Risk factors for mortality in patients with bloodstream infections during the pre-engraftment period after hematopoietic stem cell transplantation

        Igor Stoma,Igor Karpov,Natalia Milanovich,Anatoly Uss,Igor Iskrov 대한혈액학회 2016 Blood Research Vol.51 No.2

BackgroundBloodstream infections (BSI) remain a frequent complication during the pre-engraftment period after hematopoietic stem cell transplantation (HSCT), resulting in high mortality rates. This study evaluated risk factors for mortality in hematopoietic stem cell transplant recipients with BSI in the pre-engraftment period.MethodsThis prospective case control study was performed at the Center of Hematology and Bone Marrow Transplantation in Minsk, Republic of Belarus. Data relating to patient age and gender, date and type of transplantation, conditioning chemotherapy regimen, micro-organisms isolated from blood, and antibacterial therapy were prospectively collected from all hematopoietic stem cell recipients with microbiologically proven cases of BSI in the pre-engraftment period. The primary outcome was all-cause 30-day mortality after onset of febrile neutropenia. ResultsA total of 135 adult patients with microbiologically proven BSI after HSCT were studied, with 65.2% of cases caused by gram-negative microorganisms and 21.5% by non-fer-menting bacteria. Inadequate empiric antibacterial therapy and isolation of carbape-nem-resistant non-fermenting gram-negative bacteria (Acinetobacter baumannii and Pseudomonas aeruginosa) were independently associated with increased all-cause 30-day mortality in these patients. ConclusionThe risk factors for mortality in adult patients with BSI in the pre-engraftment period after HSCT were inadequacy of empirical antibacterial therapy and isolation of carbapenem-re-sistant A. baumannii or P. aeruginosa.

Cryotherapy versus radiofrequency ablation in the treatment of dysplastic Barrett’s esophagus with or without early esophageal neoplasia: a systematic review and meta-analysis

Igor Logetto Caetité Gomes,Diogo Turiani Hourneaux de Moura,Igor Braga Ribeiro,Sérgio Barbosa Marques,Alexandre de Sousa Carlos,Beanie Conceição Medeiros Nunes,Bruno Salomão Hirsch,Guilherme Henrique 대한소화기내시경학회 2024 Clinical Endoscopy Vol.57 No.2

Background/Aims: Radiofrequency ablation (RFA) is the first-line therapy for dysplastic Barrett’s esophagus (BE). Therefore, cryotherapy has emerged as an alternative treatment option. This study aimed to compare the efficacies of these two techniques based on the rates of complete eradication of intestinal metaplasia (CE-IM) and dysplasia (CE-D). Adverse events and recurrence have also been reported. Methods: An electronic search was conducted using the Medline (PubMed), Embase, LILACS, and Google Scholar databases until December 2022. Studies were included comparing cryotherapy and RFA for treating dysplastic BE with or without early esophageal neoplasia. This study was performed in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. Results: Three retrospective cohort studies involving 627 patients were included. Of these, 399 patients underwent RFA, and 228 were treated with cryotherapy. There was no difference in CE-IM (risk difference [RD], –0.03; 95% confidence interval [CI], –0.25 to 0.19; p=0.78; I2=86%) as well as in CE-D (RD, –0.03; 95% CI, –0.15 to 0.09; p=0.64; I2=70%) between the groups. The absolute number of adverse events was low, and there was no difference in the recurrence rate. Conclusions: Cryotherapy and RFA were equally effective in treating dysplastic BE, with or without early esophageal neoplasia.

The Effect of Methylphenidate on Neurological Soft Signs in ADHD

        Igor Ondrejka,Igor Hrtanek,Ingrid Tonhajzerova,Eva Snircova,Tomas Kulhan,Ivan Farsky,Gabriela Nosalova 대한신경정신의학회 2015 PSYCHIATRY INVESTIGATION Vol.12 No.4

