        The use of laser Doppler blood flow to assess the effect of acute administration of vitamin D on micro vascular endothelial function in people with diabetes

        Objective: To assess the effect of vitamin D administration on the skin blood flow response to occlusion and heat. Design: Cross-sectional study. Methods: Twenty age matched subjects; 10 who had diabetes and 10 who were controls were administered 4,000 IU of vitamin D3 for 3 weeks at breakfast. The function of the endothelial cells was evaluated in 2 ways; first, the response to 4 minutes of vascular occlusion of the skin was measured with a laser Doppler flow meter. Second, the skin blood flow response to local heat at 42 degrees C for 6 minutes was examined. Results: The results of the experiments showed that the blood flow response to heat was reduced after 3 weeks administration of vitamin D in the subjects with diabetes and in the control subjects (p<0.05). The response to occlusion was not significantly different within each group before and after vitamin D administration, but the group with diabetes had a significantly lower blood flow response to occlusion than did the controls (p<0.05). Conclusions: Acute doses of vitamin D may impair nitric oxide production and reduce blood flow to tissue during stressors in people with diabetes.

        The effect of coenzyme Q10 on endothelial function in a young population

        Objective: Coenzyme (CoQ10) is an enzymatic co factor used in normal cellular metabolism. Recent evidence shows that in people with heart disease it can reverse endothelial cell damage in the blood vessels. It is also a potent antioxidant. Design: One group pretest-posttest design. Methods: In the present study, endothelial function was evaluated using the response to occlusion and heat before and 2 weeks after administration of CoQ10, 300 mg/day. Thirty Eight subjects, who are physical therapy students, participated in a series of experiments to see if taking 300 mg of CoQ10 daily for 2 weeks would impact resting blood flow in the forearm skin and the blood flow response to 4 minutes of vascular occlusion and the response to local heat ($42^{\circ}C$) for 6 minutes. Results: The results showed that, for this population, there was no difference in the response to heat. However, the response to occlusion was improved after administration of CoQ10. Conclusions: It would appear that in a young population CoQ10 has no effect on the nitric oxide vasodilator pathway in skin but does influence other vasodilator pathways.

        A comparison of the aerobic cost and muscle use in aerobic dance to the energy costs and muscle use on treadmill, elliptical trainer and bicycle ergometry

        Objective: To determine the energy consumed and muscle use during dance compared to different standard exercise devices. Design: Longitudinal study. Methods: Fifteen female subjects were evaluated to assess the energy cost and muscle activity during a 20 minute dance video compared to treadmill, elliptical track and bicycle ergometry. The later 3 forms of exercise were accomplished in four, 5 minute bouts at different intensities of exercise. Subjects were in the age range of 22-24 years old, were free of cardiovascular disease and did not have any neurological injuries. They were not sedentary and exercised at least twice a week. During the exercise, muscle activity was measured by the electromyogram recorded by surface electrodes on 6 muscle groups. A Cosmed metabolic cart was used to measure oxygen consumption during the exercise. Results: The aerobic dance video that was tested here was equivalent to a hard workout on any of the 3 exercise modalities. The dance routine was equivalent in terms of energy consumed to running at 225 watts of work or running for 20 minutes at a speed of 2 meters per second (4.47 miles per hour). Compared to the bicycle, it was equivalent to cycling at 112 watts for 20 minutes (2.25 kpm), and for the elliptical trainer, dance was equivalent to 435 watts. Concerning muscle use, the dance routine was the most balanced for upper, core and lower body muscles. Although the elliptical trainer was close, it required muscle less muscle use. Conclusion: A good dance video can be more effective than standard exercise equipment.

        The effects of low-level laser therapy in patients with wrist pain: is this Mickey Mouse science?

        Objective: Low level laser treatment (LLLT) is widely used in physical therapy practice. It is combined with physical therapy or LLLT alone. The purpose of this study is to evaluate the effectiveness of LLLT on patients' perception of general wrist pain. Design: Longitudinal study. Methods: Forty-eight subjects with wrist pain who were in the age range of 18-70 years old were examined. The subjects were asked, via an interview and a visual analog scale, to grade their wrist pain. They were asked to rotate their wrists through full range of motion and the angle at which any pain occurred was assessed. Each subject was then exposed to one of the following: 1) treatment with an infrared laser with the power turned off (placebo), 2) treatment with an infrared therapeutic laser, 3) treatment with a red therapeutic laser, 4) treatment with an ultraviolet laser, 5) treatment with a blue laser, 6) treatment with a Mickey Mouse flashlight. The duration of the treatment was 3 sessions in 3 days. Results: The results of the experiments showed that while pain was reduced both immediately after and the next day after laser therapy (p<0.05), there was no significant difference between the laser groups and the placebo group. However, the Mickey Mouse flashlight treatment groups had a greater range of motion than the laser groups (p<0.05). Conclusions: While pain was reduced in all laser groups, it was probably a placebo effect. The Mickey Mouse flashlight group probably received benefit from the heat of the flashlight.

