      • 고관절 치환술후 무균성 해리와 골용해 조직에서 세포사멸

        목적 : 인공 고관절 치환술 후 실패의 가장 중요한 원인이 되는 무균성 해리및 골용해와 nitric oxide(NO)에 의한 염증세포 사멸의 연관성을 규명하기 위해 본 연구를 시행하였다. 대상 및 방법 : 무균성 해리와 골용해로 인해 재치환술을 받은 18명의 환자의 조직에서 세포들의 사멸을 Terminal Deoxyribonucleotidyl Transferase-Mediated dUTP Nick End Labeing (TUNEL) 방법을 이용하여 검사하였으며, inducible nitric oxide synthase(iNOS)와 미토콘드리아 관련 세포사멸 경로를 p53, Bax, Bcl2, Ki67 항체를 이용해서 면역 염색하였다. 대조군으로 대퇴 경부 골절 환자 6명의 관절낭을 이용하였다. 결과 : 16명(89%)에서 iNOS, p53, Bax 항체에 양성 반응을 보였으며, 12명(67%)에서 Bcl2, Ki-67 항체에 양성반응을, 전례에서 TUNEL에 양성반응을 보였다. 대조군에서는 1명(17%)에서 iNOS와 Bax에, 3명(50%)에서 TUNEL에 양성반응을 보였다. 무균성 해리와 골용해 조직의 세포사멸에 대한 양성반응의 발현빈도가 대조 조직에서 보다 유의하게 높았다. 결론 : Nitric oxide에 의한 염증세포의 사멸이 인공고관절 치환술 후 생기는 무균성 해리와 골용해에 작용함을 알수 있었다. 이 결과는 해리 및 골용해의 예방과 치료방법을 개발하는데 도움을 줄 수 있다. Purpose : To evaluate the relationships between the apoptosis induced by nitric oxide(NO), aseptic loosening and osteolysis, which are the most serious causes of failure after total hip arthroplasty. Materials and Methods : Apoptosis of the inflammatory cells of interface tissues from 18 patients who underwent revision hip arthroplasty was identified by Teminal Deoxyribonucleotidyl Transferase-Mediated dUTP Nick End Labeling(TUNEL). The reaction to immunostaining of inducible nitric oxide synthase(iNOS), p59, Bax, Bcl2, and Ki-67 were evaluated. Six joint capsules obtained from six patients of femoral neck fracture were studied as controls. Results : Sixteen(89%) of 18 interface tissues were positive for iNOS, p53, and Bax monoclonal antibody and twelve(67%) for Bcl2 and Ki-67 monoclonal antibody. All tissues were positive for TUNEL. In the control group of six joint capsules, only one(17%) was positive for iNOS and Bax, and three(50%) were positive for TUNEL. The incidences of apoptiotic signals at the interfacial tissue of aseptic loosening and osteolysis were significantly greater than those of the control. Conclusion : The current study suggested that the apoptosis of inflammatory cells, due to oxidative stress by NO, might be involved in the development of implant loosening and osteolysis after total hip arthroplasty. This information might be crucial for the treatment and prevention of periprosthetic osteolysis and subsequent loosening.

        Associations of serotonergic genes with poststroke emotional incontinence

        <P><B>Objectives</B></P><P>Poststroke emotional incontinence (PSEI) has been associated with serotonergic dysfunction. Polymorphisms of serotonin transporter (5‐HTT) and serotonin 2a receptor (5‐HTR2a) genes may regulate serotonergic signaling at brain synapses, and this study was to investigate associations with PSEI in an East Asian population.</P><P><B>Methods</B></P><P>In 276 stroke cases, PSEI was diagnosed by Kim's criteria. Covariates included age, gender, education, history of depression or stroke, current depression, and stroke severity and location. Genotypes were ascertained for 5‐HTT gene‐linked promoter region (5‐HTTLPR), serotonin transporter intron 2 variable number tandem repeat, 5‐HTR2a 1438A/G, and 5‐HTR2a 102 T/C. Associations with PSEI were estimated by using logistic regression models, and gene–gene interactions were investigated by using the generalized multifactor dimensionality reduction method.</P><P><B>Results</B></P><P>PSEI was present in 37 (13.4%) patients. The 5‐HTT gene‐linked promoter region <I>s</I>/<I>s</I> genotype was independently associated with PSEI. No associations with STin2 VNTR and 5‐HTR2a genes were found, and no significant gene–gene interactions were identified.</P><P><B>Conclusions</B></P><P>Stroke patients with 5‐HTTLPR <I>s</I> allele had higher susceptibility to PSEI, which underlines the potential role of serotonergic pathways in its etiology.         ‘-게 되다’ 구성의 통사와 의미에 대한 연구

        This paper aims to explain the syntactic and semantic properties of “-ge doeda” construction and to suggest its syntactic and event structure are based on the Generative Lexicon Theory proposed by Pustejovsky(1995). Like other auxiliary predicate construction, “-ge doeda” construction also conveys the ambivalence of its syntax. The auxiliary predicate “doeda” can be a predicate of the whole sentence and forms one constituent with “-ge” clause, although it can create different constituents from “-ge” clause with the same VP. Thus, the “-ge doeda” construction takes “-ge” clause as an event argument. This is evidence for Kim’s(2004, 2007) suggestion that the auxiliary predicate construction is an event function. In Korean linguistics and in Korean language education, many previous studies have explained that the “-ge doeda” construction is passive because of its affectedness and the lack of intentionality of its subject. However, passive expressions and causative expressions with similar morphemes and the passive voice are different from the change-of-state verb. According to this evidence, “-ge doeda” construction expresses change-of-state and is an unaccusative predicate construction. The change-of-state of “-ge doeda” construction can be classified as a binary-oppositional change-of-state and the polar-oppositional change-of-state can be classified by the property of the “-ge” clause that is an event argument. This is critical evidence for the principle of compositionality.

