        Prospective Phase II Study of Preoperative Chemoradiation with Capecitabine in Locally Advanced Rectal Cancer

        Purpose: Capecitabine is an attractive oral chemotherapeutic agent that has a radiosensitizing effect and tumor-selectivity. This study was performed to evaluate the efficacy and toxicity of preoperative chemoradiation therapy, when used with oral capecitabine, for locally advanced rectal cancer. Materials and Methods: A prospective phase II trial of preoperative chemoradiation for locally advanced adenocarcinomas of the lower two-thirds of the rectum was conducted. A radiation dose of 50 Gy over five weeks and a daily dose of 1650 mg/m2 capecitabine in two potions was administered during the entire course of radiation therapy. Surgery was performed with standardized total mesorectal excision four to six weeks after completion of the chemoradiation. Results: Between January 2002 and September 2003, 61 patients were enrolled onto this prospective phase II trial. The pretreatment clinical stages were T3 in 64% (n=39), T4 in 36% (n=22) and N1-2 in 82% (n=50) of these patients. Fifty-six (92%) patients completed the chemoradiation as initially planned and a complete resection performed in 58 (95%). Down-staging was observed in 45 patients (74%) and a pathologic complete response in 6 (10%). Among the 37 patients with tumors located within 5 cm from the anal verge on colonoscopy, 27 (73%) underwent a sphincter-preserving procedure. No grade 3 and 4 proctitis or hematological toxicities were observed. Conclusion: Preoperative chemoradiation therapy with capecitabine achieved encouraging rates of tumor down- staging and sphincter preservation, with a low toxicity profile. This combined modality can be regarded as a safe and effective treatment for locally advanced rectal cancer.(Cancer Res Treat. 2004;36:354-359)

        Inhalation Toxicity of Particulate Matters Doped with Arsenic Induced Genotoxicity and Altered Akt Signaling Pathway in Lungs of Mice

        In the workplace, the arsenic is used in the semiconductor production and the manufacturing of pigments, glass, pesticides and fungicides. Therefore, workers may be exposed to airborne arsenic during its use in manufacturing. The purpose of this study was to evaluate the potential toxicity of particulate matters (PMs) doped with arsenic (PMs-Arsenic) using a rodent model and to compare the genotoxicity in various concentrations and to examine the role of PMs-Arsenic in the induction of signaling pathway in the lung. Mice were exposed to PMs 124.4 ± 24.5 ㎍/㎥ (low concentration), 220.2 ± 34.5 ㎍/㎥ (middle concentration), 426.4 ± 40.3 ㎍/㎥ (high concentration) doped with arsenic 1.4 ㎍/㎥ (Low concentration), 2.5 ㎍/㎥ (middle concentration), 5.7 ㎍/㎥ (high concentration) for 4 wks (6 h/d, 5 d/wk), respectively in the whole-body inhalation exposure chambers. To determine the level of genotoxicity, Chromosomal aberration (CA) assay in splenic lymphocytes and Supravital micronucleus (SMN) assay were performed. Then, signal pathway in the lung was analyzed. In the genotoxicity experiments, the increases of aberrant cells were concentration-dependent. Also, PMs-arsenic caused peripheral blood micronucleus frequency at high concentration. The inhalation of PMs-Arsenic increased an expression of phosphorylated Akt (p-Akt: protein kinase B) and phpsphorylated mammalian target of rapamycin (p-mTOR) at high concentration group. Taken together, inhaled PMs-Arsenic caused genotoxicity and altered Akt signaling pathway in the lung. Therefore, the inhalation of PMs-Arsenic needs for a careful risk assessment in the workplace.

