        Hyperfunctioning Intrathyroidal Parathyroid: a Misleading Preoperative Diagnosis

        Hyperfunctioning parathyroid glands may be rarely located in the thyroidal parenchyma and not identified by imagingor during surgical procedures. We present three patients with primary hyperparathyroidism related to hyperfunctioningintrathyroidal parathyroid retrospectively selected among 732 cases from own Institutional parathyroid PET/CT registryfrom 2018 to 2022. Intrathyroidal parathyroids showed intense 18F-fluorocholine uptake but a variable echographic pattern,inconstant 99mTc-MIBI uptake, and atypic iodine-contrast enhancement. Although rare, the possibility of an intrathyroidalparathyroid should be considered when no hyperfunctioning gland is found on preoperative imaging and thorough bilateralneck exploration.

        The influence of mat Pilates training adherence on strength and flexibility parameters in breast cancer survivors undergoing hormone therapy

        The aim of this study was to evaluate the influence of adherence to mat Pilates training on lower and upper body strength and flexibility in breast cancer survivors. Breast cancer survivors ( ≥ 40 years) with cancer stages 0–III undergoing hormone therapy participated in this study. For this secondary investigation only the intervention group was analyzed, divided into low and high training adherence. Participants performed a 60-min session of mat Pilates, 3 times/wk, for 24 weeks. Concentric, eccentric, and isometric hip flexor-extensor muscle peak torque, and isometric maximal strength parameters of the shoulder abductors, trunk extensors, and handgrip were assessed. Physical activity level was analyzed as a control variable. The results showed that high training adherence improved (P< 0.05) left shoulder abductor strength parameters and lower and upper body flexibility compared to baseline. The low training adherence group improved (P< 0.05) trunk extensors, right and left shoulder abductor strength parameters, handgrip strength, and extensor-flexor peak torque compared to baseline. There were no differences (P> 0.05) between high and low adherence for physical activity level before and after the intervention. Therefore, it appears that higher training adherence most influences some strength parameters and flexibility, while fewer sessions enable the achievement of significant results for shoulder abductor and hip extensor-flexor muscle strength parameters.

        Glycemic Index and Glycemic Load of Two Dishes Cooked with Alache (Anoda cristata) and Chaya (Cnidoscolus aconitifolius) Plants from the Traditional Mexican Diet

        Correct nutrition is important for keeping good health; to attain that, the diet has to include vegetables such as quelites. The objective of this study was to determine the glycemic index (GI) and glycemic load (GL) of rice and a tamal prepared with and without two species of quelites: “alache” (Anoda cristata) and “chaya” (Cnidoscolus aconitifolius). The GI was measured in 10 healthy subjects, 7 women and 3 men, with the following mean metrics: age, 23 years old; body weight, 61.3 kg; height, 1.65 m; body mass index, 22.7 kg/m2; and basal glycemia, 77.4 mg/dL. Capillary blood samples were collected within 2 h after the meal. White rice (rice with no quelites) had a GI of 75.35 ± 15.6 and a GL of 36.17 ± 7.8; rice with alache had a GI of 33.74 ± 5.85 and a GL 33.74 ± 1.85. White tamal had a GI of 57.33 ± 10.23 and a GC of 26.65 ± 5.12; tamal with chaya had a GI of 46.73 ± 22.1 and a GL of 23.36 ± 11. The GI and GL values recorded for the combination of quelites with rice and tamal confirmed that quelites could be a good alternative for healthy diets.

        Relationship Between the Dose of Clodronate and Serum Level of Alkaline Phosphatase, Calcium, and Phosphate During Orthodontic Tooth Movement

        Objective : To evaluate the relationship between the dose of Clodronate and serum level of alkaline phosphatase (ALP), calcium (Ca), and phosphate (PO4) during orthodontic tooth movement MaterialS and MethodS: A total of 18 sex-matched Wistar rats (weight=180~230g, mean age=8 weeks) were allocated into the 2.5mM Clodronate (2.5C) group, 10mM Clodronate (10C) group, or control group (n=6 for each group). After the application of a nickel-titanium closed coil spring (force of 60g) between the upper central incisors and first molars (UFM), 2.5C, 10C, or saline was injected every third day into the subperiosteum of the alveolar bone adjacent to UFM for the experimental and control groups. The animals were sacrificed 17 days later. Trunk blood was quickly collected into a heparinized tube and centrifuged at 2,000 rpm for 20 min. The plasma was used for the biochemical assays of the serum level of ALP, Ca, and PO4. Kruskall-Wallis test and Mann-Whitney test with Bonferroni correction were performed for the statistical analyses. Results : Dose-dependent increase in the level of ALP (P<0.01) and decrease in the level of Ca (P<0.001) were observed among the control, 2.5C, and 10C groups. Although there was no significant difference in PO4 between the 2.5C and 10C groups, the 10C group showed a significantly higher level of PO4 than the control group (P<0.01). Conclusion : Since Clodronate induced significant dose-dependent change in the serum level of ALP, Ca, and PO4 during orthodontic tooth movement, orthodontists should consider these biochemical markers not only as a diagnostic tool for bone turnover rate but also as a monitoring tool for orthodontic tooth movement.

