        Afferent loop syndrome is an uncommon complication of a gastrectomy and Billroth Ⅱ reconstruction. It may cause symtoms at any time from the first postoperative day to many years after the gastrectomy. Afferent loop syndrome is characterized by abdominal pain, vomiting and elevation of serum amylase. Thus, it is difficult to differentiate afferent loop syndrome from other cause of acute pancreatitis. However, the history of gastrectomy can be an important clue for diagnosing afferent loop syndrome. We experienced one case of chronic afferent loop syndrome with acute pancreatitis. After appropriate management, the abdominal pain disappeared and serum amylase level decreased. We report this case with a review of relevant literatures.

        한우 myostatin 유전자의 SNP 및 발현분석

        Myostatin is a transforming growth and differentiation factor-β family member that acts as a negative regulator of muscle growth. Previously, mutations in the myostatin gene were known to be related to double muscling phenotypes in cattle. Because myostatin gene is highly related to muscle mass, also meat quality, in cattle, we sequenced whole myostatin mRNA and investigated the SNPs (Single Nucleotide Polymorphisms) in Korean cattle (Hanwoo). The results indicated that Hanwoo had an SNP in nt2385 and this mutation can be a useful marker with further verifications. We also investigated expression patterns of the myostatin gene from various muscle tissues and organs. Northern blotting results indicated that myostatin expression was restricted in muscles with variable expression levels. The results presented here can be used as a valuable information for meat quality related traits and muscle mass in cattle.

        In applying linear programming to the production cost for generation planning, the probabilistic features of random loads and random generations cannnot normally be treated. So, in the conventional methods, some simplifications or ommittances are usually made. This paper proposes a new and effieient approach to the "probabilistic" production cost by the stadard linear programming package. The random loads and the random generations are discretized so that the production costs can be in linear form with linear constraints, and the convolution of load and generation is performed. The convolution itself is also a very time-consuming process, but an efficient method is suggeted in the paper. The proposal method is tested in a real scale power system and the various results are compared. From this demonstrations, it is concluded that the proposed method is a powerful tool for the probabilistic production costing and it is expected that the proposed method can further be extended for the generation planning.

        제 3형 구상돌기 골절의 수술적 치료

        목적: 8례의 제 3형 척골 구상돌기 골절에 대해 수술적 치료 후 임상적 방사선학적 결과를 보고하고자 한다. 대상 및 방법 : 8명의 구상돌기 골절에 대한 후향적 연구를 시행하였다. 대상은 모두 남자였으며 평균 나이는 33세였다. 3례에서 척골 구상돌기 단독 골절이었으며 2례에서 주관절 탈구가, 2례에서 요골 두 및 경부 골절이, 1례에서 내측 측부인대 손상이 동반되었다. 전례에서 전방 도달법을 통해 유관나사를 이용한 관혈적 정복 및 내고정이 시행되었다. 평균 추시 기간은 31개월 이었으며 (24~60개월), Mayo Elbow Performance Score로 임상적 결과를 판정하였다. 결과 : 최근 추시에서 평균의 능동적 주관절 가동범위는 105°였다. 평균의 Mayo Elbow Performance Score는 76.9점이었다(50~95). 1례에서 최우수, 4례에서 우수, 2례에서 양호, 1례에서 불량의 결과를 보였다. 결론 : 제 3형의 척골 구상돌기 골절에서 조기의 관혈적 정복 및 안정적인 내고정으로 신뢰할 만한 치료를 제공하였으며, 손상 주관절의 동반된 인대손상과 복합골절은 중요한 예후인자로 간주되었다. Purpose: To review the clinical results of eight cases of typeⅢ coronoid process fractures which were treated operatively. Materials and Methods : Eight patients with coronoid type Ⅲ were reviewed retrospectively. All were men with an average age of 33. There were three isolated fractures, two elbow dislocations, two radial head and neck fractures, and one medial collateral ligament rupture. Open reduction and internal fixation through anterior approach with canulated screws was used. The patients were followed up for a mean of 31 months(24 to 60). Results: Average active elbow joint motion at the most recent follow up was 105°. The average Mayo Elbow Performance Score was 76.9 (50 to 95). There was one excellent result, four good, two fair, and one poor. Conclusion: Early open reduction and stable internal fixation provided a reliable method for the treatment of typeⅢ coronoid process fractures. Any associated ligament injuries to the elbow and fracture comninution were considered as important prognostic factors.

