      • A Perspective on Promoting South Korean-Canadian Relations

        Transnationalism is at the heart of any current effort to make progress in improving cross-cultural relations. The drive to develop the permanent integration of multiple identities across boundaries based on distinct national cultures is a fundamental part of multinational struggle, but the results can be inclusive; multiple identities can be added rather than subtracted from each other. Transnationalism is well-known scholarly approach to studying the increased influence nationalities have on one another as an essential part of the globalization process, but here it is meant to shed light on the enhancement, rather than the integration of Korea and Canada. Domestically, this synergistic phenomenon is referred to as the cultural “mosaic” in Canada, as it includes a multiplicity of cultures, but here the specific relationship between these two far flung nations will be discussed. The discussion here of how individuals and groups coming from their own cultural milieu approach each other, how they mix and how they understand each other is, out of necessity mostly limited to the perspective a Korean-Canadian might have of Canada, rather than the perspective a Canadian-Korean might have of Korea, but it also includes general reflections on Canadian culture, both domestic and international. To this end, the paper has been divided into sections based on three concepts: comparison, emphasis and commonality. This is meant to be an analysis of and a reflection on the positive, which presents a general blueprint for the continued development of international relations and the accruement of cultural knowledge between South Korea and Canada.

        Learning Curve for Robot-Assisted Percutaneous Pedicle Screw Placement in Thoracolumbar Surgery

        Study Design: Retrospective review of an initial cohort of consecutive patients undergoing robot-assisted pedicle screw placement. Purpose: We aimed to evaluate the learning curve, if any, of this new technology over the course of our experience. Overview of Literature: Percutaneous pedicle screws have specific advantages over open freehand screws. However, they require intraoperative imaging for their placement (e.g., fluoroscopy and navigation) and require increased surgeon training and skill with the learning curve estimated at approximately 20–30 cases. To our knowledge, this is the first study that measures the learning curve of robot-guided purely percutaneous pedicle screw placement with comprehensive objective postoperative computed tomography (CT) scoring, time per screw placement, and fluoroscopy time. Methods: We included the first 80 consecutive patients undergoing robot-assisted spinal surgery at Melbourne Private Hospital. Data were collected for pedicle screw placement accuracy, placement time, fluoroscopy time, and revision rate. Patient demographic and relevant perioperative and procedural data were also collected. The patients were divided equally into four sub-groups as per their chronological date of surgery to evaluate how the learning curve affected screw placement outcomes. Results: Total 80 patients were included; 73 (91%) had complete data and postoperative CT imaging that could help assess that placement of 352 thoracolumbar pedicle screws. The rate of clinically acceptable screw placement was high (96.6%, 95.4%, 95.6%, and 90.7%, in groups 1 to 4, respectively, p=0.314) over time. The median time per screw was 7.0 minutes (6.5, 7.0, 6.0, and 6.0 minutes in groups 1 to 4, respectively, p=0.605). Intraoperative revision occurred in only 1 of the 352 screws (0.3%). Conclusions: We found that robot-assisted screw placement had high accuracy, low placement time, low fluoroscopy time, and a low complication rate. However, there were no significant differences in these parameters at the initial experience and the practiced, experience placement (after approximately 1 year), indicating that robot-assisted pedicle screw placement has a very short (almost no) learning curve.

        Dietary and herbal supplements for fatigue: A quality assessment of online consumer health information

        Background The Internet is increasingly utilized by patients to acquire information about dietary and herbal supplements (DHSs). Previously published studies assessing the quality of websites providing consumer health information about DHSs have been found to contain inaccuracies and misinformation that may compromise patient safety.. The present study assessed the quality of online DHSs consumer health information for fatigue. Methods Six unique search terms were searched on Google, each relating to fatigue and DHSs, across four countries. Across 480 websites identified, 48 were deemed eligible and were quality assessed using the DISCERN instrument, a standardized index of the quality of consumer health information. Results Across 48 eligible websites, the mean summed score was 47.64 (SD = 10.38) and the mean overall rating was 3.06 (SD = 0.90). Commercial sites were the most numerous in quantity, but contained information of the poorest quality. In general, websites lacked discussion surrounding uncertainty of information, describing what would happen if no treatment was used, and how treatment choices affect overall quality of life. Conclusion Physicians and other healthcare professionals should be aware of the high variability in the quality of online information regarding the use of DHSs for fatigue and facilitate open communication with patients to guide them towards reliable online sources.         Bailey pairs and strange identities

