      • Poster Session : PS 0321 ; Hematology : Thromboembolism in Neurosurgical Patients: A Retrospective Cohort

        Background: Venous thromboembolism (VTE) includes deep vein thrombosis (DVT) and pulmonary embolism (PE). VTE represents the fi rst cause of preventable morbidity and mortality in neurosurgery, these patients have many additional risk factors for VTE, but thromboprophylaxis is discussed for threatened complications. Estimated annual incidence in the United States is 600, 000 cases, with a mortality of nearly 17% at 3 months after diagnosis. Objetive: To estimate the incidence of postoperative VTE in patients with neurosurgery and to describe thrombophylactic measures, assigned treatment and associated complications. Methods: Retrospective cohort of all patients (=17 years) who underwent a neurosurgical procedure from January 2010 to January 2012. All patients were followed up to 90 days to evaluate the presence of VTE and/or death. Baseline characteristics, thromboprophylaxis, treatment for VTE and complications were evaluated. Results: During two years, 321 patients met the inclusion criteria. VTE incidence was 3. 1% (10 cases, 95% CI:1. 2%-4. 9%), 5 patients developed DVT, the other half developed both DVT and PE. 52% were female, median age was 55 (RIC 35-66) years. From the total of patients, 71% (229) received thromboprophylaxis, from which 58% implemented mechanical thromboprophylaxis and 42% received associated pharmacologic prophylaxis. 4 of the 10 patients with DVT were receiving prophylaxis at the time of the event. Not receiving thromboprophylaxis increased 1. 5 times (95%, CI:0. 95-2. 38, p=0. 001) the risk of presenting VTE that in those who received thromboprophylaxis. The risk of developing PE was 4 times (95%, CI:0. 73-22%, p=0. 001) bigger in those who didn´t receive thromboprophylaxis. 80% of patients received treatment for VTE: anticoagulants (4), thrombolytics and anticoagulants (1), thrombolytics (1), thrombolytics and fi lter (1) and 1 received fi lter and anticoagulation. For complications, no ETV patients died, one required hospitalization and 6. 9% (22) had major bleedings. Conclusions: VTE remains an important complication in neurosurgical patients.

      • Evaluation of female masculinization in Pomacea canaliculata (Caenogastropoda, Ampullariidae) induced by tributyltin, heavy metals, and uranium in culture water

        All female Pomacea canaliculata develop a small, male-like copulatory apparatus a few days after birth, which growths slowly until sexual maturity, and even further in older age. Previous studies have found trace elements like mercury (Hg), arsenic (As) and uranium (U) in tap water used for snail culture, and that these elements were accumulated in snail tissues. Here, we test whether the presence of these metals at maximum allowed concentrations (Environmental Protection Agency - EPA) in aquarium water could affect the development of the copulatory apparatus in mature females. Females of different ages were used as controls, grown in reconstituted metal-free water with or without the addition of Hg, As and U, as well as tributyltin (TBT), a compound used as masculinizing agent. Six and seven months old females cultured in tap water showed a longer penis and penile sheath, and a greater overall development of the copulatory apparatus, measured by an index (DI), as compared with same-age females cultured in reconstituted water. Moreover, when females were exposed to Hg, As or U at the maximum contaminant levels for human consumption allowed by EPA regulations, there was no further development of the copulatory apparatus, while there was a clearly positive effect in TBT-exposed females. This study confirms the masculinizing effect of organotin compounds on female copulatory apparatus and discusses the usefulness of the development of these organs as a bioindicator of environmental pollution.

