      • Prognostic Significance of Human Epidermal Receptor (HER)-3 Immunohistochemical Expression in Patients with Metastatic Breast Cancer

        Background: Previous reports have shown that human epidermal receptor (HER)-3 overexpression may be associated with poor prognosis in patients with breast cancer, but results have been conflicting. In this study, we sought to investigate the prognostic significance of HER-3 immunohistochemical expression in patients with metastatic breast cancer. Methods: We retrospectively analyzed HER-3 immunohistochemical expression profiles in 45 paraffin-embedded specimens from patients who had been treated between 1996 and 2006 in the Department of Oncology of the Uludag University School of Medicine, Bursa, Turkey. Membranous or cytoplasmic dominant expression patterns of HER-3 were analyzed using the Rajkumar score and a cytoplasmic 4-point scoring system, respectively. Progression-free survival (PFS) and overall survival (OS) served as the main outcome measures. Results: The median PFS in the study participants was 9 months (interquartile range: 4.5-13 months), whereas the median OS was 20 months (interquartile range: 7.5-28 months). Categorization of the patient population according to HER-3 positive immunohistochemical expression did not reveal any statistically significant difference in terms of both PFS (p=0.70) and OS (p=0.81). The results of multivariable Cox regression analysis indicated that tumor size was the only independent predictor of PFS, whereas estrogen and progesterone receptor status was independently associated with OS. Conclusions: HER-3 immunohistochemical expression did not correlate with outcomes in Turkish patients with metastatic breast cancer. Although our results suggest that HER-3 expression in cancer specimens is not of prognostic significance, further prospective studies are warranted to confirm these results.

        $H_2O_2$/UV-C oxidation of potential endocrine disrupting compounds: a case study with dimethyl phthalate

        This paper discusses the feasibility of the ultraviolet radiation-hydrogen peroxide ($H_2O_2$/UV-C) process as an advanced oxidation process (AOP) in the treatment of endocrine disrupting compounds (EDC). Dimethyl phthalate (DMP) was chosen as the model compound owing to its classification as an EDC. Experiments have been conducted at various pH values (3.5, 6.0 and 9.0) and initial $H_2O_2$ concentrations (0-60 mM) in a batch reactor equipped with a low-pressure mercury UV-C lamp in order to evaluate the optimal operation conditions of the $H_2O_2$/UV-C process. The most effective pH value for the degradation of DMP by $H_2O_2$/UV-C treatment was found as 6.0. DMP abatement increased with increasing $H_2O_2$ concentrations from 5 to 30 mM. Further increase in initial $H_2O_2$ concentration, however, reduced both the rate and extent of DMP removal as well as chemical oxygen demand (COD) and total organic carbon (TOC) removals. A simple kinetic model was proposed for DMP, COD and TOC abatements confirmed pseudo-first-order reaction. The electrical energy per order (EE/O) values for DMP oxidation and TOC mineralization were calculated as 3.3 and $19\;kWh\;m^{-3}\;order^{-1}$ respectively for the optimum treatment conditions ($H_2O_{2,o}$ = 30 mM, $pH_o$ = 6.0, $DMP_{o}\;=\;100\;mg\;L^{-1}$). Inhibition of oxygen uptake rate by activated sludge (ISO 8192) was evaluated as a tool for assessing the acute toxicity of untreated and $H_2O_2$/UV-C treated DMP. According to the results obtained in this work, the use of the $H_2O_2$/UV-C process is recommended to achieve a complete DMP oxidation and high mineralization degree of aqueous solution of DMP.

        Esophageal Lymphoepithelioma-Like Carcinoma with Unique Daisy-Like Appearance

        Due to differences in prognosis and management, it is important to subclassify esophageal carcinoma. Esophageal lymphoepithelioma-like carcinoma (LELC) is extremely rare, with only a few cases reported to date. Review of the literature revealed case reports describing lesions with similar histology. We present a 69-year-old man with a giant pedunculated-polypoid lesion of the esophagus shrinking the lumen. Endoscopic excision of the tumor was performed and final histopathological diagnosis was confirmed to be LELC. In contrast to a previous case with a more aggressive course and a recurrent lesion, our patient died of his disease within 8 months of diagnosis. Here we discuss the endoscopic and radiologic findings of the case and a review of the literature.

        RICCI CURVATURE, CIRCULANTS, AND EXTENDED MATCHING CONDITIONS

        Ricci curvature for locally finite graphs, as proposed by Lin, Lu and Yau, provides a useful isomorphism invariant. A Matching Condition was introduced as a key tool for computation of this Ricci curvature. The scope of the Matching Condition is quite broad, but it does not cover all cases. Thus the current paper introduces extended versions of the Matching Condition, and applies them to the computation of the Ricci curvature of a class of circulants determined by certain number-theoretic data. The classical Matching Condition is also applied to determine the Ricci curvature for other families of circulants, along with Cayley graphs of abelian groups that are generated by the complements of (unions of) subgroups.