        Zagier introduced the term ``strange identity" to describe an asymptotic relation between a certain $q$-hypergeometric series and a partial theta function at roots of unity. We show that behind Zagier's strange identity lies a statement about Bailey pairs. Using the iterative machinery of Bailey pairs then leads to many families of multisum strange identities, including Hikami's generalization of Zagier's identity.

        Language, Gesture, and the Task of Theatre: András Visky’s Dramaturgical Ethics

        Beginning with András Visky’s seminal essay “In Search of a Lost Reality,” this article uncovers important insights for a 21st century understanding of theatre’s role in human culture. Moving from Visky’s essay to his Juliet and another play by Pamela Gien’s The Syringa Tree, this article locates the features of drama which correspond to Visky’s dramaturgy, and bids us understand some ethical implications of theatre. These ethics spring directly from the use of absurdist language, gesture, and the hope for theatre’s role in allowing communities to respond to violence, both political and personal. A comparison to Gien demonstrates that Visky’s dramatic approach is applicable even to plays which may, in their textual presentation, appear very different from Visky’s own, suggesting that the task of theatre envisioned by Visky applies beyond Visky’s own oeuvre. Whereas Visky as a dramaturg invites and calls actors to use gesture as a tool for enacting the ethic of theatre, Gien implies an agreement with Visky not only in her staged collaborations, but also in the textual directions for gesture which she provides in the play script itself. The comparison reveals theatre as a site for audience members to re-examine traumas within their own cultural memory and practice, whilst avoiding the worst tendencies of conscious self-victimisation. Finally, the essay briefly points to new work in the cognitive humanities which corroborates Visky’s stated aims and solidifies the claim that Visky’s theatrical ethic helps to deepen our understanding of theatre as a site of historical and political catharsis.

        Complementary and integrative medicine mention and recommendations: A systematic review and quality assessment of lung cancer clinical practice guidelines

        Background: Complementary and integrative medicine (CIM) use is widely sought by those diagnosed with cancer, with up to 50% of lung cancer patients seeking these therapies in the United States. The purpose of this study was to identify the quantity and assess the quality of CIM recommendations in clinical practice guidelines (CPGs) for the treatment and/or management of lung cancer. Methods: A systematic review was conducted to identify lung cancer CPGs. MEDLINE, EMBASE and CINAHL were searched from 2008 to 2018, along with the Guidelines International Network and the National Center for Complementary and Integrative Health websites. Eligible guidelines containing recommendations for the treatment and/or management of lung cancer were assessed with the Appraisal of Guidelines, Research and Evaluation II (AGREE II) instrument. Results: From 589 unique search results, 4 guidelines mentioned CIM, of which 3 guidelines made CIM recommendations. Scaled domain percentages from highest to lowest were: scope and purpose (82.4% overall, 76.9% CIM), clarity and presentation (96.3% overall, 63.0% CIM), editorial independence (61.1% overall, 61.1% CIM), rigour of development (62.5% overall, 54.9% CIM), stakeholder involvement (66.7% overall, 42.6% CIM) and applicability (29.9% overall, 18.8% CIM). Quality varied within and across guidelines. Conclusions: Guidelines that scored well could serve as a framework for discussion between patients and healthcare professionals regarding use of CIM therapies in the context of lung cancer. Guidelines that scored lower could be improved according to the AGREE II instrument, with insight from other guidelines development resources.
