Von Hippel-Lindau (VHL) syndrome is a rare neoplastic disorder characterized by central nervous system (CNS) and visceral tumors. We here present 68Ga-labelled [1, 4, 7, 10-tetraazacyclododecane-1, 4, 7, 10-tetraacetic acid]-1-Nal3-Octreotide positron emission tomography computed tomography findings in a 52 year old female with VHL syndrome, demonstrating both CNS and visceral tumors.

Programming in MikroBasic for Automation in Pattern Based Manufacturing

In this work the microcontroller is programmed in MikroBasic where the pattern shape is first determined using Fast Fourier Transforms offline and then the Fourier Coefficients are embedded in the program to perform calculations online to obtain the desired pattern. The stepper motors are used for accurate manufacturing. The program shows that microcontrollers do have sufficient memory to incorporate complex programs for intricate shape production.

Quick dissolving films of nebivolol hydrochloride: formulation and optimization by a simplex lattice design

Present study aims at formulation and development of quick dissolving film of nebivolol hydrochloride applying simple lattice experimental design using three hetero polymers hydroxypropyl methylcellulose, pullulan and polyvinyl pyrrolidone. Films were prepared using solvent casting technique and optimized formulation was derived by desirability function. Regression equations and contour plots were executed to narrate dependent variables,i.e. percentage drug release in 30 s (Y30), tensile strength,elastic modulus, percentage elongation and load at yield, to independent variables. Results of the experimental design revealed that the independent factors hydroxypropyl methylcellulose and pullulan significantly influenced the mechanical properties and percentage drug release from the film which was ultimately reflect in the formulation of optimized batch. The high % drug release of the film in simulated saliva and simulated gastric fluid indicated that it can be successfully used in drug delivery systems for drugs with high first-pass metabolism.

Release modulation of highly water soluble drug using solid dispersion: impact of dispersion and its compressed unit

In present day, availability of various carriers,innovative techniques of production and plenty of solventssupport the growth of solid dispersion (SD) technology inpharmaceutical industries to overcome many issues. Thepresent study was aimed to develop SD based sustainedrelease system of Milnacipran HCl (MH). The SD containingethyl cellulose, Eudragit RLPO and Eudragit RSPOat drug–polymer ratios of 1:1, 1:2, and 1:3 were developedusing solvent evaporation technique and different waxes atratio of 1:1, 1:1.25, 1:1.5 and 1:1.75 with drug weredeveloped by melting method. The physicochemicalproperties of SD were evaluated using Fourier transforminfrared spectroscopy (FTIR), differential scanning calorimetry(DSC) and X-ray diffraction (XRD). The desiredSD batch was further compressed into tablet unit to achievepredetermined once a day drug release. Tablets preparedwere evaluated for physicochemical parameters, in vitrodrug release, drug release kinetics and scanning electronmicroscopy. Out of selected carriers, bees wax had shownmaximum release retardation. The results of FTIR, DSCand XRD studies exhibited poor interaction amongst MH–bees wax and retention of crystalline state of MH in SDsystem. The presence of Benecel (HPMC K 200 M;75 mg) in tablets comprising SD (1:1.25, MH:bees wax)revealed remarkable drug release extension and it wasconsidered an optimal. The later was submitted to shortterm stability study and its results indicated the stablecharacteristic of system. Drug release from optimizedformulation fitted well into Higuchi model with anomalousdiffusion. The compressed unit of SD system of highlywater soluble drug can be successful single day regimen.

Correlation between Morphological, Chemical and RAPD Markers forAssessing Genetic Diversity in Withania somnifera (L.) Dunal

Genetic diversity among 14 Withania accessions were studied using morphological, chemical, and RAPD (Rapid Amplification ofPolymorphic DNA) markers. On the basis of morphological variation and maximum assimilation of chemical constituents, wild accessionswere considered elite as compared to cultivated. The molecular study showed that a total of 12 DNA fragments were amplifiedwith five random decamer primers 75% of which were polymorphic. Genetic similarity matrix based on the Dice index detected coefficientsranging from 0.556 to 0.941. These coefficients were used to construct a dendrogram using unweighted pair group method witharithmetic mean (UPGMA). These accessions were clustered into two major groups; the first group included AGB-009, AGB-012,AGB-042, AGB-030, AGB-053, AGB-019, AGB-017, AGB-002, and AGB-003; the second included AGB-025, AGB-001, AGB-036,AGB-015, and AGB-055. The highest similarity among the Ashwagandha accessions was observed between AGB-009 and AGB-030,AGB-009 and AGB-012, AGB-042 and AGB-030, AGB-025 and AGB-001, AGB-036 and AGB-015, AGB-015 and AGB-055 andbetween AGB-015 and AGB-055. The most distant populations in the dendrogram were AGB-053 and AGB-055. The combination ofmorphological, chemical, and molecular markers assessment will be useful in studying the genetic diversity of Withania for identification,conservation, breeding, improvement activities, and in order to achieve elite type in terms of bioactive metabolites

