        The Effect of Antioxidant Agents on Cisplatin-Induced Laryngeal Histological Alterations in Rats

        The larynx-related adverse effects that depend on cisplatin decrease patient comfort and many antioxidants have been used to eliminate these side effects. We aimed to identify the laryngeal mucosal changes imposed by cisplatin and investigated whether antioxidants, and their healing effects on these changes, may help reduce laryngeal complications in patients resulting from adverse effects in the larynx. A rat model was designed to evaluate the effects of cisplatin on the larynx and the protective role of antioxidants. Single-dose cisplatin was given both intraperitoneally alone and additionally administered with p-coumaric acid, melatonin, resveratrol, vitamin D, and oleic acid over 5 days. Whole larynges were dissected and evaluated histologically, histochemically, and immunohistochemically. Varying degrees of mucosal changes cisplatin group, but neither erosion nor an ulcer was observed. Numerous variable histological effects of antioxidants were observed on cisplatin exposed laryngeal mucosa. The most obvious effects of cisplatin were edema. The results of the study showed that resveratrol was the most preventive antioxidant agent against cisplatin-dependent mucosal changes. The highest increase in the Ki67 index was in the oleic acid group. Vitamin D increased stromal cyclooxygenase-2 expression that may have an effect on increasing mucosal damage.

        Learning a foreign language is in a way opening a door to individual’s mind and discovering different culture and language universe distant from safe arms of native language. In our day foreign language education is accepted as education of culture. The strong bond between language’s semantic and social aspects and cultural codes makes introduction of culture indispensable in foreing language education. Language and culture education gets different extents with developing technology. Great developments in the communication technology makes foreing language education easier and more common. Language and culture education occurs with different contents at different settings. Television is accepted as educational tool for language and culture education too. Television series emerge as a new tool for foreign language and culture education with increasing concern. In this study we observed contribution of South Korean television serials which are higly popular in Turkey to introduction of Korean language and culture with semi-structured interview and emphasized benefits of Television series to foreign language and culture education. Yabancı bir dil öğrenmek, bir anlamda kişinin kendi düşünce dünyasında yeni bir kapıyı aralamaktır. Ana dilinin korunaklı kollarından uzakta farklı bir kültür ve dil evreni keşfetmektir. Günümüzde yabancıdil öğretimi bir kültür öğretimi olarak kabul edilmektedir. Dilin anlamsal ve toplumsal yönleri ile o dilin sahip olduğu kültürel kodlar arasında var olan güçlü bağ, yabancı dil öğretiminde kültürün tanıtımını da vazgeçilemez kılmaktadır. Gelişen teknoloji ile birlikte dil ve kültür öğrenimi de farklı boyutlar kazanmaktadır. İletişim teknolojilerinde yaşanan büyük ilerlemeler sonucunda yabancı dil öğrenimi kolaylaşmışve yaygınlaşmıştır. Çok farklı kapsamlarda ve ortamlarda çok çeşitli şekillerde dil ve kültür öğretiminin gerçekleştiği görülmektedir. Televizyon da dil ve kültür öğretiminde kullanılan eğitsel araçlardan sayılmaktadır. Televizyon dizileri ise yabancı dil öğrenenler ya da farklıbir kültürü tanımak isteyenler için artan bir ilgi ile giderek önem kazanan yeni bir dil ve kültür öğretimi aracı olarak karşımıza çıkmaktadır. Bu çalışmada yarı yapılandırılmış görüşmeler ışığında Türkiye’de geniş bir izleyici kitlesine sahip olan Güney Kore televizyon dizilerinin Türk izleyicisine ait eğilimler temel alınarak Kore dili ve kültürünün tanıtımına olan katkısı araştırılacak ve televizyon dizilerinin medyanın yabancı dil öğretimine ve kültür tanıtımına olan faydalarıüzerinde durulacaktır.

