        Ureteral Reconstruction With Bowel Segments: Experience With Eight Patients in a Single Institute

        Purpose: Although replacement of the ureter with a bowel segment is indicated for largeureteral defects, it is still a challenging technique for urologists. We present our experienceand outcome of ureteral reconstruction using bowel segments. Materials and Methods: Ureteral reconstruction with bowel segments was performedin eight patients in our institute between 1969 and 2009. We investigated the positionand length of the ureteral defect and methods of reconstruction as well as the patients’backgrounds, postoperative complications, and clinical outcomes. Results: Five patients underwent ureteral replacement with isolated ileal segmentsalone. In one patient, the ureter was reconstructed by using the Yang-Monti procedurewith the ileum. A colon segment was used in two patients who required bladder augmentationfor tuberculous contracted bladder at the same time. Metabolic acidosis occurredin three patients having a solitary kidney and the ureter had to be replaced by a relativelylong intestinal segment. Two patients who received preoperative radiation therapywere required to undergo additional operations. Long-term cancer-free survivalwas achieved in one patient who underwent ileal substitution for low-grade renal pelviccancer. Conclusions: Although ureteral replacement with a bowel segment is a challenging anduseful procedure, attention must be paid to the possibility of metabolic acidosis, whichis likely to occur in patients having a solitary kidney with renal insufficiency or in patientsrequiring a long intestinal segment for reconstruction. In addition, preoperativeradiation therapy for the pelvic organs may cause postoperative complications.

        Cleft Lip and Palate Repair Using a Surgical Microscope

        Background Cleft lip and palate repair requires a deep and small surgical field and is usually performed by surgeons wearing surgical loupes. Surgeons with loupes can obtain a wider surgical view, although headlights are required for the deepest procedures. Surgical microscopes offer comfort and a clear and magnification-adjustable surgical site that can be shared with the whole team, including observers, and easily recorded to further the education of junior surgeons. Magnification adjustments are convenient for precise procedures such as muscle dissection of the soft palate. Methods We performed a comparative investigation of 18 cleft operations that utilized either surgical loupes or microscopy. Paper-based questionnaires were completed by staff nurses to evaluate what went well and what could be improved in each procedure. The operating time, complication rate, and scores of the questionnaire responses were statistically analyzed. Results The operating time when microscopy was used was not significantly longer than when surgical loupes were utilized. The surgical field was clearly shared with surgical assistants, nurses, anesthesiologists, and students via microscope-linked monitors. Passing surgical equipment was easier when sharing the surgical view, and preoperative microscope preparation did not interfere with the duties of the staff nurses. Conclusions Surgical microscopy was demonstrated to be useful during cleft operations.

      • Theoretical Study on Eco-Driving Technique for an Electric Vehicle with Dynamic Programming

        Eco-driving technique for electric vehicles (EVs) is investigated in this paper. Many findings on EVs have been reported; however, they did not deal with eco-driving from the viewpoint of theoretical study. The authors have developed an energy-saving driving technique - the so-called "eco-driving" technique based on dynamic programming (DP). Optimal speed profile of an EV, which minimizes the amount of total energy consumption, was determined under fixed origin and destination, running time, and track conditions. DP algorithm can deal with such complicated conditions and can also derive the optimal solution. Using the proposed method, simulations were run for some cases. In particular, the author ran simulations for the case of a gradient road with a traffic signal. The optimization model was solved with MATLAB.

        Phytobiotics to improve health and production of broiler chickens: functions beyond the antioxidant activity

        Phytobiotics, also known as phytochemicals or phytogenics, have a wide variety of biological activities and have recently emerged as alternatives to synthetic antibiotic growth promoters. Numerous studies have reported the growth-promoting effects of phytobiotics in chickens, but their precise mechanism of action is yet to be elucidated. Phytobiotics are traditionally known for their antioxidant activity. However, extensive investigations have shown that these compounds also have anti-inflammatory, antimicrobial, and transcriptionmodulating effects. Phytobiotics are non-nutritive constituents, and their bioavailability is low. Nonetheless, their beneficial effects have been observed in several tissues or organs. The health benefits of the ingestion of phytobiotics are attributed to their antioxidant activity. However, several studies have revealed that not all these benefits could be explained by the antioxidant effects alone. In this review, I focused on the bioavailability of phytobiotics and the possible mechanisms underlying their overall effects on intestinal barrier functions, inflammatory status, gut microbiota, systemic inflammation, and metabolism, rather than the specific effects of each compound. I also discuss the possible mechanisms by which phytobiotics contribute to growth promotion in chickens.       • Digital Watermarking Method Using Variance of Chrominance in the Pairs of Adjoining Blocks

        We propose a watermarking method that has robustness against geometrical slight distortions. The method is based on the variances of Cb and Cr components of an image. When a color image is geometrically distorted, Cb and Cr components on the same position cause the same distortion, and so the relation of variances of Cb and Cr components in a local region does not change easily. We embed each bit of a watermark into some adjacent local regions by changing the variances of Cb and Cr components so that the watermark may be robust to geometrical distortions.

      • Watermarking Method Resistant to Geometrical Slight Distortion Using Variance of Color Difference and Wavelet Transform

        In this paper, we propose a watermarking method that has robustness against geometrical slight distortions. The method is based on the variances of two color ceomponents of an image. We use YCbCr components. When an image is geometrically distorted, Cb and Cr components on the same position cause the same distortion, and the relation of variances of Cb and Cr components in a local domain does not change easily. So we embed a watermark by changing the variances of Cb and Cr components so that the watermark may be robust against geometrical distortions. Moreover we use discrete wavelet transform so as to be robust against JPEG compression.