      • 조혈모세포이식 후 발생한 주폐포자층 폐렴에 대한 고찰

        Background : Pneumocytitis cainii pneumonia (PCP) can occur in immunocompromised hosts especially such as AIDS or cancer patients. Although recent research had focused on PCP in AIDS patients, few studies have described the clinical presentations of PCP in recipients of stem cell transplantation (SCT). We evaluated the clinical manifestations of PCP in SCT patients admitted at St. Mary's hospital, Seoul, Korea. Methods : The medical records of 17 PCP patients undergoing SCT between Feb. 1998 and Feb. 2000 were reviewed. The diagnosis of PCP was confirmed through the demonstration of Pneumocytitis cainii via either cytology of brochoalveolar lavage (BAL) or histological technique of lung biopsy. CMV disease and CMV infection were confirmed by BAL culture and antigenemia respectively . Results : Seventeen patients were all recipients of allogeneic SCT and 7 of 17 patients were performed non-sibling SCT. Patients presented with symptoms including brief period (4 ∼23 days) of fever (76%), dyspnea (70%), cough (64%), and signs such as rare(58.8%), Sixteen patients (94%) had been receiving immunosuppressive agent such as cyclosporine A (64%) or Fk506 (35%) without PCP prophylaxis. Eleven patients (64%) were treated with corticosteroid with mean dose of 16 mg/day prednisolone and mean duration of 4.6 months after post-SCT period. Twelve patients were co-infected with CMV. Another co-infected miCroorganisms were Pseudomonas aeruginosa, Mycobacterium tuberculosis, herpes simplex virus, parainfluenza virus, Average duration of treatment with trimethoprim-sulfamethoxazole (TMP/SMX) was 21 ±9 days. Four patients died, and three of them were related with PCP. Conclusion : PCP developed frequently in patients who were taking immunosuppressive drug due to graft versus host disease or were not taking TMP/SMX prophylaxis. High risk patients showing fever, cough, or dyspnea should be considered to take early bronchoscopic intervention for detection of PCP. When treat for PCP, it also be considered to the possibility of coinfection such as CMV. (Korean J Infect Dis 33:273∼279, 2001)

        Erythrophagocytosis of Lead-Exposed Erythrocytes by Renal Tubular Cells: Possible Role in Lead-Induced Nephrotoxicity

        <P>Background: Nephrotoxicity associated with lead poisoning has been frequently reported in epidemiological studies, but the underlying mechanisms have not been fully described.</P><P>Objectives: We examined the role of erythrocytes, one of the major lead reservoirs, in lead-associated nephrotoxicity.</P><P>Methods and results: Co-incubation of lead-exposed human erythrocytes with HK-2 human renal proximal tubular cells resulted in renal tubular cytotoxicity, suggesting a role of erythrocytes in lead-induced nephrotoxicity. Morphological and flow cytometric analyses revealed that HK-2 cells actively phagocytized lead-exposed erythrocytes, which was associated with phosphatidylserine (PS) externalization on the erythrocyte membrane and generation of PS-bearing microvesicles. Increased oxidative stress and up-regulation of nephrotoxic biomarkers, such as NGAL, were observed in HK-2 cells undergoing erythrophagocytosis. Moreover, TGF-β, a marker of fibrosis, was also significantly up-regulated. We examined the significance of erythrophagocytosis in lead-induced nephrotoxicity in rats exposed to lead via drinking water for 12 weeks. We observed iron deposition and generation of oxidative stress in renal tissues of lead-exposed rats, as well as the histopathological alterations such as tubulointerstitial lesions, fibrosis, and up-regulation of KIM-1, NGAL, and TGF-β.</P><P>Conclusions: Our data strongly suggest that erythrophagocytosis and subsequent iron deposition in renal tubular cells could significantly enhance nephrotoxicity following lead exposure, providing insight on lead-associated kidney damages.</P><P>Citation: Kwon SY, Bae ON, Noh JY, Kim K, Kang S, Shin YJ, Lim KM, Chung JH. 2015.       • 개발도상국가에서 공해산업도입과 관련된 환경정책에 관한 연구

        본 연구는 생산시설의 국제적 이동과 환경보호에 대한 개발도상국가의 현행정책과 이에 따른 효율성과 공평성에 대해서 논의하고 분석하였다. 분석에 의한 결과는 다음과 같다. 생산시설의 이동은 두지역간의 상호이익과 관련이 있는데, 두 지역간 상호이익이 발생하는 원인은 두 지역간의 임금차이이다. 환경보호에 대한 국가간 차별정책도 이런 이동을 일으킬 수 있으나, 그것이 상호이익의 원인으로는 간주되지 않는다. 즉 공평성과 효율성에 영향을 주는 주요인자는 임금차, 거래비용, 관세율 그리고 환경훼손이다. 개발도상국가가 생산시설을 유지함으로서 이윤을 얻게 되는 경우는 시장임금율이 사회적 기회비용보다 높고 시장교환율이 외환의 사회가치보다 낮은 경우이다. This study has investigated efficiency and equity issues associated with the current policy of LDC's regarding environmental protecion and international movement of production facilities and the results of the analysis are as follows; There is room for mutual benefits for two countries involvep in movements of facilities. The only real source of such mutual benefits is the difference in wages between the two countries. The differential treatment of environmental effects might prompt such movement, but cannot be considered as a source of mutual benefits The key variables affecting the efficiency and distributional issues are wage differentials transaction costs, the extent of environmental damages and the rate of tariff If the market wage rate is higher than the Social opportunity cost of labor and market exchange is lower than the social value of fo reign exchange, The LDC may gain benefits from intemational movement of production facilities.
