        Flexor Tenosynovitis Caused by Neisseria gonorrhea Infection: Case Series, Literature Review, and Treatment Recommendations

        Neisseria gonorrhoeae is the most common sexually transmitted disease in the world and is known to cause disseminated disease,most commonly tenosynovitis. Classically, gonorrhea-associated tenosynovitis presents with concomitant dermatitis and arthralgias, though this is not always the case. N. gonorrhoeae-related tenosynovitis has become more commonly seen by hand surgeons. To aid in management, we present three cases of gonorrhea-induced tenosynovitis spanning a range of presentations with variable treatments to demonstrate the variety of patients with this disease. Only one of our patients had a positive gonococcal screening test and no patient had purulent urethritis, the most common gonorrhea-related symptom. A separate patient had the classic triad of tenosynovitis, dermatitis, and arthralgias. Two patients underwent operative irrigation and debridement, and one was managed with anti-gonococcal antibiotics alone. Though gonorrhea is a rare cause of flexor tenosynovitis, it must always be on the differential for hand surgeons when they encounter this diagnosis. Taking an appropriate sexual history and performing routine screening tests can assist in the diagnosis, the prescription of appropriate antibiotics, and potentially avoiding an unnecessary operation.

        Vehicular Ad-hoc Networks (VANETs) is a technology that has been recently emerged, and brings a lot of research interests. In this paper, various dimensions of VANETs including its emerging applications, security issues, challenges, security threats and the existing solutions proposed by the different researchers are studied. Also author reviewed various type of VANET simulator available and presented possible key research area of VANETs.

        Charged-particle multiplicity and transverse-energy distribution using the Weibull-Glauber approach in heavy-ion collisions

        <P>The charged-particle multiplicity distribution and the transverse-energy distribution measured in heavy-ion collisions at top RHIC and LHC energies are described using the two-component model approach based on a convolution of the Monte Carlo Glauber model with the Weibull model for particle production. The model successfully describes the multiplicity and transverse-energy distribution of minimum-bias collision data for a wide range of energies. The Weibull-Glauber model can be used to determine the centrality classes in heavy-ion collisions as an alternative to the conventional negative binomial distribution (NBD)-Glauber approach.</P>

        Influence of phytochemical composition on in vitro antioxidant and reducing activities of Indian ginseng [Withania somnifera (L.) Dunal] root extracts

        Background: Roots of Withania somnifera (WS) are a celebrated medicinal ingredient in Ayurvedic and many other indigenous systems of medicine. The present study investigates the effect of the phytochemical composition of the extracts on their antioxidant and reducing activities. Methods: WS roots were extracted with water, acetone, aqueous methanol (1:1), and methanol:- chloroform:water (1:1:1) to obtain aqueous, acetone, hydro-methanolic, and methanolechloroformewater extracts. Thereafter, phytochemical constitution and antioxidant and reducing activities of the extractswere compared using different qualitative and quantitative tests. Results: Maximum extraction recovery was obtained with 50% aqueous methanol whereas extraction with acetone yielded the poorest recovery. Methanolechloroformewater extract had the highest content of phytochemical constituents, except tannins, and also exhibited the highest antioxidant and reducing activities. Conclusion: Phytochemical composition and antioxidant and reducing activities of the extracts were positively associated with the use of organic solvents during the extraction process. Alkaloids and flavonoids were the most important contributors in the antioxidant and reducing activities of the extracts.

        Efficient finite element model for dynamic analysis of laminated composite beam

        An efficient one dimensional finite element model has been presented for the dynamic analysis of composite laminated beams, using the efficient layerwise zigzag theory. To meet the convergence requirements for the weak integral formulation, cubic Hermite interpolation is used for the transverse displacement (w0), and linear interpolation is used for the axial displacement (u0) and shear rotation (ψ0). Each node of an element has four degrees of freedom. The expressions of variationally consistent inertia, stiffness matrices and the load vector are derived in closed form using exact integration. The formulation is validated by comparing the results with the 2D-FE results for composite symmetric and sandwich beams with various end conditions. The employed finite element model is free of shear locking. The present zigzag finite element results for natural frequencies, mode shapes of cantilever and clamped–clamped beams are obtained with a one-dimensional finite element codes developed in MATLAB. These 1D-FE results for cantilever and clamped beams are compared with the 2D-FE results obtained using ABAQUS to show the accuracy of the developed MATLAB code, for zigzag theory for these boundary conditions. This comparison establishes the accuracy of zigzag finite element analysis for dynamic response under given boundary conditions.

        Molecular Imaging of CXCL12 Promoter-driven HSV1-TK Reporter Gene Expression

        The C-X-C motif chemokine 12 (CXCL12, SDF1a) and its receptor, CXCR4, play a fundamental role in several biological processes, including hematopoiesis, cardiogenesis, cancer progression, and stem cell migration. Noninvasive monitoring of CXCL12 is highly desirable for optimizing strategies that combine mobilization of therapeutic cells to combat cancer or to assist in cardiac tissue repair after myocardial infarction. Here, we report on an MRI reporter gene system for directly monitoring CXCL12 expression in vivo. Glioma cells and human adipose-derived stem cells (hADSC) were transduced with the herpes simplex virus type-1-thymidine kinase (HSV1- tk) reporter gene expressed under the CXCL12 promoter. HSV1-tk expression resulted in accumulation of the PET tracer [125I]FIAU in vitro and in vivo and induced cell death after ganciclovir treatment. Furthermore, the results show that conditional expression of the reporter gene can be induced by hypoxia in transduced cells. Transduced hADSC were incubated with the CEST MRI probe 5-methyl-5, 6- dihydrothymidine (5-MDHT) and transplanted into swine heart. Transplanted cells were clearly visible on Chemical Exchange Saturation Transfer (CEST) MRI using a 3T clinical scanner. Therefore, we conclude that it is possible to image CXCL12 expression with MRI in a large animal model, opening up a possible route to clinical translation.

        Facial feminization surgery (FFS) refers to a set of procedures aimed at altering the features of a masculine face to achieve a more feminine appearance. In the second part of this two-part series, assessment and operations involving the midface, mandible, and chin, as well as soft tissue modification of the nasolabial complex and chondrolaryngoplasty, are discussed. Finally, we provide a review of the literature on patient-reported outcomes in this population following FFS and suggest a path forward to optimize care for FFS patients.

        Facial feminization surgery (FFS) incorporates aesthetic and craniofacial surgical principles and techniques to feminize masculine facial features and facilitate gender transitioning. A detailed understanding of the defining male and female facial characteristics is essential for success. In this first part of a two-part series, we discuss key aspects of the general preoperative consultation that should be considered when evaluating the prospective facial feminization patient. Assessment of the forehead, orbits, hairline, eyebrows, eyes, and nose and the associated procedures, including scalp advancement, supraorbital rim reduction, setback of the anterior table of the frontal sinus, rhinoplasty, and soft tissue modifications of the upper and midface are discussed. In the second part of this series, bony manipulation of the midface, mandible, and chin, as well as soft tissue modification of the nasolabial complex and chondrolaryngoplasty are discussed. Finally, a review of the literature on patient-reported outcomes in this population following FFS is provided.
