        어떻게 평가할 것인가?

        Snoring is a very common sleep disorder. Approximate 20-30 percent of population and 40-60 percent of middle and old age population are reported to have symptoms of snoring. Snoring patients do not frequently recognize snoring until spouse or colleague has noticed. Hence, for most people snoring has been not a serious health issue but one of bad sleeping habits. However, it recently draws social attention because most patients with diagnosed “Obstructive Sleep Apnea Syndrome” cased by obstruction of the upper airway are reported snoring. In this point, the author attempts to review the clinical aspects and treatments about snoring and “Obstructive Sleep Apnea Syndrome” in the medical and dental perspective.

        구강 상주균에 대한 편백 피톤치드의 항균효과

        The present study was performed to observe the effect of phytoncide on oral normal microflora and the inhibitory effect of the surviving resident oral bacteria on F. nucleatum. In this study, saliva from each of 20 healthy subjects was treated with 1% phytoncide from Japanese Hinoki (Chamaecyparis obtusa Sieb. et Zucc.). The surviving salivary bacterium were isolated on blood agar plates and identified by 16S rDNA sequencing. In order to select inhibitory isolates against F. nucleatum, the isolates from the phytoncide-treated saliva were cultured with F. nucleatum. The results are as follows: 1. Among the 200 surviving resident oral bacterium, 70(35.0%) bacterium inhibit the growth of F. nucleatum on blood agar plates. 2. Among the 70 bacterium which inhibit F. nucleatum, Streptococcus salivarius was 41.3%(45/109), Streptococcus sanguinis was 28%.(7/25), Streptococcus mitis was 20%(3/15), Streptococcus parasanguinis was 33.3%(3/9), Streptococcus Alactolyticus was 100%(8/8), Streptococcus vestibularis was 28.6%(2/7) and Streptococcus sp. was 50%(2/4). Taken together, among the surviving resident oral bacterium, Streptococcus salivarius, Streptococcus sanguinis, Streptococcus mitis were mainly observed to inhibit F. nucleatum. and they may exert an additional inhibitory activity against the periodontopathic bacterium. Therefore, phytoncide can be used to prevent and cease the progress of periodontal disease, halitosis. Thus it is expected to promote oral health.

        The Therapeutic Value of Ultrasound-Guided Irrigation in Sialodochitis: A Case Report

        In the present case study, we reported a female patient with sialodochitis fibrinosa in which a conservative ultrasound (US)-guided irrigation method was used to relieve her symptoms. A 30-year-old female patient visited Kyung Hee University Dental Hospital with chief complaints of persistent swelling and discomfort in the left facial area. Her primary symptom was facial swelling that was accompanied by neuropathological symptoms, such as itching, tingling, and warmth that usually continued for 3 to 4 days. During clinical examination of orofacial area, mild swelling and fever were noted in the left face including parotid region, and mild induration could be felt at the corresponding site; sialadenitis of the left parotid gland was tentatively diagnosed. Herein, we performed intraductal irrigation of the left parotid gland in three times per month and prescribed some medication. Thus, her signs and symptoms have been improved, and she did not experience a recurrence for 12-month follow-up to date. This study can support the usefulness of the US-guided irrigation method to treat the sialodochitis fibrinosa without remarkable side effects.

      • Involvement of Nrf2-Mediated Upregulation of Heme Oxygenase-1 in Mollugin-Induced Growth Inhibition and Apoptosis in Human Oral Cancer Cells

        <P>Although previous studies have shown that mollugin, a bioactive phytochemical isolated from <I>Rubia cordifolia</I> L. (Rubiaceae), exhibits antitumor effects, its biological activity in oral cancer has not been reported. We thus investigated the effects and putative mechanism of apoptosis induced by mollugin in human oral squamous cell carcinoma cells (OSCCs). Results show that mollugin induces cell death in a dose-dependent manner in primary and metastatic OSCCs. Mollugin-induced cell death involved apoptosis, characterized by the appearance of nuclear shrinkage, flow cytometric analysis of sub-G<SUB>1</SUB> phase arrest, and annexin V-FITC and propidium iodide staining. Western blot analysis and RT-PCR revealed that mollugin suppressed activation of NF-<I><I>κ</I></I>B and NF-<I><I>κ</I></I>B-dependent gene products involved in antiapoptosis (Bcl-2 and Bcl-xl), invasion (MMP-9 and ICAM-1), and angiogenesis (FGF-2 and VEGF). Furthermore, mollugin induced the activation of p38, ERK, and JNK and the expression of heme oxygenase-1 (HO-1) and nuclear factor E2–related factor 2 (Nrf2). Mollugin-induced growth inhibition and apoptosis of HO-1 were reversed by an HO-1 inhibitor and Nrf2 siRNA. Collectively, this is the first report to demonstrate the effectiveness of mollugin as a candidate for a chemotherapeutic agent in OSCCs via the upregulation of the HO-1 and Nrf2 pathways and the downregulation of NF-<I><I>κ</I></I>B.</P>

        전신질환으로 입원한 환자의 구강질환과 전신질환의 상관관계(I)

        Purpose : To investigate the actual conditions of diagnosis and treatment of oral disease of inpatient with systemic disease. Methods : A total of 110 subjects, inpatient due to systemic disease for diagnosis and treatment of oral disease was requested to answer the medical history and dental treatment record. Results : In the main systemic disease, Endocrine, nutritional and metabolic diseases is composed of Gingivitis and periodontal diseases 44.9%, Diseases of salivary glands 22.4%, Within Normal Limit, Dental caries 12.2%, Diseases of pulp and periapical tissues 4.1%, Embedded and impacted teeth, Other diseases of hard tissues of teeth 2%. In the main oral disease, Gingivitis and periodontal diseases is composed of Non-insulin-dependent diabetes mellitus 39.2%, Cerebral infarction 29.4%, Nerve root and plexus disorders 5.6%, Intracerebral hemorrhage 3.9%, Malignant neoplasm of stomach, Thyrotoxicosis, Schizophrenia, Alcoholic liver disease, Nephrotic syndrome 2%. Conclusion : These findings indicate that inpatient due to the systemic disease is significantly correlated to the oral disease. The patients of oral disease interrelationship between inpatient and outpatient of systemic disease should be validated by future research.

