        Examination of a Nurse-led Community-based Education and Coaching Intervention for Coronary Heart Disease High-risk Individuals in China

        Purpose: Early detection and management of coronary heart disease (CHD) are embedded into many community health service and primary care practices in western countries. The Framingham CHD risk score has been used to predict CHD and mortality for nearly 20 years, and it has predicted CHD event risk accurately in multiethnic populations. The aim of this study was to access the effect of a 6-month community-based intervention on CHD risk in individuals at high risk. Methods: A randomized controlled trial of individuals with a high 10-year CHD risk were recruited from two communities in China. Individuals in the intervention group (n ¼ 53) received a 3-month group education and a 3-month coaching session. Physical examination and self-report questionnaires were used to collect both pre- and postintervention data on blood pressure, glucose, cholesterol, body mass index, smoking, depression, and health-related quality of life (HRQoL). Results: A total of 102 participants (85.0%) completed the 6-month study. Compared with the usual care group, the intervention group had a 5 mmHg greater reduction in systolic blood pressure (t = 2.01, p = .047), larger declines in glucose (t = 2.49, p = .015), cholesterol (t = 2.44, p = .017), body mass index (t = 2.58, p = .011), and depression (t = 2.05, p = .043), and better reports of HRQoL (t = 3.36, p = .001). No significant group differences in smoking behaviors were reported. Conclusion: A 6-month community-based intervention in a CHD high-risk population improved diseaserelated risk factors, depression, and HRQoL. Results provide preliminary evidence for primary prevention of cardiovascular disease risk in a community high-risk population.

        IL-2/anti-IL-2 complexes ameliorate lupus nephritis by expansion of CD4+CD25+Foxp3+ regulatory T cells

        <P>Adoptive transfer of regulatory T cells (Tregs) can delay disease progression and reduce mortality in lupus-prone mice. Here, we tested whether complex (IL-2C) consisting of IL-2 and anti -IL-2 monoclonal antibody (JES6-1) ameliorates lupus nephritis by expanding Tregs as an alternative to problematic Treg infusion therapy. IL-2C treatment of NZB/W Fl mice induced an effective and sustained expansion of CD4(+)CD25(+)Foxp3(+) Tregs in both the kidneys and spleen along with decreased renal infiltration of T cells, B cells, and innate immune cells. Compared with controls, mice treated with IL-2C showed reduced proteinuria and fewer acute and chronic renal pathological lesions with improved renal function and survival. IL-2C significantly attenuated glomerular and tubular injury, vasculitis scores, and renal deposition of IgG and C3. Disease activity markers, such as high levels of anti-dsDNA antibodies and immunoglobulin levels, and low levels of complement were improved in sera of IL-2C-treated mice. IL-2C treatment decreased renal expression of TNF-alpha and IL-6, and the frequencies of IFN-gamma(+)CD4(+) and IL-17A(+)CD4(+) T cells in both the kidneys and spleen. Depletion of Tregs by anti-CD25 antibodies abrogated the beneficial effects of IL-2C. When compared with combination therapy of steroid and mycophenolate mofetil, IL-2C treatment showed similar or better outcomes. Thus, IL-2C protected lupus-prone mice against lupus nephritis by expanding Tregs. Hence, IL-2C could have therapeutic potential in lupus nephritis.</P>

        Exploring the beneficial role of telmisartan in sepsis-induced myocardial injury through inhibition of high-mobility group box 1 and glycogen synthase kinase-3β/nuclear factor-κB pathway

        In the present experimental study, cecal ligation and puncture significantly increased the myocardial injury assessed in terms of excess release of creative kinase-MB (CK-MB), cardiac troponin I (cTnI), interleukin (IL)-6 and decrease of IL-10 in the blood following 12 h of laparotomy procedure as compared to normal control. Also, a significant increase in protein expression levels of high-mobility group box 1 (HMGB1) and decreased phosphorylation of glycogen synthase kinase-3β (GSK-3β) was observed in the myocardial tissue as compared to normal control. A single independent administration of telmisartan (2 and 4 mg/kg) and AR-A014418 (1 and 2 mg/kg) substantially reduced sepsis-induced myocardial injury in terms of decrease levels of CK-MB, cTnI and IL-6, HMGB1, GSK-3β and increase in IL-10 and p-GSK-3β in the blood in sepsis- subjected rats. The effects of telmisartan at dose 4 mg/kg and AR-A014418 at a dose of 2 mg/kg were significantly higher than the telmisartan at a dose of 2 mg/kg and AR-A014418 1 mg/kg respectively. Further, no significant effects on different parameters were observed in the sham control group in comparison to normal. Therefore it is plausible to suggest that sepsis may increase the levels of angiotensin II to trigger GSK-3β-dependent signaling to activate the HMGB1/receptors for advanced glycation end products, which may promote inflammation and myocardial injury in sepsis-subjected rats.

      • Design and Implementation of Secure Cloud Storage System Based on Elliptic Encryption

        Background/Objectives: With the rapid development of Internet technology, more and more Internet users tend to store their data and information in the cloud, and the security of the existing cloud storage system has been criticized by people, which is not conducive to protecting the data and information security of users. Therefore, how to effectively guarantee the security of cloud user data becomes more and more important. Security cloud storage system is the key to solve this problem. Methods/Statistical analysis: A secure cloud storage system is implemented for the user’s cloud data security. Its main function is: in the user login phase, the strict user identity authentication method is adopted to control the user’s access rights and prevent illegal users from logging in. Firstly, the identify authentication scheme based on zero knowledge proof is proposed. Secondly, in the user data upload stage, the user data is split. Finally, the encryption algorithm based on elliptic curve is proposed to encrypt the user’s file splitting table. Findings: The experimental results show that, in the user identify authentication stage, compared with the traditional user authentication method, the security of user identify authentication can be greatly improved by using the identify authentication method based on zero-knowledge proof. Improvements/Applications: The secure cloud storage system based on ellipse encryption developed in this thesis can realize the security of user's data by encryption while considering the efficiency of data upload and download, and has a broad application prospect.