        Stimulation of bone formation by direct electrical current in an orthopedically expanded suture in the rat

        이 실험적 연구는 직류 전류 자극(direct electrical current stimulation, DECS)을 백서의 확장된 전상악골간 봉합에 가하였을 시 골 재생에 미치는 영향을 평가하기 위해 시행되었다. 60마리의 웅성 Wistar rats를 연구에 사용하였으며 대조군과 실험군의 2개의 군으로 나누었다. 상악 절치 사이에 30-gram의 힘을 발휘하는 helical-spring을 적용하여 상악골의 확장을 두 군 모두에서 시행하였다. 실험군에서는 상악골 측면부에 금속 스크류를 식립하였고 스크류에 전극을 연결하였다. 장치는 지속적으로 $10{\mu}A$의 전류를 흘러 보내도록 조절하였으며 전류는 확장기와 초기 유지기 동안 매일 점검하였다. 골 재생을 조직형태계측학적으로 평가하기 위해 신생골 면적(${\mu}m^2$), 골 둘레길이(${\mu}m$), feret's diameter (${\mu}m$) 와 신생골(%)을 계측하였다. 결과는 유의수준 p < 0.05에서 Kruskal-Wallis rank와 Mann-Whitney U tests를 이용하여 통계 분석하였다. 실험 결과 조사한 모든 조직형태계측적 수치들에서 통계적으로 유의한 차이가 양 군 사이에서 나타났다. 신생골 면적 (p = 0.002), 골 둘레길이 (p = 0.002), feret's diameter (p = 0.002)와 신생골 비율 (p = 0.002)의 모든 계측치가 실험군에서 대조군에 비해 높게 나타났다. 이런 결과는 DECS 군에서 골 형성이 더 양호함을 의미한다. 결론적으로 악정형적으로 확장시킨 전상악골 봉합부에 DECS를 적용하면 초기 유지 기간 중에 신생골 형성이 촉진된다고 할 수 있다. Objective: The aim of this experimental study was to evaluate the effects of direct electrical current stimulation (DECS) on bone regeneration in response to an expansion of the inter-premaxillary suture in the rat. Methods: Sixteen 50 - 60 days old Wistar male rats were separated into two equal groups (control and experimental). Both groups were subjected to expansion, and 30-gram of force was applied to the maxillary incisors with helical-spring. In the experimental group, two metallic-screws were placed at lateral parts of the maxillary segments. Electrodes were connected to the screws. The device was activated with current adjustment to measure $10{\mu}A$ continuously and the current was monitored daily during the expansion and early-retention phase. Bone regeneration in the sutural area was histomorphometrically evaluated including new-bone area (${\mu}m^2$), bone perimeter (${\mu}m$), feret's diameter (${\mu}m$) and newly formed bone (%) parameters. Kruskal-Wallis rank and Mann-Whitney U tests were used for statistical evaluation at p < 0.05 level. Results: Statistical analysis showed significant differences between groups for all investigated histomorphometric parameters. New bone area (p = 0.002), bone perimeter (p = 0.004), feret's diameter (p = 0.002) and newly formed bone percentage (p = 0.002) measurements were significantly higher in the experimental group than the control group. Bone histomorphometric measurements revealed that bone architecture in the DECS group was improved. Conclusions: The application of DECS to an orthopedically expanded inter-premaxillary suture area during the early retention phase stimulated the formation of new bone.

        Effect of stress-strain curve changing with equal channel angular pressing on ultimate strength of ship hull stiffened panels

        Similar to other structures, ultimate strength values showing the maximum load that the structure can resist without damaging has great importance on ships. Therefore, increasing the ultimate strength values will be an important benefit for the structure. Low carbon steels used in ships due to their low cost and good weldability. Improving the ultimate strength values without interfering with the chemical composition to prevent of the weldability properties of these steels would be very beneficial for ships. Grain refinement via severe plastic deformation (SPD) is an essential strengthening mechanism without changing the chemical composition of metallic materials. Among SPD methods, equal channel angular pressing (ECAP) is one of the most commonly used one due to its capacity for achieving bulk ultrafine-grained (UFG) materials. When the literature is examined, it is seen that there is no study about ultimate strength calculation in ships after ECAP. Therefore, the mean purpose of this study is to apply ECAP to a shipbuilding low carbon steel to be able to achieve mechanical properties and investigate the alteration of ship hull girder grillage system’s ultimate strength via finite element analysis approach. A fine-grained (FG) microstructure with a mean grain size of 6 μm (initial grain size was 25 μm) was after ECAP. This microstructural evolution brought about a considerable increase in strength values. Both yield and tensile strength values increased from 280 MPa and 425 MPa to about 420 MPa and 785 MPa, respectively. This improvement in the strength values reflected a finite element method to determine the ultimate strength of ship hull girder grillage system. As a result of calculations, it was reached significantly higher ultimate strength values (237,876 MPa) compared the non-processed situation (192,986 MPa) on ship hull girder grillage system.

