        Mixed Nuts May Promote Satiety While Maintaining Stable Blood Glucose and Insulin in Healthy, Obese, and Overweight Adults in a Two-Arm Randomized Controlled Trial

        Multiple studies have examined the impacts of various nuts on appetitive and metabolic effects; however, fewer studies have addressed the impacts of mixed nuts. The objective was to examine the acute effects of consumption of an isocaloric (253 kcal) snack of mixed nuts or pretzels on appetite hormones, glucose and insulin responses, and subjective appetite ratings in overweight and obese adults. In a two-arm randomized controlled trial, overweight and obese adults consumed either mixed nuts (n = 27) or pretzels (n = 27) with 16 men and 11 women per group. After an overnight fast, blood glucose, insulin, and appetite hormone were measured at baseline and 60 min post snack consumption. Appetite/satiety questionnaires were completed at baseline and 20, 40, 60, 90, 120 min after snack consumption. Both snacks increased satiety and reduced hunger over time. The pretzels group exhibited higher subjective satiety and lower hunger (P < .001) following consumption than the mixed nuts group. Pretzel consumption increased glucose and insulin (P < .001), while no elevation was detected in the mixed nuts at 60 min post snack consumption. Leptin and ghrelin concentrations were significantly lower for the mixed nuts group at 60 min compared with baseline (P < .05). Mixed nuts promote satiety in overweight and obese adults while maintaining stable blood glucose and insulin levels. These results suggest that mixed nuts snack may be beneficial for those who are overweight or obese. Future mixed nuts studies are warranted to examine long-term effects.s

        Career planning is the basis of individual career development The objective of this article is to identify career planning indicators for TVET entrepreneurs from Skills Institute of MARA (IKM) and MARA Higher Skills College (KKTM) in Malaysia using Dephi Modified technique. Two rounds were conducted to obtain a quartile range (ROQ) so that the level of consensus among experts was high. ROQ value is the value obtained from the questionnaires that have been completed by entrepreneurs to get the value of the agreement on the career planning indicators. The findings show that two sub constructs found in the career development plan get a high consensus that is the focus of career planning and self efficacy in career planning where no item is eliminated and gets a high consensus. In this regard, it is important that this study is carried out to look at the items needed for an entrepreneur, especially entrepreneurs graduating from Technical Vocational Education Training (TVET) in Malaysia in planning his career well.

        A Review of Mercury Exposure and Health of Dental Personnel

        Considerable effort has been made to address the issue of occupational health and environmental exposure to mercury. This review reports on the current literature of mercury exposure and health impacts on dental personnel. Citations were searched using four comprehensive electronic databases for articles published between 2002 and 2015. All original articles that evaluated an association between the use of dental amalgam and occupational mercury exposure in dental personnel were included. Fifteen publications from nine different countries met the selection criteria. The design and quality of the studies showed significant variation, particularly in the choice of biomarkers as an indicator of mercury exposure. In several countries, dental personnel had higher mercury levels in biological fluids and tissues than in control groups; some work practices increased mercury exposure but the exposure levels remained below recommended guidelines. Dental personnel reported more health conditions, often involving the central nervous system, than the control groups. Clinical symptoms reported by dental professionals may be associated with low-level, long-term exposure to occupational mercury, but may also be due to the effects of aging, occupational overuse, and stress. It is important that dental personnel, researchers, and educators continue to encourage and monitor good work practices by dental professionals.