        Background: Rheumatoid Arthritis (RA) is the most common autoimmune disease worldwide. T cells and their cytokines play a key role in the etiopathogenesis of this illness. In the last two decades IL-17A has shown to play a major role in the disease pathogenesis. Previous research on the SNP (Single Nucleotide Polymorphism) rs3804513 has shown a link to radiological progression in patients with RA. Objetive: To determine the correlation between rs3804513 IL-17A SNP and the risk of developing RA, and assess the relation between rs3804513 SNP and the severity of the disease. Methods: Oral epithelial cells were obtained as source for genetic material. The DNA was ampli. ed using Polymerase Chain Reaction. Subsequently, a Restriction Fragment Length Polymorphism was performed. finally, in order to con. rm the SNP presence, direct sequencing of the DNA was performed. We used a case-control design. Results: 76 patients with RA and 94 healthy subjects were included. Of the 76 patients evaluated, 15% (n=12) tested positive for SNP rs3804513 vs 3 % (n=3) subjects in the control group. An odds ratio (OR) of 5.6 (p=0.01) for developing RA was obtained in RA patients carrying the studied SNP. Interestingly, a large percentage of patients who tested positive required 3 DMARDs to control the disease in comparison to patients who tested negative: 73% vs 32% (OR 5.6, p<0.01). Conclusions: The SNP rs3804513 is a risk factor for developing RA. Furthermore, carriers need more aggressive treatment to control the disease.

        Analysis of the transcripts encoding for antigenic proteins of Bovine Gammaherpesvirus 4

        The major glycoproteins of bovine gammaherpesvirus 4 (BoHV-4) are gB, gH, gM, gL, and gp180 with gB, gH, and gp180 being the most glycosylated. These glycoproteins participate in cell binding while some act as neutralization targets. Glycosylation of these envelope proteins may be involved in virion protection against neutralization by antibodies. In infected cattle, BoHV-4 induces an immune response characterized by low neutralizing antibody levels or an absence of such antibodies. Therefore, virus seroneutralization in vitro cannot always be easily demonstrated. The aim of this study was to evaluate the neutralizing capacity of 2 Argentine BoHV-4 strains and to associate those findings with the gene expression profiles of the major envelope glycoproteins. Expression of genes coding for the envelope glycoproteins occurred earlier in cells infected with isolate 10/154 than in cells infected with strain 07/435, demonstrating a distinct difference between the strains. Differences in serological response can be attributed to differences in the expression of antigenic proteins or to post-translational modifications that mask neutralizing epitopes. Strain 07/435 induced significantly high titers of neutralizing antibodies in several animal species in addition to bovines. The most relevant serological differences were observed in adult animals. This is the first comprehensive analysis of the expression kinetics of genes coding for BoHV-4 glycoproteins in 2 Argentine strains (genotypes 1 and 2). The results further elucidate the BoHV-4 life cycle and may also help determine the genetic variability of the strains circulating in Argentina.

        Background: Systemic Lupus Erythematosus (SLE) is highly associated with premature atherosclerosis and cardiovascular events. The origin of this subclinical atherosclerosis is not completely clear, but has been attributed mainly to infl ammatory nature of disease. The aim of this study was determine the effect of chronic glucocorticoids intake on cardiovascular risk profi le (CVRP) in patients with SLE. Methods: A registry-based retrospective cohort study. The study subjects consisted of all patients with SLE meeting the ACR criteria seen at the Division of Internal Medicine on a tertiary care hospital. We registered CVRP with Framingham score at baseline and at the end of follow-up, as well as cumulative dose of glucocorticoids. Then we analyzed the correlation between increase on RCVP and the cumulative dose of glucocorticoids. We also evaluated the change on Framingham score at baseline and at the end of follow-up. We used the Pearson correlation coeffi cient and one-sample T-Test. Analyses were performed with SPSS software version 19.0. Results: The cohort included 101 patients (94 female,7 male) with a mean age of 26.5±5SD years, mean age at study entry of 18.6±7SD years, mean disease duration 7.8±4.9SD years.The total follow-up was 7.8±4.9SD years. The mean daily dose of prednisone was 12±5SD mg. During 7.8 years of follow-up the Framingham score increased 8 points, from -8.1±4 at baseline to 0.8±7 at the end of study (p<0.0001).The Pearson correlation coeffi cient between the rise on RCVP and the cumulative dose of glucocorticoids was 0.74 (p<0.001). Conclusions: The chronic treatment with glucocorticoids plays a fundamental role on the high risk of cardiovascular disease in patients with SLE, and its effect is dose-dependent. The Framingham score is not an appropriate tool for predict the risk of cardiovascular disease in patients with SLE due their young age.

