Objective: The purpose of our study was to investigate the risk factors of remnant tumor growth after incomplete resection (IR) of cervical dumbbell-shaped schwannomas (DS). Methods: Twenty-one patients with IR of cervical DS with at least 2 years of follow-up were included and were divided into 2 groups: the remnant tumor growth (G) (n = 10) and no growth (NG) (n = 11) groups. The tumor location in the axial plane according to Toyama classification, the location of the remnant tumor margin, and the tumor growth rate (MIB1 index) index were compared. Results: No significant differences in Toyama classification and MIB-1 index were found. Age was significantly higher in the G group (61.4 years vs. 47.6 years; p = 0.030), but univariate logistic regression analysis revealed little correlation to the growth (odds ratio [OR], 1.08; 95% confidence interval [CI], 1.001–1.166; p = 0.047). Seventeen patients (9 in the G and 8 in the NG group) underwent the posterior one-way approach, and significant differences in the location of the remnant tumor margin were confirmed: within the spinal canal in 1 and 0 case, at the entrance of the intervertebral foramen in 7 and 1 cases, and in the foramen distal from the entrance in 1 and 7 cases, in the G and NG groups, respectively (p = 0.007). The proximal margin was identified as a significant predictor of the growth (OR, 56.0; 95% CI, 2.93–1,072; p = 0.008). Conclusion: Remnant tumors with margins distally away from the entrance of the foramen were less likely to grow after IR of cervical DS.

Recently, welfare vehicles are widely used among the aged. However, it is hard for inexperienced beginners to control the welfare vehicle in narrow places such as rooms and corridors. For the problem, the driving assist system for the welfare vehicle which is steered from an objective viewpoint with a transmittable Head Mounted Display (HMD) has been proposed in complex driving areas. Also, the autonomous driving system which can be used in safe areas for the driver has been studied. The advanced function which switches the driving assistance and the autonomous driving is needed because the welfare vehicles drive in the complex driving situations. In this paper, an autonomous driving system is proposed using the wearable MR-device which has collision avoidance function using the spatial mapping. As the system uses the virtual vehicle to avoid the obstacle, this system will be easy to switch the driving assistance and the autonomous driving. The obstacles are modeled by the spatial mapping installed in wearable HMD. Using the virtual vehicle drawn by the hologram, the autonomous system is steered using A* and NavMesh system on Unity. So, the welfare vehicle follows the virtual vehicle with the virtual platoon control constructed on the welfare vehicle. The effectiveness of proposed real-time obstacle avoidance system is evaluated by driving experiments.

Objective: This study was performed to examine differences in the association between straight and curved walking abilities of inpatients in the subacute phase and walking independence level. Design: Cross-sectional study. Methods: Subjects were divided into an independent group and a supervised group (n=10 each) by walking independence level within the ward decided by physical therapists. Inclusion criteria comprised the ability to ambulate independently within the ward, regardless of the use of walking aids. Straight walking abilities (walking velocity, stride length, and cadence) were evaluated using the 5-meter walk test. Curved walking abilities were evaluated using the Figure-of-8 Walk Test (F8W) and the 3-meter zigzag walk test (3ZW). Differences in associations between straight and curved walking abilities of inpatients were examined by calculating correlation coefficients between straight and curved walking abilities. Results: Age, walking velocity, stride length, F8W and 3ZW varied markedly between independent and supervised groups. In the independent group, F8W and 3ZW correlated significantly with walking velocity and cadence (p<0.05). In the supervised group, F8W correlated significantly with walking velocity and stride length (p<0.05), but 3ZW did not correlate significantly with straight walking abilities. Conclusions: The association between straight and curved walking abilities varied between inpatients who could ambulate independently and inpatients requiring supervision for ambulating. These differences may depend on skill on straight and curved walking abilities. There is a possibility that curved walking exercise is necessary for supervised group.
Machine Learning Approach to Drug Treatment Strategy for Diabetes Care
Kazuya Fujihara,Hirohito Sone 대한당뇨병학회 2023 Diabetes and Metabolism Journal Vol.47 No.3
Globally, the number of people with diabetes mellitus has quadrupled in the past three decades, and approximately one in 11 adults worldwide have diabetes mellitus. Since both microvascular and macrovascular diseases in patients with diabetes predispose them to a lower quality of life as well as higher rates of mortality, managing blood glucose levels is of clinical relevance in diabetes care. Many classes of antihyperglycemic drugs are currently approved to treat hyperglycemia in patients with type 2 diabetes mellitus, with several new drugs having been developed during the last decade. Diabetes-related complications have been reduced substantially worldwide. Prioritization of therapeutic agents varies according to national guidelines. However, since the characteristics of participants in clinical trials differ from patients in actual clinical practice, it is difficult to apply the results of such trials to clinical practice. Machine learning approaches became highly topical issues in medicine along with rapid technological innovations in the fields of information and communication in the 1990s. However, adopting these technologies to support decision-making regarding drug treatment strategies for diabetes care has been slow. This review summarizes data from recent studies on the choice of drugs for type 2 diabetes mellitus focusing on machine learning approaches.

In the current Internet system, there are many problems using anonymity of the network communication such as personal information leaks and crimes using the Internet system. This is why TCP/IP protocol used in Internet system does not have the user identification information on the communication data, and it is difficult to supervise the user performing the above acts immediately. As a study for solving the above problem, there is the study of Policy Based Network Management (PBNM). This is the scheme for managing a whole Local Area Network (LAN) through communication control for every user. In this PBNM, two types of schemes exist. As one scheme, we have studied theoretically about the Destination Addressing Control System (DACS) Scheme with affinity with existing internet. By applying this DACS Scheme to Internet system management, we will realize the policy-based Internet system management. In this paper, required functions in the PBNM Scheme for multiple domains as cyber physical system that utilizes data science and AI is examined.

In this paper, an adaptive control method for formation control problem is considered. We assume that there existan uncertainty in the information exchange between anagent and all of its neighbors on the network. We regard that the uncertainty in the information exchange can be described by the perturbation of the elements of adjacent matrix. Our proposed method can estimate the perturbation of the elements in the notion of adaptive control strategy. Numerical simulation results are given to illustrate the effectiveness of our proposed method.
