patients received azathioprine as a second-line agent, and 11 of these patients responded well, with half of them not requiring a third agent. Four patients developed severe infections (pneumonia, sepsis, and soft tissue ab-scess) and two had life-threatening venous thrombosis. One case of death was recorded.ConclusionAIHA is a heterogeneous disease that requires care by physicians experienced in treating these patients.

        Niosomes: a potential tool for novel drug delivery

        Vesicular drug delivery system are novel means to improve the bioavailability of the encapsulated drug along with numerous advantages over conventional drug delivery systems. Liposomes were first in such type of delivery systems but it was not so successful due to their numerous drawbacks. Niosomes or non ionic surfactant vesicles are formed from self assembly of hydrated surfactant monomers. They are formulated by non ionic surfactants but various ionic amphiphiles like di cetyl phosphate, sodium deoxycholate and stearylamine etc. are also incorporated inorder to achieve a stable vesicular suspension by inducing negative or positive charge. The most striking feature of such delivery system is that it can be used to encapsulate both hydrophobic as well as hydrophilic drug. The emphasis of this type of vesicular drug delivery system is placed over the slow release of drug in a controlled manner, resulting into sustained activity, reduced toxicity, protection of active moiety, targeting, modification in distribution profile of drugs and enhancing the bioavailability of encapsulated drug. This article summarizes the merits of niosomal drug delivery system over conventional drug therapy, its structural components, factors affecting its formation, method of preparation, evaluation techniques, therapeutic applications and patents.

        Experimental Realization of Matrix Converter Based Induction Motor Drive under Various Abnormal Voltage Conditions

        While the matrix converter has many advantages that include bi-directional power flow, a size reduction, a long lifetime, and sinusoidal input currents, it is vulnerable to the input voltage disturbances, because it directly exchanges the input voltage to the output voltage. So, in this paper, a critical evaluation of the effect of various abnormal voltage conditions like unbalanced power supply, balanced non-sinusoidal power supply, input voltage sags and short time blackout of power supply on matrix converter fed induction motor drives is presented. The operation under various abnormal conditions has been analyzed. For this, a 230V, 250VA three phase to three phase matrix converter (Me) fed induction motor drive prototype is implemented using DSP based controller and tests have been carried out to evaluate and improve the stability of system under typical abnormal conditions. Digital storage oscilloscope & power quality analyzer are used for experimental observations.
