        The Implementation of Telepractice by Malaysian Speech-Language Pathologists During the COVID-19 Pandemic

        Objectives: The Coronavirus Disease 2019 (COVID-19) led speech-language pathologists (SLPs) around the world to shift their service delivery methods from face-to-face service to telepractice. This study explored the implementation of telepractice by Malaysian SLPs and determined whether there was an association between the SLPs’ perceptions of the effectiveness of telepractice and the frequency of the telepractice services provided, as well as between the use of telepractice and prior training received. Methods: Eighty-nine SLPs responded to an 18-question online survey that inquired about demographics, telepractice during the pandemic, perceptions of telepractice and prior training received. Results: Seventy-five percent of SLPs reported providing services via telepractice during the pandemic compared to 19% before the pandemic. Most SLPs who used telepractice stated that services delivered via telepractice were comparable with face-to-face services; no association was found between the frequency of providing services via telepractice and the SLPs’ perceptions of its effectiveness. They had also received some training in telepractice, although no association was found between the type of training received and the provision of telepractice services. SLPs who did not provide services via telepractice reported awareness of telepractice but described a lack of knowledge, support, and training. All SLPs expressed interest to receive more training in telepractice to enable them to provide better services. Conclusion: Malaysian SLPs demonstrated great commitment towards service provision during the pandemic, similar to SLPs in other nations. It is hoped that Malaysian SLPs continue to have positive attitudes toward telepractice with increased familiarity, use, and training. Telepractice services, undoubtedly, are here to stay.

      • Performance of bricks and brick masonry prism made using coal fly ash and coal bottom ash

        The major problem of a coal combustion-based power plant is that it creates large quantity of solid wastes. So, to achieve the gainful use of waste materials and to avoid other environmental problems, this study was undertaken. The quantity of coal ash by-products, particularly coal fly ash and coal bottom ash has been increasing from the coal power plants around the world. The other objective of this study was to explore the possibility of utilization of coal ash in the production of ash bricks. In 15 different mixes, Mix Designation M-1 to M-15, the varying percentages of lime and gypsum were used and sand was replaced with coal bottom ash. Further, it has been noticed that the water absorption and compressive strength of mix M-15 is 13.36% and 7.85 MPa which is better than the conventional bricks. The test results of this investigation show that the prism strength of coal ash masonry prisms was more than that of the conventional bricks.

      • Strength and durability characteristics of bricks made using coal bottom and coal fly ash

        The study evaluates properties of brick having coal ash and explores the possibility of utilization of coal bottom ash and coal fly ash as an alternative raw material in the production of coal ash bricks. Lower cement content was used in the investigations to attain appropriate strength and prohibit high carbon content that is cause of environmental pollution. The samples use up to 7% of cement whereas sand was replaced with bottom ash. Bricks were tested for compressive strength, modulus of rupture, ultrasonic pulse velocity (UPV), water absorption and durability. The results showed mix proportions of bottom ash, fly ash and cement as 1:1:0.15 i.e., M-15 achieved optimum values. The coal ash bricks were well bonded with mortar and could be feasible alternative to conventional bricks thus can contribute towards sustainable development.

      • Genetic Polymorphisms of GSTM1 and GSTT1 Genes in Delhi and Comparison with other Indian and Global Populations

        The glutathione S-transferases (GSTs) are involved in the metabolism of many xenobiotics, including an array of environmental carcinogens, pollutants, and drugs. Genetic polymorphisms in these genes may lead to inter-individual variation in susceptibility to various diseases. In the present study, GSTM1 and GSTT1 polymorphisms were analysed using a multiplex polymerase chain reaction in 500 normal individuals from Delhi. The frequency of individuals with GSTM1 and GSTT1 null genotypes were 168 (33.6%) and 62 (12.4%) respectively, and 54(10.8%) were having homozygous null genotype for both the genes GSTM1 and GSTT1simultaneously. The studied population was compared with reported frequencies from other neighbouring state populations, as well as with those from other ethnic groups; Europeans, Blacks, and Asians. The prevalence of homozygous null GSTM1 genotype is significantly higher in Caucasians and Asians as compared to Indian population. The frequency of GSTT1 homozygous null genotypes is also significantly higher in blacks and Asians. We believe that due to large number of individuals in this study, our results are reliable estimates of the frequencies of the GSTM1, GSTT1 in Delhi. It would provide a basic database for future clinical and genetic studies pertaining to susceptibility and inconsistency in the response and/or toxicity to drugs known to be the substrates for GSTs.

      • Glutathione S-transferase M1 and T1 Polymorphisms, Cigarette Smoking and HPV Infection in Precancerous and Cancerous Lesions of the Uterine Cervix

        Glutathione S-transferases (GSTs) play an important role in detoxification of carcinogenic electrophiles. The null genotypes in GSTM1 and GSTT1 have been implicated in carcinogenesis. Present study was planned to evaluate the influence of genetic polymorphisms of GSTM1 and GSTT1 gene loci in cervical carcinogenesis. The study was conducted in Lok Nayak hospital, New Delhi. DNA from clinical scrapes of 482 women with minor gynaecologic complaints attending Gynaecology OPD and tumor biopsies of 135 cervical cancer cases attending the cancer clinic was extracted. HPV DNA was detected by standard polymerase chain reaction (PCR) using L1 consensus primer pair. Polymorphisms of GSTM1 and GSTT1 were analysed by multiplex PCR procedures. Differences in proportions were tested using Pearson's Chi-square test with Odds ratio (OR) and 95% confidence interval (CI). The risk of cervical cancer was almost three times in women with GSTM1 homozygous null genotype (OR-2.62, 95%CI, 1.77-3.88; p<0.0001). No association of GSTM1 or GSTT1 homozygous null genotypes was observed in women with normal, precancerous and cervical cancerous lesions among ${\leq}35$ or >35 years of age groups. Smokers with null GSTT1 genotype had a higher risk of cervical cancer as compared to non-smokers (OR-3.01, 95% CI, 1.10-8.23; p=0.03). The results further showed that a significant increased risk of cervical cancer was observed in HPV positive smoker women with GSTT1 (OR-4.36, 95% CI, 1.27-15.03; p=0.02) and GSTM1T1 (OR-3.87, 95% CI, 1.05-14.23; p=0.04) homozygous null genotypes as compared to HPV positive non smokers. The results demonstrate that the GST null genotypes were alone not associated with the development of cervical cancer, but interacted with smoking and HPV to exert effects in our Delhi population.
