        Small Size Autograft versus Large Size Allograft in Anterior Cruciate Ligament Reconstruction

        Background: A small autograft diameter negatively affects functional outcomes, knee stability, and the risk of rerupture after anterior cruciate ligament (ACL) reconstruction, whereas the strength of allograft decreases over time. Therefore, it is not clear whether the use of smaller autografts or the use of larger allografts in ACL yields better results. The aim of this study was to compare the outcome of smaller autografts and larger allografts for ACL reconstruction. Methods: Fifty-one patients who underwent ACL reconstruction with hamstring tendon autografts (size ≤ 8 mm) and 21 patients who underwent ACL reconstruction with allografts (size ≥ 10 mm) were included in our study. All patients underwent the same aggressive early postoperative rehabilitation program. There were no significant differences between the autograft and allograft groups regarding the preoperative patient age, sex, time from injury to surgery, and average follow-up time. Results: The mean diameter of the 4-stranded hamstring tendon grafts used as autografts was 7.48 ± 0.33 mm and the mean diameter of the allografts was 10.76 ± 0.67 mm. According to specific tests for the ACL (anterior drawer, Lachman, and pivot shift) and clinical evaluation tests (Lysholm knee scoring scale and International Knee Documentation Committee questionnaire), the final follow-up results were significantly better than the preoperative status in both autograft and allograft ACL reconstruction groups. Therefore, there were no significant differences between the autograft and allograft groups preoperatively and at the final follow-up. Conclusions: The large size of the graft in ACL reconstruction has been reported to affect results positively. However, in our study, we could not find any significant differences between the smaller size autografts and larger size allografts in terms of inadequacy, rerupture, and final follow-up functional results. Although allografts were significantly larger than autografts, we did not have the positive effect of larger size grafts. Smaller size autografts were as effective as the larger size allografts.

        Emulating the Turkish Experience through European Neighborhood Policy-Prospects for Democratization in the Arab and Muslim World Considered

        A widespread consensus among scholars on the Turkey-EU accession talks in recent years is their overall positive impact on the Turkish democracy. Can the European Neighborhood Policy (ENP) have a similar effect on other neighboring target Muslim countries? This paper seeks to answer the question by overviewing some of the main democratization efforts in Turkey since the 1990s. The following section then compares the Turkish experience with other Muslim countries (those with a Muslim-majority population) that are ENP partners, by stressing the differences. The final section concludes by discussing the prospects of democratization in the latter based on these findings, and the potential roles that the ENP can play to further promote the ongoing process.

        Safety and efficacy analysis of ibrutinib in 32 patients with CLL and various B-cell lymphomas: real-world data from a single-center study in Turkey

        Background Ibrutinib is an oral irreversible Bruton’s tyrosine kinase inhibitor. Here, we demonstrate the efficacy and safety of ibrutinib using real-life data from patients with marginal zone lymphoma (MZL), diffuse large B-cell lymphoma (DLBCL), Waldenström macroglobulinemia (WM), and follicular lymphoma (FL), especially in chronic lymphocytic leukemia (CLL) and mantle cell lymphoma (MCL). Methods This is a retrospective, observational, non-interventional, and single-center study on 32 patients who received ibrutinib treatment between January 2017 and March 2020 regardless of their diagnosis. Results Of the 32 patients, 11 had CLL and 21 had other B-cell lymphomas. Patients with CLL were prescribed ibrutinib for a median of 4 months (range, 2‒18). In this group, diarrhea was observed in 3 (27.3%), pneumonia in 3 (27.3%), and thrombocytopenia and/or neutropenia in 2 (18.2%) patients. The overall response rate (ORR) was 85.6 % [28.5 % complete response (CR) and 57.1 % partial response (PR)] in the final response assessment during treatment with ibrutinib. Among other types of B-cell lymphoma, seven (33.4%) of the 21 patients were diagnosed with MCL, 5 (23.8%) with DLBCL, 4 (19.0%) with MZL, 3 (14.3%) with WM, and 2 (9.5%) with FL, upon follow-up. The median treatment duration was 4 months (range, 1‒28) in this group. The most common adverse event was diarrhea: 8 (38.1%) patients. The ORR was 66.6% (20.0% CR and 46.6% PR) in the final response assessment during the treatment. Conclusion Ibrutinib is a good treatment option for CLL and other B-cell lymphomas, with an acceptable side effect profile, and high and promising CR/PR response rates. Ibrutinib treatment at an early stage decreases the burden of cytotoxic therapy in fragile patients, thereby, increasing their quality of life.

        Size effect on strength of Fiber-Reinforced Self-Compacting Concrete (SCC) after exposure to high temperatures

        This pioneer study investigates the size effect on the compressive and tensile strengths of fiber-reinforced selfcompacting concrete (FR-SCC) with different specimens, before and after exposure to elevated temperatures. 432 selfcompacting concrete (SCC) specimens with two concrete grades (50 and 80MPa) and three steel fiber ratios (0%, 0.5% and 1%) were prepared and tested. Moreover, based on the experimental results, new formulations were proposed to predict the residual strengths for different specimens. A parametric study was also carried out to investigate the accuracy of proposed formulations. Residual strength results showed that the cylinder specimen with dimensions of 100×200 mm was the most affected, while the cube with a size of 100 mm maintained a constant difference with the standard cylinder (150×300 mm). Temperature effect on the cube specimen (150 mm) was the least in comparison to other specimen sizes and types. In general, provision of steel fibers in SCC mixtures resulted in a reduction in temperature effect on the variance of a conversion factor. Parametric study results confirm that the proposed numerical models are safe to be used for all types of SCC specimens.
