        Percutaneous Microwave Ablation of Desmoid Fibromatosis

        Objective: To demonstrate the feasibility of percutaneous microwave ablation in desmoid fibromatosis with respect to tumor volume control and improvement in the quality of life. Materials and Methods: Twelve microwave ablations were performed in 9 patients with a histological diagnosis of desmoid fibromatosis between January 2010 and January 2019. The study population included 6 female and 3 male, with an age range of 21–76 years (mean = 46.6 years; standard deviation [SD] = 19.3 years). The mean major axis of the tumors was 10.9 cm (SD = 5.2 cm) and mean lesion volume was 212.7 cm3 (SD = 213 cm3). Their anatomical distribution was as follows: 3 lesions in the thigh, 2 in the gluteus, 2 in the leg and 2 in the periscapular region. We evaluated the reduction in tumor volume and improvement in the quality of life based on the Eastern Cooperative Oncology Group (ECOG) scale. Results: An average tumor volume reduction of 70.4% (SD = 24.9) was achieved, while the quality of life (ECOG scale) improved in 88.9% of patients. Conclusion: Percutaneous microwave ablation may potentially be a safe, effective, and promising technique for controlling tumor volume and improving the quality of life in patients with desmoid fibromatosis.

      • UNNOTICED MAGNETIC FIELD OSCILLATIONS IN THE VERY QUIET SUN REVEALED BY SUNRISE/IMaX

        <P>We present observational evidence for oscillations of magnetic flux density in the quiet areas of the Sun. The majority of magnetic fields on the solar surface have strengths of the order of or lower than the equipartition field (300-500 G). This results in a myriad of magnetic fields whose evolution is largely determined by the turbulent plasma motions. When granules evolve they squash the magnetic field lines together or pull them apart. Here, we report on the periodic deformation of the shapes of features in circular polarization observed at high resolution with SUNRISE. In particular, we note that the area of patches with a constant magnetic flux oscillates with time, which implies that the apparent magnetic field intensity oscillates in antiphase. The periods associated with this oscillatory pattern are compatible with the granular lifetime and change abruptly, which suggests that these oscillations might not correspond to characteristic oscillatory modes of magnetic structures, but to the forcing by granular motions. In one particular case, we find three patches around the same granule oscillating in phase, which means that the spatial coherence of these oscillations can reach 1600 km. Interestingly, the same kind of oscillatory phenomenon is also found in the upper photosphere.</P>

        Transanal Evisceration Caused by Rectal Laceration

        Transrectal evisceration caused by colorectal injury is an unusual entity. This pathology is more frequent in elderly patients and it is usually produced spontaneously. Rectal prolapse is the principal predisposing factor. An 81-year-old woman was taken to the hospital presenting exit of intestinal loops through the anus. After first reanimation measures, an urgent surgery was indicated. We observed the absence of almost every small intestine loop in the abdominal cavity; these had been moved to the pelvis. After doing the reduction, a 3 to 4 cm linear craniocaudal perforation in upper rectum was objectified, and Hartmann’s procedure was performed. We investigated and knew that she frequently manipulate herself to extract her faeces. The fast preoperative management avoided a fatal conclusion or an extensive intestinal resection. Reasons that make us consider rectal self-injury as the etiologic factor are explained.

        A parametric study of optimum tall piers for railway bridge viaducts

        This paper presents a parametric study of reinforced concrete bridge tall piers with hollow, rectangular sections. Such piers are typically used in railway construction of prestressed concrete viaducts. Twenty one different piers have been studied with seven column heights of 40, 50, 60, 70, 80, 90 and 100 m and three types of 10-span continuous viaducts, whose main span lengths are 40, 50 and 60 m. The piers studied are intermediate columns placed in the middle of the viaducts. The total number of optimization design variables varies from 139 for piers with column height of 40 m to 307 for piers with column height of 100 m. Further, the results presented are of much value for the preliminary design of the piers of prestressed concrete viaducts of high speed railway lines.

        Curcumin-loaded Polycaprolactone/Collagen Composite Fibers as Potential Antibacterial Wound Dressing

        The development of wound dressings with therapeutical benefits is of great importance in skin tissue engineeringapplications, adding bioactive molecules into biomaterials is a strategy to achieve a better biological response. In this study,four different concentrations of curcumin (CUR; 5, 10, 15 and 20 by weight in relation to the PCL content) were incorporatedinto solutions composed of polycaprolactone (PCL) and collagen (COL) for the manufacture of electrospun fibers. The PCLCOL-CUR fibers were physicochemically characterized in terms of their morphology, wettability, degradation rate,mechanical behavior, and cumulative curcumin release. The in vitro biological properties of the composite membranes werealso evaluated. The results indicated that the membranes have diameters on average of approximately 200 nm. The wateruptake was adequate for exudates remotion in a wound, and the degradation rate of the fibers was highly appropriate toachieve complete skin tissue regeneration. The addition of CUR to composite membranes produced a significant increase inthe mechanical properties which indicate a satisfactory clinical handling. The incorporation of CUR produced a significantdecrease in the planktonic growth of S. aureus over time, however, the antibacterial effect against E. coli was limited, thepresence of CUR did not cause the inhibition of its growth. Finally, the viability of human dermal fibroblasts seeded on thetop of the membranes indicated the cytotoxic dosage effect of CUR, the two highest CUR concentrations produced asignificant loss of cell viability. Overall, our results suggested that the CUR-loaded PCL-COL composite membranes arepromising candidates for use as antibacterial dressings to enhance clinical wound management.