        Cumulative survival rate and associated risk factors of Implantium implants: A 10-year retrospective clinical study

        PURPOSE. The objective of this study was to determine the cumulative survival rate (CSR) and associated risk factors of Implantium implants by retrospective clinical study. MATERIALS AND METHODS. Patients who received Implantium implants (Dentium Co., Seoul, Korea) at Korea University Guro Hospital from 2004 to 2011 were included. The period between the first surgery and the last hospital visit until December 2015 was set as the observation period for this study. Clinical and radiographic data were collected from patient records, including all complications observed during the follow-up period. Kaplan-Meier analysis was performed to examine CSR. Multiple Cox proportional hazard model was employed to assess the associations between potential risk factors and CSR. RESULTS. A total of 370 implants were placed in 121 patients (mean age, 56.1 years; range, 19 to 75 years). Of the 370 implants, 13 failed, including 7 implants that were lost before loading. The 10-year cumulative survival rate of implants was 94.8%. The multiple Cox proportional hazard model revealed that significant risk factor of implant failure were smoking and maxillary implant (P<.05). CONCLUSION. The 10-year CSR of Implantium implants was 94.8%. Risk factors of implant failure were smoking and maxillary implant.

        Comparison of Immuno-stimulatory Activities by Purification Process of Schizandra chinensis Baillon Fruits

        Two compounds from Gomisin N and Gomisin A were isolated from the fruits of Schizandra chinensis Baillon. The highest extraction yield as 21.36% was observed in the ethanol extract, compared to the yield obtained form the water extract. The extraction yields of the single compounds were measured to be 0.13 and 0.014 Gomisin A and Gomisin N, respectively. Approximately, 90% of the growth of human stomach adenocarcinoma cancer cells was inhibited after adding 1.0 g/l of the ethanol extract. The growth of the human normal lung cell was limited to 24% after adding the ethanol extract. The water extract lowered the specific secretion of TNF-á and IL-6 from T cells, 10.3×10-4 pg/cell and 12.1×10-4 pg/cell, respectively, compared to the ethanol extracts. On the other hand, a treatment with the ethanol extract increased the specific secretion of TNF-á and IL-6 from human T cells, to 11×10-4 pg/cell and 14.3×10-4 pg/cell, respectively. The crude ethanol extract had the highest effect on the differentiation of human promyelocytic leukemia cells compared to the other extracts and Gomisin A and N. In general, the biological activities of the extracts gradually decreased as the purification process proceeded, which suggests that higher immunostimulatory activities can be maintained by adding the crude extracts of the fruits rather than by adding a single compound.

        Population Dynamics and the Toxin of Anabaena in the Lower Naktong River

        Population dynamics of Anabaena and the anatoxin-a concentration were monitored with physicochemical parameters at 3 sites in the lower Naktong River from May to September in 2000. Total 4 species of Anabaena (A. flosaquae, A. smithii, A. ucrainica and A. mucosa) were identified with morphological characteristics. Anabaena flos-aquae was most abundant among the populations. The standing crop of Anabaena ranged from 10 to 11,220 cells ·ml–1 and biomass of Anabaena more 1,000 cells ·ml–1 was observed once at St. Mulgeum and St. Seonam, twice at St. Hagueon out of total 9 samplings. There were not significant correlations between the standing crop of Anabaena and other physicochemical parameters such as temperature, nitrate, total nitrogen, phosphate, total phosphorus and N/P ratios. The frequency of trichomes with akinetes was low and ranged from 0 to 4% in the total Anabaena population and A. smithii showed highest frequency of 2.8% among all species. The population at St. Seonam showed highest frequency of 1.4% among all sampling sites. The population in September showed the highest frequency of 3.0% among all sampling period. The frequency of trichomes with heterocysts was low and ranged from 1 to 87% in the total Anabaena population and A. smithii showed highest frequency of 55.1% among all species. The population at St. Mulgeum showed highest frequency of 17.6% among all sampling sites. The population in August showed the highest frequency of 21.4% among all sampling period. The frequency of trichomes with akinetes and/or heterocysts was not related to all the physicochemical parameters of temperature, nitrate, total nitrogen, phosphate, total phosphorus and N/P ratios. The anatoxin-a concentrations were determined in algal materials dominated by Microcystis and Anabaena from June though August by derivatization using 7-fluoro-4-nitro-2, 1, 3-benzoxadiazole (NBD-F) and HPLC analysis with fluorimetric detection. All the concentrations were below the detection limit of 0.1 μg · l–1 in the present study.