      • Caffeine ameliorates allergic rhinitis in an ovalbumin-induced murine model

        Caffeine is one of the most widely consumed pharmacologically active products worldwide. Caffeine exhibits various pharmacological activities in central nervous, cardiovascular, and respiratory systems. Additionally, caffeine exhibited anti-inflammatory effects in lipopolysaccharide-challenged rats. However, the potential anti-inflammatory activity of caffeine in allergic rhinitis (AR) has not been investigated yet. Therefore, we aimed to examine the anti-allergic effects of caffeine in ovalbumin (OVA)-induced AR mice in this study. Briefly, BALB/c mice were sensitized and challenged with OVA. Caffeine (5, 10, 20 mg/kg) and dexamethasone (Dex, 2.5 mg/kg) were treated by intranasal instillation of 20 µL/nasal cavity for 13 days. In this study, caffeine attenuated the nasal symptoms, including rubbing and sneezing. Caffeine ameliorated the inflammation in the lungs. In addition, caffeine reduced the thickness of the nasal mucosa, alleviated goblet cell hyperplasia and reduced mast cell recruitment in the nasal mucosa. Caffeine decreased OVA-specific immunoglobulin (Ig) E and IgG1 levels in the serum, whereas increased the level of OVA-specific IgG2a. Caffeine also reduced T helper (Th) 2 cytokines (interleukin(IL)-4, IL-5 and IL-13) and Th17 family related factors (IL-17, IL-6, retinoic-related orphan receptor gamma, signal transducer and activator of transcription-3 and phospho-signal transducer and activator of transcription-3) levels and elevated Th1 cytokines (IL-12 and interferon gamma) levels in the nasal lavage fluid. However, the level of malondialdehyde was down-regulated, the levels of superoxide dismutase, nuclear factor erythroid 2-related factor 2 and heme oxygenase-1 were up-regulated in the caffeine treatment. Collectively, we suggest that caffeine might have therapeutic effects on AR.

        Allium hookeri Inhibits Inflammation in OVA-induced Allergic Rhinitis in Mice via NF-κB Pathway

        Allium hookeri (A. hookeri) has been used as a traditional medicine for many years in the Southeast Asian region. Multiple investigations have demonstrated its role in preventing of microbial infection, immune regulation, coronary thrombosis, and atherosclerosis has been demonstrated by several studies. Therefore, this study aims to investigate the suppressing effect of A. hookeri on the ovalbumin (OVA)-induced allergic rhinitis (AR) murine model. In this study, AR was induced in BALB/c mice by using OVA emulsified in aluminum on days 1, 8 and 15. Then, nasally challenged with OVA on days 22 to 28. On days 16 to 28, OVA-induced mice were treated either with A. hookeri (50, 100, 200 mg/kg) or Dexamethasone (Dex, 2.5 mg/kg) by oral gavage. On days 22 to 28, mice received either A. hookeri (50, 100, 200 mg/kg) or Dexamethasone (Dex, 2.5 mg/kg) an hour before the OVA challenge. The mice in the control group were not sensitized, treated, or tested. In this study, A. hookeri ameliorated nasal symptoms include rubbing and sneezing; reduced thickness of nasal mucosa; alleviated goblet cell hyperplasia and eosinophil infiltration in the nasal mucosa; decreased levels of OVA-specific IgE and OVA-specific IgG1 in serum; and suppressed Th2 (IL-4, IL-5, and IL-13) cytokines level in nasal lavage fluid (NALF). In addition, A. hookeri recovered the production of Th1 (IL-12 and IFN-γ) cytokines level in NALF, inhibited the inflammation through NF-κB pathway. Collectively, our results suggest that A. hookeri effectively protects against AR by regulating Th1/Th2 cytokines imbalance and NF-κB mechanisms. These observations suggest A. hookeri as a potential therapeutic option for AR.

      • Implementation of online model updating with ANN method in substructure pseudo-dynamic hybrid simulation

        Substructure pseudo-dynamic hybrid simulation (SPDHS) is an advanced structural seismic testing method which combines physical experiment and numerical simulation. Generally, the key components which display nonlinearity first are taken as experimental substructures for actual test, and the remaining parts are modeled in simulation. Model updating techniques can be effectively applied to enhance the model precision of nonlinear numerical elements. Specifically, the constitutive model of the experimental substructure is identified online by the instantaneously-measured data, and the corresponding numerical elements with similar hysteretic behaviors are updated synchronously. Artificial neural network (ANN) can recognize the system which cannot be represented by definite numerical model, and thus avoids the structural response distortion caused by the inherent numerical model defects. In this study, a framework for online model updating in SPDHS with ANN method is expanded to implement actual test validation. Moreover, the effectiveness of ANN method is demonstrated by practical tests of a two-story frame model with bending dampers. Additionally, the unscented Kalman filter technique and offline ANN identification approach are both examined in the test validation. The experimental results show that, under the identical loading history, the online ANN method can significantly reduce the model errors and improve the accuracy of SPDHS.
