        부정유합된 상악골 골절의 처치

        Unfavorable healing of maxillary fractures may impose functional and esthetic burdens upon the trauma victim. Malunited maxillary fractures are generally a result of treatment delay, incomplete or inaccurate immobilization of the fracture fragments, or infection. Dysfunctions of mastication, distortions in speech, gross defects in facial contour, and related psychic changes are problems which may require secondary correction. When it is necessary to delay definitive treatment or when inadequate maxillary fracture reduction is recognized within the first week following injury, the maxilla can be mobilized by heavy handed dental manipulation under anesthesia or by elastic traction to an external fixation appliance attached to the maxilla by arch bars or an acrylic splint. But malunited maxillary fracture that have progressed to bony malunion require osteotomy procedure in order to establish normal anatomic relationships. This report parents two cases of malunited unilateral maxillary fracture surgically corrected by unilateral Le Fort I osteotomy.

      • 白鼠背部 實驗創傷內 亞鉛濃度 比較測定

        The purpose of this experimental tstudy was to measure zinc concentrations in the experimental wounds of the white rats which had received oral supplementation of zinc sulfate 4.4mg(equivalent to 1mg of elemental zinc) or zinc sulfate 4.4mg plus ascorbic acid 8mg daily. Twenty seven white rats were divided into three groups; control group, ad. zinc group, ad. zinc plus ascorbic acid group. . Three artificial wounds were made on the back surface of each animal and tissue biopsies were taken from the healing wounds 1st week, 2nd week, 3rd week postoperatively, and then all samples were analyzed for zinc concentration using atomic absorption spectrophotometer. The results were summarized as follows; Initial sharp decrease of zinc concentration was observed in all animal groups during the 1st experimental week. The concentration increased gradually during the 2nd and 3rd experimental week, but the final concentration didn't regain th original one. Zinc suplementation resulted in significant increase of zinc level compared with control group on 2nd and 3rd postoperative week. (P<0.05) Local zinc concention was not influenced by oral supplemention of ascorbic acid.

        시상골 골절단술시 근심골편의 변위를 방지하기위한 lingual fracture technique

        The sagittal split osteotomy of the mandibular ramus is a common procedure which has been used in the correction of mandibular deformities for a few decades. Although the technical improvements have increased the reliability and stability of SSRO procedure, the postoperative relapse is imperative and clinically more significant than any other complication. One of the major causes of the relapse is due to the displacement of the proximal segment during SSRO procedure, which is well documented in the literature. Therefore it is important to preserve the original position of the proximal segment during SSRO proced and maxillofacial fixation period. In the case of mandibular asymmetry, if one side of mandible is advanced and the other side of mandible is setback during SSRO procedure, the proximal segment in the advancement site will rotate laterally and the proximal segment in the setback site will rotate medially. For the prevention of the lateral rotation or flaring of the proximal segment in the advancment site. we deliberately fracture the posterior protion of the distal segment in green-stick fashion during SSRO procedure, and there is no need to fix the fractured lingual segment. We fix the two octeotomized bony segments in the buccal cortex area rigidly with adjustable monocortical plates and screws. During SSRO procedure the lingual fracture technique was applied to nine patients with severe mandibular asymmetry who underwent orthognathic surgery in our hospital since march, 1992. These clinical experiencies enable us to find the lingual fracture technique has the following advantages. 1. The proximal segment is displaced minimally. 2. The osteotomized bony segments are contacted intimately. 3. The postoperative relapse and the healing period are decreased.

        측두하악관절염에서 발생한 희귀한 골화현상

        Osteoarthritis is a noninflammatory degenerative disease affecting the articular surfaces and is accompanied by remodeling of the underlying bone. The sympotms of osteoarthritis of the temporomandibular joint are pain in the joint and muscles of mastication, limitation. Osteoarthritis is generally accepted that several factors can contribute to the trauma, aging process, internal derangement and MPDS. Radiographic features of the osteoarthritis are seen flattening of joint, sclerosis on flattened area and osteophyte or anterior lipping. In the past, osteoarthritis was considered to self-limiting disease. Currently, synovial chondro-membrane is part of the process of osteoarthritis secondary to trauma. Synovial chondromatosis is an uncommon disease of cartilaginous transformation of synovial membrane with formation of loose bodies within the joint space. The pathogenesis in more an active metaplastic than a neoplastic process. The cause of synovial chondromatosis is unknown. Although trauma and inflammation have all been cited as possible factors in tis pathogenesis. The clinical sign and symptoms are unilateral swelling of the joint region, pain in the joint area and crepitus seem to be the most reliable signs and symptoms. Radiographic evidence of loose bodies may or may not be present. This is a case report of 66 year old female with synovial chondromatosis, that is advanced disease of the osteoarthrits. We treated patient with surgical excision of lose bodies, diskectomy and synovectomy. The defected articular fossa area was reconstructed with temporalis fascia flap. The result was satisfactory.

      • Korean Familism Culture and Christian Ethics

        Families take up much of the Korean society. Family is the base of one’s life. Familism is a cultural value system found in the individuals of the Korean society. In Korea it still seems that the understanding of family has the power to decide an individual’s life in the present and even the future. This is because the family community way of life in the Korean society proceeds in a selfish familism and group exclusivism. The negative side of the Korean familism was formed during a period of a long time inside Korea’s traditional culture. Also even though various kinds of solutions were implemented among individuals or social systems the negative Korean familism is not being overcome easily. The negative side of the familism culture can be seen in various places of life in the Korea society. The zoom-in of familism that we can find in politics, economics, education, religion and our every day lives are retorted, and it can be a major reason why it the forming of a mature democratic society, demanded by the Korean society, struggles. Only when a family community that together pursues autonomy, rationality and community, the participants of the society would be able to experience the democratic value in their lives for real. For this to happen, we would have to practice on and on the Dae-Dong-thought that we can peek at in our traditional lives, in the lives and the places we live in today. With habitual practice to make healthy ethical value happen, the negativity of familism can be overcome. Also we have to connect the real meaning of family community with Christian values and to be responsible for the duty of the mandate that God has given us.