        Original Article : Can prosthetic limbs made too quickly cause kidney damage?: a pilot study

        Objective: The use of chemicals for building prosthetic sockets present the possibility of being hazardous and unsafe due to off-gassing. The purpose of the present study was to investigate if freshly made materials used in prosthetic sockets causes off-gassing that would penetrate the skin and cause damage to the kidneys or blood. Design: Cross-sectional study. Methods: In this research, the off-gassing effects during the initial curing process of styrene monomer, vinyl ester resin, epoxy methacrylate resin, benzene-1, 3-dimethaneamine, trimethylhexanedlamine, and paratertiarybutylphenol were analyzed. Acid detection strips were placed inside newly fabricated mock-prosthetic sockets and left overnight in a closed environment to find out if acid was present in the invisible fumes. The plastic was worn by 9 subjects and urinalysis was made after 48 hours to test for any kidney or blood toxicity of the resins. Results: After wearing the plastic cuff for 48 hours, the ratio of protein to creatinine in the urine was raised to an abnormal level in five out of nine subjects. Four out of the nine subjects showed normal protein to creatinine ratios after wearing the device. The results showed that damage to the kidney occurred from wearing the resins after curing in half of the subjects. Conclusions: It is very important to conduct patient intakes which includes the assessment of renal function. Off-gassing in vented chambers may be needed to protect both prosthetists and patients.

        Injury and inflammation detection by the application of microcurrent through the skin

        Objective: To determine the efficacy and reliability of measuring direct current microcurrent applied through the skin to determine injury in the underlying tissues. Design: Case control study. Methods: First, microcurrent was measured as decreased blood flow induced hypoxia in healthy subjects. Next, reliability was assessed by measuring over ten days with set variations in pressure and distance between the electrodes. Finally, measurements over sprained ankle were compared to measurements over comparable uninjured areas on the same injured subject. Results: For the blood flow test phase, microcurrent significantly decreased an average of 17% after 5 minutes (p<0.05), remained decreased for 30 seconds, and returned to non-occlusive levels after 2 minutes of normal circulation. The results indicate that the microcurrent decrease was not due to blood flow, and most likely from hypoxic cellular damage. For the reliability phase, the coefficients of variation averaged 10.3% for the shoulder, 14.8% for the low back, and 29.1% for the knee. Changing distance 2.5 cm between the electrodes resulted in insignificant changes. Changes in pressure had some significant effect after an increase in force of 2.6 N, affirming the need for consistent pressure for measurement. For the injury test phase, a significant 69% decrease occurred comparing injured areas to the same area on the uninjured side, and a significant 74% occurred comparing injured and non-injured areas on the same limb. Conclusions: Microcurrent through the skin shows promise as an objective method of assessing a soft tissue injury by detecting damage likely due to hypoxia.

        The effects of low level laser radiation on bacterial growth

        Objective: The low level lasers currently in the market vary in wavelength, dosage, and frequency. These devices are used with much different clinical pathology. Most notably, some studies claim that wounds heal faster with low level laser therapy due to the fact that bacteria commonly found in wounds are killed by laser light. Systemic and meta-analysis studies found the difficulty of comparison of numerous research studies because of differences in the intensities and frequencies of low level laser treatment (LLLT). The purpose of this study was to determine the effectiveness of LLLT on controlling bacterial growth. Design: Cross-sectional study. Methods: Variables included LLLT dosage and wavelength on 3 bacteria commonly seen in wounds, strains of Staphylococcus aureus, Escherichia coli, and Pseudomonas aeruginosa were used on commercially available 5.0-cm agar plates. Blue, green, and red, ultraviolet (UV) and infrared laser light sources were adjusted to either low or high intensity settings. Five Petri dishes at a time were placed directly beneath laser light sources with the exception of UV which was placed six inches below the suspended light and infrared which was placed directly on top of the Petri dish lid. Each group of five Petri dishes was irradiated for 15 minutes. Results: The results showed no effect of any of 9 different LLLT intensities or colors on bacteria growth compared to sham light. Conclusions: At least for claims of bacterial growth inhibition with LLLT, no support for this claim can be found here.
