      • Successful Use of the Hybrid Assistive Limb for Care Support to Reduce Lumbar Load in a Simulated Patient Transfer

        Study Design: Prospective experimental study in humans.Purpose: To determine whether the hybrid assistive limb (HAL) for Care Support can reduce lumbar load during a patient transfer.Overview of Literature: The prevalence of work-related low back pain (LBP) among nurses is high. In particular, transferring patients poses a high risk for LBP due to the large lumbar load. Attempts to reduce the lumbar load are crucial to avoid the risk of LBP. Therefore, we investigated the effects of the HAL for Care Support.Methods: Nineteen volunteers (16 men, three women) lifted a 60-kg doll from a seated position to a standing position. The first transfer was performed without the HAL for Care Support, and the second was performed with the HAL for Care Support assistive robot. We evaluated transfer performance, the visual analog scale (VAS) score for lumbar fatigue, and electromyogram analyses of the trunk and hip.Results: Four participants (two men, two women) succeeded with the HAL for Care Support even though they were unable to perform the task without it. The mean lumbar fatigue VAS score for all participants without the HAL for Care Support was 62 mm, while that with it was 43 mm. With lumbar assistance from the HAL for Care Support, subjective lumbar fatigue during the transfer decreased significantly. A power analysis indicated adequate statistical power to detect a difference in the VAS score for lumbar fatigue (0.99). The activity of the left gluteus maximus alone increased significantly during transfers with the HAL for Care Support. No adverse events occurred during use of the HAL for Care Support for transfers.Conclusions: The HAL for Care Support was able to reduce lumbar load in a simulated patient transfer.

        해방 후 한국에서의 사회교육학설의 형성에 관한 연구 (Ⅰ)

        한국에서는 식민지 해방 후의 혼란, 한국전쟁, 군사정권, 북한과의 군사적 대립 등과 같은 역사적 경로를 거치고 있다. 또한 ‘사회교육법’의 성립에 있어서는 1952년에 처음 ‘사회교육법 시안’이 작성되지만, 한국전쟁에서 사회가 혼란하고, 입법화 할 수 없었다. 그 후 사회교육법 시안은 작성, 계속 수정되고, 1982년에 간신히 제정·공포되게 되었다.본 연구는 이와 같은 경위를 가지는 일본 식민지 해방 후의 한국에 있어서 사회교육의 제도나 실태, 사회교육에 관한 연구가 구체화 되어 가는 데에 있어서, 사회교육학설이 어떻게 형성되어 온 것인지를 해명 해 나가는 데 관심이 있다. 여기에서는 한국 사회교육연구의 제일인자인 황종건의 사회교육론을 채택하고 있다. 황종건의 교육이념은 다음과 같은 것이다. 개인은 사회 문화의 이해, 공유에 의해서 사회의 한 구성원이 된다. 사회는 개인의 문화 형성 과정을 통해서 문화의 전승, 발전과 자기의 지속적인 발전을 실현하는 것이다. 이러한 개인의 자기교육과 사회 진보를 가능하게 하는 것이 교육이다. 황종건은 모든 인간이 자기실현의 기회를 얻어서 사회적, 문화적, 직업적으로 적응할 수 있도록, 교육 본래의 기능이 발휘할 수 있도록 하는 사회교육을 조직·운영하기 위해서 정열을 가지고 민주적, 선구적이고 지도적인 역할을 다하여 왔다. In Korea, historical circumstances such as confusion after colony release, the Korean war. a military regime, and military confrontation, are followed. Although the social education law tentative plan was created for the first time also in enactment of the social education law in 1952, in the Korea war, society was able to get confused and was not able to was enact. Then, the social education tentative plan was created, continued being corrected, and was enacted and proclaimed in 1982. This research solves how the social education theory has been formed, when the research on the system of the social education in Korea after the Japanese colony release with such circumstances, the actual condition, and social education takes shape. As on method of solving, the social education theory of Jong-Gon Hwang which is the leading authority of the Korean social education research is taken up. All the human beings got the opportunity of self-actualization, Jong-Gon Hwang had passion on order to organize and manage social education by which the function of educational original supported so that it can be socially, cultural, and occupational adapted is demonstrated, he played a role of folk, pioneering and leadership sure enough,