        Long-term evolution of continence and quality of life after sphincteroplasty for obstetric fecal incontinence

        Purpose: This study was performed to evaluate the long-term evolution of continence and patient’s quality of life after surgical treatment for obstetric fecal incontinence. Methods: A prospective longitudinal study was conducted including consecutive patients who underwent sphincteroplasty for severe obstetric fecal incontinence. The first phase analyzed changes in continence and impact on quality of life. The second phase studied the long-term evolution reevaluating the same group of patients 6 years later. Degree of fecal incontinence was calculated using the Cleveland Clinic Score (CCS). Quality of life assessment was carried out with the Fecal Incontinence Quality of Life scale. Results: Thirty-five patients with median age of 55 years (range, 28 to 73 years) completed the study. Phase 1 results: after a postoperative follow-up of 30 months (4 to 132 months), CCS had improved significantly from a preoperative of 15.7±3.1 to 6.1±5.0 (P<0.001). Phase 2 results: median follow-up in phase 2 was 110 months (76 to 204 months). The CCS lowered to 8.4±4.9 (P=0.04). There were no significant differences between phases 1 and 2 in terms of quality of life; lifestyle (3.47± 0.75 vs. 3.16±1.04), coping/behavior (3.13±0.83 vs. 2.80±1.09), depression/self-perception (3.65±0.80 vs. 3.32± 0.98), and embarrassment (3.32±0.90 vs. 3.12±1.00). Conclusion: Sphincteroplasty offers good short-medium term outcomes in continence and quality of life for obstetric fecal incontinence treatment. Functional clinical results deteriorate over time but did not impact on patients’ quality of life.

        Clinical, Psychopathological, Physical, and Sleep Evolution in Adolescents with Restrictive Anorexia Nervosa Participating in a Day Hospital Program

        Objective To analyze the clinical, psychopathological, physical, and sleep-related evolution of adolescents with restricting-type of anorexia nervosa (AN-R) after 10 weeks of a daytime hospital program. Methods Body composition, physical activity and sleep were measured objectively before and after 10 weeks of treatment. In addition, psychopathology and body image disturbances were measured with a self-report questionnaire. Results Fourteen female adolescents with AN-R (14.3±1.6 years old) participated in the study. A significant increase was found in eight of the ten variables for body composition (p<0.05). There were no significant changes in psychopathology, body image disturbances or physical activity. Concerning sleep, a significant, moderately standardized and substantial increase in night latency was found (p=0.002), and there was a significant, small standardized and substantial decrease in night efficiency (p=0.035). Conclusion After 10 weeks of follow-up with adolescent patients with AN-R who attended a day hospital program, there was a positive evolution of body composition. However, with regard to sleep patterns, there was a worsening of latency and night efficiency. Therefore, sleep care should be addressed in acute treatment programs for adolescents with AN-R.

        Performance of TMDs on nonlinear structures subjected to near-fault earthquakes

        Tuned mass dampers (TMD) are devices employed in vibration control since the beginning of the twentieth century. However, their implementation for controlling the seismic response in civil structures is more recent. While the efficiency of TMD on structures under far-field earthquakes has been demonstrated, the convenience of its employment against near-fault earthquakes is still under discussion. In this context, the study of this type of device is raised, not as an alternative to the seismic isolation, which is clearly a better choice for new buildings, but rather as an improvement in the structural safety of existing buildings. Seismic records with an impulsive character have been registered in the vicinity of faults that cause seismic events. In this paper, the ability of TMD to control the response of structures that experience inelastic deformations and eventually reach collapse subject to the action of such earthquakes is studied. The results of a series of nonlinear dynamic analyses are presented. These analyses are performed on a numerical model of a structure under the action of near-fault earthquakes. The structure analyzed in this study is a steel frame which behaves as a single degree of freedom (SDOF) system. TMD with different mass values are added on the numerical model of the structure, and the TMD performance is evaluated by comparing the response of the structure with and without the control device.

        Food Hypersensitivity in Mexican Adults at 18 to 50 Years of Age: A Questionnaire Survey

        Purpose: There is limited epidemiological evidence of food hypersensitivity (FH) in the adult population. We aimed to determine the prevalence of FH in Mexican adults, their clinical features and to establish common food involved in its appearance. Methods: We designed a cross-sectional study using a fixed quota sampling; 1,126 subjects answered a structured survey to gather information related to FH. Results: The prevalence of FH in adults was 16.7% (95% CI, 14.5% to 18.8%), without statistical significant differences related to gender (women, 17.5% and men, 15.9%) or residential location. The most common clinical manifestations in adults with FH were oral allergy syndrome (70 of 1,126) and urticaria (55 of 1,126). According to category, fruits and vegetables were the most frequent foods to trigger FH (6.12%) and were individually related to shrimp (4.0%), and cow milk (1.5%). Adults under age 25 had a higher frequency of FH (OR, 1.39; 95% CI, 1.01 to 1.91, P <0.001). Personal history of any atopic disease was significantly associated with FH (P <0.0001). Conclusions: The prevalence of FH is relatively high in Mexican adults, and FH is significantly associated with atopic diseases.
