REVIEW : Diagnosis and therapy of multiple myeloma

Many advances in the treatment of multiple myeloma have been made due to the use of transplantation and the introduction of novel agents including thalidomide, lenalidomide, and bortezomib. The first step is recognizing the symptoms and starting prompt treatment. Different strategies should be selected for young and elderly subjects. Young patients are commonly eligible for transplantation, which is now considered the standard approach for this setting, and various inductions therapies containing novel agents are available before transplantation. Elderly patients are usually not eligible for transplantation, and gentler approaches with new drugs combinations are used for their treatment.
Stability of coprecipitated natural humic acid and ferrous iron under oxidative conditions

Goethite and lepidocrocite are considered the stable species of Fe oxyhydroxides formed from Fe<SUP>2+</SUP> oxidation in mildly acidic soil environments, while maghemite is formed in mildly alkaline conditions. Complexing ligands, especially humic acid (HA), can change the pathway formation of these iron oxyhydroxides from Fe<SUP>2+</SUP> oxidation. This research aimed to assess the influence of HA on Fe<SUP>2+</SUP> oxidation and its effect on crystalline Fe mineral products. Ferrous iron was added to HA at increasing initial COOH/Fe(II) charge ratios ® at pHs of 5.0 and 8.0 and aged at 70<SUP>o</SUP>C for 1month and up to 8years at room temperature. The precipitated products after aging were analysed using XRD, FT-IR, TEM and AFM. The results indicate that oxidation of Fe<SUP>2+</SUP> at pH5.0, in the presence of large amounts of HA (R=0.1) promoted the formation of goethite together with ferrihydrite. Oxidation of Fe<SUP>2+</SUP> in slightly alkaline conditions (pH8.0) yielded maghemite with small amounts of goethite. Further, long-term aging of HA coprecipitated with Fe<SUP>2+</SUP> perturbed the stacking of the Fe hydroxyl sheets at pH5.0, favouring the formation of ferrihydrite. At pH8.0 the presence of humic acid does not influence the crystallinity of the precipitation products and maghemite was still stable after 8years of aging. This research confirms that the oxidation of Fe<SUP>2+</SUP> in soil and its subsequent hydrolysis is a very common process of Fe hydroxides/oxide formation in acidic soil environments. The type of Fe oxide formed depends strictly on the pH and secondly on the rate of oxidation, especially on the presence and amount of organic compounds that inhibit crystal growth. Humic acid are the main organic molecules in soils, particularly in cool, humid weathering environments where they lead to a decrease in crystalline perfection and promote the formation of ferrihydrite instead of goethite or lepidocrocite. These results provide significant insights into the effect of the mobility of HA in association with Fe minerals.
Kinetic Study of a Hen-Egg White Lysozyme-based Oenological Preparation

The catalytic activity of a lysozyme-basedoenological preparation toward the specific substrates i)glycol chitosan, as a non-cellular synthetic substrate, and ii)Oenococcus oeni, as a microbial substrate for muramidaseactivity, was investigated in wine-like acidic medium(tartaric buffer, pH 3.2). The good reproducibility andreliability of results revealed that both substrates are usefulfor lysozyme kinetics studies. Both the chitinolytic andmuramidase activity of lysozyme were affected byuncompetitive substrate inhibition. However, the O. oenieffect occurred at very high concentration with respect tothe typical wine bacterial content, even when microbialpopulation reaches its maximum growth level duringmalolactic fermentation. The optimum pH and temperaturefor lysozyme activity were very far from the usual winehandling conditions, but residual enzyme activity wasobserved at pH 3.2 and 20oC, confirming that theoenological application of lysozyme is a useful practice.
A 50-mA 1-nF Low-Voltage Low-Dropout Voltage Regulator for SoC Applications

In this paper, we present a low-voltage low-dropout voltage regulator (LDO) for a system-on-chip (SoC) application which, exploiting the multiplication of the Miller effect through the use of a current amplifier, is frequency compensated up to 1-nF capacitive load. The topology and the strategy adopted to design the LDO and the related compensation frequency network are described in detail. The LDO works with a supply voltage as low as 1.2 V and provides a maximum load current of 50 mA with a drop-out voltage of 200 mV: the total integrated compensation capacitance is about 40 pF. Measurement results as well as comparison with other SoC LDOs demonstrate the advantage of the proposed topology.

Granulomatous mastitis is a rare benign inflammatory disease of the breast with multiple etiologies such as tuberculosis, sarcoidosis, foreign body reaction, and mycotic and parasitic infections. In contrast, idiopathic granulomatous mastitis (IGM) is characterized by the presence of chronic granulomatous lobulitis in the absence of an obvious etiology. Clinically and radiologically it may mimic breast carcinoma and so awareness of surgeons,pathologists, and radiologists is essential to avoid unnecessary mastectomies. Cases of IGM are reported during antidepressant therapy in patients also showing high levels of prolactinemia. In these cases, we believe that surgical excision must be avoided being replaced with a conservative management of the pathological condition based on a corticosteroid treatment.