        Spectrum of Pulmonary Fibrosis from Interstitial Lung Abnormality to Usual Interstitial Pneumonia: Importance of Identification and Quantification of Traction Bronchiectasis in Patient Management

        Following the introduction of a novel pathological concept of usual interstitial pneumonia (UIP) by Liebow and Carrington in 1969, diffuse interstitial pneumonia has evolved into UIP, nonspecific interstitial pneumonia (NSIP), and interstitial lung abnormality (ILA); the histopathological and CT findings of these conditions reflect the required multidisciplinary team approach, involving pulmonologists, radiologists, and pathologists, for their diagnosis and management. Concomitantly, traction bronchiectasis and bronchiolectasis have been recognized as the most persistent and important indices of the severity and prognosis of fibrotic lung diseases. The traction bronchiectasis index (TBI) can stratify the prognoses of patients with ILAs. In this review, the evolutionary concepts of UIP, NSIP, and ILAs are summarized in tables and figures, with a demonstration of the correlation between CT findings and pathologic evaluation. The CT-based UIP score is being proposed to facilitate a better understanding of the spectrum of pulmonary fibrosis, from ILAs to UIP, with emphasis on traction bronchiectasis/bronchiolectasis.

        Pathological changes in various organs in HLA-B*57:01 transgenic mice with abacavir-induced skin eruption

        Several patients with cutaneous adverse drug reactions exhibit extracutaneous organ damages, and it becomes severe in a few patients resulting in death due to multiorgan failure. Understanding the sequential changes in various organs in patients with cutaneous eruption following drug administration will help understand disease onset and progression, aiding the development of prevention strategies and interventions. Therefore, we aimed to understand the effects of abacavir (ABC) on various organs in patients with ABC-induced eruptions by evaluating its effects in a mouse model. We found pathological changes in various organs of HLA-B*57:01 transgenic mice (B*57:01-Tg) following oral administration of ABC (20 mg/ body/day). B*57:01-Tg exhibited a significant body weight decrease from day 1 of ABC administration, and reddening of the auricle was observed from day 5, and approximately 2/3 mice died by day 7. Histopathological examination revealed severe thymic atrophy after day 3, infiltration of inflammatory cells, predominantly lymphocytes with neutrophils, not only in the skin but also in the liver, kidney, and lung after day 5, and an increased number of lymphocytes with enlarged nuclei and granulocytic hematopoiesis were observed in the spleen after day 5. Blood chemistry revealed that albumin/globulin ratio was below 1.0 on day 5, reflecting a systemic inflammatory response, and the aspartate aminotransferase concentration rose to 193 ± 93.0 U/L on day 7, suggesting that cell damage may have occurred in various organs including liver accompanying inflammatory cell infiltration. These examinations of a mouse model of ABC-induced skin eruption show that disorders in various organs other than the skin should be considered and provide insights into the unexpected early systemic responses dependent on HLA-B*57:01.

        Dynamic Chest X-Ray Using a Flat-Panel Detector System: Technique and Applications

        Dynamic X-ray (DXR) is a functional imaging technique that uses sequential images obtained by a flat-panel detector (FPD). This article aims to describe the mechanism of DXR and the analysis methods used as well as review the clinical evidence for its use. DXR analyzes dynamic changes on the basis of X-ray translucency and can be used for analysis of diaphragmatic kinetics, ventilation, and lung perfusion. It offers many advantages such as a high temporal resolution and flexibility in body positioning. Many clinical studies have reported the feasibility of DXR and its characteristic findings in pulmonary diseases. DXR may serve as an alternative to pulmonary function tests in patients requiring contact inhibition, including patients with suspected or confirmed coronavirus disease 2019 or other infectious diseases. Thus, DXR has a great potential to play an important role in the clinical setting. Further investigations are needed to utilize DXR more effectively and to establish it as a valuable diagnostic tool.