        Intraoperative Neurophysiological Monitoring in Total Hip Arthroplasty for Crowe Types 3 and 4 Hips

        Background: Crowe types 3 and 4 dysplastic hips usually need total hip arthroplasty (THA) with femoral shortening osteotomy (FSO) to facilitate reduction, equalize limb length, and decrease the traction stress in nerves. The frequency of peripheral nerve palsy after primary THA has been reported to range from 0.08% to 3.7%. Apart from direct trauma to the nerve, the excessive extension of the extremity is also reported as a common cause of nerve damage. The current study aimed to evaluate the outcomes of intraoperative neurophysiological monitoring (IONM) in THA for Crowe types 3 and 4 hips. Methods: The data of patients who underwent primary THA with IONM were retrospectively reviewed using our medical records. Patients with Crowe types 3 and 4 dysplastic hips were included in the study. Motor-evoked potentials and somatosensory-evoked potentials were assessed intraoperatively. Preoperative dislocation height and postoperative trochanter minor differences were measured using preoperative and postoperative radiographs. Results: Twenty-three hips of 19 patients (4 bilateral THAs) with a mean age of 45 years participated in the study. Ten hips (43%) were classified as Crowe type 4, whereas 13 hips (57%) were Crowe type 3. The mean preoperative dislocation height was 41.6 mm (range, 15–100 mm). Postoperatively, only 6 patients had a difference between trochanter minor levels with a mean of 8.5 mm (range, 3–17 mm). Three patients underwent a subtrochanteric FSO to achieve reduction. Postoperatively, no patient had any motor and sensory nerve dysfunction. Conclusions: According to the results acquired from this study, no nerve palsy was observed after THA for Crowe types 3 and 4 hips, and subtrochanteric FSO was not performed in all Crowe type 3 hips and 70% of Crowe type 4 hips with the aid of IONM.

      • Successful First Round Results of a Turkish Breast Cancer Screening Program with Mammography in Bahcesehir, Istanbul

        Background: The Bahcesehir Breast Cancer Screening Project is the first organized population based breast cancer mammographic screening project in Turkey. The objective of this prospective observational study was to demonstrate the feasibility of a screening program in a developing country and to determine the appropriate age (40 or 50 years old) to start with screening in Turkish women. Materials and Methods: Between January 2009 to December 2010, a total of 3,758 women aged 40-69 years were recruited in this prospective study. Screening was conducted biannually, and five rounds were planned. After clinical breast examination (CBE), two-view mammograms were obtained. True positivity, false positivity, positive predictive values (PPV) according to ACR, cancer detection rate, minimal cancer detection rate, axillary node positivity and recall rate were calculated. Breast ultrasound and biopsy were performed in suspicious cases. Results: Breast biopsy was performed in 55 patients, and 18 cancers were detected in the first round. The overall cancer detection rate was 4.8 per 1,000 women. Most of the screened women (54%) and detected cancers (56%) were in women aged 40-49. Ductal carcinoma in situ (DCIS) and stage I cancer and axillary node positivity rates were 22%, 61%, and 16.6%, respectively. The positive predictivity for biopsy was 32.7%, whereas the overall recall rate was 18.4 %. Conclusions: Preliminary results of the study suggest that population based organized screening are feasible and age of onset of mammographic screening should be 40 years in Turkey.

        Cosmetic buying behavior: examining the effective factors

        This paper reports on the results of a cross-sectional study that focused on the impact ofattitudes, subjective norms and consumer innovativeness on intention to purchasecosmetic products. Beginning with the Theory of Reasoned Action (Ajzen & Fishbein,1980; Fishbein, 1980; Fishbein & Ajzen, 1975), several models have been proposed toconceptualize the complex behavioral and social process by which individuals adoptnew products. We argue here that the inclusion of an important individual differencevariable – consumer innovativeness in conjunction with attitude and perceptions ofsubjective norms – would help us further understand both how perceptions are formedand the subsequent role they play in purchase intentions regarding cosmetic products. Data were collected through a mall-intercept questionnaire conducted in different wellknownshopping malls in Sweden. Analyses included tests of the significance impact ofthese variables on intention to purchase cosmetic products. The findings offer lessonsfor practitioners and more avenues of study for researchers. The paper advances theunderstanding of three key antecedents by investigating structural relationships amongattitudes towards new cosmetic products, novelty-seeking tendencies, behaviors andattitudes of people with the intention to purchase skin care and make up products.

