Femoral Avulsion of the Anterior Cruciate Ligament in a Multiligament Setting: A Unique Case

Anterior cruciate ligament (ACL) femoral avulsions are a rare entity mainly seen in children. We describe a unique case of a bony ACL avulsion with a medial collateral ligament (MCL) and lateral meniscus (LM) tear in a 37yearold patient who sustained a twowheeler accident. The ACL femoral avulsion was fixed arthroscopically using a Knee Scorpion and a FiberTape fixed over a suture disc on the lateral cortex. The MCL was repaired with an internal brace using a FiberTape and two SwiveLocks and the torn LM was repaired using allinside sutures. At 2 years of followup, the fragment was united, knee range of motion was 0° to 130°, and Knee Society Score was 98. This is the first case report of a femoral avulsion of the ACL combined with an MCL injury and a meniscus tear where all three were repaired with a unique technique and good results were obtained.

We describe for the first time a case of lateral femoral condyle fracture following a fall in a 13-year-old child that was reduced arthroscopically using a probe. It was definitively fixed with 0 vicryl sutures in a cruciate pattern after being initially stabilised with 1.5-mm Kirschner wires. Four beath pins carrying sutures were drilled at four opposing quadrants through the reduced fragment into the femur. The sutures were then tied on the lateral cortex of the femur. After fixation, the child was kept non-weight bearing for 6 weeks, partial weight bearing from 6 weeks to 12 weeks and then full weight bearing thereafter. Range of motion (ROM) exercises were commenced on the first postoperative day. At one-year follow-up, the ROM was from -5° to 130°, all symptoms disappeared, and complete resumption of all sports activities was allowed.

Anticoagulant-related nephropathy (ARN) was initially described in patients on warfarin (as warfarin-related nephropathy) and recently in those using dabigatran. Herein, we report clinical history and kidney biopsy findings in a patient on apixaban (Eliquis). Initiation of treatment with apixaban resulted in aggravation of preexisting mild acute kidney injury (AKI). A few days after apixaban therapy, the patient became oligoanuric, and kidney biopsy showed severe acute tubular necrosis with numerous occlusive red blood cell casts. Only one out of 68 glomeruli with open capillary loops had small segmental cellular crescent. Therefore, there was major discrepancy between the degree of glomerular injury and the glomerular hematuria. Considering that the onset of this AKI was associated with apixaban treatment initiation, we propose that this patient had ARN associated with factor Xa inhibitor (apixaban), which has not previously been described. Monitoring of kidney function is recommended after initiation of anticoagulant therapy.
Influence of Drone Self-magnetic Noise on Airborne Magnetic Exploration

Drones are already being used in various applications. In geophysical exploration, drones are expected not only as a technology to realize low-cost aerial surveys replacing aircraft, but also as a technology to open up new applications by high-density data acquisition at low altitudes, easy access to difficult-areas to enter and more efficient exploration than land surveys. However, geophysical exploration using a drone has only just begun to be considered. In this study, the authors examined magnetic exploration using a drone. A drone itself becomes a magnetic noise source. For this reason, a technique for reducing magnetic noise due to drone is required. A drone body acts as a DC noise. Driving the motor to control the drone body generates fluctuating magnetic noise. In this study, the authors carried out magnetic exploration using MagArrow which is developed by Geometrics (USA) as a magnetometer for a drone. The measurement was performed by hanging MagArrow three meters below from the drone. As a result, the magnetic noise caused by the drone was large during hovering. Swaying MagArrow like a pendulum during the measurement hardly affected as magnetic noise. On the other hand, the change in the flying speed of the drone becomes the magnetic noise, because the changes in motor power consumption generate magnetic noise. So, it was found that the hanging length of three meters from the drone was not sufficient during hovering, and it is important to keep the velocity constant during magnetic measurement.