        Percutaneous endoscopic gastrostomy in children

        Purpose: Percutaneous endoscopic gastrostomy (PEG) can improve nutritional status and reduce the amount of time needed to feed neurologically impaired children. We evaluated the characteristics,complications, and outcomes of neurologically impaired children treated with PEG. Methods: We retrospectively reviewed the records of 32 neurologically impaired children who underwent PEG between March 2002 and August 2008 at our medical center. Forty-two PEG procedures comprising 32 PEG insertions and 10 PEG exchanges, were performed. The mean follow-up time was 12.2 (6.6) months. Results: Mean patient age was 9.4 (4.5) years. The main indications for PEG insertion were swallowing difficulty with GI bleeding due to nasogastric tube placement and/or the presence of gastroesophageal reflux disease (GERD). The overall rate of complications was 47%, with early complications evident in 25% of patients and late complications in 22%. The late complications included one gastro-colic fistula, two cases of aggravated GERD, and four instances of wound infection. Among the 15 patients with histological evidence of GERD before PEG, 13 (87%) had less severe GERD, experienced no new aspiration events, and showed increased body weight after PEG treatment. Conclusion: PEG is a safe, effective, and relatively simple technique affording long-term enteral nutritional support in neurologically impaired children. Following PEG treatment, the body weight of most patients increased and the levels of vomiting, GI bleeding, and aspiration fell. We suggest that PEG with post-procedural observation be considered for enteral nutritional support of neurologically impaired children.

        ORIGINAL ARTICLE : Treatment and Outcomes for Gangliogliomas: A Single-Center Review of 16 Patients

        Background Ganglioglioma is a rare and slowly growing benign tumor. We investigated the outcomes of patients who underwent different combination treatments. Methods Between 1998 and 2012, 16 patients, including 11 men and 5 women, with a median age of 12.5 years (range, 2.5?65 years) were treated for intracranial gangliogliomas at our institution. The median follow-up period was 5.7 years (range, 48 days?15.6 years). Fifteen cases were included in the outcome assessment because one patient was lost to follow-up. Complete resection was achieved in 8 (53%) patients. Six (40%) patients underwent incomplete resection with or without adjuvant radiotherapy, and one patient with a brainstem tumor underwent only stereotactic biopsy. Results Gangliogliomas predominantly affected young (87.5%), male patients and most frequently presented with seizures (64%). Of eight patients who underwent complete resection, seven did not show recurrence, whereas only three of six with incomplete resection showed no recurrence. Four patients with recurrence received salvage treatments (two repeat surgeries and two radiosurgeries). A tumor control rate of 93% (14/15) was achieved at the last follow-up. No recurrence or malignant changes were observed after a median follow-up of 12 and 4.5 years in four patients who received gamma knife (GK) radiosurgery as adjuvant and salvage treatment. Conclusion Complete resection produced the best outcomes and incomplete resection followed by adjuvant or salvage treatments showed favorable outcomes. In patients who are not eligible for complete resection because of tumor location or potential neurologic deficits following surgery, GK radiosurgery should be considered for the treatment of residual or recurrent tumors.

      • A Novel High Step-Up Zero-Current-Switching Tapped-Inductor Boost Converter

        As global warming due to burning fossil fuels and natural resource depletion issues have emerged, the development of renewable energy sources such as photovoltaics (PV) has been brought to recent interest. Amongst the vast efforts to harvest and convert solar energy into electricity, module integrated converters (MIC) have become a worthy topic of research for small sized grid-connected photovoltaic systems. Due to the required high-boosting qualities, only a restricted amount of DC/DC converter topologies can be applied to MICs. This paper investigates the possibility of a tapped-inductor boost converter as a candidate for PV MICs. A novel tapped-inductor boost converter with zero current switching (ZCS) is proposed. By deploying an auxiliary switch and an additional capacitor, the inductor current can be resonantly shaped to achieve ZCS. This results in reduced switching losses and enhanced overall efficiency. Also, the zero crossing point of the inductor current can easily be detected, which can help digitalize the converter controller. The operation and performance of the proposed converter is verified by a 200W prototype converter.

      • Digitally Controlled Interleaving Tapped-Inductor Boost Converter for Photovoltaic Module Integrated Converters (PV MIC)

        As global warming due to burning fossil fuels and natural resource depletion issues have emerged, the development of renewable energy sources such as photovoltaics (PV) has been brought to recent interest. Amongst the vast efforts to harvest and convert solar energy into electricity, the module integrated converters (MIC) has become a worthy topic of research for grid-connected photovoltaic systems. Due to the required high-boosting qualities, only a restricted amount of DC/DC converter topologies can be applied to MICs. This paper investigates the possibility of a tapped-inductor boost converter as a candidate for PV MICs. A dual-inductor interleaving scheme operating slightly above the boundary of the two conduction modes (BCM) is suggested for reduction of input current ripple and minimization of component stress. A digital controller is used for implementation, assuring maximum power tracking and transfer while providing sufficient computational space for other grid connectivity applications, etc. For verification, a 200W converter is designed and simulated via computer software including component losses. High efficiency over a wide power range proves the feasibility of the proposed PV MIC system.