        House Dust Mite Allergy Under Changing Environments

        Environmental variations induced by industrialization and climate change partially explain the increase in prevalence and severity of allergic disease. One possible mechanism is the increase in allergen production leading to more exposure and sensitization in susceptible individuals. House dust mites (HDMs) are important sources of allergens inducing asthma and rhinitis, and experimentally they have been demonstrated to be very sensitive to microenvironment modifications; therefore, global or regional changes in temperature, humidity, air pollution or other environmental conditions could modify natural HDM growth, survival and allergen production. There is evidence that sensitization to HDMs has increased in some regions of the world, especially in the subtropical and tropical areas; however, the relationship of this increase with environmental changes is not so clear as has reported for pollen allergens. In this review, we address this point and explore the effects of current and predicted environmental changes on HDM growth, survival and allergen production, which could lead to immunoglobulin E (IgE) sensitization and allergic disease prevalence. We also assess the role of adjuvants of IgE responses, such as air pollution and helminth infections, and discuss the genetic and epigenetic aspects that could influence the adaptive process of humans to drastic and relatively recent environmental changes we are experiencing.

        Modified Soybean Affects Cholesterol Metabolism in Rats Similarly to a Commercial Cultivar

        The consumption of soy protein lowers blood cholesterol in humans and animals. Breeding may alter the physiological effects of soybeans, such as its cholesterol-lowering property. Our hypothesis is that breeding affects the hypocholesterolemic effect of soy by modulating the expression of key hepatic enzymes related to cholesterol and bile acid biosynthesis, as well as altering fecal neutral and acidic steroid excretion. Therefore the aim of this study was to evaluate the effect of a new Brazilian soybean cultivar (UFV-116), lacking lipoxygenases 2 and 3, compared with a commercial cultivar (OCEPAR-19), on 3-hydroxy-3-methylglutaryl-coenzyme A reductase (HMGR) and cholesterol 7α-hydroxylase (CYP7A) mRNA expression and fecal steroid output in rats. Thirty-six male rats were fed UFV-116, OCEPAR-19, or casein as the protein source, with or without addition of dietary cholesterol (0.25%). Blood and liver cholesterol, HMGR and CYP7A mRNA abundance, and fecal excretion of steroids were measured. Blood and liver cholesterol levels were lowered by both soybean cultivars, with and without cholesterol, but UFV-116 was more effective when included in the cholesterol-free diet. Both soy diets promoted lower levels of HMGR mRNA, higher levels of CYP7A mRNA, and higher excretion of fecal secondary bile acids. There was higher fecal neutral steroid output when cholesterol was added to all diets. These data show that both soybean cultivars acted similarly in lowering serum and hepatic cholesterol; therefore, breeding did not affect the hypocholesterolemic effect of the new cultivar.

        The university of Santo Tomas Viewed from the Lens of Total Quality Management: Implications to Total Quality Education

        Considered as a major management approach for improving organizational performance and competitive advantage, Total Quality Management (TQM) poses a challenge to dynamic institutions to adopt a systemic philosophy that places emphasis on customer needs and a commitment to a culture of excellence. Higher education institutions (HEIs) as learning organizations are expected to respond proactively to this challenge by examining closely how their educational centerpiece operates in an environment of quality. This inquiry aims to determine how the oldest university in Asia implements TQM as regards its Vision, Involvement, Continuous Improvement, Training and Education, Ownership, Reward, Yearning for Success, and Customer Focus (VICTORY-C). Moreover, it documents the best practices of the institution under study, categorized in the light of TQM principles and identifies implications for total quality education.

        Intradermal Tests for Diagnosis of Drug Allergy are not Affected by a Topical Anesthetic Patch

        The use of topical anesthesia to perform intradermal tests (IDTs) for drug allergy diagnosis was never investigated. We aimed to determine the effectsof a topical anesthetic patch containing prilocaine-lidocaine on wheal size of IDT with drugs. Patients who had positive IDT as part of their investigationprocess of suspected drug hypersensitivity were selected. IDT were performed according to guidelines. Anesthetic patch (AP) wasplaced and the same prior positive IDT, as well as positive histamine skin prick test (SPT) and negative (saline IDT) controls, were performed in theanesthetized area. Patients with negative IDT were also included to check for false positives with AP. Increase in wheals after 20 minutes both withand without AP was recorded and compared. 45 IDT were performed (36 patients), of which 37 have been previously positive (14 antibiotics, 10 generalanesthetics, 6 non-steroidal anti-inflammatory drugs, 3 iodinated contrasts, 3 anti-Hi-histamines and 1 ranitidine). Mean histamine SPT sizewithout the AP was 4.7 mm [95%CI (4.4-5.1)], and 4.6 mm [95%CI (4.2-5.0)] with anesthesia. Mean wheal increase in IDT for drugs without the anesthesiawas 4.5 mm [95%CI (3.3-5.7)] and with anesthesia was 4.3 mm [95%CI (2.8-5.8)]. No statistical significant differences were observed betweenskin tests with or without AP for histamine SPT (P=0.089), IDT with saline (P=0.750), and IDT with drugs (P=0.995). None of the patientswith negative IDT showed positivity with the AP, or vice-versa. The use of an AP containing prilocaine-lidocaine does not interfere with IDT to diagnosedrug allergy, and no false positive tests were found.