Factors Affecting Trial Without Catheter for First Spontaneous Acute Urinary Retention

Purpose: To find the association of trial without catheter (TWOC) outcome for first spontaneous acute urinary retention (AUR) in benign prostatic obstruction with age, prior lower urinary tract symptoms (LUTS), retention volume at catheterization (RV), and size of prostate. Methods: Our prospective observational analytical (interventional) study enrolled 77 cases of spontaneous AUR over 24 months. After clinical evaluation, digital rectal examination, and transabdominal ultrasonography, all patients were catheterized per urethra and their RV was recorded. TWOC was administered after 2 or 3 doses of 0.4 mg tamsulosin-oral absorption control system and after 48-72 hours had passed. A successful endpoint was defined as a maximum flow-rate, >5 mL/sec; voided volume, >100 mL; postvoid residue, <200 mL; and voiding within 6 hours of catheter removal. Data obtained from 58 patients were analyzed after excluding the cases lost to follow-up and secondary exclusion. Age, RV, duration of LUTS, and prostate volume on examination and ultrasonography (PUSG) were recorded and statistically analyzed. Prostate-specific antigen levels were obtained on follow-up and cases of cancer, as seen on transrectal ultrasound-guided biopsy, were secondarily excluded. Results: The patients had a mean age of 65.89±8.67 years. Prior LUTS was seen in 35 patients (2.07±2.91 months). The mean PUSG and RV were 46.81±20.58 mL and 854.8±36.26 mL, respectively. Thirty patients underwent a successful TWOC; a mean age of 63.13±8.58 years (mean±standard deviation; unpaired t-test; P=0.0053) and a PUSG of ≤45 mL (Pearson chi-square test; P=0.0427) were significantly associated with a successful outcome. Conclusion: There is a significant association between TWOC outcome, age (P=0.0053), and PUSG (P=0.0427).

Late Port Site Metastasis from Occult Gall Bladder Carcinoma After Laparoscopic Cholecystectomy for Cholelithiasis: The Role of 18F-FDG PET/CT

Late port site metastasis of gall bladder carcinoma(GBC) after laparoscopic cholecystectomy is a rare finding. Rarer still is such a presentation where the GBC remainedoccult at histopathology. 18F-flurodeoxyglucose (18F-FDG)positron emission tomography/computed tomography (PET/CT) can play an important role in this setting by supporting thediagnosis of port site metastasis, by demonstrating additionalsites of metastasis, if any, and by ruling out any other primarysite. We here present two such patients with late port sitemetastasis of occult GBC after laparoscopic cholecystectomyfor cholelithiasis and discuss the role of 18F-FDG PET/CT inthis setting.

Posterior Urethral Valve: Delayed Presentation in Adolescence

Posterior urethral valves (PUV) are now commonly suspected on antenatal ultrasound, but can present with a broad spectrum of severity postnatally. Rarely, the diagnosis is missed until adolescence or adulthood when the patient usually presents with lower urinary tract symptoms. We describe an even rarer case of PUV in a late adolescent who first presented with preserved renal function and urinary incontinence. We review the literature on presentation, natural history and outcomes of both early and late presenting PUV cases.

Improved Feature Selection Techniques for Image Retrieval based on Metaheuristic Optimization

Content-Based Image Retrieval (CBIR) system plays a vital role to retrieve the relevant images as per the user perception from the huge database is a challenging task. Images are represented is to employ a combination of low-level features as per their visual content to form a feature vector. To reduce the search time of a large database while retrieving images, a novel image retrieval technique based on feature dimensionality reduction is being proposed with the exploit of metaheuristic optimization techniques based on Genetic Algorithm (GA), Extended Binary Cuckoo Search (EBCS) and Whale Optimization Algorithm (WOA). Each image in the database is indexed using a feature vector comprising of fuzzified based color histogram descriptor for color and Median binary pattern were derived in the color space from HSI for texture feature variants respectively. Finally, results are being compared in terms of Precision, Recall, F-measure, Accuracy, and error rate with benchmark classification algorithms (Linear discriminant analysis, CatBoost, Extra Trees, Random Forest, Naive Bayes, light gradient boosting, Extreme gradient boosting, k-NN, and Ridge) to validate the efficiency of the proposed approach. Finally, a ranking of the techniques using TOPSIS has been considered choosing the best feature selection technique based on different model parameters.