      • Carcinoma of the Tongue: A Case-control Study on Etiologic Factors and Dental Trauma

        Background: Carcinoma of the tongue is the most common intra-oral malignancy in Western countries. Incidence and mortality rates have increased in recent years, and survival has not improved. This study aimed to determine etiologic factors for tongue cancer with age-sex matched case-control data. Materials and Methods: 47 patients with carcinoma of the tongue referred to our oral medicine clinic between years 2005-2006 were analyzed and compared with control group data. The medical records, including family history of cancer, dental trauma, and history of abuse of alcohol and tobacco products was recorded for all subjects. Chi square comparison tests and linear regression analysis were performed using the SPSS program for statistics. Results: Patient and randomly selected control groups each consisted of 30 male and 17 female subjects with mean ages 53.2 (${\pm}12.6$) and 52.6 (${\pm}11.5$) years respectively. Smoking and alcohol abuse proportions were significantly higher in the patient group (p=0.0001, p<0.0001 respectively). Chronic mechanical trauma was observed in 44.7% of the patients and 17.0% of the control group (p=0.004). Similarly, family history of cancer of any type (for the first degree relatives) was found to be more common in the patient group (p=0.009). On regression analysis, alcohol abuse, family history of cancer, smoking, chronic mechanical traumas appeared as significant etiologic factors (p=0.0001). Conclusions: We believe that field cancerization may become evident in oral and oropharyngeal mucosa with multiple steps of molecular changes starting from the first sign of dysplasia with chronic exposure to etiological factors. Chronic trauma cases need particular attention to search for very early signs of cancer.

      • Is the MDR1 C3435T Polymorphism Responsible for Oral Mucositis in Children with Acute Lymphoblastic Leukemia?

        Background and Aim: Although the functional consequences of MDR-1 polymorphisms have been the subject of numerous studies, to the best to our knowledge, associations with clinical side effects of anticancer drugs have yet to be assessed. Our aim was to clarify any role of the C3435T polymorphism of the MDR1 gene in oral mucositis and its relation with elevated reactive oxygen species (ROS) levels, in children with acute lymphoblastic leukemia (ALL). Materials and Methods: The distribution of the MDR-1 C3435T polymorphism in 47 patients with ALL was determined by RFLP and compared with that of 68 healthy controls. Results: There were no association in distribution of genotypes of MDR-1 C3435T polymorphism and the risk of ALL. Oral mucositis were detected in 78.7% (n=37) of the patients and significantly related to the MDR-1 CT genotype (p=0.042), as confirmed by logistic regression analysis. Conclusion: Our preliminary data suggest that children carrying the CT genotype are more prone to develop oral mucositis, which might mean that the heterozygous genotype leads to accumulation of more reactive oxygen species. Since a limited number of patients was investigated, further studies are needed to confirm these findings.

        Potentially Inappropriate Medication Use among Nursing Home Residents: Medication Errors Associated with Pro re nata Medications and the Importance of Pill Burden

        Background: The use of potentially inappropriate medications (PIM) has become more common among nursing home residents (NHR). This study focused on drugs initially prescribed as pro re nata (PRN) medications and pill burden in association with PIM among NHR. Methods: This observational cross-sectional study was conducted between March and April 2019 on 225 adult NHR aged ≥60 years. Results: The prevalence of PIM was 47.6% among NHR according to the Screening Tool of Older Persons' Prescriptions (STOPP) criteria version 2. The most frequent PIM was the use of any drug prescribed without evidence-based clinical indication; most medication errors were associated with PRN medications. The prevalence rates of PRN in non-PIM and PIM users were 12% and 62.4%, respectively. PRN medications that most commonly caused PIM were non-steroidal anti-inflammatory drugs and proton pump inhibitors. The cut-off value for both medications and pills to correctly identify participants with PIM was 5.5. Pill burden had a similar sensitivity to polypharmacy in identifying individuals with PIM. Conclusion: Medication errors associated with PRN medications were overlooked as factors that increased the risk of PIMs. The most common error related to PRN medications was the continued daily use despite symptom resolution.