        6 시그마 기법을 이용한 임상약동학 자문 업무의 효율성 개선 : 신속성과 정확성을 중심으로 Focusing on Speed and Accuracy of CPS

        연구 배경 : 적정한 약물요법을 순발력 있게 시행하기 위해서는 CPS 업무가 신속, 정확하게 수행되는 것이 필수적임에도 불구하고 지금까지 CPS 업무의 신속성, 정확성을 향상시키기 위한 객관적인 업무 표준은 물론 그의 측정까지도 전무한 실정이었다. 이에 수요자인 의사의 측면에서, CPS 자문결과의 신임도에 영향을 미치는 업무의 순발력과 정확성을 향상시키기 위해 CPS 업무의 신속성, 정확성에 영향을 미치는 요인을 파악, 분석하고 이를 향상시킬 수 있는 업무 표준을 개발하는 것을 목표로 6-Sigma 기법을 사용하여 본 연구를 진행하였다. 방 법 : CPS 업무의 신속성 평가를 위해 전체 업무 과정과 부분별 업무 과정에 대한 소요 시간을 측정하였고, 정확한 평가를 위해 예측되는 Cpeak와 실제 측정된 Cpeak를 비교하였다. CPS 업무에 대해 6-Sigma 기법을 사용하여 개선이 필요한 사항을 파악하고 이에 대한 개선활동을 한 후, 개선 활동 전과 후의 CPS 업무의 신속성과 정확성의 σ 수준을 각각 측정하여 이의 향상 정도를 잠재적인 경제적 이익으로 환산하였다. 결 과 : 신속성은 제 1차 자료수집 기간(대조군, 현황 파악 기간) 중 총 소요시간은 평균 930.6±583.2이었으며, 채혈-검체 접수 : 140.4±8705분, 검체접수-약물농도 결과보고: 48.4±23.1분, 결과보고-CPS 보고서 작성 : 741.8±510.6분으로 측정되었다. σ 수준은 총 소요시간의 경우는 0.56σ, 단계별로는 각각 0.45σ, 5.70σ, 0.30σ이었다. 제 2차 자료수집 기간(개선군, 개선활동 평가 기간) 중 총 소요시간은 평균 659.1±344.1분이었고 단계별로는 각각 92±66.2분, 48±28.0분, 518.9±340.7분이 소요되었다. σ 수준은 총 소요시간은 1.75σ이고, 단계별로는 1.32σ, 4.70 σ, 1.10σ로서 총 소요시간과 단계별 소요시간이 1차 자료수집기간에 비해 향상되었다. 정확성 측면에서는, 대조군의 경우, 예측 Cpeak와 실측 Cpeak의 차이는 평균은 0%, 분산 ±27.10%, σ 수준은 -0.56σ이었고 개선군의 평균과 분산 그리고 σ 수준은 각각 0%, ±15.10%, 0.14σ로 향상되었다. CPS 신속성, 정확성 향상으로 인한 경제적 이익은 정확성 측면에서 380만원/년, 신속성 측면에서 1350만원/년으로 총 1730만원/년의 비용절감 효과가 있었다. 결 론 : CPS 업무 흐름에 따라 CPS 효율성에 영향을 미치는 요인을 분석한 결과 CPS 효율성 향상을 위해서는 투약관련 의무 기록의 정확성, 임상병리 검사의 정도관리 향상, 그리고 관련 부서와의 원활한 업무 협조가 가장 중요하였다. The effectiveness of CPS (Clinical Pharmacokinetics Service), one of pharmacist's clinical services, was emphasized by many studies since 1985 it was introduced Korea about. But in the view of clinicians (medical doctors), customers, it's convenience or reliability was not properly evaluated for applying it to patient's care. To improve quality of Clinical Pharmacokinetics Service (CPS), the speed and the accuracy of CPS, the key points of efficiency in CPS, were evaluated by using 6-Sigma method. The speed of CPS was evaluated using total hours it took from blood sampling by a nurse to sending a CPS report to the patient's chart by a clinical pharmacist. Total hours were divided into blood sampling-to-sampling registering, sampling registering-to-C_(peak) result reporting, and C_(peak) result reporting-to-pharmacist's CPS report. The accuracy of CPS was evaluated by the difference between expected C_(peak) and measured C_(peak). Then the speed and the accuracy of CPS were expressed as σ level. After the campaign for improvement by Six Sigma method, the speed was improved by 1.19σ (before and after the campaign; 0.56σand 1.75σ, respectively) and the accuracy was improved by 0.7σ (before and after the campaign, -0.56σand 0.14σ, respectively). Exact recordings of dosing time, drug infusion hours and blood sampling time were shown to have profound effect on the accuracy of CPS. The improvement in the speed and the accuracy of CPS could produce potential financial benefit of U.S.$ 15,300 a year. There have not been studies on evaluating and objectifying as numerical value for the speed and the accuracy of CPS; thus, 6-Sigma method for improving efficiency in manufacturing process was applied to CPS process by expressing as σlevel. The speed and the accuracy of CPS could be improved by tightly controlling many factors related to CPS efficiency.

