        Prospective evaluation of fiducial marker placement quality and toxicity in liver CyberKnife stereotactic body radiotherapy

        Background: Evaluate morbidities and “quality” of fiducial marker placement in primary liver tumours (hepatocellular carcinoma [HCC]) for CyberKnife. Materials and Methods: Thirty-six HCC with portal vein thrombosis (PVT) were evaluated for “quality” of fiducial placement, placement time, pain score, complications, recovery time and factors influencing placement. Results: One hundred eight fiducials were placed in 36 patients. Fiducial placement radiation oncologist score was “good” in 24 (67%), “fair” in 4 (11%), and “poor” in 3 (8%) patients. Concordance with radiologist score in “poor”, “fair”, and “good” score was 2/2 (100%), 4/5 (80%), and 24/27 (89%), respectively (p=0.001). Child-Pugh score (p=0.080), performance status (PS) (p=0.014) and accrued during “learning curve” (p=0.013) affected placement score. Mean placement time (p=0.055), recovery time (p=0.025) was longer and higher major complications (p=0.009) with poor PS. Liver segment involved (p=0.484) and the Barcelona Clinic Liver Cancer (BCLC) stage did not influence placement score. “Good” placement score was 30% in first cohort whereas 93% in last cohort (p=0.023). Time for placement was 42.2 and 14.3 minutes, respectively (p=0.069). Post-fiducial pain score 0-1 in 26 patients (72%) and pain score 3-4 was in 2 (6%). Five patients (14%) admitted in “day-care” (2 mild pneumothorax, 3 pain). Mortality in 1 patient (3%) admitted for hemothorax. Conclusion: Fiducial placement is safe and in experienced hands, “quality” of placement is “good” in majority. Major complications and admission after fiducial placement are rare. Complications, fiducial placement time, recovery time is more during the “learning curve”. Poor Child-Pugh score, extensive liver involvement, poor PS have higher probability of complications.

        Musculoskeletal Applications of Elastography: a Pictorial Essay of Our Initial Experience

        Elastography is an ultrasound-based newer imaging technique that is currently being used for the evaluation of breast lesions and hepatic pathology. It is also being evaluated for characterizing lesions of the prostate, thyroid, cervix and lymph nodes. We have applied real-time sonoelastography to a variety of musculoskeletal pathologies and here we report the findings of elastography for the evaluation of various musculoskeletal pathologies. Elastography of musculoskeletal lesions is not yet being routinely used in clinical practice, but it is being extensively researched.

        COMPARATIVE RESPONSES OF RICE (ORYZA SATIVA) STRAW TO UREA SUPPLEMENTATION AND UREA TREATMENT

        Twenty five 75% Holstein Friesian cross bred bullocks fed rice straw (Oryza sativa) of long form, were fed with the following five treatments. 1. Rice straw, untreated (RS) 2. RS + water (1:1), stored for 24 hours (WRS) 3. RS (100 kg) + urea solution (4 kg urea/100 litre water) and dried (USRS) 4. RS (100 kg) + urea solution (as in 3) stored in wet condition for 24 hours (UWRS) 5. RS (100 kg) + urea solution (as in 3) stored in pit for 21 days (UTRS). Potential digestibility of treatments of RS was evaluated by monitoring (in vitro) Simulating Rumen like Fermentation (SRLF). The results indicated that Dry Matter Intake (DMI), digestibility of nutrients, N utilization were of the order UTRS > UWRS > USRS > WRS and RS (p < 0.05 to p < 0.01). SRLF index was high (255.84) for UTRS and least (145.58) for USRS. It was intermediary (199.66) for UWRS. The acetyl content (AC) of UTRS with higher hemicellulose (HCE) digestibility (80.8%) was low compared to UWRS, USRS, RS and WRS. The acetate content was of the order UTRS < UWRS < USRS < WRS and RS thereby indicating that reduction in acetyl content was an index of positive response of urea-treatment of RS. In addition, the ratio of HCE/AC in faeces of UTRS was 0.87 as against the ratios (2.26-2.48) observed in other treatments recording reduction in AC due to urea-treatment. Among the treatments, USRS only supplemented N while UTRS in addition to utilization N, increased the digestibility of structural carbohydrates. Reduction in treatment time from 21 days to 1 day (UWRS) resulted in improvements similar to those of UTRS.

        Optical, thermal and gamma ray attenuation characteristics of tungsten oxide modified: B2O3–SrCO3–TeO2–ZnO glass series

        The glass series modified by tungsten oxide was created using the compounds (75-x) B2O3– 10SrCO3– 8TeO2– 7ZnO - xWO3, where x = 0, 1, 5, 10, 22, 27, 34, and 40% mole percentage. A UV–visible spectrophotometer and thermogravimetric-differential thermal analysis (TG-DTA) methods were employed to characterize the specimen’s optical and phase transition attributes, respectively. The mass-attenuation coefficient (AC) of all created glasses from BSTZW0 to BSTZ7 was estimated using Geant4 code from 0.05 to 3 MeV and compared to the XCOM software results, with a relative difference of less than 2% between the two results. The increase of WO3 percentage lead to an increase in the Linear-AC at each studied energy, and this is mainly due to the fact that the higher the percentage of WO3 in the glass increases its density which causes an increase in the Linear-AC, so an energy of 0.06 MeV, as an example, the values of the Linear-AC was 4.009, 4.509, 5.442, 6812, 8.564, 9.856, 10.999 and 11.628 cm 1 form BSTZW0 too BSTZW7, respectively. The Half-VL (value layer), Mean-FP (free path), Tenth-VL, and Radiation attenuation performance (RAP) were also calculated for the current BSTZW-glass samples and revealed that BSTZW7 had the best gamma ray attenuation performance at all discussed energies when compared to other studied glass samples.
