Is intravesical stent position a predictor of associated morbidity?

Purpose: Temporary drainage of the upper urinary tract by use of internal ureteral stents is a common procedure that is often associated with a variety of symptoms. The role of intravesical stent position in associated morbidity is controversial. Materials and Methods: The German version of the ureteral stent symptom questionnaire (USSQ) was completed by 73 patients with an indwelling ureteral stent the day before stent removal. Intravesical stent position was classified into 3 categories by x-ray before stent removal. The influence of intravesical stent position on USSQ score was analyzed, including subscores and single items. Results: Intravesical stent position showed no significant influence on associated morbidity. The median USSQ total score in all patients was 77.5 (range, 30–147). Patients with ipsilateral stents (69.0; range, 30–122) tended to have lower total scores than did those with tangential (86.5; range, 30–122) or contralateral (77.0; range, 31–147) stents, but the differences were not statistically significant (p=0.35). The USSQ subscores for urinary symptoms (p=0.80), body pain (p=0.80), general health (p=0.16), work performance (p=0.07), additional problems (p=0.81), and all of the USSQ single items of interest in the context of stent length also did not differ significantly between the three groups. Conclusions: Intravesical stent position did not significantly influence associated morbidity in our study. An appropriate stent length should be chosen to avoid dislocation. However, complex calculations of optimum stent length, time-consuming manipulations, and costly stock holding of various stent sizes to obtain the perfect stent position do not seem worthwhile.

Anatomical variations of carotid arteries may be related to their development(agenesis, aplasia, hypoplasia) or course (coiling, kinking, tortuosity). Partial ortotal aberrancies in carotid vessel anatomy rarely occur. We describe the case of a95-year-old woman presented with sudden onset of confusion and disorientationtogether with upper limb clonus. Computed tomography (CT)-scan revealed aleft frontal brain injury with a not conclusive carotid doppler ultrasound. CTangiography reported a bovine aortic arch with bilateral retroesophageal courseof both common carotid arteries and left severe (>70%) internal carotid arterystenosis. The knowledge of anatomical variations of the course of carotid arteries isrelevant for possible surgical or endovascular repair or in case of otolaryngology orintubation procedures.
Stress and strain state in the segmental linings during mechanized tunnelling

The application of the mechanized tunnelling has been extended in recent years. There are at present different approaches that are used in the design of segmental tunnel linings supported in mechanized tunnels. Even though segmental lining is utilized for mechanized tunnels, its behaviour is still quite unclear under in situ stress and there is a lack of data regarding the distribution of stresses inside segmental linings. So far no single effective calculation method exists for segmental lining design. The lack of clear solutions makes the use of segmental lining to be more expensive due to the adoption of greater safety factors. Therefore, a particular attention must be given in order to obtain data from monitored tunnels which permits to validate design methods. In this study, strain measurements, which were conducted during the construction of twin tunnels in the Bologna-Florence railway line, have been presented. The behaviour of segmental lining during the excavation and the influence of a new tunnel excavation on an existing tunnel have been shown through the measured data. The data are then compared with the results obtained with Einstein and Schwartz's method and Duddeck and Erdmann's method, which permits to highlight the fact that the two analytical methods underestimate structural forces induced in the segmental lining and then must be used with caution.

Nucleoside analogues play an important role inantiviral, antibacterial and antineoplastic chemotherapy. Herein we report the synthesis, structural characterizationand biological activity of some 40-C-methyl- and -phenyldioxolane-based nucleosides. In particular, a and banomers of all natural nucleosides were obtained andcharacterized by NMR, HR-MS and X-ray crystallography. The compounds were tested for antimicrobial activityagainst some representative human pathogenic fungi, bacteriaand viruses. Antitumor activity was evaluated in alarge variety of human cancer cell-lines. Although most ofthe compounds showed non-significant activity, 23aweakly inhibited HIV-1 multiplication. Moreover, 22a and32a demonstrated a residual antineoplastic activity, interestinglylinked to the unnatural a configuration. Theseresults may provide structural insights for the design ofactive antiviral and antitumor agents.