        지역사회 구강진료기관 치위생사 집단의 직무만족에 관한 조사

        This study is intended for suggesting some methods or measures to improve the working environment and dental service consequently by finding out general characteristics and various factors that affects the job satisfaction in the organization according to dental hygienists' job peculiarities and supplying dental hygienists' improvement in quality and good dental treatment with dental hygienists who work in the dental clinics in the community as study objects. In this study, to research and analyze dental hygienists' job satisfaction to improve dental hygienists' job satisfaction and promote efficiency in personnel management, 50 dental clinics where there were more than 5 dental hygienists were chosen at random and 250 dental hygienists who work there were given questionnaires and the questionnaires were analyzed and finally the result was as follows. Firstly, there are some general characteristics. The ages of 2.0~2.5 cover 68% of the dental hygienists. The unmarried occupied 81.2%. The size of the working place is the dental clinic(joint opening of a dental clinic), which occupied 54.2%. Secondly, there is a meaningful difference working ability, a sense of belonging and job stability or potentiality in case of job characteristics according to the marriage or singleness, and the married questionees are higher than the unmarried ones in the average value in all question sections. Thirdly, in case of the working Place that affects dental hygienists' job satisfaction there is a difference in the working region and no difference in the working place. Fourthly, it was found out that the difference between variables according to the region is meaningful. In working ability and job stability or potentiality, there is a meaningful difference according to the region. Lastly, the things that should be improved in the dental clinics are guarantee of job stability or potentiality, harmony in the group, publicity work, geographical conditions, improvement in dental treatment level, degree of reliability and kindness for patients , equipment. and finally hygiene.

        TFSCAN 검색 프로그램 TFSCAN의 개발

        TFD는 기능이 알려진 짧은 DNA sequence(signal)들과 그와 연관된 저널 자료로 구성된 데이타베이스이다. 임의의 DNA에서 이 데이타베이스의 sequence들을 검색하여 signal을 찾는 프로그램으로 Dan S. Prestridge가 개발한 SIGNAL SCAN이라는 프로그램이 사용되고 있는데, 이는 간단한 문자열 비교 알고리즘을 사용한다. 본 연구에서는 계산상 보다 효율적인 검색을 위해서, TFD의 sequence를 검색하기 위한 automata를 구성하는 프로그램과, 이 automata에 따라 signal을 검색하도록 하는 TFSCAN이라는 프로그램을 개발하였다. 검색된 signal에 대한 관련 문헌의 검색에서도 인덱싱 방법을 이용하여 계산 속도를 향상시켰다. 프로그램의 사용을 단순화시켰고, 결과 내용을 signal과 관련된 모든 정보를 일목요연하게 보여줄 수 있도록 구성하였다. 이 프로그램을 Web을 통해서도 사용할 수 있도록, GINet Web 서버에 TFSCAN 입력용 form과 CGI 프로그램을 개발·설치하였다. 본 연구의 특정 Motif 패턴으로 구성된 데이타베이스 검색에서, automata를 응용한 알고리즘을 이용하여 계산상 급격히 향상된 결과를 얻을 수 있음을 알 수 있었는데, 이는 생물학의 여러 패턴 검색에 응용될 수 있을 것이다. 더욱 민감한(sensitive) signal 검색을 위해서, 이와 같이 automata를 활용하고, 이 automata를 최적화하는 알고리즘 연구를 계속하고 있다. TFD is a transcription factor database which consists of short functional DNA sequences called as signals and their references. SIGNAL SCAN, developed by Dan S. Prestridge, is used to determine what signals of TFD may exist in a DNA sequence. This program searches TFD database by using a simple algorithm for character string comparison. We developed TFSCAN that aims at searching for signals in an input DNA sequence more efficently than SIGNAL SCAN. Our algorithms consist of two parts, one constructs an automata by scanning sequences of TFD, the other searches for signals through this automata. Searching for signal-related references is radically improved in time by using an indexing method. Usage of TFSCAN is very simple and its output is obvious. We developed and installed a TFSCAN input form and a CGI program in GINet Web server, to use TFSCAN. The algorithm applying automata showed drastical results in improvement of computing time. This approach may apply to recognizing several biological patterns. We have been developing our algorithm to optimize the automata and to search more sensitively for signals.

