        물리치료사의 직무유형이 직무만족도와 조직몰입도에 미치는 영향 : 부산지역을 중심으로

        Purpose : The purpose of this survey was to investigate the effect on job satisfaction and organizational commitment according to the job types of Pusan area physical therapists figuring out structural causality between job satisfaction and organizational commitment. Method : This survey was conducted through structured questionnaire for the physical therapists working in Pusan area, and data from 217 therapists were utilized in the final analysis. Data collection was deployment questionnaire using Google Docs (online survey), The SPSS 21.0 program was used according to for research purposes independent t-test was used for individual and job characteristics, was used according to job types and job satisfaction and organizational commitment are logistic regression analysis, correlation analysis and chi-square test. Result : The results of this study were as follows. 1. Overall job satisfaction according to the job types was appeared musculoskeletal physical therapist more 4 times. 2. Job types according to the organizational commitment did not make direct effect. Conclusion : Number of musculoskeletal physical therapist than neurological physical therapist was fewer and there were many per day treatment of patients, but overall job satisfaction was appeared that is higher. This is thought that job types affects more than working environment.

      • 영양불량환자의 중증도 적용 향상을 위한 영양지원팀 협의진료체계 개선활동의 중요성

        Purpose: The grade of complexity in the diagnosis related group (DRG) payment system is influenced by the secondary diagnosis of specific complication and comorbidity level, in which moderate or severe malnutrition is included. This study examined an existing proportion of patients with malnutrition who were supposed to be qualified for the complexity level and devised quality improvement measures to increase the proportion of qualifying complexity payments. Methods: The goal of the activities was to increase the rate of complexity payment claims for patients with malnutrition (%). Cases ineligible for the DRG payment system and cases with no diagnosis of malnutrition were excluded. We established a collaborative system between the nutrition support team and departments related to each improvement factor (i.e., patient care, medical records, insurance review, and medical information). Results: Before implementing the activities, this study investigated the current level of complexity payment claims for malnutrition patients who were discharged within a specific period (June 1, 2015∼August 31, 2015). The results showed that complexity payment claims were filed in 10.00% (2 of the 20 malnutrition cases). After the activities, the rate of complexity payment claims for the patients with malnutrition within the study period (June 1, 2016∼August 31, 2016) was 46.43% (26 out of 56), showing an approximately 364% increase from the pre activity rate. This change was statistically significant according to the chi-square test on Microsoft Excel 2010 (P<0.01). Conclusion: Collaborative efforts by the related departments enabled the smooth implementation of each activity. In addition, moderate or severe malnutrition was revealed to be a variable in the complexity-specific payment system. In the future, hospital-wide awareness and effort are crucial to promot the steady practice of these activities and expand their implementation.

      • Evaluating the short message service alerting system for critical value notification via PDA telephones.

        <P>Physicians need to respond to critical value alerts in a timely and appropriate manner. We evaluated the clinical usefulness of a short message service (SMS) system for notifying physicians of critical values by sending a text message to their personal data assistant (PDA) phones. The clinical response times and clinical response rates to notifications of critical hyperkalemia in inpatients in 2001, when the callback system alone was used to alert physicians, were compared to those in 2005, when the SMS alert system was used in addition to the callback system. The clinical response time to SMS alerts of critical hyperkalemia sent to PDA phones in 2005 was remarkably shorter than the response time to alerts sent using the callback system alone in 2001. With the help of the SMS system for sending critical value alerts to PDA phones, corrective actions could be taken more promptly and patient care was improved.</P>

      • Photoluminescent Green Carbon Nanodots from Food-Waste-Derived Sources: Large-Scale Synthesis, Properties, and Biomedical Applications

        <P>We have developed a simple approach for the large-scale synthesis of water-soluble green carbon nanodots (G-dots) from many kinds of large food waste-derived sources. About 120 g of G-dots per 100 kg of food waste can be synthesized using our simple and environmentally friendly synthesis approach. The G-dots exhibit a high degree of solubility in water because of the abundant oxygen-containing functional groups around their surface. The narrow band of photoluminescence emission (400–470 nm) confirms that the size of the G-dots (∼4 nm) is small because of a similar quantum effects and emission traps on the surfaces. The G-dots have excellent photostability; their photoluminescence intensity decreases slowly (∼8%) under continuous excitation with a Xe lamp for 10 days. We carried out cell viability assay to assess the effect of cytotoxicity by introducing G-dots in cells such as Chinese hamster ovary cells (CHO-K1), mouse muscle cells (C2C12), and African green monkey kidney cells (COS-7), up to a concentration of 2 mg mL<SUP>–1</SUP> for 24 h. Due to their high photostability and low cytotoxicity, these G-dots are excellent probes for in vitro bioimaging. Moreover, the byproducts (not including G-dots) of G-dot synthesis from large food-waste derived sources promoted the growth and development of seedlings germinated on 3DW-supplemented gauze.       • Clinical proteomic analysis of scrub typhus infection