      • Exploring new treatment windows for immune checkpoint inhibitors in NSCLC

        With the success of immune checkpoint inhibitors in advanced NSCLC, we have seen a shift in its development to earlier stage disease. The interaction in earlier stage disease with treatments such as radiation and chemotherapy have the potential to enhance interactions between the immune system in tumour, with lower disease burden and more intact immunity in these patients versus in later stages. The presence of macroscopic tumour may be important to drive a durable immune response, thus timing of checkpoint inhibitors may be important in combination with radiation, chemotherapy and surgical therapy. Both chemotherapy and radiation have the potential to engage the innate and adaptive arms of the immune system, and potentially convert the tumour into an in-situ vaccine. In unresectable stage III NSCLC, durvalumab improves survival with maintenance durvalumab for 1 year post chemoradiation. Median survival in the durvalumab arm has not been reached while the hazard ratio for mortality is 0.69 (95% confidence interval 0.55-0.86). At 1, 2 and 3 years, 83%, 66% and 57% of patients were alive in the durvalumab arm versus 75%, 55% and 43% in the placebo arm. Subgroup analyses suggest that those with PDL1 tumour expression of 1% or more derive most of the benefit. In North America, durvalumab is offered to all patients while in Europe, its use is restricted to PD-L1 positive patients only. In subgroup analyses of EGFR mutation positive NSCLC patients, progression-free survival was improved but survival data have not been released. Ongoing studies looking at concurrent anti-PD-1 therapy with chemoradiation (CRT) and maintenance immunotherapy include Checkmate 73L (nivolumab + CRT +/- ipilimumab, Pacific 2 (Durvalumab + CRT + durvalumab), DECIPHER (Atezolizumab +CCRT v Atezolizumab + CT) and Rationale-001 with teslelizumab + CRT). Also there are trials looking at duration of therapy and immunotherapy after sequential chemoradiation. The recent NICOLAS and DETERRED phase II studies show the feasibility of nivolumab and atezolizumab with phase III trials ongoing. In resectable stage I-III NSCLC, several studies have now demonstrated the efficacy of anti-PD-1 monotherapy, including a preoperative study of nivolumab with 2 doses administered over 4 weeks yield a major pathologic response rate (MPR) of 45% and pathologic complete response (pCR) in 10%. Similar data have been shown in the NEOSTAR study with nivolumab (MPR 17%, pCR 9%) and LCMC3 (N=77; MPR 19%, pCR 5) with atezolizumab. Adding ipilimumab to nivolumab increases the response rate and highlights that CT scan alone is insufficient to identify pathologic response, and that nodal flare may signal benefit rather than progression. Combining chemotherapy with anti-PD-1 therapy yields very promising response rates. In the NADIM study from the Spanish Lung Cancer Group, preliminary response data included an overall response rate of 78.5% (N=46), MPR in 85% and pCR in an unprecented 61% of patients. This remains a very exciting area, with multiple ongoing trials of induction anti-PD-1 plus chemotherapy followed by adjuvant immunotherapy in resectable disease. Ongoing trials include Checkmate 77T (nivolumab), Keynote 671 (pembrolizumab), IMpower030 (atezolizumab) and AEGEAN (durvalumab). Whether this approach will be superior to upfront resection and adjuvant immunotherapy is unknown, with readout expected from adjuvant trials of PD-1 inhibitors in the next few years. These include the Korean-Canadian collaboration in CCTG BR.31 (adjuvant durvalumab), ANVIL (adjuvant nivolumab), PEARLS (adjuvant pembrolizumab) and IMPower 010 (Adjuvant atezolizumab). More recently, interest in novel immune therapies has led to the CANOPY-A study of adjuvant canakinumab. Thus while the phase III studies are ongoing, there is clear potential to dramatically improve outcomes by shifting immunotherapy into earlier stage disease, to help transform outcomes for more of our patients into cure.

      • The journey of immuno-oncology; changing landscape for cancer treatment

        First line therapy in advanced or metastatic non-small cell lung cancer (NSCLC) has undergone rapid changes in the past 15 years. Previously, doublet based chemotherapy was the standard of care, with the recent evolution of targeted therapy and now immunotherapy. Treatment selection depends on tumor pathologic subtype, the presence of targetable genomic aberrations (EGFR, ALK, ROS1, BRAF, NTRK), and PD-L1 tumour proportion score (TPS). If the patient’s tumor harbors an oncogenic driver including activating EGFR mutations, ALK, ROS-1 or TRK fusions, the optimal first-line treatment is targeted therapy. In the absence of oncogenic drivers, treatment selection is based on PD-L1 TPS. If PD-L1 TPS is ≥50%, pembrolizumab monotherapy is preferred, although combination chemotherapy-immunotherapy is an option. If PD-L1 TPS is <50%, combination chemotherapy-immunotherapy is recommended in good performance status patients, with different options for those with squamous and non-squamous pathology. Patient factors play a key role in treatment decision-making, first and foremost patient preference, but also performance status, comorbidities such as autoimmune disease, and steroid use.

      • New perspectives for the future treatment option in NSCLC

        With the exciting evolution of treatment in advanced non-small cell lung cancer to incorporate either targeted therapy or immune-oncology, the future of lung cancer treatment continues to evolve. Currently we have seen the transition of immunotherapy from the palliative stage IV setting to the curative stage III setting after radical chemoradiation for unresectable stage III NSCLC. Also we have seen the impact of immunotherapy in stage IV small cell lung cancer, with approximately 15% of patients deriving major long term benefit from anti-PD-1 agents in the IMPower 133 and CASPIAN trials. The use of immunotherapy is moving towards earlier stage disease, with dramatic benefits seen with single agent (preoperative nivolumab) and combination chemo-immunotherapy as preoperative treatment in resectable NSCLC (NADIM trial). There are multiple ongoing trials exploring the impact of adjuvant and peri-adjuvant checkpoint inhibitors either as anti-PD-1 monotherapy, combination with chemotherapy or combination anti-PD-1/anti-CTLA-4 therapy in the curative setting. In advanced disease, we must continue to develop our understanding of whether tumours are potentially sensitive to checkpoint inhibitors, and if not, why not. Is it a failure of antigen presentation or MHC machinery, a failure of T cell activation and infiltration or other challenges? With a better understanding of the immunogram of each person’s tumour, we will be able to design strategies to overcome treatment resistance, turning “cold” tumours “hot”. These may include combinations of chemotherapy and immunotherapy, combinations of checkpoint inhibitors (e.g. LAG-3 and PD-1), and targeted agents with immunotherapy, (e.g. VEGF and PD-1 inhibitors). All of these areas have growing promise with novel agents and ongoing trials. Cell therapeutics and vaccines have been more challenging to harness in lung cancer therapeutic development but ongoing studies may yet yield promise, including bespoke vaccines.