      • 腹部腫瘍(72例)에 있어서의 血管撮影 및 手術所見

        Aortography with selective visceral injections has been recognized as highly diagnostic and easy technique for the evaluation of various abdominal mass lesions. At department of Radiology, National Medical Center, Seoul, Korea seventy-two (72) patients whose chief complaints were slowly growing abdominal mass were studied with angiography after conventional examinations (barium study, I. V. P. etc) for last several years(1965-1972). The angiographic patterns were comprehensively analized. Almost cases (about 64 cases) were retroperitoneal expanding lesions which were not well detected with conventional radiological study (barium study, I. V. P. etc). The all expanding lesions were composed of renal expanding lesions(42 cases), other retroperitoneal expanding lesions (17cases), and liver, spleen, pancreatic lesions (13 cases). The angiography with selective visceral injections was the most diagnostic and easy technique without any permament complication than any other conventional studies.

        대전광역시 양축농가의 축분뇨 관리 실태 및 VOCs 농도 조사

        This study was carried out to investigate the style of livestock house, concentration of malodorous substances of livestock feces and livestock houses in Daejeon area. Among the livestock houses investigated, as most of cow pens(94.5%) have sawdust or chaff on the bottom, there was no leakage of feces out of pen. Most pig pens adopted slury style, but some of them currently use buffering material on the bottom. It is thought that there will be no possible contamination leakage. When it comes to hen house, all the broiler house use litters on the bottom and all the layer house use scrapper. It is also thought that there will be no possible contamination leakage. 3 out of 12 deer pens used buffering material on the bottom, 10 places were maintained in a traditional method, and 7 places left possibility of contamination leakage considering whether the roof was installed or not. The contents of ammonia, amine and volatile fatty acid in fresh feces were lower compared to rotten feces, but the concentration of sulfur-containing matter - hydrogen sulfide, methylmercapthan and ethylmercapthan were higher compared to rotten feces. In the case of malodorous ingredient in livestock houses, only small amount of ammonia and hydrogen sulfide were detected in pig pen and hen house, and other ingredients were not detectable. And those who are engaged in animal husbandry reacted negatively to the use of feed additives for decreasing malodor. In conclusion, it is not worrisome that contamination can be leaked out of animal raising facilities. But if we take into consideration that the point of investigation time is wintry season, there should be more considerate attitude. And feed additives for decreasing malodor need establishing criteria in the manufacturing process.

        In the literatual study on Holotrichia, the results were obtained as follows ; 1. Holotrichia is larva of Holotrichia diomphalia Bates etc. powder or liquor of Holotrichia is used medically. 2. Appearance of Holotrichia is shape of kidney, yellowish color. 3. The oriental characters of Holotrichia is warm, toxicant, salty. 4. The significant efficancy of Holotrichia is breaking the stagnant blood. 5. Holotrichia can be applied to the diseases related to thrombosis, and recover the demage of liver. 6. Holotrichia avails Liver diseases such as Hepatitis, Liver cirrhosis, Hepatosplenomegaly, Hepatoma etc.
