Background/Aims: Acute exacerbations of chronic hepatitis B (CHB-AEs) are common in endemic areas and are often presumed to be acute hepatitis B (AHB) due to their similarities in clinical and serological pictures, presenting a major diagnostic dilemma. This study aimed to identify laboratory markers for differentiating between the two groups, and to establish the cut-off value for significant markers. Methods: A retrospective analysis of records was conducted for patients who presented with clinical features of acute hepatitis along with hepatitis B surface antigen (HBsAg) and IgM antibody to hepatitis B core antigen (IgM anti-HBc) positivity from May 2015 to May 2017. A total of 172 patients were enrolled and grouped as AHB (n=89) and CHB-AE (n=83) based on their history of hepatitis B virus infection and duration of HBsAg persistence. Virological and biochemical parameters were analyzed and compared. Cut-off values, sensitivity, and specificity of the variables were calculated. Results: The median value of signal by cut-off (S/Co) ratio for IgM anti-HBc was significantly higher in AHB group (30.44) compared to CHB-AE group (8.63) with a sensitivity and specificity of 97% and 84%, respectively, at a cut-off of 20.5 (P<0.01). The mean international normalized ratio (INR) was significantly greater in CHB-AE (1.88±1.24) group compared to AHB group (1.62±0.17) with a sensitivity and specificity of 57.9% and 45.1%, respectively, at a cut-off value of 1.27. Conclusions: A value of 20.5 S/Co of IgM anti-HBc and 1.27 INR could be helpful in differentiating between AHB and CHB-AE.
Dietary Lysine Requirement of Juvenile Yellowtail Flounder Pleuronectes ferrugineus

The lysine requirements of juvenile yellowtail flounder (Pleuronectes ferrugineus) having 19.5 g initial body weight were estimated by feeding six practical-type diets containing graded levels of lysine (1.21 to 2.69% of dry diet). Dietary amino acid profile simulated that of whole body of yellowtail flounder. Most of amino acids in the diets were provided by corn gluten meal, herring meal and gelatin. Protein efficiency ratio (PER) improved significantly until lysine level increased up to 2.1% (4.3% of protein). Same trend was observed in feed:gain ratio (FGR) which maintained constant in fish groups fed diets containing lysine above 2.1%. The highest nitrogen gain (0.34 g/fish) in whole body was found in fish fed 2.1% lysine, though the value was not different from those of fish fed above the level of lysine. Fish fed 2.1% lysine also showed the best nitrogen retention efficiency of 24.6%. The broken-line analysis of protein efficiency ratio and body nitrogen gain against dietary lysine level yielded an estimated lysine requirement of 2.2% (4.5% of protein) and 2.3% (4.7% of protein), respectively.

Twenty small-scale farms of two villages (A and B) were surveyed to identify the feeding traits, milk productivity and nutritional status of lactating cattle and buffalo in Terai, Nepal. Constituents and dry matter (DM) of feed supplied, body condition score (BCS), heart girth (HG), bodyweight (BW), milk yield (MY) and plasma metabolites were obtained in the pasture-sufficient, pasturedecreasing and fodder-shortage periods. Milk yield of 305-day lactation was estimated by the daily MY. The supplies of rice straw and native grass were lower and higher in the pasture-sufficient period than in the other periods, respectively (5.5 kg/day vs. 9.8 kg/day and 3.2 kg/day vs. 0.4 kg/day, respectively, p<0.01). The roughage-supplement rates of the animals were higher in village A than in village B (5.0 vs. 2.2 in cattle and 9.3 vs. 1.8 in buffalo, p<0.01). The variance of feed constituents among the periods and between the villages induced different supplies of CP, NDF and TDN. The concentrations of CP and TDN in the cattle feed were higher in the pasturesufficient period than in the other periods (9.1% vs. 7.3% and 57.4% vs. 51.0%, respectively, p<0.01). The supplies of CP for cattle and buffalo, and of TDN for buffalo were lower in village A than in village B (7.5% vs. 8.7% and 6.6% vs. 9.1% [p<0.01], and 53.1% vs. 56.2% [p<0.05], respectively). The BCS, HG and BW of the animals were lower in village A than in village B (2.51 vs. 2.86, 156 cm vs. 170 cm and 300 kg vs. 318 kg, respectively in cattle, 2.83 vs. 4.00, 186 cm vs. 216 cm and 429 kg vs. 531 kg, respectively in buffalo, p<0.01). The cattle yielded more milk in the pasture-sufficient period than in the other periods (7.9 liters/day vs. 6.6 liters/day, p<0.01). The 305-day MY of cattle that calved in the fodder-shortage period was lower than that of cattle that calved in the other periods (1,900 liters vs. 2,251 liters, p<0.01). The MYs of cattle and buffalo were lower in village A than in village B (6.2 liters/day vs. 8.1 liters/day and 3.7 liters/day vs. 7.7 liters/day, respectively, p<0.01). The 305-day MY of cattle was lower in village A than in village B (1,935 liters vs. 2,409 liters, p<0.01). The concentrations of plasma albumin and urea nitrogen in cattle were lower in village A than in village B (3.2 g/dl vs. 3.4 g/dl [p<0.01] and 7.4 mg/dl vs. 10.2 mg/dl [p<0.05], respectively). The different supplies of CP, NDF and TDN among the periods and between the villages might have affected MY and nutritional status in cattle and buffalo. It was likely that the lower supplies of CP and TDN for cattle that calved in the fodder-shortage period and in village A lowered the 305-day MY of cattle.
Altered Doppler flow patterns in cirrhosis patients: an overview

Doppler ultrasonography of the hepatic vasculature is an integral part of evaluating precirrhotic and cirrhotic patients. While the reversal of the portal venous flow is a well-recognized phenomenon, other flow patterns, although not as easily understood, may play an important role in assessing the disease status. This article discusses the different characteristic flow patterns observed from the portal vein, hepatic artery, and hepatic vein in patients with liver cirrhosis or related complications and procedures. Knowledge of these different flow patterns provides additional information that may reinforce the diagnosis of cirrhosis, help in staging, and offer prognostic information for determining the direction of therapy. Doppler ultrasonography is invaluable when liver transplantation is being considered and aids in the diagnosis of cirrhosis and portal hypertension.
The Market Development of PM Aluminum

The Application of powder metallurgy (PM) aluminum structural parts is at its early growing stage, despite of some automotive applications. The market potential for PM aluminum, however, is large. Growth is expected from the market expansion of the existing applications and new applications, including the replacements of aluminum and zinc based castings and some ferrous PM automotive parts by PM aluminum. Compared to castings, PM is an efficient mass production technology. The PM aluminum is more competitive than die casting for some automotive applications. Besides weight saving leading to performance improvement, the total cost increase for aluminum PM parts is less than 15% compared to ferrous PM automotive parts. The future is promising for PM aluminum.