        Antecedents of Online Impulse Buying Behavior: An Empirical Study in Indonesia

        This study aims to determine and analyze the effect of social shopping, adventure shopping, value shopping, relaxation shopping, and idea shopping in influencing impulsive online buying behavior moderated by scarcity and serendipity information. The research method used is the quantitative research paradigm using surveys as a medium to obtain primary data. The paper examines the theoretical research model and tested fifteen hypotheses. The questionnaire was developed based on indicators from previous research. A non-probability sampling framework is used in this study. The data collection method uses electronic and online questionnaires to collect primary data with a total sample of 330 taken with the criteria of having made transactions in e-commerce Shopee in the last three months. Data analysis tools using Structural Equation Modelling (SEM) approach. The results showed that 8 out of 15 hypotheses were accepted and supported. The results show that there is a relationship between the value of hedonic shopping, scarcity, and serendipity information on impulsive online buying behavior. Therefore, analyzing the needs of customers, optimizing customer satisfaction, service excellence, website quality, and the ease of use of e-shopping itself especially in the e-commerce industry should be taken seriously nowadays.

        An Interdisciplinary Examination of the Material Effects of Deceptive Sport Beverage Advertisements

        Sport companies frequently make product claims in advertise- ments to influence consumer purchase decisions. Sport beverages, in particular, often tout health benefits and performance claims. Unfortunately, some sport beverage claims may be false or mis- leading, persuading consumers to erroneously purchase products due to incorrect information. The Federal Trade Commission (FTC) prohibits any deceptive advertising resulting in a material effect on the consumer. Consumer behaviorists, however, rarely consider the FTC’s guidelines in measuring consumer deception. Therefore, drawing upon consumer behavior theory and the FTC’s guide- lines, this paper examines the material effects of deceptive sport beverage advertisements. The authors identify and define three types of materiality: cognitive, affective, and behavioral. Employing a within-subjects experimental design, two studies were conducted: print and Facebook. The results reveal both types of deceptive advertisements had positive and statistically significant effects on all three types of materiality. These findings substantiate the determination of a violation by the National Advertising Division (NAD) and support further investigation of deceptive sport beverage advertising and marketing practices by the FTC.

        Social service purchasing in China: Rationale, features, and risks

        progressing an agenda of purchasing child welfare and other social services from the nongovernment sector, primarily to expand capacity and address vast unmet need. This paper draws on current research evidence to explore the approaches to purchasing emerging in China, examining the rationale for purchasing and models of supply, competition, and regulation. While some approaches are modeled on direct service contracting, direct purchasing of social service “posts” is also used, aimed at achieving goals of professionalization alongside service expansion. Overall, the review shows purchasing is helping to rapidly expand service scale and capacity; however, regulatory strategies for managing and mitigating risks to quality and access appear lacking. This highlights the need for further scholarship aimed at developing the robust risk management strategies which are required to support high quality, sustainable provision of purchased services.

        Nanoparticle technology and its implications in endodontics: a review

        Background: The era of Nanomaterials has had a long lasting impression in the field of medical science. It’s excellent use in medicine has led to its application in dental science. Serious concerns regarding the eradication of microbial biofilms from the root canal system still exists in the field of endodontics. Nanoparticles have proven to be much more efficient with good bonding capabilities and surface chemistry as compared to the conventional materials. The practical applications of nanotechnology in endodontics has led to future prospects in research in this field. Main body: Nanoparticles in endodontics have shown promising results. The various nanoaprticles like graphene, silver nanoparticles, chitosan, hydroxyapaptite nanoparticles, Iron compound, zirconia, Poly (lactic) co-glycolic acid, bioactive glass, mesoporous calcium silicate, titanium dioxide nanoparticles, Magnesium, Calcium oxide and Copper oxide have been discussed. These nanoparticles have fetched and shown great results in various application in endodontics like incorporation of nanoparticles in selaers, obturating materials, irrigation, and intracanal medicament. Conclusion: The application of nanoparticles from natural and synthetic materials is rapidly evolving in dentistry. These biomaterials have helped in treatment of oral diseases, in eradication of smear layer and biofilms, have been incorporated in various dental materials for their antimicrobial effects. Combining all their beneficial aspects, these nanoparticles will provide new paradigm shift in dentistry. This review on nanoparticles will provide the reader with the latest knowledge of these materials, their mechanism of action and its implications in endodontics.
