Transarterial chemoembolization (TACE) is recommended as the first line treatment option for the patients with Barcelona Clinic Liver Cancer (BCLC) stage B hepatocellular carcinoma (HCC), however, treatment strategy and evaluation of effects after TACE has not been fully established. Recently, sub-stage of BCLC stage B has been proposed and validated, but it should be validated including a large number of the patients and its refinement should be discussed. We have validated the sub-stage of BCLC stage B (B1-B4) by comparing overall survival after TACE, and there was no statistically significant difference in overall survival after TACE between B1 and B2. After excluding the patients with Child-Pugh point 7 from B1, the overall survival was significantly better than that of B2. Therefore, up-to-seven criteria is shown to be a reliable tool for the treatment strategy in the patients with intermediate stage of HCC. Refinement of sub-stage of BCLC stage B has been proposed by some other institutes, and it is important to establish novel treatment strategy for the patients with BCLC stage B after TACE to improve the prognosis of the patients after TACE, and to define the best timing for conversion to sorafenib or liver transplantation should be discussed. 어느 나라이건 근대이전의 거의 모든 회화에는, 그려진 이유와 기대되었던 기능이 있다. 도시가 회화화의 대상이 되는 경우도 마찬가지이다. 京都를 그린 洛中洛外 라는 畵題 역시 성립하게 된 데에는 필연성이 있었다. 이글에서는 洛中洛外圖가 어떤 경위로 성립되었고, 어떠한 용도로 사용되었는가에 대해 생각해 보고자 한다. 우선 제Ⅰ장에서는 京都 시가지를 그린다는 것이 어떠한 의미를 지니고 있는가를 개관하고, 동시에 洛中洛外圖의 성립시기에 대해서 고찰한다. 제Ⅱ장에서는 洛中洛外 의 기본적인 구성을 기술하고 그 구성의 특징을 두가지 시점으로 분석하고자 한다. 제Ⅲ장에서는 그러한 구성을 가지고 성립된 洛中洛外圖가 어떠한 기능을 가지고 있었는지 고찰하고, 최종적으로 洛中洛外圖의 기능과 京都를 그리는 것이 江戶時代가 되어 어떻게 변질되었는가를 개관하고자 한다.

<P>We discovered Dentilabus Heinrich (Icneumoninae: Platylabini) for the first time from the Eastern Palearctic region. Two new species, D. nigripodus sp. n. [Japan and Korea] and D. iyoensis sp. n. [Japan] are described. Dentilabus nigripodus resembles its Western Palearctic congener, D. variegatus (Wesmael), but they can be easily distinguished by the uniformly black hind femora in D. nigripodus. With the inclusion of D. iyoensis, we also redefine the generic limits of Dentilabus. A key to Palearctic species of the genus is provided. </P>

Primary penile melanomas are rare tumors that represent less than 0.1% of all melanomas. We report a case of a 60-year-old Japanese male with a mucosal penile melanoma and describe an increased CD8+ T cell infiltration in brain after dacarbazine (DTIC) administration. After partial penectomy and left inguinal lymphadenectomy, he developed multiple lung, bone, spleen, brain and skin metastases. He was treated with interferon-β, DTIC and nivolumab. However, the metastases were not reduced in size. Immunohistochemistry showed an increase of CD8+ T cell infiltration and programmed death-ligand 1 (PD-L1) expression after the administration of DTIC, but the expression of programmed cell death protein 1 (PD-1) was negative. We speculate that DTIC exerted immunostimulatory effects, but nivolumab was ineffective due to the negative expression of PD-1 and/or an insufficient infiltration of CD8+ T cells. Although this is only one case, this case report could be the first step to discuss the development of effective therapies against melanoma to take advantage of the increased CD8+ T cell in-filtration elicited by chemotherapeutic agents. It would be beneficial to pay more attention to the relationship between DTIC and immune checkpoint modulators. (Ann Dermatol 28(4) 486∼490, 2016)

Background: Most non-penetrating sham and placebo acupuncture needles comprise a traditional pedestal for fixing the guide tube that makes these needles difficult to operate independently. We developed a simple sham acupuncture needle to overcome this problem and focused on managing the needling sensation. Objectives: To ascertain how differently participants feel sham and real needles and to evaluate whether sham needles are effective in clinical trials. Methods: After enrolling 64 healthy volunteers who had experienced and were knowledgeable about acupuncture, the practitioner randomly used real and sham needles at four sites (bilateral LI4 and LI10) on the participants’ forearms. A custom-made sham blunt stainless acupuncture needle (40 mm, 0.80 mm) that was inserted into a guide tube was used. Immediately after needling, the participants were questioned in regard to their identification of the needle, skin penetration sensation, and de qi. Results: The sham needle resulted in 62.5% blinding. Inequivalent sensations were elicited by the sham needles compared to the real needles. Women reported similar needling sensations from the sham and real needles. Conclusion: This study achieved comparatively higher sham-needle blinding and is therefore worthy of use in clinical trials. The mutual independence of the sham needle from the real needle was mediated presumably by interindividual differences among the participants and the needling sites. Sex differences in sensation were likely related to the blinding capability of the sham needle.
Late-Onset Hypogonadism Syndrome and Lower Urinary Tract Symptoms

Androgen replacement therapy (ART) is a widely accepted form of treatment worldwide for aging men with late-onset hypogonadism (LOH) syndrome. Concurrent with the progressive decline in testosterone from middle age, there is a gradual increase in prostate volume, reflecting the development of benign prostatic hyperplasia (BPH). Prostate growth is dependent on the presence of androgens, and conversely, antiandrogen agents or orchidectomy can decrease prostate volume in patients with BPH. Thus, it is important to investigate whether ART could have any negative effects on prostatic disease or lower urinary tract symptoms (LUTS). Although only limited amounts of information on the correlations between androgen levels in aging men and clinical manifestations of LUTS are available, a few recent studies have suggested that testosterone levels may have some beneficial effects on various urinary functions in men. Androgen receptors are found in the urothelium, urinary bladder, prostate, and urethra, and testosterone could have an impact on the autonomic nervous system, bladder smooth muscle differentiation, nitric oxide synthase, phosphodiesterase-5 and Rho/Rho-kinase activities, and pelvic blood flow. In addition, some previous studies demonstrated that ART had little effect on LUTS or urinary function in aging men with LOH syndrome. Furthermore, some recent randomized controlled trials indicated that short-term ART may be effective in the improvement of LUTS in hypogonadal men with mild BPH. However, only limited information is available regarding the effects of longer-term ART or the safety of ART in men with severe BPH and LUTS, and further studies are required to reach more definitive conclusions.
