        The role of Lactobacillus reuteri DSM 17938 for the absorption of iron preparations in children with iron deficiency anemia

        Purpose: To determine whether Lactobacillus reuteri DSM 17938 plays a role in absorption of iron preparations given to children with iron deficiency anemia (IDA). Methods: We performed a quasi-experimental study involving pre- and postintervention tests using a control group in North Sulawesi province, Indonesia, between July and September 2017. We conducted a single-blind controlled trial that included primary school children who were diagnosed with IDA based on reticulocyte hemoglobin equivalent (Ret-He) levels <27.8 pg/L. Results: A total of 66 children were randomized into 2 groups. Thirty-four children received iron preparations with the addition of L. reuteri DSM 17938 (group 1), whereas the other 32 received iron preparations alone (group 2). The baseline Ret-He levels before intervention were similar in both groups. After 14 days of intervention, mean Ret-He level in group 1 changed from 24.43±1.64 to 28.21±1.72 pg/L (P=0.000). Mean Ret-He level in group 2 changed from 24.31±1.42 to 27.03±2.14 pg/L (P=0.000). Statistical analysis showed a significant increase in Ret-He levels in both groups; Ret-He levels were significantly higher in the experimental group than in the control group (P<0.05). Conclusion: Children with IDA receiving iron preparations with L. reuteri DSM 17938 for 14 days show higher Ret-He levels than those receiving iron preparations alone.

      • The Exigency of Being Parrhèsiastic: On Truth-telling in Socratic Dialogue Groups with Survivors of Cancer

        A philosophical dialogue about the existentially disruptive situation left by cancer can be constructive for life post cancer. In this article I suggest that a Foucauldian interpretation of parrhèsia is a valuable lens that can magnify the kind of dialogue that takes place among survivors of cancer participating in a Socratic Dialogue Group (SDG) as well as conceptualize a fundamental precondition underlying a good philosophical dialogue. After a brief account of the concept of parrhèsia, the article will identify its particular modalities within a SDG and analyze these by using illustrative examples taken from my research. Through these examples I study how parrhèsia can help articulate the relation between truth and subject and display the philosophical objective in a SDG and, finally, the article will show what parrhèsia presents in a setting of this nature both in terms of its potential and its problems.

      • Staging and Performing Dialogue: Employing the Theater Metaphor as a Methodological Strategy for Facilitating a Socratic Dialogue Group

        A Socratic Dialogue Group (SDG) harbors human drama in the stories and reflections of its participants. Through the use of a theater metaphor, I theoretically expand the method and practical action of SDG, whereby a distinctive approach is developed that emphasizes the art and craftsmanship involved in facilitating Socratic dialogue. It also allows me to bring forth the particular dramatic circumstances of the participants and the communal effort of each individual player in the ensemble. The micro cosmos of SDG incarnates the living, moving world that theater represents. It connects the drama of an individual life to the larger-scale human stories of moral tragedies and the empowering resilience that we see depicted in great plays. In 2012-2013, I conducted three SDGs in connection with a research project involving seventeen rehabilitating cancer patients. In this article, I use these dialogues as a paradigmatic case to illustrate my theater metaphor.

        Information Structure and Referential Givenness/Newness: How Much Belongs in the Grammar?

        This paper is concerned with such concepts as `topic`, `focus` and `cognitive status of discourse referents`, which have been included under the label `information structure`, as they relate in some sense to the distribution of given and new information. It addresses the question of which information structural properties are best accounted for by grammatical constraints and which can be attributed to non-linguistic constraints on the way information is processed and communicated. Two logically independent senses of given-new information are distinguished, one referential and the other relational. I argue that some phenomena pertaining to each of these senses must be accounted for in the grammar, while others are pragmatic effects that do not have to be represented in the grammar, since they result from interaction of the language system with general pragmatic principles that constrain inferential processes involved in language production and understanding.

      • An Assessment of the Viability of Producing True-to-type, Virus-free, Tissue-cultured Garlic in the Philippines

        Considering that the Philippines’ garlic production cannot cope up with the local demand requirements, this supply gap presents itself as a bright spot in garlic farming. As the demand for garlic far outpaces production levels which remain low due to poor yields exacerbated by traditional growing practices of garlic farmers, there is a need to demonstrate whether producing true-to-type, virus-free, tissue-cultured garlic propagation is the answer to bridge such a supply gap. This study, which is part of a larger research study focused on looking into the technology utilization of producing true-to-type, virus-free, tissue-cultured (TC) garlic planting materials for farmers, aimed to evaluate the feasibility of producing in vitro bulblets from multiplied shoots and bulb production under greenhouse and field conditions so that better quality and high volumes of garlic planting materials may be available to garlic farmers. Data gathering was done through face-to-face interviews with tissue culture laboratory personnel, farmers and municipal agriculture officers and from secondary and online sources. Using comparative analysis with traditional garlic production methods, cost and return, benefit-cost ratio and NPV, it was revealed that TC garlic planting materials are more costly and beyond the reach of farmers. Moreover, current traditional garlic practices are more profitable compared to TC garlic production (whether as G0 in vitro bulblets from multiple shoots or as G2 bulblets). Though the sales potential of farmers using TC planting materials is about three times more than that of traditional farmers, their net income is only half of that of traditional farmers because challenges remain in terms of bringing the production cost of TC planting materials down in view of lack of economies of scale. It is imperative that continuing improvements in research laboratory protocols be done to bring down the gestation period of TC planting materials from 3 years to one year and address the high contamination rates in the production process. In addition, economies of scale must be considered in choosing at which production level to operate.

        Erectile Dysfunction in Men with Adult Congenital Heart Disease: A Prevalent but Neglected Issue

        Background and Objectives: For adult men with congenital heart disease (ACHD), data on erectile dysfunction (ED) is limited. We aimed to assess the frequency of ED, its role in patient-physician communication and to identify parameters predicting ED. Methods: Male ACHD ≥18 years registered at the German National Register for Congenital Heart Defects were invited to participate in an online questionnaire about sexual health. Participants with presumed ED according to International Index of Erectile Function Score were compared to patients without ED. Results: The 371 patients responded to the questionnaire (83% with moderate to highly complex ACHD). The 43% presented with more than mild ED. When ED was present, patients complained about general anxiety to be sexually active more often (p<0.05) and underwent sexual activity less frequently compared to those without ED (p<0.05). Age ≥40 years (odds ratio [OR], 3.04; p=0.002), being single (OR, 6.82; p<0.0001), anxiety to be sexually active (OR, 2.64; p=0.0002) and psychiatric disease (OR, 4.33; p<0.0007) emerged as independent predictors for ED. Overall, patients sought medical advice in 6.7% of cases, whilst 29.6% would appreciate an active approach by the physician to address this sensitive topic. Conclusions: ED is affecting one third to one half of male ACHD according to a questionnaire-based analysis. Older age, being single, fear of sexual activity due to ACHD and psychiatric disorder emerged as independent predictors for ED. These parameters can easily be assessed to identify patients at risk. ED should be addressed proactively by health professionals.