        <P><B>Background</B></P><P> Scrub typhus is an acute and febrile infectious disease caused by the Gram-negative α-proteobacterium <I>Orientia tsutsugamushi</I> from the family Rickettsiaceae that is widely distributed in Northern, Southern and Eastern Asia. In the present study, we analysed the serum proteome of scrub typhus patients to investigate specific clinical protein patterns in an attempt to explain pathophysiology and discover potential biomarkers of infection.</P><P><B>Methods</B></P><P>Serum samples were collected from three patients (before and after treatment with antibiotics) and three healthy subjects. One-dimensional sodium dodecyl sulphate–polyacrylamide gel electrophoresis followed by liquid chromatography-tandem mass spectrometry was performed to identify differentially abundant proteins using quantitative proteomic approaches. Bioinformatic analysis was then performed using Ingenuity Pathway Analysis.</P><P><B>Results</B></P><P>Proteomic analysis identified 236 serum proteins, of which 32 were differentially expressed in normal subjects, naive scrub typhus patients and patients treated with antibiotics. Comparative bioinformatic analysis of the identified proteins revealed up-regulation of proteins involved in immune responses, especially complement system, following infection with <I>O. tsutsugamushi</I>, and normal expression was largely rescued by antibiotic treatment.</P><P><B>Conclusions</B></P><P>This is the first proteomic study of clinical serum samples from scrub typhus patients. Proteomic analysis identified changes in protein expression upon infection with <I>O. tsutsugamushi</I> and following antibiotic treatment.         <P><B>Abstract</B></P> <P><B>Background</B></P> <P>Limited data exist regarding the impact of aortic valve calcification (AVC) eccentricity on the risk of paravalvular regurgitation (PVR) and response to balloon post-dilation (BPD) after transcatheter aortic valve replacement (TAVR). We investigated the prognostic value of AVC eccentricity in predicting the risk of PVR and response to BPD in patients undergoing TAVR.</P> <P><B>Methods</B></P> <P>We analyzed 85 patients with severe aortic stenosis who underwent self-expandable TAVR (43 women; 77.2±7.1years). AVC was quantified as the total amount of calcification (total AVC load) and as the eccentricity of calcium (EoC) using calcium volume scoring with contrast computed tomography angiography (CTA). The EoC was defined as the maximum absolute difference in calcium volume scores between 2 adjacent sectors (bi-partition method) or between sectors based on leaflets (leaflet-based method).</P> <P><B>Results</B></P> <P>Total AVC load and bi-partition EoC, but not leaflet-based EoC, were significant predictors for the occurrence of ≥moderate PVR, and bi-partition EoC had a better predictive value than total AVC load (area under the curve [AUC]=0.863 versus 0.760, <I>p</I> for difference=0.006). In multivariate analysis, bi-partition EoC was an independent predictor for the risk of ≥moderate PVR regardless of perimeter oversizing index. The greater bi-partition EoC was the only significant parameter to predict poor response to BPD (AUC=0.775, <I>p</I> =0.004).</P> <P><B>Conclusion</B></P> <P>Pre-procedural assessment of AVC eccentricity using CTA as “bi-partition EoC” provides useful predictive information on the risk of significant PVR and response to BPD in patients undergoing TAVR with self-expandable valves.</P> <P><B>Highlights</B></P> <P> <UL> <LI> AV calcification has been suggested to be associated with PVR after TAVR. </LI> <LI> We developed a bi-partition method to assess eccentricity of AV calcification (EoC). </LI> <LI> Bi-partition EoC predicted the risk of PVR and response to balloon post-dilation. </LI> <LI> Bi-partition EoC had better predictive power than the conventional leaflet-based method. </LI> <LI> Measurement of the bi-partition EoC may be helpful in a patient undergoing TAVR. </LI> </UL> </P>

        Nanocrack-regulated self-humidifying membranes

        <P>The regulation of water content in polymeric membranes is important in a number of applications, such as reverse electrodialysis and proton-exchange fuel-cell membranes. External thermal and water management systems add both mass and size to systems, and so intrinsic mechanisms of retaining water and maintaining ionic transport(1-3) in such membranes are particularly important for applications where small system size is important. For example, in proton-exchange membrane fuel cells, where water retention in the membrane is crucial for efficient transport of hydrated ions(1,4-7), by operating the cells at higher temperatures without external humidification, the membrane is self-humidified with water generated by electrochemical reactions(5,8). Here we report an alternative solution that does not rely on external regulation of water supply or high temperatures. Water content in hydrocarbon polymer membranes is regulated through nanometre-scale cracks ('nanocracks') in a hydrophobic surface coating. These cracks work as nanoscale valves to retard water desorption and to maintain ion conductivity in the membrane on dehumidification. Hydrocarbon fuel-cell membranes with surface nanocrack coatings operated at intermediate temperatures show improved electrochemical performance, and coated reverse-electrodialysis membranes show enhanced ionic selectivity with low bulk resistance.</P>

      • Highly catalytic and reflective dual-phase nickel sulfide electrodes for solar energy conversion

        <P><B>Abstract</B></P> <P>A simple and practical method is presented for the fabrication of mirror-like reflective nickel sulfide (NiS) films via a hydrothermal reaction to serve as efficient transition metal chalcogenide catalysts. The NiS films on fluorine-doped tin oxide (FTO) glass substrates exhibit dual-phase crystalline structure with tunable α-NiS/Ni<SUB>3</SUB>S<SUB>4</SUB> composition. The α-NiS/Ni<SUB>3</SUB>S<SUB>4</SUB> ratio could be tuned by varying the concentration of the sulfur source, thioglycolic acid in the reaction mixture. The key role of thioglycolic acid is to control the morphology and crystal phase structure as well as determine the nickel to sulfur ratio. The NiS films, especially those fabricated with an adequate amount of the sulfur source, show multifunctional properties with significantly improved electrical conductivity, catalytic activity, and light reflectance due to the multiple layers of the stacked structure. Owing to the unique features of the fabricated films, solid polymer electrolyte dye-sensitized solar cells (DSSC) based on NiS counter electrodes exhibit photovoltaic energy conversion efficiency of 6.22%, which is much higher than that achieved with commercial Pt counter electrodes (5.42%).