        Resistance to Hypoosmotic Shock of Liposomes Containing Novel Pigments from an Antarctic Bacterium

        Although the antioxidant capacity of carotenoids and their role in regulating membrane fluidity have been well studied, their ability to confer resistance to hypoosmotic shock is poorly understood. In this work, we analyzed the effect of a mixture of carotenoid pigments obtained from an Antarctic microorganism belonging to the genus Pedobacter on liposomal resistance to hypoosmotic conditions. Intercalation of pigments intoliposomal structures resulted in an improvement of membrane resistance by decreasing the percentage of calcein released in comparison to that by liposomes without pigments. Due to these properties, such pigments could be useful for biotechnological applications.

        Antioxidant Capacity of Novel Pigments from an Antarctic Bacterium

        In Antarctica microorganisms are exposed to several conditions that trigger the generation of reactive oxygen species, such as high UV radiation. Under these conditions they must have an important antioxidant defense system in order to prevent oxidative damage. One of these defenses are pigments which are part of the non-enzymatic antioxidant mechanisms. In this work we focused on the antioxidant capacity of pigments from an Antarctic microorganism belonging to Pedobacter genus. This microorganism produces different types of pigments which belong to the carotenoids group. The antioxidant capacity of a mix of pigments was analyzed by three different methods: 1,1-diphenyl-2-picrylhydrazyl, ROS detection and oxygen electrode. The results obtained from these approaches indicate that the mix of pigments has a strong antioxidant capacity. The oxidative damage induced by UVB exposure to liposomes was also analyzed. Intercalated pigments within the liposomes improved its resistance to lipid peroxidation. Based on the analysis carried out along this research we conclude that the antioxidant properties of the mix of pigments protect this bacterium against oxidative damage. These properties make this mix of pigments a powerful antioxidant mixture with potential biotechnological applications.

        Pseudotumoral Presentation of Cerebral Amyloid-Beta Angiopathy: Case Report and Review of Literature

        Objective Cerebral amyloid angiopathy-related inflammation (CAA-RI) is a rare and potentially treatable encephalopathy that usually affects people older than 50 years old and has an acute or subacute clinical presentation characterized by rapidly evolving cognitive decline, focal deficits and seizures. In a small subset of patients the disease can adopt a pseudotumoral form in the neuroimages that represents a very difficult diagnostic challenge. Methods Here in we report a patient with a tumour-like presentation of histopathologically confirmed CAA-RI. Results We also conducted a search and reviewed the clinical and radiological features of 41 cases of pseudotumoral CAA-RI previously reported in the literature in order to identify those characteristics that should raise diagnostic suspicions of the disease, there by avoiding unnecessary surgical treatments.Conclusion The therapy of CAA-RI with steroids is usually effective and clinical and radiological remission can be achieved in the first month in approximately 70% of cases.

        Treatment of Osteochondritis Dissecans of the Knee with Autologous Iliac Bone Graft and Hyaluronic Acid Scaffold

        Osteochondritis dissecans (OCD) is a condition that corresponds to an idiopathic focal lesion affecting the subchondral bone with possible compromise of the stability of the adjacent cartilage. Treatment depends on the size of the lesion, cartilage stability, and the physeal status. The case reported is about an 18­year­old male patient who complained of suffering from knee pain for a period of ten months. Magnetic resonance imaging (MRI) revealed a lesion of 2 cm2 in the medial femoral condyle that compromised the subchondral bone, compatible with OCD. He underwent surgery that consisted of filling the subchonral defect with an iliac crest autograft and sealing the defect with a hyaluronic acid scaffold. At the 12­month follow­up, the MRI shows complete healing and the patient has resumed sports activities. Management with autologous iliac crest graft and hyaluronic acid scaffold represents an effective alternative treatment for OCD.