      • Clinicopathological Characteristics of Hepatocellular Carcinoma in Turkey

        Background: Hepatocellular carcinoma (HCC), the main malignant tumor of the liver, is very common and highly lethal. The aim of this study was to determine its clinicopathologic characteristics and risk factors in Turkey. Materials and methods: In this study, patients who were diagnosed as suffering from HCC in the period between August 2004 and December 2011 were evaluated retrospectively. Results: A total of 98 patients were included, with a median age 61 (range: 16 to 82). Seventy nine (80.6%) were male 59 (60.2%) were infected with hepatitis B virus (HBV) and 15 (15.3%) with HCV, another 15 (15.3%) being alcohol abusers. Seventy two (73.5%) were at advanced stage and 54 (55.1%) had elevated serum alpha-fetoprotein (AFP). Surgery, chemoembolization, systemic chemotherapy and application of the tyrosine kinase inhibitor sorafenib were the major treatment options. Conclusions: According to our findings HCC is mostly diagnosed in advanced stage and age, being five times more common in males than females. Main risk factors of HCC are HBV infection, HCV infection and alcohol abuse. Elevation in AFP may facilitate early diagnosis of HCC in high risk groups.

        Expansion of the mandibular arch using a trombone appliance

        Objective: This case report describes orthodontic treatment of contracted mandibular arch using a trombone appliance. Methods: A 14-year-old girl with Class II division 2 malocclusion, retroclined maxillary incisors, and buccally displaced maxillary canines required dental expansion in 3 spatial directions to correct the contracted maxillary and mandibular arches. In the initial phase of treatment, the maxillary arch was expanded and distalized using a quad-helix appliance and cervical headgear. Following the expansion and leveling of the maxillary arch, a trombone appliance was used to expand the mandibular arch. On correction of the mandibular arch and provision of sufficient space to level the mandibular teeth, fixed orthodontic treatment phase was initiated. Results: A trombone appliance proved effective in correcting the contracted mandibular arch. Because of labiolingual and transversal expansion, the mandibular dental arch perimeter was increased by 7.4 ㎜; the misalignment of the mandibular teeth was corrected successfully. Conclusions: A trombone appliance may serve as an appropriate clinical alternative for treating moderate mandibular arch crowding caused by the contraction of the dental arch.

        Stimulation of bone formation in the expanding inter-premaxillary suture by vitamin E, in rat

        Objective: The aim of this study was to evaluate the effects of vitamin E (α-tocopherol) administration on bone formation in response to expansion of the inter-premaxillary suture, in rats, histomorphometrically. Methods: Thirty 50 - 60 day old Wistar rats were separated into five equal groups (one control and four experimental). All groups were subjected to inter-premaxilla expansion with 50-gram of force. Six control animals received saline solution (Group I) and three experimental groups were treated with a single dose of α-tocopherol injected into the inter-premaxillary suture after one day after appliance placement (Group II: 2 mg/kg; Group III: 10 mg/kg; and Group IV: 50 mg/kg). A further group of six animals received three injections of 10 mg/kg α-tocopherol, one each on days 3, 6, and 9 (Group V). Bone formation in the suture was evaluated by bone histomorphometry. Kruskal-Wallis rank and Mann-Whitney U tests were used for statistical evaluation at p < 0.05 level. Results: New bone area, bone perimeter, feret?s diameter and newly formed bone measurements were significantly higher in the experimental groups than the control (p < 0.001). Bone architecture in α-tocopherol administrated groups was improved, and bone formation during the expansion period was stimulated significantly, in a dose-dependent manner. Conclusions: The application of α-tocopherol during the early stages to orthopedically expanded inter-premaxillary suture areas may stimulate bone formation and shorten the retention period, in rats. 본 연구는 백서에서 전상악골 봉합부의 확장시 비타민 E (α-토코페롤) 복용의 골형성 효과를 조직계측학적으로 평가하기 위해 시행되었다. 30마리의 50 - 60일된 백서를 6마리씩 5개 집단(대조군과 실험군 4개 집단)으로 분류하였다. 모든 실험 집단에서 50 gm의 힘으로 전상악부를 확장하였다. 장치 장착 1일 후, 대조군(Group I)에는 생리식염수를, 실험군(Group II, Group III, Group IV)에는 α-토코페롤을 각각 2 mg/kg, 10 mg/kg, 50 mg/kg씩 전상악골 봉합부에 주사하였다. 다른 실험군(Group V)에는 장치 장착 3, 6, 9일 후 각각 10 mg/kg씩의 α-토코페롤을 주사하였다. 10일간 유지한 뒤 봉합부에서의 골형성을 조직계측학적 방법으로 평가하였다. Kruskal-Wallis rank 및 Mann-Whitney U test를 사용하여 유의수준 0.05에서 통계분석하였다. 신생골 면적, 둘레, 직경의 계측치에서 실험군이 대조군보다 통계적으로 유의성있게 높은 수치를 나타내었다 (p < 0.001). α-토코페롤을 주사한 집단에서 골 내부구조가 향상되었고, 확장으로 인한골형성은 주사한 α-토코페롤 용량에 비례하여 유의성있게 증가되었다. 백서에서 전상악골 봉합부의 확장시 초기 단계에서의 α-토코페롤 주입은 골형성을 촉진하고 유지기간을 단축시킬 수 있을 것이다.