ObjectiveaaNeurological soft signs are very common in children with the attention deficit hyperactivity disorder (ADHD), and the first line medication of this disorder is methylphenidate. The aim of the study was to assess the effect of methylphenidate on the neurological soft signs in children and adolescents suffering from ADHD depending on the dose of methylphenidate. MethodsaaThirty five patients with ADHD were investigated by the ADHD RS-IV parent version questionnaire and the Revised Neurological Examination for Subtle Signs before treatment adjustment and after four weeks of methylphenidate medication. The changes in hyperactivity symptomatology, neurological soft signs during therapy and the influence of the methylphenidate dose were statistically analyzed. ResultsaaA significant decrease in hyperactivity symptomatology was found after one month of methylphenidate medication (p= 0.0001) and significant decrease in neurological soft signs was demonstrated in 21 from a total of 26 items (p<0.05). Correlation analysis showed no relationship between the dose of methylphenidate and the improvement of neurological soft signs. Similarly, the improvement of ADHD symptomatology had not correlation with the improvement of neurological soft signs. ConclusionaaThe study demonstrated the positive effect of methylphenidate on neurological soft signs in which improvement occurred independently of the dose, indicating that their progress may be due to methylphenidate treatment of any dose. The unrelated effect of methylphenidate on the attention deficit hyperactivity disorder and neurological soft signs suggest that methylphenidate might be useful in the therapy of clumsy child syndrome and in ADHD treatment of non-responders.

Data-Driven Healthcare and Cybercrime: A Threat We Are Not Aware Of?

Igor Vuletić 이화여자대학교 생명의료법연구소 2018 Asia Pacific Journal of Health Law & Ethics Vol.11 No.2

This paper deals with the problem of cybercrime as a rising threat to the healthcare sector. Security incidents involving different forms of cyberattacks are on the rise and are becoming one of the top security threats nowadays. The healthcare system is not an exception: moreover, it seems that it is one of the most vulnerable sectors for cyberattacks. Hackers prefer targeting healthcare organizations (especially hospitals) because they are more willing to pay the ransom to gain their crucial data back. Cyberattacks on hospitals paralyze their entire operational process and cause damage to both institutions and patients. A typical example is the recent attack on several U.S. hospitals, when one of the targeted hospitals paid around $17,000 USD in ransom for the decryption of their digital information. The purpose of this paper is to draw attention to scientific debates on this important aspect of Big Data in the healthcare system. The author describes the phenomenology of cyberattacks in healthcare and points to certain legal issues—such as conflict of jurisdictions, inadequate defining of criminal offences and the general lack of consensus on the catalog of incriminations—that can adequately respond to the needs of contemporary cybercrimes.

Numerical analysis on the behaviour of reinforced concrete frame structures in fire

        Igor M. Džolev,Meri J. Cvetkovska,Đorđe Ž. Lađinović,Vlastimir S. Radonjanin 사단법인 한국계산역학회 2018 Computers and Concrete, An International Journal Vol.21 No.6

Numerical approach using finite element method has been used to evaluate the behaviour of reinforced concrete frame structure subjected to fire. The structure is previously designed in accordance with Eurocode standards for the design of structures for earthquake resistance, for the ductility class M. Thermal and structural response are obtained using a commercially available software ANSYS. Temperature-dependent nonlinear thermal and mechanical properties are adopted according to Eurocode standards, with the application of constitutive model for the triaxial behaviour of concrete with a smeared crack approach. Discrete modelling of concrete and reinforcement has enabled monitoring of the behaviour at a global, as well as at a local level, providing information on the level of damage occurring during fire. Critical regions in frame structures are identified and assessed, based on temperatures, displacements, variations of internal forces magnitudes and achieved plastic deformations of main reinforcement bars. Parametric analyses are conducted for different fire scenarios and different types of concrete aggregate to determine their effect on global deformations of frame structures. According to analyses results, the threedimensional finite element model can be used to evaluate the insulation and mechanical resistance criteria of reinforced concrete frame structures subjected to nominal fire curves.

The Study on Highly Expressed Proteins as a Function of an Elevated Ultraviolet Radiation in the Copepod, Tigriopus japonicus

        Igor Z. Zubrzycki,이승한,이강현,Magdalena Wiacek1,이원철 한국해양과학기술원 2012 Ocean science journal Vol.47 No.2

The objective of the study was to analyze constantlyhighly expressed proteins as a function of elevated midultraviolet (UVB, 280-315 nm) radiation in Tigriopus japonicus sensu lato (T. japonicus s.l). We also analyzed associations between kinetics of radiation avoidance, measured as a covered distance per time unit, and highly expressed proteins. The obtained results indicate an increase in T. japonicus s.l. mobility between the control (no radiation) and mild UV radiation levels (15 kJ·m-2). Two-dimensional gel electrophoresis combined with MALDI-MS-MS resulted in 2D protein map comprising of 686 protein spots, of which 19 were identified as highly expressed proteins across all experimental conditions. Obtained results indicate that calpain, vitellogenin, and collagenase are housekeeping protein that are expressed at a constant level independently of environmental changes and that adoption of a locomotive system for the avoidance of a UV source may be, at least partially, supported by hepatopancreas-driven metabolism.