        편백 피톤치드가 Candida albicans에 미치는 영향에 대한 연구

        Phytoncide, essential oil of trees, has microbicidal, insecticidal, acaricidal, and deodorizing effect. The present study was performed to examine the effect of phytoncide on Candida albicans, which is a commensal colonizer of the mucous membranes but has become an opportunistic pathogen. C. albicans was incubated with or without phytoncide extracted from Hinoki (Chamaecyparis obtusa Sieb. et Zucc.; Japanese cypress) and then changes were observed in its optical density, cell viability and morphology. As concentrations of phytoncide added to the culture medium increased, optical density and cell viability of C. albicans decreased. Minimum inhibitory concentration of phytoncide for C. albicans was observed to be 0.25%, and minimum fungicidal concentration was 0.5%. Numbers of morphologically atypical cells with electron-dense cytoplasm and granules and increased with increasing concentration of the phytoncide. At higher concentrations of phytoncide, compartments and organelles in the cytoplasm became indistinguishable. The overall results indicate that the phytoncide used for this study has a strong antimicrobial activity against C. albicans. Therefore, the phytoncide may be used as a candidate for prevention and therapeutic agent against oral candidiasis.

        P. gingivalis에 대한 피톤치드의 항균효과

        Trees emit phytoncide into atmosphere to protect them from predation. Phytoncide from different trees has its own unique fragrance that is referred to as forest bath. Phytoncide, which is essential oil of trees, has microbicidal, insecticidal, acaricidal, and deodorizing effect. The present study was performed to examine the effect of phytoncide on Porphyromonas gingivalis, which is one of the most important causative agents of periodontitis and halitosis. P. gingivalis 2561 was incubated with or without phytoncide extracted from Hinoki (Chamaecyparis obtusa Sieb. et Zucc.; Japanese cypress) and then changes were observed in its cell viability, antibiotic sensitivity, morphology, and biochemical/molecular biological pattern. The results were as follows: 1. The phytoncide appeared to have a strong antibacterial effect on P. gingivalis. MIC of phytoncide for the bacterium was determined to be 0.008%. The antibacterial effect was attributed to bactericidal activity against P. gingivalis. It almost completely suppressed the bacterial cell viability (>99.9%) at the concentration of 0.01%, which is the MBC for the bacterium. 2. The phytoncide failed to enhance the bacterial susceptibility to ampicillin, cefotaxime, penicillin, and tetracycline but did increase the susceptibility to amoxicillin. 3. Numbers of electron dense granules, ghost cell, and vesicles increased with increasing concentration of the phytoncide, 4. RT-PCR analysis revealed that expression of superoxide dismutase was increased in the bacterium incubated with the phytoncide. 5. No distinct difference in protein profile between the bacterium incubated with or without the phytoncide was observed as determined by SDS-PAGE and immunoblot. Overall results suggest that the phytoncide is a strong antibacterial agent that has a bactericidal action against P. gingivalis. The phytoncide does not seem to affect much the profile of the major outer membrane proteins but interferes with antioxidant activity of the bacterium. Along with this, yet unknown mechanism may cause changes in cell morphology and eventually cell death.

        Secondary Dental Pain and Facial Pain Due to Pansinusitis : A Case Report

        Toothache is a common complaint in the dental office. Most toothaches have their origin in the pulpal tissues or periodontal structures1). These odontogenic pains are managed well and predictably by dental therapies. One of the most frequent encounters and most confusing phenomena with which the dental diagnostician must deal is the problem of referred pain. The most important step toward proper management of a toothache is to consider that the pain may not be of dental origin. And Patients with orofacial pain, especially those in chronic pain, present a diagnostic and management challenge for the practitioners. There are many structures in the head and neck that can produce heterotopic pains felt in the teeth and other structures. Once referred pain is suspected, the true source of the pain must be located in order to render effective therapy. With increased interest in temporomandibular disorders and orofacial pain, many studies of accurate diagnosis and differential diagnosis about orofacial pain have been established. The purpose of this paper is to present a case for pansinusitis which produced pain referral in teeth and mimicked the symptoms of migraine.

        피톤치드의 입냄새 제거효과

        Antimicrobial action of phytoncide in the mouth decrease odor-producing microorganisms. Also phytoncide has malodor effect by reaction with volatile sulfur compounds. Phytoncide has excellent malodor effect in microbiologically and chemically. This study prove the malodor effect of phytoncide by use ferrous sulfate. So I try to make new treatment method for halitosis. I get the results as follows. 1. The difference of mean value of absorbancy was 0.849 between the mean absorbancy of deposition by add phytoncide to saliva and the saliva only. 2. The difference of mean value of absorbancy was 0.701 between the mean absorbancy of deposition by add phytoncide to distilled water and the distilled water only. 3. The difference of mean value(0.849) in saliva by existence of phytoncide was larger than in double distilled water(0.701) by existence of phytoncide. Therefore, phytoncide make more deposition in saliva than double distilled water by reaction with sulfur compounds. As the results, phytoncide reaction with sulfur compounds in saliva. It take malodor action in liquid state effectively. It is thought, only the toothpaste it knows from in the limit which does not have a side effect by the human body it adds in the oral cavity of the mouth rinse and with the fact that it will be able to use positively in clinic.
